ABSTRACT

Objective: To report on the experience and impressions of the Brazilian orthopedic trauma surgeons attending the Leadership Development Program (LDP) hosted by the Sociedade Brasileira do Trauma Ortopédico (SBTO) in Sao Paulo, Brazil on November 4, 2022. Methods: Forty-eight orthopedic trauma surgeons from five different regions throughout Brazil were provided a link to complete The Big Five Test, a validated online personality assessment. The questionnaire was available in Portuguese and was intended to provide a background on individual personality traits and their influence on interpersonal interactions. The LDP integrated content from literature reviews specific to Latin America, established leadership programs from leading business schools, and various subject matter experts. Prior to the start of the LDP, participants received a pre-course survey evaluating demographic information, a needs assessment, and the prioritization of leadership topics utilizing a 5-point Likert-scale. Attendees participated in the one-day, interactive LDP focusing on the fundamental principles of leadership development, communication, personal development, emotional intelligence and negotiation. Following the LDP, a post-course evaluation was administered to determine the participants' overall experience, and suggestions for LDP improvement. Results: Forty-one of the forty-eight course participants completed the pre-course evaluation, whereas forty-six of the forty-eight participants completed the post-course evaluations. Overwhelmingly, the lack of opportunity was most prevalently reported as the main obstacle to attending a leadership course, as cited by 56% of respondents. Conclusion: Expanding the accessibility, diversity, and customizability of leadership programs can facilitate the development of personal tools needed to move healthcare forward. Critical topics include emotional intelligence and other differentiating leadership qualities that distinguish true transformational and servant leaders. Advancing leadership skills can stimulate networking, expose learners to experiential learning styles, inspire others to create positive change, and engender creative solutions for systematic improvements and health outcomes. Os participantes participaram do PDL interativo de um dia com foco nos princípios fundamentais de desenvolvimento de liderança, comunicação, desenvolvimento pessoal, inteligência emocional e negociação. Após o PDL, foi realizada uma avaliação pós-curso para determinar a experiência geral dos participantes e sugestões para melhoria do PDL. Resultados: Quarenta e um dos quarenta e oito participantes do curso concluíram a avaliação pré-curso, enquanto quarenta e seis dos quarenta e oito participantes concluíram a avaliação pós-curso. A falta de oportunidade foi relatada com maior prevalência como o principal obstáculo para frequentar um curso de liderança, conforme citado por 56% dos entrevistados. Conclusão: Expandir a acessibilidade, a diversidade e a personalização dos programas de liderança podem facilitar o desenvolvimento de ferramentas pessoais necessárias para fazer avançar os cuidados de saúde. Os tópicos críticos incluem inteligência emocional e outras qualidades de liderança diferenciadas, que distinguem verdadeiros líderes transformacionais e servidores O avanço das competências de liderança pode estimular o networking, expor os alunos a estilos de aprendizagem experiencial, inspirar outros a criar mudanças positivas e gerar soluções criativas para melhorias sistemáticas dos resultados na saúde. ABSTRACT

Vertically unstable pelvic injuries associated with sacroiliac disruption are challenging. Although percutaneous iliosacral fixation using two screws at S1 vertebral body has been shown beneficial, the use of two transiliac screws at S1 has been proposed to increase the fixation strength of the construct. In the herein study, the finite element method (FEM) was performed to analyse the biomechanical behaviour of five different constructions using iliosacral screws only, transiliac screws only, and combining an iliosacral and a transiliac screw. A vertically unstable AO 61C1.2 type pelvic injury was produced for the evaluation of the posterior pelvic displacement and implant stress, and the anterior implant stress using FEM. The symphysis pubis was fixed with a 3.5-mm reconstruction plate in all cases. The model was axially loaded with 800N applied in the centre of S1 body, perpendicular to the ground (Y-axis), simulating the bipodal stance moment. There was a statistically significant reduction in both posterior displacement and implant stresses in the groups fixed with at least one transiliac screw compared to the groups fixed with iliosacral screws. In our FEM study, the construct using two transiliac screws in S1 is biomechanically superior for stabilizing the sacroiliac joint in vertically unstable pelvic ring injuries compared to the other configurations. Lateral displacement, posterior displacement, and von Mises stress were reduced with this construct. A good option can be the use of one iliosacral screw and one transiliac screw in S1.

