ABSTRACT

The posterior mandible is often regarded as a reliable area for achieving primary stability during implant placement. However, in rare instances, implant displacement can occur in this region because of a significant decrease in bone density. Typically, surgical procedures such as the lateral or crestal approach have been used to remove displaced implants, requiring extensive bone removal and postponing implant placement for between 3 and 6 months. This clinical report presents an uncommon occurrence of implant displacement in the posterior mandible and introduces a less invasive approach to recovering it. This technique utilized the long screw of an open-tray impression post and an abutment to retrieve the displaced implant while simultaneously performing implant placement at the same site.

ABSTRACT

Introduction: The manifestations of herpes simplex virus keratitis range from epithelial keratitis to vision-threatening stromal keratitis. There are limited studies done on our part regarding stromal keratitis. The aim of the study was to find out the prevalence of stromal keratitis among herpes simples keratitis patients in a tertiary eye hospital. Methods: This descriptive cross-sectional study was conducted among patients who presented with herpes simplex virus keratitis in a tertiary eye hospital between 1 January 2020 to 28 February 2022. Ethical clearance was taken from Ethical Review Board (Reference number: 1/2079/80). Data was collected from hospital records which was reviewed and visual acuity at presentation and at one-month follow-up, clinical details on examination were recorded and, diagnosis of the stage of disease given in the case record was noted. Convenience sampling was used. Point estimate and 95% Confidence Interval were calculated. Results: Among 112 Herpes simplex keratitis patients, the prevalence of stromal keratitis was 38 (33.93%) (25.16-42.70, 95% Confidence Interval). Conclusions: The prevalence of stromal keratitis among patients of herpes simplex keratitis was similar to studies conducted in similar settings. ABSTRACT

BACKGROUND: Patient satisfaction is an individual's positive evaluation of a distinct dimension of health care. This study aimed to assess client satisfaction with maternity care in a tertiary hospital in the western region of Nepal. METHODS: This cross-sectional study was conducted among 405 delivered women. A structured questionnaire of 21 items was used as a tool during exit interviews at their discharge. Each item was assessed on 5 points Likert scale. Student's t-test and one-way ANOVA tests were applied to analyse parametric data. Binary logistic regression analysis was applied to identify the association of predictors with the satisfaction level. Strength of association was interpreted using odds ratio and 95% confidence interval. RESULTS: The mean waiting time was 16.94 +5.57 minutes. Education level of the clients was statistically significant with the mean satisfaction score (p<0.001). Women with unwanted pregnancies were more satisfied than those with wanted pregnancies (p<0.001). Lower complete satisfaction rates were observed for pain control (6.4%), information at discharge (14.6%) and the degree of privacy in the ward (13.6%). Mothers who underwent instrumental delivery were less satisfied with health worker's attitudes than those having vaginal deliveries (odds ratio 0.20, 95% confidence interval:0.05-0.84). Those having uncomplicated deliveries were 3.5 times more likely to be satisfied (odds ratio 3.50, 95% confidence interval: 1.1-11.5) with health workers communication. Mothers with waiting time less than 15 minutes were more satisfied. CONCLUSIONS: Lower educational level, unwanted pregnancy and shorter waiting time were associated with higher maternal satisfaction.

ABSTRACT

The present study aimed to investigate the effects of chitinase-3-like protein 1 (YKL-40) gene RNA interference on the biological behaviors and enhanced chemosensitivity of endometrial cancer (EC) HEC-1A cells. YKL-40 small interfering (si)RNA was transduced into EC HEC-1A cells using a lentivirus. The experiment was divided into three groups: The experimental group was transfected with YKL-40 siRNA (si-YKL-40); the mock-treatment group was transfected with transfection reagent only; and the blank control group was left untreated. A reverse transcription-quantitative polymerase chain reaction was performed to investigate the mRNA expression levels of YKL-40. The biological behaviors, including cell proliferation, migration, invasion and apoptosis, were detected by MTT and Transwell assays, and flow cytometry (FCM) analysis, respectively. The results of the present study demonstrated that the mRNA expression levels of YKL-40 were downregulated within HEC-1A cells upon transfection with si-YKL-40 (P<0.05). The proliferative, migratory and invasive abilities of HEC-1A cells were inhibited by si-YKL-40 (P<0.05). The mRNA expression levels of YKL-40 were upregulated within HEC-1A cells following treatment with cisplatin (P<0.05). FCM analysis revealed that the average cellular apoptosis rate increased following the inhibition of YKL-40 gene expression via siRNA (P<0.05). Therefore, the YKL-40 gene may be associated with the proliferative, migratory, invasive and anti-apoptotic ability of HEC-1A cells. YKL-40 downregulation may enhance the sensitivity of human EC HEC-1A cells to chemotherapy.

ABSTRACT

Recognizing clinical style is essential for generating intelligent guidance in virtual reality simulators for dental skill acquisition. The aim of this study was to determine the potential of Dynamic Time Warping (DTW) in matching novices' tooth cutting sequences with those of experts. Forty dental students and four expert dentists were enrolled to perform access opening to the root canals with a simulator. Four experts performed in manners that differed widely in the tooth preparation sequence. Forty students were randomly allocated into four groups and were trained following each expert. DTW was performed between each student's sequence and all the expert sequences to determine the best match. Overall, the accuracy of the matching was high (95%). The current results suggest that the DTW is a useful technique to find the best matching expert for a student so that feedback based on that expert's performance can be given to the novice in clinical skill training.
