ABSTRACT

BACKGROUND: Mortality rate estimation in small areas can be difficult due the low number of events/exposure (i.e. stochastic error). If the death records are not completed, it adds a systematic uncertainty on the mortality estimates. Previous studies in Brazil have combined demographic and statistical methods to partially overcome these issues. We estimated age- and sex-specific mortality rates for all 5,565 Brazilian municipalities in 2010 and forecasted probabilistic mortality rates and life expectancy between 2010 and 2030. METHODS: We used a combination of the Tool for Projecting Age-Specific Rates Using Linear Splines (TOPALS), Bayesian Model, Spatial Smoothing Model and an ad-hoc procedure to estimate age- and sex-specific mortality rates for all Brazilian municipalities for 2010. Then we adapted the Lee-Carter model to forecast mortality rates by age and sex in all municipalities between 2010 and 2030. RESULTS: The adjusted sex- and age-specific mortality rates for all Brazilian municipalities in 2010 reveal a distinct regional pattern, showcasing a decrease in life expectancy in less socioeconomically developed municipalities when compared to estimates without adjustments. The forecasted mortality rates indicate varying regional improvements, leading to a convergence in life expectancy at birth among small areas in Brazil. Consequently, a reduction in the variability of age at death across Brazil's municipalities was observed, with a persistent sex differential. CONCLUSION: Mortality rates at a small-area level were successfully estimated and forecasted, with associated uncertainty estimates also generated for future life tables. Our approach could be applied across countries with data quality issues to improve public policy planning.

ABSTRACT

α, ß amyrin (ABAM) is a natural mixture of pentacyclic triterpenes that has a wide range of biological activities. ABAM is isolated from the species of the Burseraceae family, in which the species Protium is commonly found in the Amazon region of Brazil. The aim of this work was to develop inclusion complexes (ICs) of ABAM and ß-cyclodextrin (ßCD) and hydroxypropyl-ß-cyclodextrin (HPßCD) by physical mixing (PM) and kneading (KN) methods. Interactions between ABAM and the CD's as well as the formation of ICs were confirmed by physicochemical characterization in the solid state by Fourier transform infrared (FTIR), scanning electron microscopy (SEM), X-ray diffraction (XRD), thermogravimetry (TG) and differential scanning calorimetry (DSC). Physicochemical characterization indicated the formation of ICs with both ßCD and HPßCD. Such ICs were able to induce changes in the physicochemical properties of ABAM. In addition, the formation of ICs with cyclodextrins showed to be an effective and promising alternative to enhance the anti-inflammatory activity and safety of ABAM.

ABSTRACT

The novel 2-aminothiophene derivative 2-amino-4,5,6,7-tetrahydrobenzo[b]thiophene-3-carbonitrile (6CN) has shown potential anti-proliferative activity in human cancer cell lines. However, the poor aqueous solubility of 6CN impairs its clinical use. This work aimed to develop binary 6CN-ß-cyclodextrin (ßCD) systems with the purpose of increasing 6CN solubility in water and therefore, to improve its pharmacological activity. The 6CN-ßCD binary systems were prepared by physical mixing, kneading and rotary evaporation methods and further characterized by FTIR, XRD, DSC, TG and SEM. In addition, molecular modeling and phase solubility studies were performed. Finally, MTT assays were performed to investigate the cytostatic and anti-proliferative effects of 6CN-ßCD binary systems. The characterization results show evident changes in the physicochemical properties of 6CN after the formation of the binary systems with ßCD. In addition, 6CN was associated with ßCD in aqueous solution and the solid state, which was confirmed by molecular modeling and the aforementioned characterization techniques. Phase solubility studies indicated that ßCD forms stable 1:1 complexes with 6CN. The MTT assay demonstrated the cytostatic and anti-proliferative activities of 6CN-ßCD binary systems and therefore, these might be considered as promising candidates for new anticancer drugs.

ABSTRACT

Hecogenin acetate (HA) is an acetylated sapogenin that has shown potential antihyperalgesic activity, inhibiting descending pain and acting in opioid receptors. However, HA exhibits poor aqueous solubility, which may limit its application. This study aims to develop amorphous solid dispersions (ASD) using five hydrophilic polymers, to characterize them and to evaluate their antihyperalgesic activity. Physicochemical characterization was performed by X-Ray Diffraction (XRD), Scanning Electron Microscopy (SEM) and Fourier Transformed Infrared (FTIR) spectroscopy. In order to evaluate the hyperalgesia of the ASD, sciatic nerve crush injury (NCI) was induced in mice followed by administration of the ASD, where three parameters were evaluated: mechanical and thermal hyperalgesia as well as grip strength. XRD and SEM showed that ASD of HA with HPMC obtained by kneading (KND) presented an amorphous profile, unlike the others polymers, indicating interaction between HA and HPMC. FTIR analysis evidenced the strong interaction between HA and HPMC. Although the results of mechanical hyperalgesia were slightly improved on the groups treated with ASD of HA with HPMC, the thermal hyperalgesia showed that the incorporation of HA into HPMC matrix significantly improved its antinociceptive activity.