ABSTRACT

PURPOSE: although locking plates have led to important changes in fracture management, becoming important tools in the orthopedic surgeon's arsenal, the benefits of locking plates for traumatic diastasis of the pubic symphysis have not been established. This study was conducted to assess the quality of life in its different domains among patients with traumatic diastasis of the pubic symphysis managed either with locking or nonlocking plate. METHODS: a prospective cohort study was undertaken at 3 level 1 trauma centres in Brazil. Patients presenting traumatic diastasis of the pubic symphysis treated with plate fixation with a minimum follow-up of 12 months were eligible for inclusion. Through a Pfannenstiel approach, the pubic symphysis was reduced and fixed with a superiorly positioned 4.5mm four to six hole reconstruction locked plate or 3.5mm four to six hole reconstruction nonlocked plate. Posterior injury was managed during the same procedure. Outcome measures were adequate healing of the pelvic injuries, return to pre-injury level on daily activities, and quality of life at the last follow-up visit. Complications and modes of failure were summarized and reviewed. Bivariate linear regression was used to assess individual factors affecting patients' health-related quality of life. A p value of <5% was considered significant. RESULTS: a total of 31 adult patients (29 males and 2 females) were eligible for the study. Thirteen patients were managed with a reconstruction locked plate and 18 patients with a nonlocked reconstruction plate. Average postoperative follow-up time was 24 months. Adequate healing of the pelvic injuries was achieved in 61.5% of patients treated with locking plates and 94.4% of patients treated with nonlocking plates (p=0.003). Radiographic failure of fixation with minor complications occurred in 46.1% of patients after locked plating versus 11.1% of patients in the nonlocking plate group (p=0.0003). In bivariate analysis, abnormal gait (p=0.007) was associated with a reduced long-term quality of life as measured with the EQ-5D-3L. CONCLUSION: internal fixation of traumatic diastasis of the pubic symphysis with locking plates has no clinical advantage when compared to nonlocked plating. Mechanical failure and inadequate healing are significantly increased after locked plating of the pubic symphysis. Therefore, we do not recommend routine use of locking plates for managing patients presenting traumatic diastasis of the pubic symphysis. LEVEL OF EVIDENCE: II (prospective, cohort study).

ABSTRACT

Septic arthritis of the pubic symphysis is a rare condition. Risk factors include trauma, low-grade infection, urological or gynecological procedures, malignant tumors of the pelvis, sports, and intravenous drug abuse. This report describes a case of septic arthritis of the pubic symphysis in a 23-year-old male patient with no history of pelvic surgery, previous infections, or intense physical activity. Resumo A artrite séptica da sínfise púbica é uma condição rara. Os fatores de risco são trauma, infecção de baixo grau, procedimentos urológicos ou ginecológicos, tumores malignos da pelve, prática de esportes e uso de drogas intravenosas. O presente relato descreve um caso de artrite séptica da sínfise púbica em um paciente do sexo masculino, de 23 anos, sem história de cirurgias pélvicas, infecções prévias ou atividade física intensa. A artrite foi diagnosticada pela hemocultura que revelou crescimento de Enterococcus sp + leveduras, e o paciente foi tratado com antibioticoterapia. Este caso enfatiza a importância de exames complementares no auxílio do tratamento da artrite séptica da sínfise púbica, e demonstra que procedimentos invasivos, tais como a punção da sínfise púbica, podem não ser necessários.

