ABSTRACT

One thousand and thirty patients with proved L(5)/S(1)disc herniations were examined. The dynamics of disease in acute stage was studied using ultrasonography and electromyography in 3 groups of patients: 280 with lumboischialgia, 520 with the root syndrome and 230 controls without back pain. It has been shown that edematous epiduritis and venous stas in the superincumbent spinal movement segment is a main pathogenetic mechanism of acute phase of the disc-radicular conflict. Focal myelopathy is a pathogenetic mechanism of radiculopathy in the disc-recticular conflict. Compression and reflex syndromes are caused by the common mechanisms and their differentiation by clinical presentations has a conditional character.

ABSTRACT

Ethiopathological and clinical factors of vertebral blood flow insufficiency development are discussed in the article. Distinct connection between pathogenesis and stages of clinical manifestation is proved and nosological independence of vertebrobasilar disease is validated. Used in combined therapy of patients with the disease, cavinton is shown to exert a positive effect on the main clinical symptoms.

ABSTRACT

50 patients with different neurological manifestations of lumbar osteochondrosis were examined. A new method of transabdominal ultrasonographic investigation of both the discs and the surrounding tissues was applied. 30 cases of discogenic disease and 20 cases of reflex algesic syndrome were revealed. Possibilities of transabdominal ultrasonography for staging the diseases were demonstrated as well as the advantage of the method over computer magnetic-resonance tomography. Ultrasonography allows to visualize rupture of the disc and fibrous changes. The results of the study can be used for diagnosis of the severity of disease as well as for its therapy and prognosis.

ABSTRACT

Based on the sufficient clinical material with the use of the current research methods including computer-aided tomography the author provides evidence in favour of a possibility of carrying out successful conservative therapy in the majority of patients with the compression syndromes of lumbar osteochondrosis. Such conservative therapy is of paramount socioeconomic importance, since the percentage of invalidism in the youngest capable age often increases just after the surgical intervention.

ABSTRACT

The paper is concerned with the development and specification of diagnostic approaches to patients with the title pathology. 168 patients with vascular syndromes of cervical osteochondrosis were under observation. According to the readings of ultrasound echocardiography and ultrasound double scanning, the patients were distributed into two groups: with occlusions and hemodynamically significant stenoses (from 50 to 99%) in the vertebral arteries, and without hemodynamically significant stenoses. On comparison of these two groups of patients with the disease clinical picture the first group patients demonstrated the signs of static or dynamic ataxia of varying gravity, thus allowing confirmation of the existence of the compression form of the vertebral artery syndrome--vertebrobasilar insufficiency. The authors have elaborated a complex of the treatment measures for patients with vascular syndromes of spinal osteochondrosis, including the use of medicamentous and nontraditional means (manual therapy, acupuncture), providing a possibility of carrying out successful conservative therapy. The role of vegetative formations located in the cervical part of the spine in regulation of the cerebral blood flow has been revealed.

ABSTRACT

Using electromyography the authors studied the functional status of the segmentary apparatus of the spinal cord in 58 patients with compression of S1-radicle secondary to vertebral osteochondrosis. The findings obtained included changes in the interferential EMG and reciprocal reflexes and decreased reflectory activity of alpha-motoneurons predominantly on the side of radicular involvement with the simultaneous intensification of gamma-loop effect. Acupuncture was followed by pain alleviation and normalization of the functional activity of the segmentary apparatus of the spinal cord mainly at the expense of this activity reduction on the healthy side. Clinical recovery was observed following the compensation of neuroreflectory activity of the segmentary centres on a new functional level. The advisability of the use of acupuncture in the treatment of the radicular syndrome of lumbar osteochondrosis is justified.

ABSTRACT

A new approach to the treatment of cerebral strokes is discussed. This approach consists in exposing the pathological focus in the brain to the action of decimeter electromagnetic waves (DW). The results of treating 112 patients with cerebral circulation disorders have demonstrated a sufficiently high effectiveness and a broad range of the therapeutic action of this new method. The decimeter waves contributed to restoration of the motor and sensor functions in the patients with the cerebral strokes; they produced a regulatory effect on the cerebral ad peripheral blood circulation, the brain bioelectric activity, the functions of the neuromuscular apparatus, lipid metabolism, the blood coagulation and anticoagulation system and the neuromediator metabolism. The data obtained suggest that it is a diminution (achieved with the use of DW) of the pathological impulsation flow from the pathological focus in the brain that plays the main role in the restoration of the impaired functions in patients with cerebral strokes. This conclusion is very important for understanding the pathogenesis of cerebral circulation disturbances.

ABSTRACT

The central and peripheral hemodynamics was examined in patients with the radicular syndrome of lumbar osteochondrosis who were treated by reflexotherapy methods. In addition to the direct favourable effect of microacupuncture and classical acupuncture on the circulatory system a longer duration of the positive effect of microacupuncture was revealed. An interconnection between the favourable changes of the patient's state and the improvement of the systemic and local circulation was noted.
