ABSTRACT

【Objective】 To investigate the association between HLA-A, B, DRB1 alleles and chronic renal failure (CRF) in Han population of Shandong Peninsula. 【Methods】 Sequence specific oligonucleotide probe-polymerase chain reaction (PCR-SSO) was used to genotype 880 patients with CRF in the Han population of Shandong Peninsula. The allele frequencies of HLA-A, B and DRB1 were compared with 865 hematopoietic stem cell voluntary donors, and the association between HLA gene polymorphism and CRF was analyzed. 【Results】 A total of 33 HLA-A alleles, 76 HLA-B alleles and 39 HLA-DRB1 alleles were detected in the study group. DRB1*11∶01 (6.70% vs 4.45%) and DRB1*12∶02 (8.69% vs 5.90%) in CRF group were significantly higher than those in the control(Pc<0.05), and B*15∶11 (1.82% vs 3.64%) among CRF group was significantly lower compared with the control(Pc<0.05). The frequency of three loci haplotypes A*30∶01 -B*13∶02 -DRB1*07∶01 (16.61% vs 7.61%), A*33∶03 -B*58∶01 -DRB1*03∶01 (4.57% vs 1.62%) and A*02∶07 -B*46∶01 -DRB1*09∶01 (4.06% vs 1.09%) in CRF patients were significantly higher than that of the control(Pc<0.05), which were strongly correlated with CRF. 【Conclusion】 The data on the association of HLA-A, B, DRB1 alleles and haplotype polymorphisms with CRF in Shandong Peninsula has been obtained in this study. DRB1*11∶01 and DRB1*12∶02 may be the susceptibility risk factors for development of CRF, and B*15∶11 may be protective genes against development of CRF, and A*30∶01 -B*13∶02 -DRB1*07∶01, A*33∶03 -B*58∶01 -DRB1*03∶01 and A*02∶07 -B*46∶01 -DRB1*09∶01 may be the susceptible haplotypes in Han population of Shandong Peninsula.

ABSTRACT

Objective@#To compare the predictive value of 10 prehospital assessment scales for large vessel occlusion in patients with acute ischemic stroke.@*Methods@#From January 2016 to December 2018, patients with acute ischemic stroke within 24 h of onset admitted to the Department of Neurology, Nanfang Hospital, Southern Medical University were enrolled retrospectively. The scores of various scales were calculated based on clinical data, including the National Institutes of Health Stroke Scale (NIHSS), Los Angeles Motor Scale (LAMS), and 3-item stroke scale (3I- SS), Prehospital Acute Stroke Severity Scale (PASS), Ambulance Clinical Triage For Acute Stroke Treatment (ACT-FAST), and Stroke Vision, Aphasia, and Neglect Assessment Scales (VAN), etc. The predictive threshold of the NIHSS score was determined, and the receiver operating characteristic (ROC) curve and the area under the curve (AUC) were used to evaluate the effectiveness of various prehospital scales to predict large vessel occlusion.@*Results@#A total of 705 patients with acute ischemic stroke within 24 h of onset were enrolled, including 252 (35.7%) with large vessel occlusion. The best predictive cutoff value for judging large vessel occlusion by the NIHSS score was 9, sensitivity was 81.7%, specificity was 79.7%, positive likelihood ratio was 4.851, and negative likelihood ratio was 0.260. LAMS score ≥4 (sensitivity 88.1%, specificity 81.0%, positive likelihood ratio 4.640, and negative likelihood ratio 0.247), VAN positive (sensitivity 83.7%, specificity 82.3%, positive likelihood ratio 4.741, and negative likelihood ratio 0.198) and NIHSS score ≥9 were more accurate in identifying large vessel occlusion. The AUC values of the 8 quantitative scores were all > 0.7, and the AUC of LAMS was the largest (0.852, 95% confidence interval 0.825-0.878).@*Conclusions@#In patients with acute ischemic stroke within 24 h of onset, the NIHSS score ≥9 can be used as the best cutoff value for predicting large vessel occlusion events. LAMS, VAN, and NIHSS scales are more accurate in predicting large vessel occlusion. The predictive power of the 8 quantitative scales is higher, and the AUC of LAMS is the largest, which can be used for clinical prediction of large vessel occlusion in patients with acute ischemic stroke.

ABSTRACT

Objective To explore the effects of all-trans-retinoic acid (ATRA) on the proliferation of human lung adenocarcinoma cell line A549 and the expression of APLNR (apelin receptor) gene.Methods The inhibition of proliferation of human lung adenocarcinoma cell lines A549 cultured in vitro with or without ATRA was measured by MTT (methyl thiazolyl tetrazolium,MTT) method.The morphological changes in the cells were observed by light microscopy.The cell cycle and apoptosis were analyzed by flow cytometry.The levels of APLNR,cyclin D1 and p16 proteins were detected by western blot.Results After treatment of ATRA,the proliferation of A549 cells was obviously inhibited in dose-and time-independent manner (P < 0.01).The cell morphology was significantly changed.The cycle of A549 cells was blocked at G0/G1 phase and the apoptosis rate was increased.With the increasing concentration of ATRA,the expressions of cyclin D1 and APLNR were down-regulated but the expression of p16 was up-regulated (P < 0.01).Conclusion ATRA could inhibit the proliferation of A549 cells by retardant cell cycle of A549 cells at G0/G1 phase and inducing the apoptosis,and down-regulate the expression of APLNR gene.

