ABSTRACT

With the advent of osseointegrated dental implant therapy in the last few decades, complete edentulism of the mandible can be treated and restored predictably and successfully. However, due to medical and financial considerations, many patients cannot avail themselves of this therapy. This article presents a case report of such a patient treated with advanced yet traditional denture construction protocols. These conventional protocols should still be considered for addressing the needs of a patient with medical or financial restrictions.

ABSTRACT

BACKGROUND: Patient transfers among medical facilities are high-risk situations. Despite this, there is very little training of physicians regarding the medical and legal aspects of transport medicine. OBJECTIVES: To examine the effects of a one hour, educational intervention on Emergency Medicine (EM) residents' and Critical Care (CC) fellows' knowledge regarding the medical and legal aspects of interfacility patient transfers. METHODS: Prior to the intervention, physician knowledge regarding 12 key concepts in patient transfer was assessed using a pre-test instrument. A one hour, interactive, educational session followed immediately thereafter. Following the intervention, a post-intervention test was given between two and four weeks after delivery. Participants were also asked to describe any prior transportation-medicine-related education, their opinions as they relate to the relevance of the topic, and their comfort levels with patient transfers before and after the intervention. RESULTS: Only a minority of participants had received any formal training in patient transfers prior to the intervention, despite dealing with patient transfers on a frequent, often daily, basis. Both groups improved in several categories on the post-intervention test. They reported improved comfort levels with the medicolegal aspects of interfacility patient transfers after the intervention and felt well-prepared to manage transfers in their daily practice. CONCLUSION: A one hour, educational intervention objectively increased EM and CC physician trainees' understanding of some of the medicolegal aspects of interfacility patient transfers. The study demonstrated a lack of previous training on this important topic and improved levels of comfort with transfers after study participation.

ABSTRACT

The American College of Prosthodontists (ACP) has developed a classification system designed for use by dental professionals in the diagnosis and treatment of completely dentate patients. This classification is the third in a series and is similar to the Classifications for Complete Edentulism and Partial Edentulism previously developed by the ACP. These guidelines are intended to aid practitioners in the systematic diagnosis of each patient which, in turn, should lead to an appropriate treatment. Four categories of a completely dentate situation are defined (Class I-IV), differentiated by specific diagnostic criteria, with Class I representing an uncomplicated clinical situation and Class IV representing the most complex clinical situation. Potential benefits of the system include (1) improved intraoperator consistency, (2) improved professional communication, (3) insurance reimbursement commensurate with complexity of care, (4) an improved screening tool for dental school admission clinics, (5) standardized criteria for outcomes assessment and research, (6) enhanced diagnostic consistency, and (7) a simplified aid in the decision-making process associated with referral.

ABSTRACT

This classification system is similar to the classification system for complete edentulism previously developed by the ACP. These guidelines are intended to help practitioners determine appropriate treatments for their patients.
