ABSTRACT

Artificial intelligence models represented in machine learning algorithms are promising tools for risk assessment used to guide clinical and other health care decisions. Machine learning algorithms, however, may house biases that propagate stereotypes, inequities, and discrimination that contribute to socioeconomic health care disparities. The biases include those related to some sociodemographic characteristics such as race, ethnicity, gender, age, insurance, and socioeconomic status from the use of erroneous electronic health record data. Additionally, there is concern that training data and algorithmic biases in large language models pose potential drawbacks. These biases affect the lives and livelihoods of a significant percentage of the population in the United States and globally. The social and economic consequences of the associated backlash cannot be underestimated. Here, we outline some of the sociodemographic, training data, and algorithmic biases that undermine sound health care risk assessment and medical decision-making that should be addressed in the health care system. We present a perspective and overview of these biases by gender, race, ethnicity, age, historically marginalized communities, algorithmic bias, biased evaluations, implicit bias, selection/sampling bias, socioeconomic status biases, biased data distributions, cultural biases and insurance status bias, conformation bias, information bias and anchoring biases and make recommendations to improve large language model training data, including de-biasing techniques such as counterfactual role-reversed sentences during knowledge distillation, fine-tuning, prefix attachment at training time, the use of toxicity classifiers, retrieval augmented generation and algorithmic modification to mitigate the biases moving forward.

ABSTRACT

Research has identified a positive relationship between physical activity and neuropsychological functioning across the lifespan. The present study further examined the relationship between physical activity, depression, anxiety, and cognitive functioning in community-dwelling older adults (ages 65-96) who completed an outpatient neuropsychological evaluation (N = 526). Psychometrically sound and validated measures were used to assess depression, anxiety, and cognitive functioning. Analyses of covariance (ANCOVA) were conducted to examine differences between individuals who reported regularly engaging in physical activity and those who did not, after adjusting for demographic variables (age, education, and gender). Results indicated that physical activity was associated with better scores on measures of depression, anxiety, and cognitive functioning. Effect sizes for total scores on all measures were large, but there was a sizeable range of effect sizes (from small to large) for various cognitive domains. Smallest effect sizes were observed for subtests measuring language skills and visuospatial abilities, whereas largest effect sizes were seen in processing speed and memory. Results suggest that engaging in physical activity may be a beneficial non-pharmacological intervention for older adults. These findings underscore the importance of integrating physical activity programs in community and healthcare settings to foster mental and cognitive health in older populations.

ABSTRACT

Theory of mind (ToM) has been argued to be a multidimensional construct, with ToM inferences depending on distinct processes across affective and cognitive ToM tasks and across first-order cognitive and second-order cognitive ToM tasks. Behavioural evidence for a multidimensional account has primarily depended on dissociations identified via analysis of variance, a statistical approach insufficient for assessing dimensionality. Instead, state-trace analysis (STA) is a more appropriate statistical technique to uncover dimensionality. The current study first applied STA to two summary datasets that had previously identified key dissociations between cognitive and affective ToM; these reanalyses did not support a multidimensional account of ToM. Next, STA was applied to a more detailed dataset to reveal whether ToM is based on multiple processes in a sample of 115 older adults aged 60-85 years (M = 68.5, SD = 5.92, 61.7% female) with higher or lower emotion perception ability. Participants made ToM judgements about different social exchanges (e.g., sarcasm or lying). STA results supported a multidimensional account of ToM across first-order cognitive, second-order cognitive, and affective ToM subdomains. These results lay a more rigorous foundation for subsequent studies to further examine the dimensionality of ToM and to apply formal modelling, progressing the field's understanding and measurement of the cognitive processes driving ToM judgements.

ABSTRACT

De Neys offers a welcome departure from the dual-process accounts that have dominated theorizing about reasoning. However, we see little justification for retaining the distinction between intuition and deliberation. Instead, reasoning can be treated as a case of multiple-cue decision making. Reasoning phenomena can then be explained by decision-making models that supply the processing details missing from De Neys's framework.

ABSTRACT

BACKGROUND: Failure of eruption of the maxillary permanent incisor teeth usually presents in the mixed dentition between the ages of 7 and 9 years. Missing and unerupted maxillary incisors can be regarded as unattractive and have a potentially negative impact on facial and dental aesthetics. The presence of a supernumerary tooth (or odontoma) is commonly responsible for failed eruption or impaction of the permanent maxillary incisors. The primary objective of this trial is to investigate the success of eruption associated with maxillary incisor teeth that have failed to erupt because of a supernumerary tooth in the anterior maxilla. METHODS: This protocol describes an interventional multicentre two-arm randomised clinical trial. Participants meeting the eligibility criteria will be randomised (unrestricted equal participant allocation [1:1]) to either space creation with an orthodontic appliance, removal of the supernumerary tooth and application of direct orthodontic traction or space creation with an orthodontic appliance, removal of the supernumerary tooth and monitoring. The primary outcome of this trial is to determine the prevalence of successfully erupted maxillary central permanent incisors at 6 months following removal of the supernumerary tooth. Secondary outcome measures include (1) the effect of initial tooth position (assessed radiographically) on time taken for the tooth to erupt, (2) time taken to align the unerupted tooth to the correct occlusal position, (3) gingival aesthetics and (4) changes in the self-reported Oral Health Related-Quality of Life (OHRQoL) (pre-and post-treatment). DISCUSSION: There is a lack of high-quality robust prospective studies comparing the effectiveness of interventions to manage this condition. Furthermore, the UK national clinical guidelines have highlighted a lack of definitive treatment protocols for the management of children who present with an unerupted maxillary incisor due to the presence of a supernumerary tooth. The results of this trial will inform future treatment guidelines for the management of this condition in young children. TRIAL REGISTRATION: ISRCTN Registry ISRCTN12709966 . ABSTRACT

