ABSTRACT

BACKGROUND: The aim of this study was to construct a novel prediction model for the pathological complete response (pCR) to neoadjuvant chemotherapy (NAC) using immune-related gene expression data. PATIENTS AND METHODS: DNA microarray data were used to perform a gene expression analysis of tumor samples obtained before NAC from 117 primary breast cancer patients. The samples were randomly divided into the training (n = 58) and the internal validation (n = 59) sets that were used to construct the prediction model for pCR. The model was further validated using an external validation set consisting of 901 patients treated with NAC from six public datasets. RESULTS: The training set was used to construct an immune-related 23-gene signature for NAC (IRSN-23) that is capable of classifying the patients as either genomically predicted responders (Gp-R) or non-responders (Gp-NR). IRSN-23 was first validated using an internal validation set, and the results showed that the pCR rate for Gp-R was significantly higher than that obtained for Gp-NR (38 versus 0%, P = 1.04E-04). The model was then tested using an external validation set, and this analysis showed that the pCR rate for Gp-R was also significantly higher (40 versus 11%, P = 4.98E-23). IRSN-23 predicted pCR regardless of the intrinsic subtypes (PAM50) and chemotherapeutic regimens, and a multivariate analysis showed that IRSN-23 was the most important predictor of pCR (odds ratio = 4.6; 95% confidence interval = 2.7-7.7; P = 8.25E-09). CONCLUSION: The novel prediction model (IRSN-23) constructed with immune-related genes can predict pCR independently of the intrinsic subtypes and chemotherapeutic regimens.

ABSTRACT

Fractal structures and non-Gaussian velocity distributions are characteristic properties commonly observed in virialized self-gravitating systems, such as galaxies and interstellar molecular clouds. We study the origin of these properties using a one-dimensional ring model that we propose in this paper. In this simple model, N particles are moving, on a circular ring fixed in three-dimensional space, with mutual interaction of gravity. This model is suitable for the accurate symplectic integration method by which we argue the phase transition in this system. Especially, in between the extended phase and the collapsed phase, we find an interesting phase (halo phase) that has negative specific heat at the intermediate energy scale. Moreover, in this phase, there appear scaling properties and nonthermal and non-Gaussian velocity distributions. In contrast, these peculiar properties are never observed in other gas and core phases. Particles in each phase have a typical time scale of motions determined by the cutoff length xi, the ring radius R, and the total energy E. Thus all relaxation patterns of the system are determined by these three time scales.

ABSTRACT

We created a self-study program using HTML browser on the Clinical Nursing General Remarks Course, Eighty-three students each selected a published book on a personal history (written personal reflections from individuals who had undergone medical treatment and hospitalization), read it and submitted reports of their impressions of the histories. Their reports were arranged from a nursing perspective and entered on the home page of our college using HTML browser. We intended that the students would become more interested in reading of the personal histories, and that they would acquire new self-study skills and increase their interest in Internet through use of our program. In addition, we hoped that this program would encourage positive communication and mutual sharing of information. The students were able to easily refer to a personal history according to their interest from a nursing perspective. Therefore this program realized the mutual learning among students and other users.
