ABSTRACT

Questions of conservative and operative treatment are considered on the basis of studying 259 patients with spontaneous pneumothorax. In most of the patients the lung was spreaded by a single or permanent active aspiration of air from the pleural cavity. However, due to great amount of recurrences after the conservative treatment it should be more often followed by operative methods of treatment. Indications to operation are: failure of conservative spread of the lung, repeated collapse, recurrent pneumothoraxes and detection of subpleural bullas during thoracoscopy.

ABSTRACT

The authors observed 7 patients aged from 19 to 74. Five of them were operated upon. One patient died from anaerobic neclostridial phlegmon of the anterior abdominal wall which appeared after an injury of the not noticed urinary duct. Laparotomic accesses passing aside form the area of possible localization of the urinary duct were more justified. When operating on the paraumbilical area it is expedient to make a revision for the detection of the urachus and associated abnormalities on the anterior abdominal wall.

ABSTRACT

The observation of 107 patients with spontaneous non-specific pneumothorax has shown that more active surgical tactics should be used. In 48 patients treated for spontaneous pneumothorax during the last three years 27 operations were performed which completely cured the patient. There were no relapses.

ABSTRACT

An analysis of 11127 appendectomies has been made, among which carcinoid of the appendicular process was revealed in 8 cases. The carcinoid was diagnosed histologically. It is the authors' opinion that determination of the serotonin level in blood and 5-hydroxy-indol acetic acid in urine would permit to judge the activity of the carcinoid process. In case of progressing carcinoid a pararectal incision with a revision of the abdominal organs is recommended. Patients operated upon should be under medical observation.
