ABSTRACT

BACKGROUND: Administration of allergen mixtures of many components comprises the most common approach for American allergists regarding the management of polyallergic patients. European allergists, however, are more reluctant to this type of treatment due to the potential drawbacks of mixing extracts. RESEARCH DESIGN AND METHODS: To assess the efficacy and safety of subcutaneous immunotherapy (SCIT) with polymerized allergen mixtures without dilutional effect in polyallergic patients.This observational, prospective, multicenter study included patients (between 5 and 60 years) with respiratory allergic diseases that had been prescribed with SCIT with mixtures of two pollen or mite extracts. Changes in Symptoms and Medication Score (SMS) and in rhinitis quality of life questionnaire (RQLQ), subjective clinical improvement, treatment satisfaction and tolerability were assessed after the 1-year treatment. RESULTS: A total of 115 patients were included in the assessment. Mean global SMS decreased from 3.5 (SD = 1.1) to 1.6 (SD = 1.2) points, with a mean absolute reduction of 1.6 (SD = 1.3) points in the RQLQ score (p < 0.001, Wilcoxon test). General subjective clinical improvements and a good treatment satisfaction and tolerability were observed. CONCLUSION: SCIT with polymerized allergen mixtures from either pollen or mite extracts proved to be an effective and safe treatment option for polyallergic patients suffering from allergic respiratory diseases.

ABSTRACT

We describe a recent case of lymphatic filariasis in Colombia caused by Wuchereria bancrofti nematodes. Our study combines clinical-epidemiologic findings with phylogenetic data. Resurgence of lymphatic filariasis may be linked to increasing urbanization trends and migration from previously endemic regions. Fieldwork can be a beneficial tool for screening and containing transmission.

ABSTRACT

En el Campeonato Nacional de Paratletismo, en Cuba, los resultados competitivos de las balistas con discapacidad físico-motriz por amputación muestran un decrecimiento, en varias categorías. Se detectaron insuficiencias en la dosificación de las cargas, en la utilización de métodos, medios y procedimientos; además de una inadecuada orientación metodológica. Se trazó como objetivo diseñar una metodología para la planificación de la capacidad fuerza rápida en la etapa de preparación especial de balistas con discapacidad físico-motriz por amputación. Se realizó una investigación de tipo descriptiva y cuantitativa en el periodo comprendido entre el 2019-2022, el estudio se centró en el proceso de planificación y entrenamiento de la fuerza rápida en la etapa de preparación especial analizado y descrito, a través de métodos empíricos como la revisión de documentos, encuesta, entrevista y la triangulación metodológica, ello permitió evaluar su estado actual. Colaboraron como muestra atletas, entrenadores y directivos de nivel provincial y nacional. La metodología propuesta es viable y factible, con una efectividad esperada para el contexto deportivo actual y con posibilidades de generalización en otros eventos de lanzamiento. La propuesta diseñada enriquece la teoría del entrenamiento deportivo en atletas con discapacidad.

Foram detectadas insuficiências na dosagem de cargas, no uso de métodos, meios e procedimentos, bem como uma orientação metodológica inadequada. O objetivo foi elaborar uma metodologia para o planejamento da capacidade de força rápida na fase de preparação especial de balistas com deficiência físico-motora decorrente de amputação. Foi realizada uma pesquisa descritiva e quantitativa no período entre 2019-2022, o estudo focou no processo de planejamento e treinamento da força rápida na fase de preparação especial analisada e descrita, por meio de métodos empíricos como revisão de documentos, pesquisa, entrevista e triangulação metodológica, o que permitiu avaliar seu estado atual. Atletas, técnicos e gerentes em nível provincial e nacional colaboraram como amostra. A metodologia proposta é viável e factível, com uma eficácia esperada para o contexto esportivo atual e com possibilidades de generalização em outros eventos de arremesso. A proposta elaborada enriquece a teoria do treinamento esportivo em atletas com deficiências.


In the National Para-Athletics Championship in Cuba, the competitive results of shot put athletes with physical-motor disabilities due to amputation show a decrease in several categories. Deficiencies were detected in the dosage of loads, in the use of methods, means and procedures; in addition to inadequate methodological guidance. The objective was to design a methodology for planning rapid strength capacity in the special preparation stage of shot put athletes with physical - motor disabilities due to amputation. A descriptive and quantitative research was carried out in the period between 2019-2022, the study focused on the planning and training process of rapid strength in the special preparation stage analyzed and described, through empirical methods such as the review of documents, survey, interview and methodological triangulation made it possible to evaluate its current state. Athletes, coaches and managers from provincial and national levels collaborated as a sample. The proposed methodology is viable and feasible, with expected effectiveness for the current sporting context and with possibilities of generalization in other throwing events. The designed proposal enriches the theory of sports training in athletes with disabilities.