RESUMO Justificativa e Objetivo: embora as placas bloqueadas tenham levado a mudanças importantes no tratamento de fraturas, tornando-se ferramentas importantes no arsenal do cirurgião ortopédico, os benefícios para a fixação da lesão da sínfise púbica não foram adequadamente estabelecidos. Este estudo foi realizado para avaliar a qualidade de vida em diferentes domínios de pacientes com disjunção traumática da sínfise púbica tratados com placas bloqueadas e não bloqueadas. Métodos: trata-se de estudo de coorte prospectivo, realizado em três centros de trauma nível 1, no Brasil. Foram elegíveis para inclusão no estudo pacientes com disjunção traumática da sínfise púbica tratados com redução aberta e fixação interna com placa, com seguimento mínimo de 12 meses. Por meio de abordagem de Pfannenstiel, a sínfise púbica foi reduzida e fixada com uma placa bloqueada de reconstrução de 4,5mm de quatro a seis orifícios posicionada superiormente ou com uma placa não bloqueada de reconstrução de 3,5mm de quatro a seis orifícios. A lesão pélvica posterior foi tratada durante o mesmo procedimento. Os desfechos analisados na última visita de acompanhamento foram cicatrização da lesão pélvica, retorno às atividades diárias para nível pré-lesional e qualidade de vida. Complicações e modos de falha foram observados e descritos. Foi utilizada regressão linear bivariada na avaliação dos fatores individuais que afetaram a qualidade de vida relacionada à saúde dos pacientes, com valor p <5% considerado significativo. Resultados: foram incluídos no estudo 31 pacientes adultos (29 homens e 2 mulheres). Treze pacientes foram tratados com placa de reconstrução bloqueada e 18 com placa de reconstrução não bloqueada. O tempo médio de seguimento pós-operatório foi de 24 meses. A cicatrização adequada da lesão do anel pélvico foi alcançada em 61,5% dos pacientes tratados com placas bloqueadas e em 94,4% dos pacientes tratados com placas não bloqueadas (p=0,003). Falha radiográfica de fixação com complicações menores ocorreu em 46,1% dos pacientes tratados com placa bloqueada contra 11,1% dos pacientes no grupo de placas não bloqueadas (p=0,0003). Na análise bivariada, marcha anormal (p=0,007) foi associada à redução da qualidade de vida em longo prazo, medida com o EQ-5D-3L, embora não tenha sido observada relação direta destas com os implantes utilizados. Conclusão: a fixação interna da disjunção traumática da sínfise púbica com placas bloqueadas não apresenta vantagem clínica quando comparada com placas não bloqueadas. Falha mecânica e cicatrização inadequada aumentam significativamente após o uso de placas bloqueadas na sínfise púbica. Portanto, não recomendamos o uso rotineiro de placas bloqueadas para o tratamento de pacientes com disjunção traumática da sínfise púbica. Nível de evidência: II (estudo de coorte prospectivo).

Method This was a prospective, cross-sectional study of a series of 125 consecutive hip arthroscopies performed between May 2016 and May 2017. After applying the exclusion criteria, 107 hips of 92 patients submitted to surgical treatment for mixed and CAM FAI were analyzed. For purposes of analysis, the present study considered groups with lesions considered mild and deep, which were associated with symptom score, lateral coverage angle, α angle, age, gender, and radiological classification of arthrosis. Results with a p -value < 0.05 were considered statistically significant. Results Patients whose hips had lesions considered deep had significantly higher nonarthritic hip scores (NAHSs) than those whose hips presented lesions considered mild or who did not present chondral lesions (67.9 ± 19.4 versus 57.0 ± 21.9, p = 0.027). The prevalence of deep lesions was higher in hips with Tonnis 1 compared with hips with Tonnis 0: 15 (55.6%) versus 10 (12.7%), respectively, p < 0.001. Men presented a higher prevalence of grades III and IV lesions than women, 23 (34.3%) versus 2 (5.0%), p = 0.001, and had significantly higher functional scores (65.6 ± 19.6 versus 49.3 ± 21.6, p < 0.001). Conclusion Men presented a higher prevalence of deep lesions. Hips classified as Tonnis 1 presented a 4.4-fold higher probability of presenting these lesions. Patients with deep chondrolabral lesions had a better preoperative functional score.

