ABSTRACT

Objective:To study the clinical characteristics and death risk factors of the patients with fall-related injuries in parts of Kashgar Prefecture during 2019-2020.Methods:The clinical data were retrospectively analyzed of the 894 patients with fall-related injuries who had been admitted to Department of Trauma Center, The First People's Hospital of Kashgar Prefecture, Xinjiang Uygur Autonomous Region from January 2019 to December 2020. Recorded were the patient's gender, age, location of fall, month of fall, fall height, major injury site, injury severity score (ISS) and Glasgow score (GCS). The clinical characteristics and death risk factors of the fall patients were analyzed.Results:Of the 894 patients, 72.3%(646/894) were male and 86.9%(777/894) fell from a height from 1 to 6 meters. Their ages ranged mainly from 15 to 59 years old (74.3%, 664/894). Home was the most frequent site for falls (60.2%, 538/894) and the patients who fell in summer months (from June to August) were the most (32.3%, 289/894). Twenty-one patients (2.3%, 21/894) died. There were significant differences in the major injury site, blood transfusion, ISS score and GCS score between the dead and survival patients ( P<0.05). The multivariate logistic regression analysis showed that the injury to the head, face and neck [ OR=10.936, 95% CI: 1.177 to 101.627, P=0.035] and GCS score ≤12 [ OR=5.640, 95% CI: 2.658 to 11.968, P< 0.001] were the death risk factors for the patients with fall-related injuries in parts of Kashgar Prefecture during 2019-2020. Conclusions:In the patients with fall-related injuries in parts of Kashgar Prefecture during 2019-2020, males aged from 15 to 59 years old were the high-risk group of falls. Months with a high incidence of falls were from June to August. The fall patients with injuries to the head, face and neck and with a GCS score of ≤12 were at a high risk of death.

ABSTRACT

The clinical data of a child with bronchial Dieulafoy disease treated in Wuhan Children′s Hospital Affiliated to Tongji Medical College, Huazhong University of Science and Technology, was analyzed retrospectively.The patient was a 9-month-old boy, who was admitted to hospital due to " intermittent hematemesis vomiting blood for 6 hours" . Chest CT suggested ground-glass opacity in both lungs.Electronic bronchoscopy showed that the neoplasm bulged into the lumen at the opening of the right inferior lobar bronchus, and fresh blood oozed from the basal segment of the neoplasm during the operation.Bronchial arteriography and transcatheter bronchial artery embolization were performed due to recurrent hemoptysis, during which the patient was diagnosed with bronchial arterial vascular malformation and finally diagnosed with bronchial Dieulafoy disease after consulting the relevant literatures.The disease is infrequent and characterized by rupture hemorrhage of bronchial submucosal malformed artery, the etiology and pathogenesis of which are still unclear, and it may be related to congenital vascular malformation in children.Bronchoscopy for hemoptysis of unknown cause in children should be performed with caution.If small and smooth protruded nodular lesions are seen under the bronchoscope, the bronchial Dieulafoy disease should be considered, and the lesions should not be touched too much or subjected to biopsy blindly.Fatal massive hemorrhage can be avoided by bronchial arteriography and bronchial artery embolization.

ABSTRACT

The drawbacks of conventional inactivated Foot and Mouth Disease (FMD) vaccine, such as escaping of the virus during manufacture processes prompted researchers to explore novel types of vaccine to overcome these disadvantages. Listeria ivanovii (LI) is an intracellular microorganism that possesses immune-stimulatory properties, making it appropriate for use as a live bacterial vaccine vector. The Foot and mouth disease virus (FMDV) VP1 protein is the most immunogenic part of FMDV capsid, it has most of the antigenic sites for viral neutralization. The expression of antigen gene cassette in vitro was confirmed by Western blot analysis. Mice were able to eliminate LI△actAplcB-vp1 from the liver and spleen within few days revealed a safety of the candidate vaccine. Two doses of LI△actAplcB-vp1 with 14 days of interval were injected into mice. High levels of specific IgG antibodies and CD8+ and CD4+ T cells secreted cytokines including IFN-γ, TNF-α and IL-2 against FMDV-VP1 were achieved. Based on the obtained results, LI△actAplcB-vp1 candidate vaccine utilizing Listeria ivanovii as a live vector-based vaccine could enhance a specific cellular and humoral immune responses against the inserted FMDV-vp1 heterologous genes. LI△actAplcB-vp1 candidate vaccine could be a modern tool to overcome the disadvantages of the traditional inactivated FMD vaccine.