ABSTRACT

Introduction: Hematopoietic stem cell transplantation (HCT) can cure chronic granulomatous disease (CGD). However, transplant-associated morbidity or mortality may occur, and it is still controversial which patients benefit from this procedure. The aim of this retrospective study was to evaluate the outcome of pediatric patients who received HCT in one of the Spanish pediatric transplant units. Results: Thirty children with a median age of 6.9 years (range 0.6-12.7) were evaluated: 8 patients received a transplant from a sibling donor (MSD), 21 received a transplant from an unrelated donor (UD), and 1 received a haploidentical transplant. The majority of the patients received reduced-intensity conditioning regimens based on either busulfan plus fludarabine or treosulfan. Relevant post-HCT complications were as follows: i) graft failure (GF), with a global incidence of 28.26% (CI: 15.15-48.88), 11.1% in patients with MSD (1.64-56.70) and 37.08% in unrelated donors (19.33-63.17); and ii) chronic graft-versus-host disease (GVHD), with an incidence of 20.5% (8.9-43.2), 11.1% in patients with MSD (1.64-56.70) and 26.7% in unrelated donors (10.42-58.44). Post-HCT infections were usually manageable, but two episodes of pulmonary aspergillosis were diagnosed in the context of graft rejection. The 2-year OS was 77.3% (55.92-89.23). There were no statistically significant differences among donor types. Discussion: HCT in patients with CGD is a complex procedure with significant morbidity and mortality, especially in patients who receive grafts from unrelated donors. These factors need to be considered in the decision-making process and when discussing conditioning and GVHD prophylaxis.

ABSTRACT

Introduction: Healthcare-associated infections pose a significant challenge, contributing to hospital morbidity and mortality. Objective: To describe the behavior of Healthcare Associated Infections before and during the pandemic reported to a high-complexity health institution in Colombia. Material and Methods: In our retrospective observational study on Healthcare-Associated Infections (HAIs), we analyzed data from all in-patients diagnosed with HAIs between 2018 and 2020. This included clinical, demographic, microbiological, and microbial susceptibility information collected from the Committee on Nosocomial Infections' prospective database. Data from 391 isolates were obtained using Whonet software for antimicrobial resistance surveillance. Results: We found 504 cases of HAIs (2018-2020) with an overall in-hospital infection rate of 2.55/1000 patient-days. The median age for pediatric patients was 5 years, and for adults, 56 years, with 57% male. The leading admission diagnoses were oncologic disease complications (31%). Bacteremia had a 30-day mortality rate of 13%, predominantly catheter-associated (37%). Gram-negative bacilli, notably Klebsiella pneumoniae, Escherichia coli, and Pseudomonas aeruginosa, represented 58% cases of HAI. Discussion: The critical need for specific interventions and antimicrobial management to control HAIs, especially given the challenges posed by the COVID-19 pandemic, is highlighted. Conclusions: This is the first report on HAIs incidence at a tertiary hospital in Bucaramanga, Santander (Colombia). Bacteremia was predominant; 75% of HAIs patients had comorbidities. Gram-negative bacilli prevailed; a notable rise in ICU respiratory infections occurred during the 2020 COVID-19 pandemic. Resistance to cephalosporins and carbapenems was prevalent.

ABSTRACT

Targeted next-generation sequencing (NGS) enables the identification of genomic variants in cancer patients with high sensitivity at relatively low costs, and has thus opened the era to personalized human oncology. Veterinary medicine tends to adopt new technologies at a slower pace compared to human medicine due to lower funding, nonetheless it embraces technological advancements over time. Hence, it is reasonable to assume that targeted NGS will be incorporated into routine veterinary practice in the foreseeable future. Many animal diseases have well-researched human counterparts and hence, insights gained from the latter might, in principle, be harnessed to elucidate the former. Here, we present the TiHoCL targeted NGS panel as a proof of concept, exemplifying how functional genomics and network approaches can be effectively used to leverage the wealth of information available for human diseases in the development of targeted sequencing panels for veterinary medicine. Specifically, the TiHoCL targeted NGS panel is a molecular tool for characterizing and stratifying canine lymphoma (CL) patients designed based on human non-Hodgkin lymphoma (NHL) research outputs. While various single nucleotide polymorphisms (SNPs) have been associated with high risk of developing NHL, poor prognosis and resistance to treatment in NHL patients, little is known about the genetics of CL. Thus, the ~100 SNPs featured in the TiHoCL targeted NGS panel were selected using functional genomics and network approaches following a literature and database search that shielded ~500 SNPs associated with, in nearly all cases, human hematologic malignancies. The TiHoCL targeted NGS panel underwent technical validation and preliminary functional assessment by sequencing DNA samples isolated from blood of 29 lymphoma dogs using an Ion Torrent™ PGM System achieving good sequencing run metrics. Our design framework holds new possibilities for the design of similar molecular tools applied to other diseases for which limited knowledge is available and will improve drug target discovery and patient care.

