ABSTRACT

Vancomycin is a bactericidal antibiotic used for various infections but can cause hypersensitivity reactions, including vancomycin flushing syndrome (VFS) and anaphylaxis. VFS, previously known as red man syndrome, is a pseudoallergic reaction characterized by flushing, erythema, and pruritus. We present a case of VFS in a female patient with recurrent Methicillin-resistant Staphylococcus aureus (MRSA) infections receiving vancomycin for back abscesses. Following the second dose, she developed a pruritic rash on her face, neck, and torso, which resolved with treatment. The differential diagnosis included hydromorphone allergy, ruled out due to previous tolerance. Anaphylaxis was unlikely due to the absence of respiratory distress, hypotension, or angioedema. Management involved discontinuing vancomycin, administering corticosteroids and antihistamines, and monitoring for anaphylaxis. The patient was transferred for surgical intervention and alternative antibiotic therapy. This case highlights the importance of recognizing and managing VFS, the significance of differential diagnoses, and the need for enhanced documentation and clinical support in managing vancomycin hypersensitivity reactions.

ABSTRACT

This comprehensive review explores the evolution and clinical impact of MitraClip intervention in the management of mitral regurgitation. Mitral regurgitation results from dysfunction in the mitral valve (MV) apparatus. The MitraClip Clip Delivery System was approved by the Food and Drug Administration (FDA) in 2013. The discussion delves into the procedural foundation of MitraClip intervention, primarily based on Alfieri's technique of edge-to-edge leaflet approximation. As highlighted by key clinical trials, including Endovascular Valve Edge-to-Edge Repair (EVEREST) II Trial, Cardiovascular Outcomes Assessment of the MitraClip Percutaneous Therapy for Heart Failure Patients with Functional Mitral Regurgitation (COAPT) Trial, and Percutaneous Repair with the MitraClip Device for Severe Functional/Secondary Mitral Regurgitation (MITRA-FR) trial, the efficacy and safety of MitraClip were evaluated in comparison to surgical interventions and guideline-directed medical therapy. Notably, the COAPT demonstrated significant benefits in reducing all-cause mortality and heart failure hospitalization, while the MITRA-FR presented contrasting results, emphasizing the importance of patient selection. An analysis of the EVEREST II trial underscores MitraClip's potential to achieve comparable outcomes to surgical intervention, emphasizing its role in reducing mitral regurgitation and improving clinical status. However, limitations and complications, such as device-related issues and the potential impact on future MV surgery, are discussed. The study also explores the evolving landscape of MV interventions, reflecting advancements and the growing acceptance of MitraClip. In conclusion, the MitraClip device represents a significant advancement in the treatment of mitral regurgitation. The data presented highlights its promising results in terms of reduced hospitalization rates, improved in-hospital mortality, and enhanced quality of life for patients. However, challenges remain, and careful consideration of patient selection and underlying pathology is crucial in determining the optimal treatment approach. Ongoing research and clinical experience will continue to refine our understanding of MitraClip's role in the evolving landscape of MV interventions.

ABSTRACT

The physiologic impact of pulsatile flow (PF) on end-organ perfusion during cardiopulmonary bypass (CPB) is controversial. Using an intra-aortic balloon pump (IABP) to maintain PF during CPB for patients undergoing heart transplantation (HT) may impact end-organ perfusion, with implications for postoperative outcomes. A single-center retrospective study of 76 patients bridged to HT with IABP was conducted between January 2018 and December 2022. Beginning in May 2022, patients received IABP-generated PF during CPB at an internal rate of 80 beats/minute. Fifty-eight patients underwent HT with the IABP turned off (IABP-Off), whereas 18 patients underwent HT with IABP-generated PF (IABP-On). The unmatched IABP-On group experienced shorter organ ischemia times (180 vs. 203 minutes, p = 0.015) and CPB times (104 vs. 116 minutes, p = 0.022). The cohort was propensity matched according to age, organ ischemia time, and CPB time. Elevations in postoperative lactates in the immediate (2.8 vs. 1.5, p = 0.062) and 24 hour (4.7 vs. 2.4, p = 0.084) postoperative periods trended toward significance in the matched IABP-Off group. There was no difference in postoperative vasoactive inotropic score (VIS), postoperative creatinine, or length of stay. This limited preliminary data suggest that maintaining counterpulsation to generate PF during CPB may improve end-organ perfusion in this patient population as suggested by lower postoperative lactate levels.