ABSTRACT

Objective:To investigate the acid resistance of tooth hard tissues irradiated by different power outputs of Er,Cr∶YSGG laser.Methods:Samples in laser groups were irradiated using Er,Cr∶ YSGG laser with the power outputs of 2.5,3.5 and 5 W for enamel and 2 W,3 W and 4 W for dentin,respectively.The calcium and phosphate ion dissolved was measured after decalcification in lactate buffer solution for 24 h.The atomic percentage of calcium and phosphate on the surface of samples was examined by X-ray energy dispersive spectroscopy and the morphological changes were investigated by SEM.Results:In all laser groups of enamel samples except 2.5 W group calcium and phosphate ion dissolved less than in control group and block group (P<0.05).There's no statistic difference between different power groups.Compared with control group and block group,Ca/P ratio increased(P<0.05).There's no statistic difference of the atomic percentage of Ca and P on the surface of dentin samples between each 2 groups.SEM observation showed that the surface of the laser irradiated samples was rouph,the space among enamel fibers was increased,the dentin around dentinal tuble orifice was protruded and looked like collar flange.Conclusion:Er,Cr∶YSGG laser irradiation with a range of power is effective in increasing acquired acid resistance of dental hard tissue.There was no relationship between laser power outputs and acid resistance.

ABSTRACT

OBJECTIVE:To evaluate and compare cost-effectiveness of levofloxacin and cefoxitin in the treatment of adult non-severe community-acquired pneumonia(CAP). METHODS:71 cases of adult non-severe CAP were selected and divided into levofloxacin group (group A,33 cases) and cefoxitin group (group B,38 cases). Gronp A received Mesylate levofloxacin injec-tien 200 ml inotraveously,once a day;group B received Cefoxitin for injection 2.0 g intravenously,3 times a day,for 8 days. Clinical efficacy,excellence time and ADR of 2 groups were observed,and cost-effectiveness analysis was conducted. RESULTS:The total effective rate of group A and group B were 87.88% and 65.79%,and excellence time(5.53±2.56)d and(3.87±3.03)d, with statistical significance(P0.05). The total costs were 6 748.93 yuan and 8 110.34 yuan;the cost-effectiveness ratios were 76.80 and 123.28;incremental cost-effectiveness ratio was -61.63%. Results of sensitivity analysis matched results of cost-effectiveness analysis. CONCLU-SIONS:Levofloxacin save more and is economical in the treatment of adult non-severe CAP.

ABSTRACT

OBJECTIVE@#To detect the expression of miR-126 in different tissues and organs and the change of peripheral blood glucose in microRNA-126 knock down (miR-126 KD) mouse, and to explore the pathological significance.@*METHODS@#Total RNAs were isolated from twelve kinds of tissues and organs in wild-type mouse (WT) and miR-126 KD mouse respectively. Th en, the expression level of miR-126 was detected by real-time PCR assay. Th e levels of peripheral blood glucose and body weight of miR-126 KD mice were measured. Th e pathologic changes of pancreas and lung tissue were observed by HE staining.@*RESULTS@#Compared with the WT mice, the relative expression of miR-126 in spleen, liver, muscle and lung from the miR-126 KD mice were dramatically decreased respectively (P<0.05). The level of peripheral blood glucose in the miR-126 KD mouse increased significantly at seven week and sixteen week after the birth (P<0.05). HE staining showed that the pathological structure of pancreas and liver were abnormal. The body weight of miR-126 KD mice was increased obviously from thirteen week after birth (P<0.05).@*CONCLUSION@#Peripheral blood glucose levels in the miR-126 KD mouse were dramatically elevated, which might be related to the pathological changes in the structure of pancreas and liver. These results suggest that miR-126 may play an important role in the metabolism of blood glucose and the development of type 2 diabetes mellitus.
