ABSTRACT

A new direction with the use of endoscopic hemostasis (EH) was offered in the complex program of metabolic rehabilitation of patients with ulcerous gastroduodenal bleedings (UGDB) and hemorrhagic shock (HS). There was a study of the results of treatment of 44 patients (29 men and 15 women at the age from 61 to 77) with severe concurrent diseases and UGDB complicated with HS. Recurrences of UGDB in case of HS are a factor of high operative-anesthetic risk. Endoscopic preventive metabolic hemostasis is a prospective direction in rehabilitation of patients with UGDB complicated with HS. It is necessary to improve the EH methods for treatment of continued arterial bleedings from giant gastroduodenal ulcers.

ABSTRACT

Submitted in the article are medical causes of unfitness of those men called up for military service in peace-time. These include psychic dysfunctions (22%), traumata (18.5%), disorders of the nervous system and sensory organs (14.5%), of the osteomuscular system and connective tissue (13.3%), digestive diseases (8.6%). Mental disorders, those of the nervous system and sensory organs, the endocrine system and digestive organs rank first among causes of striking the serviceman off the register, coming up to 40.9%, 31.2%, and 6.8% respectively. Age has been established at which disease manifestations causing unfitness for military service come to reveal themselves: in 58.4 percent of registrants the above manifestations were first diagnosed in childhood, in 5.4 percent--at 16 to 17 years of age, in 36.2 percent--at call-up age.

ABSTRACT

The authors analyse the results of treatment of 185 patients with acute cholecystitis coexisting with affection of the bile ducts. In view of the high risk of operative treatment, endoscopic papillosphincterotomy was undertaken as the first stage in 83 patients and was completed by nasobiliary drainage in 46 of them, in still another 12 patients decompression was accomplished by means of laparoscopic cholecystostomy . Surgical intervention was carried out in the second stage after the patients recovered from a grave condition and obstructive jaundice and intoxication caused by purulent cholangitis were corrected. Another 90 patients underwent surgical operation without preliminary decompression in a postponed order (67 patients) and for emergency and urgent indications (19 patients) in vital conditions due to peritoneal phenomena. The changes in the bile ducts were corrected in them in the second stage, which reduced the risk of the development of postoperative complications. The authors emphasize the importance of two-stage treatment in coexistence of lesions and of a differential approach to the treatment of this contingent of patients according to the character and severity of the inflammatory process and the severity of the patients' condition.
