ABSTRACT

To augment information at digital subtraction angiography, 17 patients with 22 intracranial aneurysms underwent three-dimensional-display magnetic resonance (MR) angiography. Reconstructions created to simulate vascular endoscopy provided additional information in 18 aneurysms about the aneurysmal sac, its orifice and neck, and/or its relation to the parent vessel and nearby vasculature. No additional information was provided in two aneurysms, and two aneurysms were not depicted.

ABSTRACT

Hepatocellular carcinoma remains a disease with poor prognosis. Liver resection, although the optimal method of management, is associated with a high incidence of intrahepatic tumour recurrence ranging between 50-70%, 12 to 18 months following surgery. This study assesses prospectively the results of liver resection as compared to liver resection combined with pre- and post-operative locoregional chemotherapy-immunotherapy in 40 patients suffering from hepatocellular carcinoma. Patients were randomly assigned to two groups. Group A (20 patients) had liver resection only, while Group B (20 patients) had liver resection combined with pre- and past-operative targeting locoregional chemotherapy-immunotherapy. Five (5) patients died in total: two from Group A and two from Group B, during the first 30 days following surgery due to reasons related to the procedure (post-operative liver failure in three, pulmonary embolism in one). The remaining patient from Group A died 10 months following liver resection due to intrahepatic tumour recurrence. From Group A, 17 patients are alive from 3 to 26 months after surgery. Of the 17 alive patients, 10 are free of disease and 7 show intrahepatic recurrence. Thus, tumour intrahepatic recurrence occurred in 8 patients of group A. From Group B, all 18 patients are alive and free of disease from 4 to 27 months after surgery. No patient died because of the disease nor has any patient shown intrahepatic tumour recurrence. As a conclusion of the present results, liver resection combined with pre- and post-operative targeting locoregional immunotherapy-chemotherapy appears to offer substantial advantages for patients undergoing surgery because of hepatocellular carcinoma.

ABSTRACT

Our aim was to demonstrate the orifice of intracranial aneurysms by magnetic resonance angiography (MRA). The aneurysmal orifice is that part of its lumen that communicates with the parent vessel. We studied 12 patients with 17 intracranial aneurysms using three-dimensional display (3DD) MRA; 15 of the aneurysms had previously been shown by digital subtraction angiography (DSA). The overall image quality of 3DD MRA was excellent in 10 patients. The orifice was clearly demonstrated in 13 aneurysms (76%) providing unique information about its size, shape and orientation. The orifice of 2 aneurysms (12%) was not demonstrated. Two aneurysms (12%) were not detected on MRA. Although 3DD MRA has limitations, this study suggests that it can accurately define the orifice of aneurysms and could be used to provide information crucial for endovascular treatment.

ABSTRACT

This is the first report in which aspirin (ASA) has failed to inhibit aggregation of mammalian platelets. Preincubation of citrated sheep platelet rich plasma with a final concentration of 500 microM ASA for 3 minutes at 37 degrees C did not inhibit aggregation induced by either arachidonic acid (AA; 1.6 mM), ADP (2.5 microM), collagen (5.6 micrograms/ml) or thrombin (0.04U). Instead, ASA potentiated the aggregation response produced by these agents except AA. Platelet aggregation that was reversible with ADP became irreversible after adding ASA. The inhibitory properties of ASA was confirmed with human platelets, challenged with AA, ADP, adrenalin and collagen. These findings suggest that sheep platelets have an ASA resistant cyclo oxygenase and may be able to aggregate by a pathway that is independent of arachidonic acid.

ABSTRACT

1. A non-invasive tail cuff method was validated against direct intra-arterial blood pressure measurements (r2 greater than 0.9) and then used to measure the systolic blood pressure (SBP) responses to air-jet stimulation in mature conscious Sprague-Dawley (SD) and spontaneously hypertensive rats (SHR). The experiments with the SHR were conducted in parallel with age matched SD rats. 2. All rats were trained to remain in perspex holders and when their SBP had stabilized, a jet of air of 10 min duration was directed at the rat's nose via rubber tubing. The blood pressure was measured during the first, fifth and tenth minute. 3. The maximum SBP response normally occurred during the first min of air-jet stimulation and averaged +30 mmHg in the SHR and +21 mmHg in the SD. Although of higher magnitude in the SHR, when expressed as a percentage of the control SBP, the rise was similar for both strains, 13% and 14%, respectively. 4. The pressor response of three SD rats stimulated daily for up to 12 days did not show any evidence of habituation to the stimulus. 5. The pressor responses to air-jet stimulation were abolished in anaesthetized rats, suggesting that they are primarily higher centre responses.

ABSTRACT

Heparin, aspirin and dipyridamole were given alone or in combination to groups of sheep which had polyethylene catheters inserted into facial and saphenous veins. Animals were killed after 3 or 9 days. Neither aspirin nor dipyridamole reduced the amounts of thrombus on the catheters at autopsy. Heparin reduced thrombus only when sufficient was given to reduce blood coagulation.

ABSTRACT

Three experiments were performed to measure thrombus formation on plastic cannulae inserted by cut-down into veins of sheep. On polyethylene tubing, considerable thrombus was present after one day and the amount increased in the subsequent 13 days. When different sized polyethylene tubings were tested, thrombus tended to form in relation to the surface area. Smaller tubings had smaller proportions of their lengths covered with thrombus. There were differences between types of plastics with one batch of polyvinylchloride tubing being virtually non-thrombogenic.

ABSTRACT

The present investigation shows that the capacity of thyrolymphography for demonstrating the morphologic features of various thyroid lesions is not inferior to that of isotope scanning. On the contrary, lymphography is an effective procedure for detecting small nodules that remain undetected by other methods. Contrast medium is more rapidly distributed to lymph nodes in patients with diffuse goiter and also in patients pretreated with Lugol's solution. Finally, it is shown that the contrast medium employed (Lipiodol Ultra Fluid) remains in the lymph nodes for a period of several months.