BACKGROUND: The benefits of fast-track extubation in the intensive care unit (ICU) after cardiac surgery are well established. Although extubation in the operating room (OR) is safe in carefully selected patients, widespread use of this strategy in cardiac surgery remains unproven. This study was designed to evaluate perioperative outcomes with OR vs ICU extubation in patients undergoing nonemergency, isolated coronary artery bypass grafting (CABG). METHODS: The Society of Thoracic Surgeons (STS) data for all single-center patients who underwent nonemergency isolated CABG over a 6-year interval were analyzed. Perioperative morbidity and mortality with ICU vs OR extubation were compared. RESULTS: Between January 1, 2017 and December 31, 2022, 1397 patients underwent nonemergency, isolated CABG; 891 (63.8%) of these patients were extubated in the ICU, and 506 (36.2%) were extubated in the OR. Propensity matching resulted in 414 pairs. In the propensity-matched cohort, there were no differences between the 2 groups in incidence of reintubation, reoperation for bleeding, total operative time, stroke or transient ischemic attack, renal failure, or 30-day mortality. OR-extubated patients had shorter ICU hours (14 hours vs 20 hours; P < .0001), shorter postoperative hospital length of stay (3 days vs 5 days; P < .0001), a greater likelihood of being discharged directly to home (97.3% vs 89.9%; P < .0001), and a lower 30-day readmission rate (1.7% vs 4.1%; P = .04). CONCLUSIONS: Routine extubation in the OR is a feasible and safe strategy for a broad spectrum of patients after nonemergency CABG, with no increase in perioperative morbidity or mortality. Wider adoption of routine OR extubation for nonemergency CABG is indicated.

ABSTRACT

Side-channel disassembly attacks recover CPU instructions from power or electromagnetic side-channel traces measured during code execution. These attacks typically rely on physical access, proximity to the victim device, and high sampling rate measuring instruments. In this work, however, we analyze the CPU instruction-level power side-channel leakage in an environment that lacks physical access or expensive measuring equipment. We show that instruction leakage is present even in a multitenant FPGA scenario, where the victim uses a soft-core CPU, and the adversary deploys on-chip voltage-fluctuation sensors. Unlike previous remote power side-channel attacks, which either require a considerable number of victim traces or attack large victim circuits such as machine learning accelerators, we take an evaluator's point of view and provide an analysis of the instruction-level power side-channel leakage of a small open-source RISC-V soft processor core. To investigate whether the power side-channel traces leak secrets, we profile the victim device and implement various instruction opcode classifiers based on both classical machine learning algorithms used in disassembly attacks, and novel, deep learning approaches. We explore how parameters such as placement, trace averaging, profiling templates, and different FPGA families (including a cloud-scale FPGA) impact the classification accuracy. Despite the limited leakage of the soft-core CPU victim and a reduced accuracy and sampling rate of on-chip sensors, we show that in a worst-case scenario for the evaluator, i.e., an attacker breaching physical separation, we can identify the opcode of executed instructions with an average accuracy as high as 86.46%. Our analysis shows that determining the executed instruction type is not a classification bottleneck, while leakages between instructions of the same type can be challenging for deep learning models to distinguish. We also show that the instruction-level leakage is significantly reduced in a cloud-scale FPGA scenario with higher soft-core CPU frequencies. Nevertheless, our results show that even small circuits, such as soft-core CPUs, leak potentially exploitable information through on-chip power side channels, and users should deploy mitigation techniques against disassembly attacks to protect their proprietary code and data.