ABSTRACT

α,ß Amyrin (ABAM) is a natural mixture of pentacyclic triterpenes that has shown a variety of pharmacological properties, including anti-inflammatory effect. ABAM is isolated from Burseraceae oilresins, especially from the Protium species, which is commonly found in the Brazilian Amazon. This work aimed to develop solid dispersions (SD) of ABAM with the following hydrophilic polymers: polyvinylpyrrolidone (PVP-K30), polyethylene glycol (PEG-6000) and hydroxypropylmethylcellulose (HPMC). The SDs were prepared by physical mixture (PM), kneading (KND) and rotary evaporation (RE) methods. In order to verify any interaction between ABAM and the hydrophilic polymers, physicochemical characterization was performed by Fourier transform infrared (FTIR), scanning electron microscopy (SEM), powder X-ray diffraction (XRD), thermogravimetry (TG) and differential scanning calorimetry (DSC) analysis. Furthermore, an in vitro anti-inflammatory assay was performed with ABAM alone and as SDs with the hydrophilic polymers. The results from the characterization analysis show that the SDs were able to induce changes in the physicochemical properties of ABAM, which suggests interaction with the polymer matrix. In vitro anti-inflammatory assay showed that the SDs improved the anti-inflammatory activity of ABAM and showed no cytotoxicity. In conclusion, this study showed the potential use of SDs as an efficient tool for improving the stability and anti-inflammatory activity of ABAM without cytotoxicity.

ABSTRACT

α-Amyrenone and ß-amyrenone are triterpenoid isomers that occur naturally in very low concentrations in several oleoresins from Brazilian Amazon species of Protium (Burseraceae). This mixture can also be synthesized by oxidation of α,ß-amyrins, obtained as major compounds from the same oleoresins. Using a very simple, high yield procedure, and using a readily commercially available mixture of α,ß-amyrins as substrate, the binary compound α,ß-amyrenone was synthesized and submitted to physico-chemical characterization using different techniques such as high-performance liquid chromatography, nuclear magnetic resonance (¹H and 13C), mass spectrometry, scanning electron microscopy, differential scanning calorimetry, thermogravimetry and derivative thermogravimetry, and Fourier transform infrared spectroscopy (FTIR). Biological effects were also evaluated by studying the inhibition of enzymes involved in the carbohydrate and lipid absorption process, such as α-amylase, α-glucosidase, lipase, and their inhibitory concentration values of 50% of activity (IC50) were also determined. α,ß-Amyrenone significantly inhibited α-glucosidase (96.5% ± 0.52%) at a concentration of 1.6 g/mL. α,ß-Amyrenone, at a concentration of 100 µg/mL, showed an inhibition rate on lipase with an IC50 value of 82.99% ± 1.51%. The substances have thus shown in vitro inhibitory effects on the enzymes lipase, α-glucosidase, and α-amylase. These findings demonstrate the potential of α,ß-amyrenone for the development of drugs in the treatment of chronic metabolic diseases.

ABSTRACT

INTRODUCTION: Given the major impact in terms of morbidity and mortality that episodes of early neonatal sepsis (ENS) have on both newborns and health systems, this study aimed to identify the etiological profile of early neonatal bacterial sepsis by a multiplex quantitative real-time polymerase chain reaction (qPCR). METHODOLOGY: Blood samples from newborns diagnosed with clinical ENS and hospitalized in neonatal intensive care units (NICUs) were collected and analyzed using the multiplex qPCR method to detect Streptococcus agalactiae, Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa, Enterobacter sp., Serratia sp., and Staphylococcus aureus. A universal primer was used in the analysis. RESULTS: A total of 150 neonates with clinical sepsis and 10 newborns as healthy controls were included in the study. The group with clinical sepsis was 100% positive for the presence of bacterial genomic DNA through the universal primer. The control group showed negativity by qPCR. The multiplex qPCR analysis showed that 76% of the samples were positive for Escherichia coli, 34% for Staphylococcus aureus, 13.3% for Streptococcus agalactiae, 7.3% for Pseudomonas aeruginosa, and 0.7% for Enterobacter sp. and Serratia sp. Multiplex qPCR of patients with clinical sepsis matched with 8.1% of the blood samples that tested positive by the microbiological method. CONCLUSIONS: Rapid and sensitive detection of the pathogens causing ENS by this new multi-target approach based on multiplex qPCR could potentially excel compared to microbiological methods, with the simple objective of facilitating the progression to a more rapid and specific antimicrobial therapy, avoiding the abuse of antibiotics in NICUs.

