ABSTRACT

Background. Benign ovarian tumors (BOT) occupy the 2nd place in the structure of diseases of the female genital organs. In 20% of women of reproductive age, BOT are associated with infertility. One of the causes of infertility caused by ovarian tumors is morphofunctional inferiority with impaired endometrial receptivity. OBJECTIVE: To reveal the morphological and functional features of the endometrium and the level of receptivity to sex hormones in patients with BOT before and after organ-preserving operations. MATERIAL AND METHODS: The study included 77 patients with epithelial ovarian tumors (EOT) - I group, 52 with mature teratomas (MT) - II group. Before and 6-12 months after laparoscopic cystectomy aspiration biopsy of endometrium was performed in the middle stage of secretory phase. The percentage and degree of maturity of pinopodes were determined, and the level of expression of estrogen (ER) and progesterone (PR) receptors in the glands and stroma of the endometrium was assessed. RESULTS: At the preoperative stage, a decrease in the number of mature pinopodes in patients with EOT was revealed. Normal levels of ER were determined in glands and stroma of endometrium, PR was reduced both in stroma and glands of uterine mucosa. In patients with MT all markers corresponded to those of healthy women. In the postoperative period an increase in the number of developed pinopodes on the apical surface of endometrium in patients of I group was found. The ER level did not differ from control values, PR remained reduced in stroma. In II group a persistent decrease in quantity of mature pinopodes, ER in stroma, PR in glands and stroma of uterine mucosa was recorded. CONCLUSION: The presence of BOT and unintentional intraoperative removal of healthy ovarian tissue lead to indirect disorders of the morphofunctional state and endometrial receptivity.

ABSTRACT

A series of square planar [Pt(N^C)(NHC)L] complexes containing cyclometallated N^C ligands (phenylpyridine and benzoquinoline) and N-heterocyclic carbene (NHC)--N^C = 2-phenylpyridine, 7,8-benzoquinoline; NHC = 1,3-dibenzylbenzimidazolium, 1,3-diethylbenzimidazolium, 1,3-dibenzylimidazolium; L = Cl, Br, -C2Ph--have been synthesized in moderate to good yields. The complexes obtained were characterized using chemical analysis, MS-ESI spectrometry, NMR spectroscopy and X-ray crystallography. The complexes display moderate to strong phosphorescence in solution (Q.Y. 0.3-7.9%) and in the solid state (Q.Y. 2.7-16.0%), which is related to metal modulated intraligand π-π* transitions located at the aromatic system of cyclometallated ligands with some contribution of the MLCT excited state. Emission lifetimes fall in the range of 0.2-1.5 µs in solution and amount up to 13 µs in the solid state. Analysis of the spectroscopic data together with the density functional theory (DFT) and time-dependent density functional theory (TDDFT) calculations clearly support this assignment and show negligible contribution of the auxiliary ligands to the emissive excited states. The compounds obtained were also used to prepare organic light emitting diode (OLED) devices, which display good luminance efficiency emitting in the green area of the visible spectrum.

ABSTRACT

An assessment of the radiological situation due to exposure to gamma radiation, radon and thoron was carried out at the former uranium mining and processing sites in Shekaftar, Minkush and Kadji Sai in Kyrgyzstan. Gamma dose rate measurements were made using various field instruments and radon/thoron measurements were carried out using discriminative radon ((222)Rn)/thoron ((220)Rn) solid state nuclear track detectors (SSNTD). The detectors were exposed for an extended period of time including at least three seasonal periods in a year, in different outdoor and indoor public and residential environments at the selected uranium legacy sites. The results showed that gamma, Rn and Tn doses were in general low, which consequently implies a low/relatively low radiological risk. The major radiation hazard is represented by abandoned radioactive filtration material that was being used as insulation by some Minkush residents for a longer period of time. Annual radiation doses of several hundred mSv could be received as a consequence of using this material in their houses. The radiation doses deriving from external radiation (gamma dose rate), indoor radon and thoron with their short-lived progenies in several cases exceeded national as well as international standards. Current doses of ionizing radiation do not represent any serious hazard to the health of the resident public, but this issue should be adequately addressed to further reduce needless exposure of resident public to ionizing radiation.

