ABSTRACT

Una nueva técnica utilizando el cierre asistido con vacío o presión negativa (VAC, por sus siglas en inglés) se utilizó exitosamente en un paciente masculino de 57 años, con una herida de esternotomía dehiscente e infectada. Posterior a su desbridamiento y drenaje de absceso, el tratamiento regular con gasa iodoformada que utilizó por 10 días, fue sustituido con la aplicación de presión negativa con succión controlada durante 7 días. Esta novedosa técnica, para el cierre deheridas, utilizada por primera vez en Latinoamérica, en este tipo de lesión, hizo posible reducir el tiempo de cicatrización y los costos hospitalarios inherentes a este tipo de complicación.


A new technique using vacuum assisted closure (VAC) was succesfully applied to a 57 year old, male patient, with a dehisced infected sternotomy wound. After debridment and abscess drainage, a standard treatment with iodorform gauze strip was applied during 10 days, then it was substituted with by the application of negative pressure by controlled suction through a porous derssing, during 7 days. This novel technique, the first time applied in Latinamerica in this kind of lesion, has made possible to reduce healing time and hospital costs.

ABSTRACT

El pneumotorax catamenial es aquel que ocurre en el periódo perimenstrual, generalmente en la tercera o cuarta década de la vida. Se presenta de manera recurrente y se asocia o no a endometriosis intratorácica, o diafragmática, y ocasionalmente a perforaciones del mismo. Es una entidad poco frecuente, y hasta donde podemos investigar, el presente es el primer caso de esa entidad, reportado en nuestro país. Revisamos aquí las teorías etiopatogénicas, y el tratamiento de ésta condición.

ABSTRACT

The surgical excision of lung metastasis is a procedure with clear indications. Some patients not only survive longer periods, but are actually cured of their malignant disease. We present here the experience at the Hospital Mexico with the surgical treatment of metastatic pulmonary involvement. During the last 6 years 12 patients underwent resection of lung metastasis.There were 5 men and 7 females, with an average age of 42 years. A total of 15 procedures were performed because 3 patient had recurrent disease and were reoperated. A single lobectomy was done in 1 patient and from one to multiple wedge excisions were carried out in the other 14 without surgical mortality. There was only a prolonged air leak(8.3 porcent) that stopped spontaneously. This group is compared to one previously reported, convering from 1969 to 1983. Eleven patient were operated upon, their sex and distribution were just about the same as in the present study. The lesions were unilateral and were treated by lobectomy in 6 cases and wedge excision in 5. There was no mortality and the morbility was 18 porcent.In conclusion, the number of cases has more than doubled. Now we tend to be lesions, even bilaterally, and at the same time sparing more lung tissue.