ABSTRACT

Poverty is a multidimensional phenomenon that encompasses privation of education, health or housing. Women show more positive perceptions of poor people, making external attributions for the causes of poverty or the circumstances that explain it. The aim of this study is to analyse perceptions of poverty, identifying the differences in attitudinal profiles between women and men, and the influence of their political and religious beliefs. The sample consists of 278 participants (154 women and 124 men), who completed a sociodemographic questionnaire and the Scale of Attitudes and Stereotypes toward Poverty. The results showed two attitude profiles for women and men, with differences in the first profile, where women or men did not have religious beliefs, had left-wing or centre-left political ideas and favourable attitudes about poverty.

ABSTRACT

Although children and adolescents with acute lymphoblastic leukaemia (ALL) have high survival rates, approximately 15-20% of patients relapse. Risk of relapse is routinely estimated at diagnosis by biological factors, including flow cytometry data. This high-dimensional data is typically manually assessed by projecting it onto a subset of biomarkers. Cell density and "empty spaces" in 2D projections of the data, i.e. regions devoid of cells, are then used for qualitative assessment. Here, we use topological data analysis (TDA), which quantifies shapes, including empty spaces, in data, to analyse pre-treatment ALL datasets with known patient outcomes. We combine these fully unsupervised analyses with Machine Learning (ML) to identify significant shape characteristics and demonstrate that they accurately predict risk of relapse, particularly for patients previously classified as 'low risk'. We independently confirm the predictive power of CD10, CD20, CD38, and CD45 as biomarkers for ALL diagnosis. Based on our analyses, we propose three increasingly detailed prognostic pipelines for analysing flow cytometry data from ALL patients depending on technical and technological availability: 1. Visual inspection of specific biological features in biparametric projections of the data; 2. Computation of quantitative topological descriptors of such projections; 3. A combined analysis, using TDA and ML, in the four-parameter space defined by CD10, CD20, CD38 and CD45. Our analyses readily extend to other haematological malignancies.

El insuficiente estudio de los patrones de lateralidad en el deporte limita el diagnóstico del tipo de motricidad y obstaculiza notablemente los procesos de planificación del entrenamiento, desde las individualidades de los futbolistas, para buscar el rendimiento óptimo. El objetivo del estudio fue caracterizar los patrones de lateralidad predominantes en futbolistas masculinos del equipo Habana, categoría juvenil. Se desarrolló un estudio exploratorio y descriptivo que aplicó métodos de carácter teórico, empírico (entrevistas y encuestas), y estadístico-matemático (por medio de hojas de cálculo de Excel). Fueron aplicados seistest (Ojo director, Mano directora, Lateralidad de cadera, Lateralidad de hombro, Pierna dinámica y Pie director), con instrumentos internacionales contextualizados y avalados en investigaciones especializadas para su uso en Cuba. La heterogeneidad de la muestra mostró la tendencia genérica en el comportamiento preferencial de los futbolistas estudiados.


O estudo insuficiente dos padrões de lateralidade no esporte limita o diagnóstico do tipo de motricidade e dificulta significativamente os processos de planejamento do treinamento, a partir das individualidades dos jogadores de futebol, a fim de buscar um desempenho ideal. O objetivo do estudo foi caracterizar os padrões predominantes de lateralidade em jogadores de futebol masculino da equipe Habana, categoria juvenil. Foi realizado um estudo exploratório e descritivo com métodos teóricos, empíricos (entrevistas e pesquisas) e estatístico-matemáticos (por meio de planilhas do Excel). Foram aplicados seis testes (Olho Direcionado, Mão Direcionada, Lateralidade do Quadril, Lateralidade do Ombro, Perna Dinâmica e Pé Direcionado), com instrumentos internacionais contextualizados e aprovados em pesquisas especializadas para uso em Cuba. A heterogeneidade da amostra mostrou a tendência genérica no comportamento preferencial dos jogadores de futebol estudados.


