ABSTRACT

OBJECTIVE: To assess the quality of services offered by health service providers, according to the perception of consumers. METHODS: A cross-sectional study with 360 consumers from seven health service providers in the metropolitan area of Curitiba, PR, in 2008, was performed. An individual questionnaire measuring the consumers' preferences in relation to six attributes (location of service points; effectiveness of doctors, clinics and hospitals; promptness and kindness when caring for patients and family members; ease of access to the authorization forms for consultations; price; diversity of available doctors, clinics and hospitals) for each one of the analyzed health service providers was carried out. The Analytic Hierarchy Process (AHP), a multiple criteria tool for decision analysis and planning, was used to analyze the responses. RESULTS: The attribute most valued by the consumers was "price". The companies were grouped into two sets, regarding the mentioned attributes: two had lower preference (between 19% and 23%) and five, higher preference (around 10% each). CONCLUSIONS: With this type of research, health service providers could reshape their structures, processes, prices and accredited networks, in order to improve their market strategy.

OBJETIVO: Analisar a qualidade dos serviços oferecidos por empresas operadoras de planos de saúde, segundo a percepção de usuários. MÉTODOS: Estudo transversal com 360 usuários de sete operadoras de planos de saúde da cidade de Curitiba, PR, e região metropolitana em 2008. Foi aplicado questionário sobre as preferências dos usuários em relação a seis atributos (localização dos pontos de atendimento; efetividade da ação dos médicos, clínicas e hospitais; rapidez e amabilidade no atendimento; facilidade na liberação de guias; preço; abrangência da rede credenciada) de cada uma das empresas operadoras. Para a análise das respostas foi utilizado o método Analytic Hierarchy Process (AHP, ou Processo Analítico de Hierarquia), ferramenta de análise de decisão e planejamento de múltiplos critérios. RESULTADOS: O atributo mais valorizado pelos usuários foi "preço". As empresas foram agrupadas em dois conjuntos de preferências em relação aos atributos: dos sete planos de saúde, dois apresentaram menor preferência (entre 23% e 19%) e cinco, maior preferência (em torno de 10%). CONCLUSÕES: Com esse tipo de pesquisa, as empresas operadoras de planos de saúde poderiam reformular suas estruturas, processos, preços e redes credenciadas com o objetivo de melhorar seu posicionamento no mercado.

MÉTODOS: Estudio transversal con 360 usuarios de siete operadoras de seguros de salud de la ciudad de Curitiba, Sur de Brasil, y región metropolitana en 2008. Se aplicó cuestionario sobre las preferencias de los usuarios con relación a seis atributos (localización de los puntos de atención; efectividad de la acción de los médicos, clínicas y hospitales; rapidez y amabilidad en la atención, facilidad en la liberación de documentos de aprobación, precio; alcance de la red acreditada) de cada una de las empresas operadoras. Para el análisis de las respuestas fue utilizado el método Analytic Hierarchy Process (AHP, o Proceso Analítico de Jerarquía), herramienta de análisis de decisión y planificación de múltiples criterios. RESULTADOS: El atributo más valorizado por los usuarios fue "precio". Las empresas fueron agrupadas en dos conjuntos de preferencias con relación a los atributos: de los siete seguros de salud, dos presentaron menor preferencia (entre 23% y 19%) y cinco, mayor preferencia (en torno de 10%). CONCLUSIONES: Con este tipo de investigación, las empresas operadoras de seguros de salud podrían reformular sus estructuras, procesos, precios y redes acreditadas con el objetivo de mejorar su posicionamiento en el mercado.

ABSTRACT

This 12-week, double-blind study evaluated the effectiveness of risperidone (4 mg/day), quetiapine (400 mg/day), or fluphenazine (12.5 mg/day) in a stringently defined treatment-resistant population of people with schizophrenia. No differences were noted in total Brief Psychiatric Rating Scale (BPRS) or Clinical Global Impression scores among the drug groups (n = 38). More subjects tended to complete the study on risperidone (69%) or quetiapine (58%) than those treated with fluphenazine (31%; P value not significant). Eighty-nine percent of those who discontinued on fluphenazine (8 of 9) were due to lack of efficacy. Discontinuation due to adverse effects was low, with only 2 subjects (both on quetiapine) stopping due to side effects. Three of 13 risperidone-treated subjects (23%) and 3 of 12 quetiapine-treated subjects (25%) met response criteria (decrease of 20% of total BPRS score), whereas 2 of 13 subjects (15%) responded to fluphenazine. Side effect occurrence was similar among drug groups and EPS ratings on the Simpson Angus Scale improved in all drug groups (quetiapine, 1.64; risperidone, 1.30; fluphenazine, 0.69; P value not significant). Despite the newer class of second-generation antipsychotic medications, this treatment-resistant population remains difficult to treat. Many people have only minimal to modest improvements with antipsychotic treatment and most continue to have residual psychotic symptoms. Treatment with first- and second-generation antipsychotics may demonstrate similar efficacy; however, patients treated with second-generation antipsychotics may be more likely to adhere to treatment.

ABSTRACT

Como parte das atividades de treinamento de um grupo de monitores do Departamento de Medicina Social da Faculdade de Medicina de Ribeiräo Preto (USP), foram realizada uma investigaçäo sobre a adequaçäo do uso profilático de imunoglobulina, humana antitetânica (IGHAT) na Unidade de Emergência do Hospital das Clínicas da mesma Faculdade, durante o período compreendido entre junho de 1987 e setembro de 1988. Numa amostra de 74 pacientes que receberam IGHAT foram investigados o tipo de lesäo que motivou o atendimento, as condiçöes da mesma no momento do acidente e na chagada ao hospital, ocorrência de interrogatório do responsável pelo atendimento sobre antecedentes vacinais e sobre medidas de assepsia adotadas por ocasiiao do ferimento. Observou-se que pouca atençäo foi dada às variáveis que definem a necessidade de aplicaçäo profilática de IGHAT (antecedentes vacinais e tipo condiçöes da lesäo), fazendo com que aplicaçöes desnecessárias tenham ocorrido em 61,1% dos casos, com proporçöes variáveis segundo a clínica e o nível de formaçäo da pessoa responsável pelo atendimento (acadêmicos de medicina, residentes ou docentes). Os achados säo discutidos à luz de suas implicaçöes para o ensino médico e dos custos acarretados ao Serviço, tendo em vista que 2580 ampolas de IGHAT foram aplicadas no mesmo em 1988, a um custo unitário de US$3,00