ABSTRACT

Objective: Totally endoscopic intracardiac robotic surgery is generally limited to uncomplicated mitral valve surgery. With experience, our team has developed a more aggressive approach to robotic cardiac surgery that allows for repair of a broad spectrum of mitral valve pathologies. We report complex subvalvular procedural advancements associated with this approach secondary to enhanced team experience and capabilities. Methods: All robotic mitral procedures performed by a 2-surgeon team in a quaternary care medical center from July 2011 to May 2022 were reviewed. Natural language-processing techniques were used to analyze operative reports for subvalvular repair techniques. Complex subvalvular techniques included papillary muscle repositioning, division of secondary anterior leaflet chordae, septal myomectomy, division of aberrant left ventricular muscle band attachments, and left ventricular patch reconstruction. The surgical experience was divided into 2 periods: early robotic experience (pre-2018) versus late (2018 onwards). Baseline demographics, outcomes, and subvalvular techniques were analyzed and compared. Results: A total of 1287 intracardiac robotic operations were performed by a 2-surgeon team. Thirty-day mortality was 0.6% (8/1287). Mitral valve repair was performed in 1024 patients. The mean age was 61 years (range, 18-90 years), and 15% were >75 years old; 29 patients (2.8%) had previously undergone cardiac surgery. There was a significant increase with experience in the application of advanced subvalvular techniques between the early versus late period (52.3% [268/512] vs 74.2% [380/512] (P < .001)). Conclusions: An experienced 2-surgeon team can perform progressively more complex robotic subvalvular repair techniques. These subvalvular techniques are a surrogate for team proficiency and capabilities.

Introduction Intra-axial brain tumors are a significant health problem and present several diagnostic and treatment challenges. Conventional magnetic resonance imaging (MRI) has posed several limitations, such as the inability to delineate the detailed anatomy of fibers in structures like the brainstem and the inability to accurately judge the extent of tumor infiltration. Diffusion tensor imaging (DTI), based on the concepts of isotropic and anisotropic diffusion, is capable of visualizing and segmenting white fiber bundles in high detail and providing crucial information about tumor boundaries, extent, neighboring tracts, and more. This information can be very useful in initial non-invasive diagnosis, preoperative tumor grading, biopsy planning, surgical planning, and prognosis. Methods and materials This is a cross-sectional observational study in a tertiary care setup, conducted over a one-year period. The study was performed in Seth Gordhandas Sunderdas Medical College (Seth G.S. Medical College) and King Edward VII Memorial Hospital (K.E.M. Hospital), a tertiary care hospital located in Mumbai, India. Fiber tractography was performed and was used to visualize the corticospinal tracts passing through the length of the brainstem. Changes in the degree of infiltration, destruction, and displacement of the corticospinal tracts were observed carefully. Adult patients who were diagnosed with brain tumors, willing to participate in the study, and capable of providing written informed consent prior to study registration were included. The DTI findings along with information from other investigations were used to decide the best course of management for each case. Results The study included 30 participants with a mean age of 46.0 ± 17.1 years, 63.3% and 37.7% being male and female, respectively. According to the lesion's location, the pons was found to be the most often affected area in 23.33% of cases, followed by the temporo-parietal region (13.3%) and the frontal region (13.3%). These lesions had heterogenous enhancement in 63.3% of the instances and homogeneous enhancement in 36.7% of the cases, according to a contrast study. According to their consistency, the lesions were further divided into two categories: solid lesions, which were present in 66.7% of instances, and cystic lesions, which were present in 90% of cases. Results from the diffusion tensor technique revealed that infiltration accounted for 40.0% of cases, displacement for 76.7%, and loss of white fiber tracts for 20.0%. DTI findings were significantly associated with the type of planned management and with the presence of post-management neurological deficit. Conclusion DTI played a complementary role in the assessment of tumors and can be used to improve surgical planning and therapeutic decision making. Preservation of corticospinal tracts is vital to prevent motor impairment. Availability of qualitative data with the depiction of corticospinal tracts in a three-dimensional projection and their relation with the brain tumors by DTI greatly helps in preoperative decision making and surgical approach.

