ABSTRACT

Experience of treatment of chronic wounds in 11 patients in 2014 ­ 2016 yrs was analyzed. The impact of negative pressure, using domestically manufactured apparatus «ÐGАТ- Dnepr¼, in complex of treatment as a local therapy, was applied. Vacuum-therapy was conducted preoperatively and postoperatively together with autodermoplasty. The injection material for the skin regeneration "lacerta" was applied subcutaneously intraoperatively. The Ialuset-cream bandages were used in 10 - 12 days postoperatively during 2-3 weeks. Еfficacy of vacuum-therapy was estimated, basing on analysis of the wound process course dynamics, clinic-laboratory criteria, terms of the wound defect complete closure. The patients' stationary treatment duration have constituted 38.3 days at average. Complex treatment of chronic wounds have had guaranteed complete healing of the wound defect in 7 (63%) patients in terms up to 1 - 5 weeks, and in 3 (26%) - 6 weeks.

ABSTRACT

In 57 patients with severe burns the dynamic state of peripheral blood neutrophil granulocytes was studied. It was established that the effect of combined detoxication therapy expresses the rise of the cells dynamic activity, attended by the rise of the organism natural resistance, what correlates with infection complications frequency diminution from 14 to 2%.

ABSTRACT

A cytological investigation of erythropoiesis and the peripheral blood was performed in 66 patients with a severe burn disease. It was established that the ultraviolet photomodification of the autoblood in complex with detoxication optimized hemopoiesis and allowed the nontransfusion correction of burn anemia to be performed at the expense of using the reserves of medullary hemopoiesis. The maintenance of basic parameters of the peripheral blood at the adaptational level requires 20% less erythrocyte-containing preparations of blood which naturally reduces the risk of post-transfusional complications.

ABSTRACT

The burn disease influence on the adaptational reactions of the organism was studied up in 192 injured persons. The stress-reaction was suggested the most unfavorable for the prognosis. The severity of pathological reactions is lowering, correlating with the complications occurrence frequency, while simultaneous application of conventional methods of treatment with combined detoxication and nonmedicinal immunomodulation.

ABSTRACT

The possibility to reduce intoxication of organism in critical burn disease by using application sorbents of the "Dnieper" type was studied. It was found that in acute period of burn disease there occurs resorption of toxic products of middle molecular weight from the lesion focus and accumulation of them in blood resulting in polyvalent alterations of functions of the internal organs. The using of application sorption allows the degree of toxicosis to become 20% less, the course of the wound process and the disease to be improved and to make the time of treatment 55.5% shorter.

ABSTRACT

Traditional surgical treatment of deep burns of the scalp and skull involve the excision of necrotic bone and soft tissues with trephanation of the bone to permit granulation tissue formation and subsequent grafting. This approach prolongs wound time, adds additional trauma and, even after initial healing, necessitates secondary soft tissue and bone reconstruction. The treatment described here is an alternate and more aggressive one involving early excision of necrotic soft tissue without bone resection followed by immediate coverage with well-vascularized axial flaps from adjacent intact scalp. This method provides an optimal environment for healing and regeneration of areas of destruction to the skull. This surgical method was used in the treatment of 22 patients with deep burns of the scalp with satisfactory clinical results. Bone regeneration was confirmed by roentgenological investigations.

ABSTRACT

The indices of burn traumatism in the Dnepropetrovsk region for the 10-year period are presented, and lethality in burns at the regional burn department is analysed. Improvement of the antishock therapy led to reduction of lethality at the period of shock, and didn't change essentially the general postoperative lethality.