The objective of the search was evaluation of the difference between treatment with the probiotic 1 (P1) in relation when treatment with probiotic 2 (P2) in the larvae cultivation of the Litopenaeus vannamei, these were culture for 18 days. The treatment with P1 consisted in the addition of the commercial probiotic Epicin G2 (Epicore Networks Eastampton, United State) containing Bacillus subtilis, B. licheniformis, Lactobacillus acidophilus, B. pumilus and Saccharomyces cerevisiae. In the treatment with P2, was utilized commercial probiotic ProBacyl (Bern&Roc Aquacultura Ltda, Ceará-Mirim, Brazil) with a mixture of B. subtilis, B. licheniformis, Pediococcus acidilactici and probiotic bacteria (total). The experimental units consisted of six tanks of 15,000 L, and each treatment had three repetitions. The probiotics were applied daily in the water, evaluating the zootechnical potential and resistance to stress larvae and post-larvae (PL). The parameters salinity, temperature, pH and oxygen dissolved were controlled. The post-larvae submitted for P2 presented medium values higher (p < 0,05) to length (PL 1 and PL 5), humid and dry weights (PL 5) and metamorphosis percentage (90%) compared to the other treatment, but as other variables did not differ. The final survival was 56,4% and 64,9% to post-larvae submitted to P1 and P2, respectively. Thus, the probiotic 2 showed better results than P1 for cultivation of L. vannamei incommercial hatchery system at some stages of development.(AU)

ABSTRACT

A possibilidade da restauração imediata de um elemento dental perdido favorece a manutenção da arquitetura óssea e gengival, além de reduzir o tempo de tratamento. O objetivo deste trabalho foi descrever a sequência de procedimentos para a reabilitação imediata de um dente incisivo central superior que apresentava uma fratura radicular longitudinal e com uma perda total da tábua óssea vestibular. A exodontia do elemento 11 foi realizada, e a instalação de um implante osteointegrado tipo cone-morse foi feita. Um intermediário protético anatômico foi selecionado para a reabilitação protética imediata provisória. Neste mesmo momento, uma imediata reconstrução óssea da parede alveolar vestibular, com uma lâmina de enxerto autógeno oriunda do túber de maxila associado a enxerto particulado heterógeno, foi executada. Após o período de osteointegração, foi realizada uma adequação do pilar protético anatômico e a instalação da prótese definitiva total cerâmica. Este procedimento se mostrou eficaz, promovendo uma recuperação da estrutura anatômica dentoalveolar perdida em um único procedimento, apesar de uma limitada reconstrução em espessura do rebordo alveolar, com o mínimo de morbidade pós-operatória, mantendo uma estética dentogengival satisfatória após dois anos de reabilitação definitiva.


The chance to immediate restore a missing tooth favors maintenance of bone and gingival architecture and still shortens treatment time. The aim of this report was to describe a sequence for immediate rehabilitation of a lost maxillary central incisor with longitudinal root fracture and complete loss of the buccal bone plate. Upon atraumatic extraction, a cone Morse dental implant was inserted. Then, an anatomic abutment was selected for the provisional restoration concomitantly to a tuberosity bone/heterogeneous material grafts. After osseointegration, the anatomic prosthetic abutment was redesigned and the definitive prosthesis delivered. This therapy was effective to recover the dentoalveolar unit in a single session with minimal postoperative morbidity, providing satisfactory dentogingival esthetic aspect after two years of the definitive restoration.

Objective: To evaluate the impact of gait training with neuromuscular electrical stimulation on urodynamic parameters of patients with neurogenic bladder. Methods: Eight male quadriplegic patients with complete cervical injury level ranging from C4 to C7 comprised the study population. They underwent treadmill gait training with neuromuscular electrical stimulation for six months, only after having their quadriceps and tibialis anterior muscles stimulated for five months in order to support at least 50% of their body weight (pre-gait training). Urodynamic testing was performed before the treadmill gait training and six months after. Results: The mean time after cervical lesion was 74.63 months. The urodynamic parameters before and after neuromuscular training by electrical stimulation did not show significant difference. Conclusion: This study demonstrated that neuromuscular training with electrical stimulation can benefit the urinary tract. This promising minimally invasive requires further and more complete studies to confirm a possible benefit to the low urinary tract.