Over the past seven decades, significant advancements and innovations have occurred in the field of percutaneous atrial septal defect (ASD) closure using transcatheter-based devices. This article focuses on the current literature surrounding the three Food and Drug Administration (FDA)-approved devices for ASD and patent foramen ovale (PFO) closure in the United States, namely, the Amplatzer Septal Occluder (ASO), Amplatzer Cribriform Occluder, and Gore Cardioform ASD Occluder. The ASO has been widely used since its FDA approval in 2001. Studies have shown its high success rate in closing ASDs, especially small-sized defects. The RESPECT trial demonstrated that PFO closure using the ASO reduced the risk of recurrent ischemic stroke compared to medical therapy alone. The Closure of Atrial Septal Defects With the Amplatzer Septal Occluder Post-Approval Study (ASD PMS II) evaluated the safety and effectiveness of ASO in a large cohort of patients, reporting a high closure success rate and rare hemodynamic compromise. The Amplatzer Cribriform Occluder is designed for the closure of multifenestrated ASDs and has shown promising results in small-scale studies. It successfully closed the majority of fenestrated ASDs, leading to improved right ventricular diastolic pressure without major complications. The REDUCE trial compared PFO closure using the Gore Helex Septal Occluder and Gore Cardioform Septal Occluder with antiplatelet therapy alone. The study demonstrated that PFO closure significantly reduced the risk of recurrent stroke and brain infarction compared to antiplatelet therapy alone. However, the closure group had a higher incidence of atrial fibrillation or atrial flutter. There is a risk of atrial fibrillation with the use of ASO as well. The FDA-approved Gore Cardioform ASD Occluder showed excellent performance in the ASSURED clinical study. The device achieved high technical success and closure rates, with low rates of serious adverse events and device-related complications. A meta-analysis comparing transcatheter ASD closure with surgical closure revealed that the transcatheter approach had a high success rate, lower rates of adverse events, and shorter hospital stays compared to surgery, without any mortality. Complications associated with transcatheter ASD closure have been reported, including femoral arteriovenous fistulas, device embolization, cardiac erosion, aortic incompetence, and new-onset migraine. However, these complications are relatively rare. In conclusion, transcatheter ASD closure using FDA-approved devices has proven to be safe and effective in the majority of cases. These devices offer excellent closure rates, reduced risk of recurrent stroke, and shorter hospital stays compared to surgery. However, careful patient selection and follow-up are necessary to minimize complications and ensure optimal outcomes.

Introduction Age estimation has been an area of special interest in the medicolegal context because of its necessity in various criminal and civil cases like assaults, murders, rapes, inheritance, insurance claims, etc. While legal documents are useful in daily activities that require age identity, they cannot be relied on for criminal and civil proceedings because of being falsifiable and inaccessible to some people. Scientific methods of age determination like physical, dental, and radiological examinations are used for reliable age estimation due to their universal and non-falsifiable nature. The skeletal examination is of great importance here because the human skeleton provides many sites for age estimation in different age groups. The xiphisternal joint between the xiphoid process and the body of the sternum provides one such opportunity in participants of 35-50 years of age. The ossification in this joint proceeds gradually in approximately the third to fifth decade of life; this natural variation in the morphology of the joint can be leveraged for age estimation. Previous studies showed that the mean age of fusion varied with the ethnicity of individuals and environmental factors. Thus, it is critical to have statistical information for the concerned population to avoid errors. Also, the relation of gender with the mean age of complete fusion remained ambiguous with the previous studies. The xiphisternal joint can be studied by radiological techniques like computed tomography (CT) and plain radiographs. Radiological methods have the benefit that they can be used on both living and dead participants and are non-invasive. The present study aims at gathering data relevant for use in India (Maharashtra) and to find out the reference age group in which there is complete ossification of the xiphisternal joint in males and females.  Methods and materials This was a cross-sectional observational study in a tertiary care setup over a period of one year. High-resolution computed tomography (HRCT) was used for assessing joint fusion due to its high spatial resolution. The participants were included in the study if they were referred for HRCT chest by a physician for some pathology, did not have any trauma or lesion of the sternum and consented to the use of their information for the purpose of this study. Results The study included a total of 384 participants, out of whom 195 (50.8%) were males and 189 (49.2%) were females. The mean age of participants was 42.87 years. The mean age of complete xiphisternal joint fusion was observed to be 46.31 years (95% CI: 45.61 to 47.00) in males and 45.57 years (95% CI: 44.73 to 46.42) in females. Similarly, the mean age of participants with an unfused xiphisternal joint was observed to be 38.42 years (95% CI: 37.47 to 39.39) in males and 37.85 years (95% CI: 37.14 to 38.57) in females. There was no statistically significant difference in the age above which males and females show complete ossification of the xiphisternal joint. Conclusion The xiphisternal joint fusion can be used to determine the chronological age of an individual. It can be estimated as lesser than or equal to 45 years if the xiphisternal joint is unossified and greater than or equal to 37 years if the joint is ossified, with a 95% level of confidence.