Resumo Objetivo Avaliar os fatores associados à presença de lesões condrais profundas (graus III e IV de Konan/Haddad) em pacientes submetidos à artroscopia do quadril para tratamento do impacto femoroacetabular (IFA). Método Estudo transversal, prospectivo, de uma série de 125 artroscopias consecutivas do quadril feitas entre maio de 2016 e maio de 2017. Depois de aplicados os critérios de exclusão, foram analisados 107 quadris de 92 pacientes submetidos a tratamento cirúrgico do IFA dos tipos misto e CAM. Para fins de análise, os grupos foram divididos entre lesões consideradas leves e profundas, e foi feita associação comescore de sintomas, ângulo de cobertura lateral, ângulo alfa, idade, gênero, e classificação radiológica de artrose. Foramconsiderados como estatisticamente significativos testes com valor de probabilidade < 0,05. Resultados Pacientes cujos quadris apresentaram lesões consideradas profundas tiveramescores de quadril não artrítico (NAHSs, na sigla em inglês) significativamentemaiores do que aqueles cujos quadris apresentavam lesões consideradas leves ou não apresentavamlesão condrolabral (67,9 ± 19,4 versus 57,0 ± 21,9; p= 0,027). Aprevalência de lesões profundas foi maior nos quadris Tonnis 1 do que nos que apresentaramTonnis 0: 15 (55,6%) versus 10 (12,7%), respectivamente; p< 0,001. Homens apresentaram melhores escores funcionais e maior prevalência de lesões graus III e IVdoque as mulheres: 65,6 ± 19,6 versus 49,3 ± 21,6; p< 0,001, e 23 (34,3%) versus 2 (5,0%), p= 0,001, respectivamente. Conclusão Homens apresentaram maior prevalência de lesões profundas. Quadris Tonnis 1 tiveram um risco 4,4 vezes maior de apresentar essas lesões. Pacientes com lesões condrolabrais profundas apresentaram melhor escore funcional pré-operatório.

OBJECTIVE: To analyse the glenopolar angle (GPA) at different inclinations of the scapula using 3D CT, to test the hypothesis that the result could change the indication from conservative treatment to surgery. MATERIALS AND METHODS: Analysis of 30 3D CT images of patients' scapulae, measuring the GPA. The GPA was measured with scapulae at 0° and at 20° and 30° of internal and external rotation. Angles were compared by age, sex and examiner for the different angles of rotation of the scapulae. RESULTS: The GPA of scapulae in rotation tended to be smaller than the GPA without rotation, and the larger the degree of rotation, the more the angle was underestimated. Additionally, for the same degree of rotation, internal rotation was associated with greater underestimation of the GPA than external rotation. Two different examiners achieved an excellent level of agreement between angle measurements. The GPA with the scapula at 0° was significantly higher among elderly patients. The variation in GPA with the scapula in rotation (20° and 30°) in relation to the GPA without rotation was significantly greater for female patients. CONCLUSIONS: As the rotation of the scapula was displaced from the scapula in the coronal position (GPA 0°), both in internal rotation and in external rotation, the GPA reduced. Therefore, rotational displacement may lead to an error in GPA measurement, resulting in incorrect indication of treatment. It is recommended that whenever possible, GPA measurements should be taken in neutral rotation, with the scapula in a neutral position at 0°.

BACKGROUND: Several so-called casting indices are available for objective evaluation of plaster cast quality. The present study sought to investigate four of these indices (gap index, padding index, Canterbury index, and three-point index) as compared to a reference standard (cast index) for evaluation of plaster cast quality after closed reduction of pediatric displaced distal forearm fractures. METHODS: Forty-three radiographs from patients with displaced distal forearm fractures requiring manipulation were reviewed. Accuracy, sensitivity, specificity, false-positive probability, false-negative probability, positive predictive value, negative predictive value, positive likelihood ratio, and negative likelihood ratio were calculated for each of the tested indices. RESULTS: Comparison among indices revealed diagnostic agreement in only 4.7% of cases. The strongest correlation with the cast index was found for the gap index, with a Spearman correlation coefficient of 0.94. The gap index also displayed the best agreement with the cast index, with both indices yielding the same result in 79.1% of assessments. CONCLUSION: When seeking to assess plaster cast quality, the cast index and gap index should be calculated; if both indices agree, a decision on quality can be made. If the cast and gap indices disagree, the padding index can be calculated as a tiebreaker, and the decision based on the most frequent of the three results. Calculation of the three-point index and Canterbury index appears unnecessary.

Patients with history of hip pain, even non-athletes or military recruits, should be analyzed to achieve an early diagnosis and prevent possible complications from the surgical treatment. This report describes a 43-year-old male patient, non-athlete, without previous diseases, who developed bilateral stress fracture of femoral neck without displacement. He had a late diagnosis; bilateral osteosynthesis was made using cannulated screws. Although the diagnosis was delayed in this case, the study highlights the importance of the diagnosis of stress fracture, regardless of the activity level of the patients, for the success of the treatment.