Objetivo: Avaliar o impacto do treinamento neuromuscular, por estimulação elétrica da marcha, nos parâmetros urodinâmicos de pacientes com bexiga neurogênica. Métodos: A população do estudo compreendeu oito pacientes quadriplégicos do sexo masculino com lesão cervical completa nos níveis C4 a C7. Eles foram submetidos a um treinamento neuromuscular por estimulação elétrica da marcha por seis meses, somente após estimulação dos músculos quadríceps e tibial anterior, por cinco meses, de modo a poder suportar 50% de seu peso corporal (pré-treinamento). Testes urodinâmicos foram feitos antes do treinamento e seis meses depois. Resultados: O tempo médio após a lesão cervical foi de 74,63 meses. Os parâmetros urodinâmicos antes e depois do treinamento neuromuscular por estimulação elétrica da marcha não mostraram diferença significante. Conclusão: Este estudo mostrou que o treinamento neuromuscular com estimulação elétrica pode ser benéfico para o trato urinário. Esse campo, promissor e minimamente invasivo, requer outros estudos mais completos para confirmar um possível benefício no trato urinário inferior.

ABSTRACT

OBJECTIVE: To evaluate the impact of gait training with neuromuscular electrical stimulation on urodynamic parameters of patients with neurogenic bladder. METHODS: Eight male quadriplegic patients with complete cervical injury level ranging from C4 to C7 comprised the study population. They underwent treadmill gait training with neuromuscular electrical stimulation for six months, only after having their quadriceps and tibialis anterior muscles stimulated for five months in order to support at least 50% of their body weight (pre-gait training). Urodynamic testing was performed before the treadmill gait training and six months after. RESULTS: The mean time after cervical lesion was 74.63 months. The urodynamic parameters before and after neuromuscular training by electrical stimulation did not show significant difference. CONCLUSION: This study demonstrated that neuromuscular training with electrical stimulation can benefit the urinary tract. This promising minimally invasive field requires further and more complete studies to confirm a possible benefit to the low urinary tract.

O ronco e a apnéia obstrutiva do sono podem levar a diversos problemas de saúde. Hipertensão arterial e pulmonar, infarto, acidente vásculo-cerebral, impotência e depressão são alguns exemplos de doenças que se instalam mais facilmente quando ocorre a apnéia do sono. O cirurgião-dentista inserido em uma equipe multidisciplinar é um profissional altamente requisitado para o diagnóstico e tratamento destes distúrbios do sono. Dentre os tratamentos recomendados, a terapia com placas intrabucais se mostra uma excelente alternativa não-invasiva, alcançando ótimos resultados

ABSTRACT

O intercâmbio entre componentes de diferentes marcas é uma prática comum entre profi ssionais. Isso pode levar a desadaptações da estrutura metálica comprometendo o prognóstico do tratamento. Este trabalho analisou a tolerância de desadaptação de três empresas.De acordo com os resultados obtidos, a desadaptação entre o pilar/transferente variou de 6,69 µm a 11,94 µm. Já entre a interface análogo/transferente variou de 6,35 µm a 17,99 µm. Baseado nesses resultados e nos encontrados na literatura concluiu-se que os componentes avaliados se mostraram com fendas aceitáveis para uso clínico e que o cuidado com o torneamento final contribui com o ótimo assentamento dos componentes.

ABSTRACT

Tem por objetivo apresentar os resultados da pesquisa realizada com alguns Comitês de Ética em Pesquisa (CEPs) de São Paulo, com o intuito de verificar se esses comitês estão conseguindo efetivar a finalidade a que se propõem, isto é, garantir a proteção dos direitos e da dignidade dos seres humanos envolvidos em experimentações científicas. Mais especificamente, tem a intenção de refletir sobre aqueles aspectos relacionados à vulnerabilidade dos sujeitos de pesquisa. Procurou-se colher o relato de experiências dos envolvidos nas atividades dos Comitês, em particular, dos secretários responsáveis, e levantar os documentos de arquivo disponíveis. Os CEPs escolhidos foram: Pontifícia Universidade Católica de São Paulo (PUC-SP), PUC-Sorocaba, Centro Universitário São Camilo, Hospital Dante Pazzanese, Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (USP), Universidade Federal de São Paulo (Unifesp) e Faculdade de Saúde Pública da USP. De acordo com as informações obtidas, percebe-se a preocupação com a proteção do sujeito de pesquisa, para se evitar abusos e desrespeito à sua dignidade. Observou-se também, que o principal motivo da não aprovação de projetos de pesquisa, tem sido o preenchimento inadequado ou insuficiente do Termo de Consentimento Livre e Esclarecido (TCLE). De qualquer maneira, os CEPs estão ainda num processo de evolução e aprimoramento. Ainda resta, como desejável, ampliar o pluralismo interno e os vínculos entre os comitês. Só então a reflexão bioética terá atingido a razão de sua existência como linguagem mediadora de conflitos e interesses no campo da pesquisa e experimentação com seres humanos.