Dementia is one of the major causes of disability and hospitalization in the elderly. As far as non-invasive markers of dementia are concerned, we only have age and Apolipoprotein-E (Apo-E) gene, which can be considered as clinically relevant. Modifiable risk factors have been found to be the cause in one-third of the patients who develop dementia. The compatible data supporting the same, in particular for dyslipidemia, is limited, which in turn makes it difficult to devise prevention and interventional methods for both dementia and mild cognitive impairment. Hence, the objective of the review is to summarize the findings on the relation established between the high-density lipoprotein type C( HDL-C) levels and lower the chance of dementia in the elderly, and the possible role of HDL-C as a potential predictive biomarker for cases of dementia in elderly people. Dyslipidemia, a known risk factor for the occurrence of cardiovascular diseases, seems to be linked to Alzheimer's disease. Elevated levels of serum cholesterol in mid-adult life increases the risk of dementia in older age. But elevated high-density lipoprotein (HDL) level and its principal apolipoprotein A-I (ApoA-I ) equates with a low risk of dementia in the elderly population HDL cholesterol has been found to promote endothelial nitric oxide synthase activity which in turn reduces the neural and vascular inflammation and suppresses vascular adhesion thereby exhibiting its vasoprotective function. It has been believed that all these factors have a role to play in the pathogenesis of dementia. The relation between the higher levels of HDL cholesterol or its key protein component ApoA-I and the lower dementia prevalence in the elderly had been documented in numerous observational studies. Some studies have reported conflicting results. Yet, observational studies measuring the baseline HDL level in middle age found a significant association between HDL level and dementia risk in the elderly, whereas those studies measuring HDL cholesterol level only in old age found no association. Likewise, a significant association between HDL cholesterol and dementia risk has been reported with studies that carry through to 10 years or longer. However, the studies with follow-up of fewer than 10 years had failed to document any such association between HDL cholesterol and dementia.  HDL assays may also be used as a predictive biomarker for dementia patients to target the interventions. Although statins do not target HDL directly but can be an area of interest for dementia.

Ischemic colitis (IC) is one of the most feared complications after abdominal aortic aneurysm (AAA) repair. Though the complication is seen in only a handful of total repairs, the mortality rates after IC is very high. Due to infrequent presentation, attending doctors may overlook this possibility leading to a delay in diagnosis. Open repair and endovascular aneurysm repair (EVAR) are the two most common methods used for repairing aneurysms and both of these procedures have their implications in the development of IC. While the incidence of IC was greater after open repair, it also harbored more patients with ruptured aneurysms, emergency repairs, and patients in shock. Similarly, a significant proportion of patients having ruptured aneurysms developed IC. Controlling minor variables like acidosis, hypothermia, hypovolemia, and clamp time duration can play a cumulative role in diminishing this hazardous complication. Medical advancements and minimally invasive technologies have improved the quality of care and operation success considerably, but researchers have not identified a statistically significant association in the prevention of postoperative IC. Therefore, early diagnosis and prompt management become crucial in reducing mortality rates. This can be achieved by being aware of impending signs and symptoms especially in patients with risk factors and being proactive in medical management.

Patients with severe symptomatic aortic stenosis (AS) are categorized into high risk, intermediate risk, and low risk. The identification of risk status is done using the Society of Thoracic Surgeons mortality score. Various factors are considered such as clinical symptoms, ejection fraction, age, left ventricle measurements, severity of AS, associated comorbid factors, and any other associated cardiac diseases. Surgery is still a standard practice in many countries. However, it has its own complications, especially in high-risk patients. Transcatheter intervention is getting precipitous recognition as an alternative mode of treatment in selected cases to mitigate complication rates and improve quality of life. In this article, transcatheter aortic valve replacement and surgical aortic valve replacement are compared in patients with different surgical risks. The impact of the cost of the procedure and quality of life are of paramount importance in choosing the type of intervention. Structural valve degeneration is an independent risk factor affecting patient outcomes. Modifications in valve designs are being constantly implemented as well. The standard analytical methods are in accordance with randomized clinical trials to determine the efficacy and outcome of procedures. Primary and secondary endpoints were considered to evaluate the data. The results were tabulated to derive statistical significance of the studies. In high-risk surgical patients, transcatheter intervention has been proven as the procedure of choice for valve replacement. However, intermediate-risk and low-risk categories need further studies.