The insufficient study of laterality patterns in sport limits the diagnosis of the type of motor skills and significantly hinders the training planning processes, from the individualities of the soccer players, to seek optimal performance. The objective of the study was to characterize the predominant laterality patterns in male soccer players of the Havana team, youth category. An exploratory and descriptive study was developed that applied theoretical, empirical (interviews and surveys), and statistical-mathematical methods (using Excel spreadsheets). Six tests were applied (Directing eye, Directing hand, Laterality of the hip, Laterality of the shoulder, Dynamic leg and Directing foot), with contextualized international instruments and endorsed by specialized research for their use in Cuba. The heterogeneity of the sample showed the generic trend in the preferential behavior of the soccer players studied.

El trabajo que se presenta parte de un estudio de tipo descriptivo, de naturaleza cualitativa y cuantitativa; para ello, se aplicaron métodos teóricos como el inductivo-deductivo y el analítico-sintético, métodos empíricos como la revisión documental, la observación, la encuesta y la entrevista; además de la medición para la obtención de los datos de la muestra, a partir de la aplicación de los test. Se resaltaron las características del tenis de mesa como deporte, así como las lesiones que se presentan en el mismo. Se indagó acerca de las bases teóricas de los músculos estabilizadores que son aquellos que permiten fijar una articulación para poner ciertos grupos musculares en movimiento. Los tenimesistas categoría sub 13 de La Habana carecen de un instrumento metodológico que sustente el entrenamiento de la resistencia a la fuerza de los músculos estabilizadores, por lo que se propuso como objetivo de esta investigación evaluar la resistencia a la fuerza de los músculos estabilizadores de los tenimesistas categoría sub 13 de La Habana. Se diseñó el instrumento eval-lumbar tennis que permitió valorar el nivel de resistencia a la fuerza de los músculos estabilizadores de la muestra seleccionada. Como resultados, el 100 % de la muestra presentó una resistencia a la fuerza deficiente, donde se encontraron débiles los siguientes músculos: recto abdominal, oblicuo abdominal, multífidos, cuadrado lumbar, intertransverso, serrato anterior, erectores espinales y deltoides en su porción lateral y frontal.


O trabalho que se apresenta assenta num estudo descritivo, de natureza qualitativa e quantitativa; Para isso, foram aplicados métodos teóricos como indutivo-dedutivo e analítico-sintético, métodos empíricos como revisão documental, observação, levantamento e entrevista; além da medição para obtenção dos dados amostrais, a partir da aplicação dos testes. Foram destacadas as características do tênis de mesa enquanto esporte, bem como as lesões que nele ocorrem. Foram investigadas as bases teóricas dos músculos estabilizadores, que são aqueles que permitem fixar uma articulação para colocar em movimento determinados grupos musculares. Os tenistas da categoria sub 13 de Havana carecem de um instrumento metodológico que apoie o treinamento de força resistida dos músculos estabilizadores, por isso o objetivo desta pesquisa foi avaliar a força resistiva dos músculos estabilizadores dos tenistas da categoria sub 13 de Havana. O instrumento de tênis eval-lombar foi projetado para avaliar o nível de resistência à força dos músculos estabilizadores da amostra selecionada. Como resultados, 100% da amostra apresentou resistência à força deficiente, onde se encontravam fracos os seguintes músculos: reto abdominal, oblíquo abdominal, multífido, quadrado lombar, intertransverso, serrátil anterior, eretores da coluna vertebral e deltóides em sua porção lateral e frontal.


The work that is presented is based on a descriptive study, of a qualitative and quantitative nature; for this, theoretical methods such as inductive-deductive and analytical-synthetic, as well as empirical methods such as documentary review, observation, survey, interview and the measurement to obtain the sample data, from the application of the tests were applied. The characteristics of table tennis as a sport were highlighted, as well as the injuries that occur in it. The theoretical bases of the stabilizing muscles were studied, which are those that allow fixing a joint to put certain muscle groups in motion. The sub 13 category tennis players in Havana lack a methodological instrument that supports the strength endurance training of the stabilizer muscles, so the objective of this research was to evaluate the strength endurance of the stabilizer muscles of the sub 13 category tennis players from Havana. The eval -lumbar tennis instrument was designed to assess the level of endurance to strength of the stabilizing muscles of the selected sample. As a result, 100 % of the sample presented a deficient endurance to strength, where the following muscles were found to be weak: rectus abdominis, oblique abdominis, multifidus, quadratus lumborum, intertransversus, serratus anterior, spinal erectors and deltoids in its lateral portion and frontal.