A trombose venosa subclávio-axilar é um evento trombótico pouco frequente, relacionado a esforço físico excessivo com o membro superior dominante. Ocorre predominantemente em indivíduos jovens do sexo masculino, e apresenta-se com quadro agudo de trombose venosa profunda, devendo ser lembrada no diagnóstico diferencial de patologias que causam dor e edema em membro superior. O diagnóstico e a terapêutica precoces têm fundamental importância para minimizar a morbidade desta condição


The axillary-subclavian vein thrombosis is an infrequent thrombotic event, related to excessive physical effort with the dominant upper extremity. It happens predominantly in young men and appears as deep vein thrombosis that must be reminded on the differential diagnosis of pathologies that cause pain and swelling in the upper extremity. The early diagnosis and therapy have great importance to minimize the morbidity of this condition

ABSTRACT

A perda dos dentes naturais provoca distorção morfológica e funcional tanto em tecidos moles quanto no esqueleto facial. Tendo conhecimento de que a prótese total tem como finalidade restabelecer a harmonia do sistema estomatognático e que a mucosa bucal está sujeita a várias lesões em conseqüência do uso de dentaduras artificiais, este trabalho tem como objetivo relacionar o uso de próteses totais com lesões provocadas pela mesma, no que diz respeito ao tempo de uso das próteses, adaptação e higienização, entre outros. Este trabalho foi desenvolvido no Lar para Idosos da Sociedade São Vicente de Paulo, no município de Bauru, SP, com 30 pacientes acima de 50 anos, de ambos os sexos e portadores de prótese total. À inspeção visual com o auxílio de espelho clínico plano e espátula de madeira, foi observada a existência de 42,86 por cento de candidíase ou estomatite por dentadura; 28,57 por cento de hiperplasias mucogengivais; 23,8 por cento de lesões associadas e 4,76 por cento de úlceras traumáticas. Foi observada adaptabilidade das próteses confeccionadas em um período de 20 anos, sendo que 80 por cento foram consideradas parcialmente satisfatórias. Houve associação positiva entre o uso de próteses insatisfatórias e a presença de lesões localizadas no palato

ABSTRACT

Com a propagação de doenças infecto-contagiosas como a AIDS, pesquisadores clínicos têm se preocupado mais com a desinfecção e esterilização, tanto de instrumentais como de materiais. Em função disto, há a necessidade de se prevenir a contaminação cruzada, tanto em consultórios como em laboratórios de prótese. A mesma importância deve ser dada para a prevenção de outras doenças como a Tuberculose e a Hepatite B (HBV), sendo a última mais transmissível, especialmente por estar presente no sangue, em alta concentração. O objetivo deste estudo foi avaliar a importância da desinfecção dos moldes e modelos de gesso para evitar a ocorrência de contaminação cruzada. No final, os autores sugerem um protocolo simples de ações, relacionando cada tipo de material de moldagem com a solução desinfectante mais adequada

ABSTRACT

O objetivo deste estudo foi avaliar o resultado do tratamento dos pacientes com desordens crânio-mandibulares (DCM) realizado na clínica de DCM do curso de graduaçäo da Faculdade de Odontologia da Universidade do Sagrado Coraçäo. Os pacientes com envolvimento muscular, tratados com a terapia de placas miorrelaxantes, foram reavaliados após seis meses através do questionário anamnético para DCM, segundo as alterações nos sintomas. A partir daí, foram novamente classificados de acordo com o nível de disfunçäo (severa, moderada, leve e sem DCM) e os resultados foram comparados com os achados antes do tratamento. Os resultados mostraram que a terapia com placas miorrelaxantes melhoram o quadro de disfunçäo, sendo que os pacientes precisam de contoles periódicos
