ABSTRACT

Anti-amastigote polyclonal antibody (IgG) was incubated with solutions of stannous chloride and sodium borohidride. After that, 3.7 MBq of technetium-99m (99mTc) was added. A labeling yield of the antibody about 84% was obtained. After filtration of 99mTc-IgG, the radiochemical purity increased from 84 to 95%. The labeling of IgG with 99mTc did not modify the immunoreactivity of the antibody, since it was able to identify in vitro and in vivo the specific antigen of Leishmania amazonensis.

ABSTRACT

Several studies show the ability of macrophages to remove particles injected into the bloodstream. This function seems to be increased in the presence of acute renal failure. The objective of the present study was to assess the phagocytic function of the main organs (spleen, liver and lung) of the mononuclear phagocytic system in renal and postrenal failures. Fifteen rats (250-350 g) were divided into three groups (N = 5): group I - control; group II - ligature of both ureters, and group III - bilateral nephrectomy. On the third postoperative day, all animals received an iv injection of 1 ml/kg 99mTc sulfur colloid. Blood samples were collected for the assessment of plasma urea, creatinine, sodium, and potassium concentrations and arterial gasometry. Samples of liver, spleen, lung and blood clots were obtained and radioactivity was measured. Samples of liver, spleen, lung and kidney were prepared for routine histopathological analysis. Plasma urea, creatinine and potassium concentrations in groups II and III were higher than in group I (P<0.05). Plasma sodium concentrations in groups II and III were lower than in group I (P<0.05). Compensated metabolic acidosis was observed in the presence of postrenal failure. Group II animals showed a lower level of radioactivity in the spleen (0.98) and lung (2.63), and a higher level in the liver (105.51) than control. Group III animals showed a lower level of radioactivity in the spleen (11.94) and a higher level in the liver (61.80), lung (11.30) and blood clot (5.13) than control. In groups II and III liver steatosis and bronchopneumonia were observed. Renal and postrenal failures seem to interfere with blood clearance by the mononuclear phagocytic system

OBJECTIVE: To assess the lung deposition pattern of radioaerosol and the clinical performance of a spacer developed and made in Brazil. METHODS: Qualitative - in a patient with cystic fibrosis - and semi-quantitative - in two healthy volunteers - assessment of pulmonary deposition of (99)mtechnetium was done using the Aerogama Medical oxigen driven nebulizer system attached to the spacer and a gama-camera (Siemens, model Orbiter) connected to a microcomputer. In the next step, clinical assessment was carried out in 50 asthmatic children, aged from four months to 13 years old with an acute attack, using conventional doses of albuterol through a metered dose inhaler attached to the spacer device. RESULTS: Qualitative assessment revealed a lung silhouette comparable with those obtained in the inhalation scintigraphy and semiquantitative assessment reveals that 7.5% to 8.0% of the inhaled (99m)technetium reached the volunteers lungs. Statistically significant differences (p < 0.001) were observed comparing clinical scores at admission with those verified 20 and 40 minutes after albuterol inhalation; conversely, no significance was obtained for scores taken at 60 and 80 minutes. CONCLUSIONS: Although we used an alternative method, the scintigraphic assessment reveals an expected pattern of pulmonary deposition. Similarly, clinical performance in the treatment of an acute attack showed results comparable with those obtained with other spacers devices.

ABSTRACT

BACKGROUND: Hypertension may be a cause of sepsis, especially among cirrhotic patients. METHODS: In the present investigation we studied the response of the mononuclear phagocytic system in 25 female Holtzman rats divided into five groups of 5 animals each: A, sham-operated animals; and animals with portal hypertension induced by calibrated ligature of the portal vein at different times of evolution: B, 15 days; C, 30 days; D, 45 days; E, 60 days. After the period of observation, 1 MBq/kg 99mTc colloidal sulfur was injected through the caudal vein and the animals were sacrificed 30 minutes later. Blood samples were obtained for GOT, GPT and albumin determination and radioactive counts, and liver, lung, spleen and bone marrow samples were obtained for the determination of the proportional phagocytosis index per gram organ. The results were analyzed statistically by the Kruskal-Wallis test. RESULTS: Increased radioactivity was observed in bone marrow and spleen and higher GPT levels were detected after 15 days. A reduction of splenic phagocytosis, an increase of pulmonary phagocytosis and increased GPT levels occurred after 30 days. In the 45-day group there was an increase in non-phagocytized particles remaining in blood, and in the 60-day group there was only and increase in GPT values. CONCLUSIONS: We conclude that portal hypertension causes phagocytic alterations during the early phases in this experimental model.

ABSTRACT

Samples of a culture of unlabeled Escherichia coli were incubated with different concentrations of stannous chloride for various time periods. 99mTc (26.0 MBq) was added to each preparation and the results showed a labelling yield of 98% for E. coli. Since the bacterial viability of 99mTc-E. coli and E. coli did not show any statistical differences, these results demonstrate that labelling of E. coli with 99mTc does not modify the bacterial viability, and the radiolabelled bacteria may be a good model to study bacterial translocation.