En México, el creciente sedentarismo de la población escolar constituye un serio motivo de preocupación para las instituciones de la salud pública y deportiva. Incluir el béisbol como parte del deporte extraescolar es uno de los retos de las autoridades de la escuela deportiva matutina "Enrique Conrado Rébsamen" de Xalapa, Veracruz. A partir del diagnóstico preliminar, con la aplicación de entrevistas y encuestas a docentes, deportivos y directivos, se constató la escasa documentación, bibliografías, programas e indicaciones metodológicas relacionadas con la enseñanza de los fundamentos básicos del béisbol, y en específico del bateo. Por lo que se plantea como objetivo proponer acciones para la enseñanza del bateo en la escuela deportiva de Xalapa, Veracruz. Se emplearon métodos teóricos como el analítico-sintético, histórico-lógico, inductivo-deductivo y la modelación. Del nivel empírico se utilizó la observación, la entrevista, la encuesta, la revisión de documentos, la técnica Iadov, la triangulación metodológica y el criterio de expertos; además, se utilizó el método estadístico-matemático y se calculó el coeficiente de concordancia de Kendall (W). El resultado demuestra las fortalezas de las acciones para la integración de conocimientos teóricos, prácticos y metodológicos que permite diseñar una herramienta para el trabajo de los docentes deportivos.


No México, o crescente estilo de vida sedentário da população escolar é uma séria preocupação para as instituições de saúde pública e esportivas. A inclusão do beisebol como parte do esporte extracurricular é um dos desafios para as autoridades da escola de esportes matutina "Enrique Conrado Rébsamen" em Xalapa, Veracruz. A partir do diagnóstico preliminar, com a aplicação de entrevistas e pesquisas com professores, esportistas e diretores, foi confirmada a escassez de documentação, bibliografias, programas e indicações metodológicas relacionadas ao ensino dos fundamentos básicos do beisebol e, especificamente, do rebatimento. Portanto, o objetivo foi propor ações para o ensino do rebater na escola de esportes de Xalapa, Veracruz. Foram utilizados métodos teóricos como o analítico-sintético, histórico-lógico, indutivo-dedutivo e modelagem. No nível empírico, foram usados observação, entrevista, pesquisa, revisão de documentos, técnica de Iadov, triangulação metodológica e julgamento de especialistas; além disso, foi usado o método estatístico-matemático e calculado o coeficiente de concordância de Kendall (W). O resultado demonstra os pontos fortes das ações para a integração de conhecimentos teóricos, práticos e metodológicos que permitem a elaboração de uma ferramenta para o trabalho dos professores de esportes.


In Mexico, the increasing sedentary lifestyle of the school population is a serious concern for public health and sports institutions. Including baseball as part of extracurricular sports is one of the challenges of the authorities of the morning sports school "Enrique Conrado Rébsamen" in Xalapa, Veracruz. From the preliminary diagnosis, with the application of interviews and surveys to teachers, sport trainers and managers, it was verified the scarce documentation, bibliographies, programs and methodological indications related to the teaching of the basic fundamentals of baseball, and specifically batting. Therefore, the objective is to propose actions for the teaching of batting in the sports school of Xalapa, Veracruz. Theoretical methods such as analytical-synthetic, historical-logical, inductive-deductive, and modeling were used. From the empirical level, observation, interview, survey, document review, the Iadov technique, methodological triangulation and expert criteria were used; in addition, the statistical-mathematical method was used and the Kendall coefficient of concordance (W) was calculated. The result demonstrates the strengths of the actions for the integration of theoretical, practical and methodological knowledge that allows designing a tool for the work of sports teachers.

Crofton weed (Ageratina adenophora) is a global and highly invasive weed, with ingestion causing severe respiratory disease in horses, leading to irreversible and untreatable pulmonary fibrosis and oedema. While reports of equine pneumotoxicity remain common in Australia and New Zealand, equine pneumotoxicity may be underdiagnosed in other countries where Crofton weed is endemic but poorly differentiated. The pathogenesis of Crofton weed toxicity following ingestion has been well described in a number of different animal models, including rodents, rabbits, and goats. However, induced toxicity is organ-selective across different animal species, and these vastly differ from the pathogenesis described in horses, both clinically and after experimental exposure. Sources of variation may include species-specific susceptibility to different toxins present in the plant, different mechanistic processes of toxicity, and species differences in toxin biotransformation and bioactivation across different organs. Considering disease severity and Crofton weed's invasiveness globally, assessing published toxicological and exposure data is necessary to advance research, identify specific toxins for horses, and possible prophylactic and therapeutic strategies. This review presents an overview of the available literature on equine toxicity, parallels between toxicity in horses and other animal species, and important aspects to be included in the future research agenda.