ABSTRACT

Objective To investigate the vancomycin trough concentration in elderly patients with severe infection and provide references for clinical use of drugs effectively .Methods According to the Ccr of elderly patients (56 cases), who were ≥65 years old, the patients were divided into two groups , one with Ccr≥50 ml/min, which named group A , the other with Ccr between 20 ml/min and 50 ml/min, which named group B .The use of vancomycin , vancomycin steady-state plasma trough concentration and the differences of renal function were analyzed before and after in both two groups .Results There were 31cases of patients in group A (25 cases of them use vancomycin 1 g,q 12 h;other 6 cases use 0.5 g,q 12 h), 25 cases of patients in group B (15 cases of them use vancomycin 1 g, q 12 h;other 10 cases use 0.5 g,q 12 h) .The percentage that the trough concentration of the patients who use two different scheme of using drugs in group A was between 10 mg/L and 20 mg/L is 12%(3/25) and 66.67%(4/6).The percentage that the trough concen-tration of the patients who used two different scheme of using drugs in group B was between 10 mg/L and 20 mg/L is 20%(3/15) and 60%(6/10).The percentage that trough concentrationis more than 20 mg/L is 73.33%(11/15) and 30%(3/10).The serum creat-inine value of the 15 patients of all cases except for group B of which vancomycin dosage was 1 g, q 12 h had significantly risen after using the drug(P<0.05).The urea nitrogen had no obvious change (P>0.05).The serum creatinine value and urea nitrogen of oth-er patients had no significant change after using the drug (P>0.05).Group B had kidney toxicities observed in 5 patients, the dosage of vancomycin was 1 g, q 12 h, valley concentration >30 mg/L.There was no kidney toxicities observed in group A .Conclusion The use of vancomycin in elderly patients must depend on their Ccr .If the Ccr was larger than 50 ml/min, the use of vancomycin should be 1 g,q 12 h, and the Ccr was between 20 ml/min and 50 ml/min, the use should be 0.5 g.In addition, blood trough concentration need to be paid more attention in elderly patients in order to regulate the regimens according to the individual differences .

ABSTRACT

OBJECTIVE To study the relationship between Gardnerella vaginalis and male infertility. METHODS To test G.vaginalis in semen by nested PCR and to carry out correlation analysis with sperm parameter,volumetric count of leukocytes and AsAb.Totally 383 male cases with infertility,set up into positive group,other 297 cases set up into negative group. RESULTS G.vaginalis in semen was positive in 86 cases in positive group,their semen concentration,motility,a+b grade motile,and the swelling rate of hypoosmotic swelling test were observed to be evidently lower compared to negative group(P0.05). CONCLUSIONS G.vaginalis has an effect on sperm quality.

BACKGROUND: Although partner notification (PN) has long been considered as an important component in sexually transmitted disease (STD) control programs and has been implemented in both developed and developing world, little information is known about the acceptability and efficacy of the strategy in China. GOAL: The goal of this study was to assess the acceptability and outcome of self-referral for male patients with urethral discharge and to identify the characteristics associated with successful outcome based on a specialized STD clinic. METHODS: From November 2001 to November 2002, 730 patients with confirmed urethral discharge were selected and asked by STD clinicians to bring their sexual contacts to treatment in a specialized STD clinic. The demographic and sexual characteristics of returned and unreturned index patients were analyzed. Univariate and multivariate analyses were applied to identify the characteristics associated with the outcome of self-referral. RESULTS: Of 723 eligible index patients, 294 (40.7%) who returned for follow up identified 534 sexual partners and 429 (59.3%) who did not return for follow up identified 759 sexual partners. Of the total named 1293 sexual partners, 301 (23.3%) were notified by index male patients and 265 (20.5%) presented at the clinic. Of the 265 partners tested, 165 (62.3%) were infected with gonorrhea, chlamydia, or both; of them, 78 (47.3%) were asymptomatic. When the partnerships were married and steady, the informed rate was higher than that when the partnership was casual. For commercial sexual partnerships, only 1.8% of the partners were informed. In multivariate analysis, a prior STD history and condom use during the last sexual contact were associated with successful outcome of notification. CONCLUSION: Patient referral was accepted both by STD clinic attendees and STD clinicians. The results of the study show that use of self-referral for partners of men with urethral discharge in a Chinese STD clinic was, with minimal effort, moderately successful. Further efforts to improve case-finding outcomes are warranted.

ABSTRACT

With the decline in prevalence of leprosy, social and economic rehabilitation (SER) has become a major priority in leprosy control programme in Shandong Province. In the preparative phase of an SER programme, a province-wide survey was conducted with a semi-structured questionnaire in order to provide policy makers and programme managers with some basic information on the disability, and social and economic situation of the people affected by leprosy. This paper presents the results of a study in the people affected by leprosy living in the communities.

ABSTRACT

In the late phase of the leprosy control programme in Shandong Province, People's Republic of China, there are a few old and disabled ex-patients living in 54 leprosy villages/leprosaria. The small, and declining number of patients makes the running of these leprosy villages/leprosaria uneconomic. In this paper, we review the history and the role of leprosy villages/leprosaria in the care of leprosy patients and the control programme in Shandong province. We then analyse the present situation of the 643 people still living in these leprosy villages/leprosaria, using information collected from a questionnaire-based survey. Finally, we offer some suggestions and recommendations for policy makers and leprosy control managers, in order to improve the present situation and make better use of existing resources.

Objective To evaluate the consistency of global acne grading system (GAGS) and acne lesion counting. Methods After training 3 dermatologists evaluated 60 acne patients by GAGS and skin lesion counting respectively in randomized order sequentially, and evaluated again 5 days later. Results The kappa-value of GAGS (consistency) was 0.675 among 3 different dermatologists, and 0.847 for the same desmatologist. The kappa-value among the 3 different dermatologists and the same dermatologist for the total lesion count was 0.426 and 0.413 respectively; for pustule count was 0.446 and 0.407 respectively.