RESUMO A fratura de estresse bilateral do colo do fêmur em pacientes adultos sadios é uma entidade extremamente rara, cujo diagnóstico e tratamento representam um grande desafio. Pacientes com história de dor no quadril, mesmo se não forem atletas ou militares, devem ser analisados para se obter um diagnóstico precoce e prevenir possíveis complicações provenientes do tratamento cirúrgico. Este relato descreve um paciente de 43 anos, não atleta, do gênero masculino, sem doenças prévias, que desenvolveu fratura de estresse do colo do fêmur bilateral sem desvio, diagnosticado e tratado tardiamente com osteossíntese bilateral com parafusos canulados. Apesar de o diagnóstico ter sido tardio nesse caso, enfatiza-se a importância de se obter diagnóstico de fratura de estresse, independentemente do nível de atividade dos pacientes, para o sucesso do tratamento.

ABSTRACT

OBJECTIVE: To analyze knowledge of the anatomic location of the piriform fossa using a questionnaire with anatomic figures. MATERIALS AND METHODS: Participants taking AO Trauma Brasil courses were requested to complete a questionnaire containing a photograph of the superior surface and a photograph of the lateral surface of the femur and answer a question asking which of four numbered points corresponded to the piriform fossa. RESULTS: Just 4.5% of respondents correctly chose point 2 (the piriform fossa) in both images, while 75.4% of respondents chose point 4 (the trochanteric fossa) as the correct anatomic structure. The subset of 4th-year residents' answers was significantly different from those of the other subsets, with 7.5% of correct answers. CONCLUSIONS: The low rate of correct answers indicates a tendency for the respondents to be influenced by illustrations in text books or examples in scientific publications that indicate the site of the piriform fossa incorrectly. Interest in the specialty of traumatology is possibly the reason why the subset of 4th-year residents had a better- than-average rate of correct answers.

ABSTRACT

Dois pacientes com tumor de células gigantes (TCG) no terço distal do rádio foram tratados com a ressecçäo em bloco do tumor e medializaçäo da ulna em relaçäo ao punho, associado a artrodese. O tempo de evoluçäo pós-operatório é de 11 e cinco anos, respectivamente. Em ambos os casos näo houve recidiva do tumor e a artrodese consolidou, permitindo boa funcionalidade da mäo e ausência de dor.

ABSTRACT

Quarenta e duas fraturas acetabulares em 40 pacientes foram submetidas a reduçäo cirúrgica no Hospital Säo Vicente de Paulo de Passo Fundo entre outubro de 1978 e fevereiro de 1995. Foram avaliados os resultados radiológicos imediatos no pós-operatório e as complicaçöes precoces de todos os pacientes. Vinte e oito pacientes retornaram para o último seguimento (média: 4,1 anos - 12 a 209 meses), 22 dos quais puderam ser avaliados de acordo com o escore clínico de Harris e o critério radiológico de Matta. Dos 42 quadris operados houve reduçöes anatômicas em 26,1 por cento, satisfatórias em 33,4 por cento e insatisfatórias (> de 3mm de deslocamento) em 40,5 por cento. A avaliaçäo radiológica tardia mostrou 60 por cento de resultados satisfatórios, enquanto que os resultados clínicos foram excelentes e bons em 75 por cento dos casos. As lesöes associadas foram em alto número, por tratar-se de trauma de alta energia. As complicaçöes compreenderam trombose venosa (30 por cento), ossificaçäo heterotópica (25 por cento), infecçäo profunda (4,8 por cento), necrose avascular (9,5 por cento) e paralisia transitória do nervo ciático (18 por cento).

ABSTRACT

Este trabalho investiga o efeito do traumatismo e infiltraçao de xilocaína com corticóide, traumatismo e infiltraçao de xilocaína e traumatismo apenas, no peritendao da unidade musculotendínea do tríceps sural em 20 ratos adultos machos. Foram obtidos o estudo histológico e o teste de resistência da unidade musculotendínea em cinco séries. As três primeiras séries foram de infiltraçao após trauma inicial com intervalos semanais (24 horas, uma e duas semanas). As duas últimas séries (três e quatro semanas) tiveram o descontínuo do uso de infiltraçoes, tendo sido realizado apenas estudo histológico e biomecânico. Observou-se o efeito deletério nos aspectos anatomopatológico e biomecânico na unidade músculo-tendao dos animais experimentais em que a associaçao de infiltraçao de xilocaína com corticóide foi utilizada.