Abstract Aim: To analyse the potential usefulness and clinical relevance of the assessment by echocardiography with left atrial strain, based on the myocardial atrial deformation curves with speckle-tracking velocity vector imaging (VVI), in the analysis of short-form recurrent atrial extra systoles in ambulatory patients not suffering from organic cardiopathy. Methods: We designed a descriptive, prospective, and observational study including 270 patients between the ages of 18 and 75 assessed during an outpatient cardiology consultation attended due to palpitations over a period of two years. Using ambulatory electrocardiographic monitoring, we selected cases with short forms of repetitive atrial extrasystole, isolated or recurrentatrial fibrillation and a control group formed by those patients without repetitive ectopia. All patients underwent a thorough echocardiographic study during their first cardiological visit. Results: The analysis of the dynamic curves segmental deformation generated after an atrial extrasystole can reveal different points of origin of the extrasystole and detect specific anatomical alterations in the interatrial conduction at the level of the Bachmann's fascicle showing different models of electro anatomical activation possibly involved in the appearance of repetitive forms. Higher values of dyssynchrony between the septal and lateral wall and elongation in the time of interatrial electromechanical conduction could also be related to the existence of repetitive ectopic beats. Conclusions: Our ambulatory study employing the left atrial longitudinal strain, particularly in its segmental analysis, provides new insights into its the usefulness and potential clinical relevance.


Resumen Objetivo: Analizar la utilidad y relevancia clínica de la evaluación mediante ecocardiografía basada en las curvas de deformación auricular miocárdica con imágenes vectoriales de velocidad (VVI) de speckle-tracking, en el análisis de las extrasístoles auriculares recurrentes de corta duración en pacientes ambulatorios sin cardiopatía orgánica. Métodos: Se diseñó un estudio descriptivo, prospectivo y observacional que incluyó a 270 pacientes de entre 18 y 75 años evaluados durante una consulta externa de cardiología a la que acudieron por palpitaciones durante un periodo de dos años. Mediante el uso de monitorización electrocardiográfica ambulatoria, se seleccionaron casos con formas cortas de extrasistolia auricular repetitiva, fibrilación auricular aislada o repetitiva y un grupo control formado por aquellos pacientes sin ectopia repetitiva. Todos los pacientes se sometieron a un estudio ecocardiográfico exhaustivo durante su primera visita cardiológica. Resultados: El análisis de las curvas dinámicas de deformación segmentaria generadas tras un extrasístole auricular diferentes modelos de activación electroanatómica posiblemente implicados en la aparición de formas repetitivas. Valores mayores de disincronía entre la pared septal y lateral y el alargamiento en el tiempo de conducción electromecánica intraauricular pudieran también relacionarse con la existencia de latidos ectópicos repetitivos. Conclusiones: Nuestro estudio ambulatorio empleando la deformación longitudinal auricular izquierda, particularmente en su análisis segmentario, proporciona nuevas perspectivas sobre su utilidad y potencial relevancia clínica.

ABSTRACT

One-anastomosis gastric bypass has now become the third most commonly performed bariatric technique worldwide. However, as a consequence of the configuration of this surgery, it can present some chronic complications (anastomotic mouth ulcers and biliary reflux) that physicians must come to better understand and assess. In this narrative review, we aimed to update our knowledge of both the diagnosis and treatment of these two complications in the context of bariatric surgeries. We concluded that a series of pre-, intra-, and postoperative preventive strategies should be considered by surgeons to help reduce the appearance of these complications.

RESUMEN Introducción: El trabajo de investigación que se muestra en este artículo, ofrece una respuesta a la carencia de documentos orientadores para el direccionamiento del proceso de enseñanza aprendizaje de los pasos y carreras como elementos técnicos de enlace en los ejercicios competitivos de gimnasia rítmica; lo que a su vez, constituye un apoyo a la aplicación del Programa Integral de Preparación del Deportista (PIPD 2017-2020) y de soporte de capacitación a las entrenadoras encargadas de la preparación de las gimnastas. Objetivo: Ello encaminó el objetivo de la investigación a la elaboración de una guía metodológica en formato multimedia que sirva a las entrenadoras en el proceso de enseñanza de estos elementos contenidos en el PIPD. Materiales y métodos: Los métodos de investigación de nivel teórico utilizados (análisis y síntesis, enfoque de sistema) y los de nivel empírico (la encuesta, la entrevista y el criterio de especialista, el análisis de documentos y la observación de videos). Resultados: Se demuestra la afectación agravada de la enseñanza por los diversos modelos técnicos metodológicos actuantes. Conclusiones: Se concluye planteando que una guía metodológica debe contener la descripción técnica y su metodología de enseñanza, los errores más comunes, elementos de preparación especial para la consecución y las recomendaciones.