ABSTRACT

During 1949-1967, a U mine, a coal-fired thermal power plant and a processing plant for the extraction of U from the produced ash were operated at the Kadji Sai U mining site in Tonsk district, Issyk-Kul County, Kyrgyzstan. The Kadji Sai U legacy site represents a source of contamination of the local environment by naturally occurring radionuclides and associated trace elements. To assess the environmental impact of radionuclides and trace metals at the site, field expeditions were performed in 2007 and 2008 by the Joint collaboration between Norway, Kazakhstan, Kyrgyzstan, Tajikistan (JNKKT) project and the NATO SfP RESCA project. In addition to in situ gamma and Rn dose rate measurements, sampling included at site fractionation of water and sampling of water, fish, sediment, soils and vegetation. The concentrations of radionuclides and trace metals in water from Issyk-Kul Lake were in general low, but surprisingly high for As. Uptake of U and As was also observed in fish from the lake with maximum bioconcentration factors for liver of 1.6 and 75, respectively. The concentrations of U in water within the Kadji Sai area varied from 0.01 to 0.05 mg/L, except for downstream from the mining area where U reached a factor of 10 higher, 0.2 mg/L. Uranium concentrations in the drinking water of Kadji Sai village were about the level recommended by the WHO for drinking water. The (234)U/(238)U activity ratio reflected equilibrium conditions in the mining pond, but far from equilibrium outside this area (reaching 2.3 for an artesian well). Uranium, As and Ni were mainly present as low molecular mass (LMM, less than 10 kDa) species in all samples, indicating that these elemental species are mobile and potentially bioavailable. The soils from the mining sites were enriched in U, As and trace metals. Hot spots with elevated radioactivity levels were easily detected in Kadji Sai and radioactive particles were observed. The presence of particles carrying significant amount of radioactivity and toxic trace elements may represent a hazard during strong wind events (wind erosion). Based on sequential extractions, most of the elements were strongly associated with mineral matter, except for U and As having a relatively high remobilization potential. Low Kd was obtained for U (3.5 × 10(2) L/kg d.w.), intermediate Kds (~3 × 10(3) L/kg d.w.) were obtained for (226)Ra, As and Ni, while a high Kd (2.2 × 10(5) L/kg d.w.) were obtained for Pb. The accumulation of metals in fish gills reflected the LMM species in the Issyk-Kul water, and did not show any bioaccumulation. The muscle Hg concentrations in all fish species were low and did not represent any health risk even for groups at risk. Total gamma and Rn dose rate to man amounted to about 12 mSv/y, while the highest calculated dose rate for non-human species based on the ERICA Assessment Tool were obtained in terrestrial plants (164 µGy/h) due to the Ra exposure. The results obtained showed that radiation doses to resident public at all of the investigated sites in the Kadji Sai area were in general relatively low. Low radiological risk and no detrimental health impact on resident public can be expected at these sites. However, exposure to Rn and Tn in the living environment can be further reduced by implementing simple countermeasures such as ventilation of dwelling cellars. More focus in the Kadji Sai area should probably be put on trace elements, especially the As uptake in fish in Lake Issyk-Kul.

ABSTRACT

High vascularization is noted in serous papillary ovarian tumors of a low malignancy grade, particularly in malignant serous ovarian tumors with low vascularization in benign neoplasms. Pronounced morphological changes in the vascular wall are found mainly in malignant tumors and tumors of a low malignancy grade where primitive blood vessels of a sinusoid type with a thin muscle wall prevail. Types of vascularization in the tumors studied are variable and this correlates with results of color doppler mapping.

ABSTRACT

Morphometric study of blood vessels in serous tumors of the ovaries was carried out. Vascularization of benign, borderline, and malignant tumors is different, which agrees with the data of color Doppler mapping. Pronounced morphological changes in vascular wall (degenerative changes, sclerosis, and hyalinosis) were detected mainly in borderline and malignant tumors.

ABSTRACT

A total of 97 patients with laparoscopically confirmed tubal and peritoneal sterility were examined. Thirty-six patients presented with a normal biphasic menstrual cycle, the rest 61 with the lutein phase insufficiency. Follicular presence and size were examined by ultrasonography, the dominant follicle and endometrium volumes were estimated. Only in 17 of the 97 examinees all the examined parameters were found normal, in the rest deviations from the normal echographic picture were revealed. More marked changes were detected in the patients with the lutein phase insufficiency. The findings evidenced a direct correlation between ultrasonic data and rectal temperature, adhesion process severity and the results of ovarian biopsy specimen histologic examination. The authors recommend ultrasonic examination for the assessment of ovarian function in patients suffering from tubal and peritoneal sterility.

ABSTRACT

The authors analyze the usefulness of ultrasonic examination for the assessment of ovarian and endometrial status in 84 patients operated on for tubal and peritoneal sterility. After surgery the patients were recommended nootropic agents and cyclic vitamin therapy for 3 months. In cases with concomitant diseases (endometriosis, small pelvis varicose veins) synthetic estrogen-gestagen drugs, anticoagulants, disaggregants were prescribed. If the treatment did not result in pregnancy in 2 months, ovulation stimulants (clostilbegit, choriogonin) were prescribed. Drug therapy had a favorable effect on ovarian and endometrial function. Ultrasonic examination may be recommended for monitoring ovarian and endometrial status.

ABSTRACT

Small pelvic organs were examined in 61 postmenopausal females suspected for having an intrauterine abnormality and 25 females matched for age who had no clinical signs of endometrial abnormalities. The findings were compared with the results of hysteroscopic and histological examinations of their curettage specimens. There was a 85% agreement of echographic and histological findings in the patients with suspected intrauterine abnormalities. The echography of small pelvic organs is a screening tool for the diagnosis of intrauterine diseases in postmenopausal females. When M-mode echography shows high values, hysteroscopy and separate diagnostic curettage are indicated. M-mode echography is a useful method for checking up the endometrial status in postmenopausal females receiving hormonal therapy for proliferative endometrial processes.

ABSTRACT

A total of 65 women in labor complicated with uterine inertia were investigated for tissue pO2 and cardiac performance of fetuses under the effect of oxytocin. The authors stated that the aggravation of fetal status in drug-induced labors resulted from poorer tissue oxygenation caused by the activation of uterine contractility. In this line, hypoxic changes of the fetus were more pronounced in pregnancies complicated by nephropathy or prolonged pregnancy. Accurate monitoring of the fetal status and the character of induced labor course was found to be mandatory. Before oxytocin induction the fetus should be protected with diazepam.