ABSTRACT

Subtotal splenectomy preserving the upper splenic pole vascularized only by the splenogastric vessels has been performed by us in 91 patients since 1984. This procedure was combined with central splenorenal shunt or portal variceal disconnection to treating schistosomatic portal hypertension (n = 67); it was combined with distal pancreatectomy to treat pancreatic cystadenoma (n = 1), and it was carried out for severe splenic trauma (n = 14), and to control severe Gaucher's disease (n = 4) and myeloid metaplasia due to myelofibrosis (n = 5). All patients are alive. In the present paper, we present the late postoperative follow up of 32 patients operated in a period that ranges from 1 to 12 years. All patients are satisfied with the operation. Increased counts of white blood cell (n = 10) and platelets (n = 7) were the only hematological alterations. Esophageal varices were present in 23 of 27 patients operated due to portal hypertension, but all of them were shorter than in the preoperative period. The ultrasound confirmed the presence of the splenic remnant (30 of 32) without significant size changes. Splenic scintigraphies were positive in 30 of 32 patients. Subtotal splenectomy seems to be a safe procedure that can be useful in portal hypertension, some metabolic and hematological diseases, and benign growth of the distal pancreas.

ABSTRACT

O objetivo deste trabalho foi preparar, em condiçöes adequadas, o fármaco (DTPA) marcado com isótopo radioativo para estudos de cinrilografia renal. Além de avaliar a funçäo renal, este fármaco permite diagnosticar, precocemente, alteraçöes que indicam rejeiçäo de tecido, em casos de transplante de rins. Inicialmente, foram realizados experimentos com camundongos para investigar a distribuiçäo biológica do radiofármaco. Os resultados mostraram que, alguns minutos após a administraçäo, os rins apresentavam uma captaçäo de radioatividade. Estes dados e os resultados dos testes de esterilidade, apirogenicidade e pureza radioquímica confirmaram a qualidade do radiofármaco, permitindo o seu emprego em cintilografias de pacientes com comprometimento renal

ABSTRACT

We studied 13 children with 21-hydroxyalse deficiency to explore the immediate potential suppressive effect of hydrocortisone dose schedule on the adrenal cortex. They were given 20 mg/m2 daily in a controlled trial. After random administration of a greater dose in the morning (7 patients) or at night (6 patients), we measured plasma levels of 17-hydroxyprogesterone, testosterone, and androstenedione at times-24, 0, 2, 4, and 6h. Considerable fluctuation of the steroid levels, unrelated to the drug intake, was observed. There was no statistically significant differences between the "morning dose" and "night dose" groups for any steroid. We conclude that; (i) the greater night dose did not avoid the 17-hydroxyprogesterone morning peaks, and (ii) the variation in plasma steroid levels is so marked that a single morning sample is unreliable to reflect the degree of adrenal suppression.

ABSTRACT

1. A comparative study of the inhibitory action of topically applied indomethacin and aspirin (1% (w/v) 2 h before and immediately after trauma, totaling 4 mg each) on myosis, increase in intraocular pressure and breakdown of the blood aqueous barrier was performed. 2. Argon laser photocoagulation on the anterior surface of the left iris of pigmented rabbits, totaling 2,250 mJ, was used as traumatic stimulus. The untraumatized eye was used as a reference for measurements. 3. Four groups of 10 animals each (control, photocoagulated, photocoagulated plus indomethacin and photocoagulated plus aspirin) were studied. 4. Both drugs similarly inhibited the increase in the total protein concentration in aqueous humor and in intraocular pressure at 10, 20 and 40 min. Only aspirin showed some inhibitory action on the pupillary response 10 min after photocoagulation.

ABSTRACT

Este trabalho vem acrescentar mais uma entidade etiológica ao capítulo das Uveites - a Coroidite Esquistossomótica - fato inédito na literatura mundial. Apresentamos 6 casos clínicos, os quais säo descritos através de estudo laboratorial, anamnéstico, biomicroscópico, fundoscópico e angioretinográfico. O aspecto morfológico em 5 dos casoselhante e bilateral, caracterizado por nódulos branco-amarelados, de tamanhos variados, distribuidos pela coroide, e 1 caso foi unilateral, com nódulos e papilite. Além desses casos, descrevemos 1 caso no qual tivemos a felicidade de constatar, pela histologia de 100 cortes, a presença de S. mansoni na coroide. Também fazemos uma discussäo sobre os diagnósticos diferenciais e as possíveis vías pelas quais o parasita alcança a coroide, e outras formas de uveite

ABSTRACT

This paper presents the results of biomicroscopy and funduscopy on five patients with hepatosplenic schistosomiasis mansoni. Fluorescein angioretinography was performed on two patients. All cases showed yellowish white multiple billateral nodules of various sizes, located in the choroidal plane. The nature and differential diagnosis of these nodules is discussed, and the suggestion is made that they represent cases of schistosomotic nodular choroiditis.