RESUMO Introdução: O trabalho de pesquisa mostrado neste artigo, oferece uma resposta à falta de documentos orientadores para a direção do processo de ensino-aprendizagem das etapas e corridas como elementos técnicos de ligação nos exercícios competitivos da ginástica rítmica; o que, por sua vez, constitui um apoio para a aplicação do Programa Integral de Preparação do Atleta (PIPD 2017-2020) e de apoio ao treinamento dos treinadores encarregados da preparação dos ginastas. Objetivo: Isto levou o objetivo da pesquisa à elaboração de um guia metodológico em formato multimídia que serve aos treinadores no processo de ensino destes elementos contidos no PIPD. Materiais e métodos: Os métodos de pesquisa utilizados foram teóricos (análise e síntese, abordagem de sistema) e empíricos (pesquisa, entrevista e critérios especializados, análise de documentos e observação de vídeo). Resultados: A afetação agravada do ensino pelos vários modelos metodológicos de atuação técnica é demonstrada. Conclusões: Conclui-se propondo que um guia metodológico deve conter a descrição técnica e sua metodologia de ensino, os erros mais comuns, elementos de preparação especial para a realização e recomendações.


ABSTRACT Introduction: The research work shown in this article offers a response to the lack of guiding documents for the direction of the teaching-learning process of the steps and runs as technical elements of connection in the competitive exercises of rhythmic gymnastics; which, in turn, constitutes support for the application of the Comprehensive Program for the Preparation of the Athlete (PIPD 2017-2020) and training support for the coaches in charge of preparing the gymnasts. Objective: This directed the objective of the research to the elaboration of a methodological guide in multimedia format that serves the trainers in the process of teaching these elements contained in the PIPD. Materials and methods: The theoretical level research methods used (analysis and synthesis, system approach) and the empirical level (survey, interview and specialist criteria, document analysis and video observation). Results: The aggravated affectation of teaching by the various acting methodological technical models is demonstrated. Conclusions: It is concluded by stating that a methodological guide must contain the technical description and its teaching methodology, the most common errors, elements of special preparation for achievement and recommendations.

ABSTRACT

Objetivo: Aplicar la teoría del cambio al diseño y la evaluación de un programa para promover la actividad física en 11 zonas básicas de salud. Método: Realización de cuatro grupos focales siguiendo la metodología de la teoría del cambio. Se identifican los cambios (a largo, medio y corto plazo) que se pretende alcanzar con el programa «La Ribera Camina» según la opinión de los agentes de interés: profesionales de atención primaria, concejalas/es y personal técnico municipal deportivo, y ciudadanía participante. A través de un análisis temático se identifican las acciones que se deben realizar para alcanzar estos cambios, y las dificultades y los facilitadores para la sostenibilidad del programa. Resultados: Los cambios identificados se clasificaron en cuatro apartados: 1) cambios en la salud física y social (mejoría en la condición física, hábitos saludables, autoestima y sensación de bienestar); 2) cambios organizativos y relacionales (mejor coordinación entre instituciones); 3) cambios específicos del programa (incorporación de más «activos» y asociaciones, sobre todo de hombres, y ampliación de rutas y horarios); y 4) cambios en el entorno (mejora de infraestructuras y seguridad de las rutas). Conclusiones: La teoría del cambio permite identificar y clasificar los cambios que se esperan, las acciones que deben realizarse y los vínculos entre elementos del programa. Esto servirá de base para la evaluación del programa. Dicha metodología podría aplicarse en otros programas que deseen incorporar la intersectorialidad y la participación comunitaria en su diseño y evaluación. (AU)


Objective: To develop a theory of change of a program to promote physical activity in eleven health districts, in order to improve its design and plan its evaluation. Method: Four focus groups were carried out, to develop a participatory theory of change, to identify the expected changes (long, medium and short term) of “La Ribera Camina” program, according to the following stakeholders: primary healthcare professionals, local government representatives and community members. A thematic analysis was used to identify the actions to be taken to achieve these changes, as well as the difficulties and facilitators to enhance the sustainability of the program. Results: The identified changes were classified into four themes: 1) changes in physical and social health (improved physical condition, healthy habits, self-esteem and perceived well-being); 2) organizational and relational changes (better coordination between institutions); 3) specific changes to the program (incorporation of more “assets” and local associations, especially male participants, more trails and schedules); and 4) changes in the environment (improved trails’ infrastructures and safety). Conclusions: The theory of change allows to identify and classify the changes that are expected, the actions to be carried out and the links between elements of the program. This will serve as the basis for its evaluation. This methodology could be applied to other programs interested in incorporating intersectorality and community engagement in their design and evaluation. (AU)

ABSTRACT

Dogs suffering from Myxomatous Mitral Valve Disease (MMVD) show a potential heart enlargement, especially in the left atrium, detectable by radiography. Due to digital radiography, different radiographic measurements estimate cardiac size quite uncomplicatedly. The Vertebral Heart Size (VHS), Radiographic Left Atrial Dimension (RLAD), Left Atrial Width (LAWidth), and the Vertebral Left Atrial Size (VLAS) used anatomical landmarks for measuring cardiac size in relation to the vertebral column. This study aimed to compare VHS, RLAD, LAWidth, and VLAS measured in conventional and inverted radiographs by veterinarians with different levels of experience in healthy dogs and dogs with MMVD. The reliability and user-friendliness of these measurements were evaluated, and the staging was compared to the echocardiography staging. A total of 50 unaffected dogs and 150 dogs with MMVD in stages B1, B2, and C were assessed. Three veterinarians with different levels of experience examined 200 conventional radiographs and their corresponding inverted radiographs blinded to the echocardiographic and clinical examination results. Analyses were performed to compare the measurements' grading and determine anatomical landmarks with measurement difficulties. Additionally, inter- and intraobserver agreement was assessed using intraclass correlation coefficient, and the agreement between radiographic and echocardiographic staging was compared using the kappa coefficient. The VHS, LAWidth, and VLAS were easier to define than the RLAD. The interobserver agreement was almost perfect for VHS (0.962) and good for the other radiographic measurements (RLAD: 0.778, LAWidth: 0.772, VLAS: 0.858). The VHS assigned the most dogs to the correct stage. However, VHS, RLAD, LAWidth, and VLAS presented an almost perfect intraobserver agreement. The dorsal left atrial margin of the RLAD was the most difficult measurement point to identify. The VHS is the most reproducible radiographic method for measuring the canine heart size and shows the highest agreement with echocardiography. An observer-related influence could be detected for RLAD, LAWidth, and VLAS.

ABSTRACT

The molecular heterogeneity of feline mammary carcinomas (FMCs) represents a prognostic and therapeutic challenge. RNA-Seq-based comparative transcriptomic profiling serves to identify recurrent and exclusive differentially expressed genes (DEGs) across sample types and molecular subtypes. Using mass-parallel RNA-Seq, we identified DEGs and performed comparative function-based analysis across 15 tumours (four basal-like triple-negative [TN], eight normal-like TN, and three luminal B fHER2 negative [LB fHER2-]), two cell lines (CL, TiHo-0906, and TiHo-1403) isolated from the primary tumours (LB fHER2-) of two cats included in this study, and 13 healthy mammary tissue controls. DEGs in tumours were predominantly upregulated; dysregulation of CLs transcriptome was more extensive, including mostly downregulated genes. Cell-cycle and metabolic-related DEGs were upregulated in both tumours and CLs, including therapeutically-targetable cell cycle regulators (e.g. CCNB1, CCNB2, CDK1, CDK4, GTSE1, MCM4, and MCM5), metabolic-related genes (e.g. FADS2 and SLC16A3), heat-shock proteins (e.g. HSPH1, HSP90B1, and HSPA5), genes controlling centrosome disjunction (e.g. RACGAP1 and NEK2), and collagen molecules (e.g. COL2A1). DEGs specifically upregulated in basal-like TN tumours were involved in antigen processing and presentation, in normal-like TN tumours encoded G protein-coupled receptors (GPCRs), and in LB fHER2- tumours were associated with lysosomes, phagosomes, and endosomes formation. Downregulated DEGs in CLs were associated with structural and signalling cell surface components. Hence, our results suggest that upregulation of genes enhancing proliferation and metabolism is a common feature among FMCs and derived CLs. In contrast, the dissimilarities observed in dysregulation of membrane components highlight CLs' disconnection with the tumour microenvironment. Furthermore, recurrent and exclusive DEGs associated with dysregulated pathways might be useful for the development of prognostically and therapeutically-relevant targeted panels.