Renal transplantation is the ultimate treatment for end-stage renal disease patients. However, vascular complications can impact renal allograft outcomes. Extrarenal pseudoaneurysms (EPSA) are a rare complication occurring in 1% of transplant recipients. We report a case series of extrarenal pseudoaneurysm after kidney transplant with different clinical presentations and management strategies. Given the rarity of EPSA, literature describing this complication is limited to single case reports or small retrospective case series. We also provide an up-to-date review of 76 articles on mycotic, bacterial, and idiopathic EPSAs. Allograft removal is considered standard treatment, but new endovascular alternatives may allow allograft salvage. EPSA should be managed with a multidisciplinary approach. Surveillance with renal ultrasound is recommended in patients considered high risk.

ABSTRACT

Much recent research and theorizing in the field of reasoning has been concerned with intuitive sensitivity to logical validity, such as the logic-brightness effect, in which logically valid arguments are judged to have a "brighter" typeface than invalid arguments. We propose and test a novel signal competition account of this phenomenon. Our account makes two assumptions: (a) as per the demands of the logic-brightness task, people attempt to find a perceptual signal to guide brightness judgments, but (b) when the perceptual signal is hard to discern, they instead attend to cues such as argument validity. Experiment 1 tested this account by manipulating the difficulty of the perceptual contrast. When contrast discrimination was relatively difficult, we replicated the logic-brightness effect. When the discrimination was easy, the effect was eliminated. Experiment 2 manipulated the ambiguity of the perceptual task, comparing discrimination performance when the perceptual contrast was labeled in terms of rating "brightness" or "darkness". When the less ambiguous darkness labeling was used, there was no evidence of a logic-brightness effect. In both experiments, individual sensitivity to the perceptual discrimination was negatively correlated with sensitivity to argument validity. Hierarchical latent mixture modeling revealed distinct individual strategies: responses based on perceptual cues, responses based on validity or guessing. Consistent with the signal competition account, the proportion of those responding to validity increased with perceptual discrimination difficulty or task ambiguity. ABSTRACT

Cytomegalovirus (CMV) is a double-stranded DNA virus, which infects a large portion of the adult population. In immunocompetent patients, it typically is asymptomatic or manifests as mild and self-limiting flu-like illness symptoms, whereas in immunocompromised patients, CMV can cause significant disease. Herein we report an unusual case of CMV pancreatitis in an immunocompetent 75-year-old female. Patient developed severe significant pancreatic necrosis that failed non-operative management, and ultimately underwent pancreatic necrosectomy. Later on, she developed three spontaneous gastric perforations. The first two perforations were managed operatively, but after the third perforation family decided not to undergo another operation. The CMV pancreatitis diagnosis was based on pancreatic histopathology and confirms by a prompt response to ganciclovir. Patient was promptly started on intravenous (IV) ganciclovir which resulted in clinical recovery and she remained asymptomatic more than one-year post op. This is a rare case of CMV pancreatitis with gastric perforations in an immunocompetent patient. High degree of suspicion and appropriate treatment are important for such clinical scenarios.

ABSTRACT

Dual-process theories posit that separate kinds of intuitive (Type 1) and reflective (Type 2) processes contribute to reasoning. Under this view, inductive judgments are more heavily influenced by Type 1 processing, and deductive judgments are more strongly influenced by Type 2 processing. Alternatively, single-process theories propose that both types of judgments are based on a common form of assessment. The competing accounts were respectively instantiated as two-dimensional and one-dimensional signal detection models, and their predictions were tested against specifically targeted novel data using signed difference analysis. In two experiments, participants evaluated valid and invalid arguments, under induction or deduction instructions. Arguments varied in believability and type of conditional argument structure. Additionally, we used logic training to strengthen Type 2 processing in deduction (Experiments 1 & 2) and belief training to strengthen Type 1 processing in induction (Experiment 2). The logic training successfully improved validity-discrimination, and differential effects on induction and deduction judgments were evident in Experiment 2. While such effects are consistent with popular dual-process accounts, crucially, a one-dimensional model successfully accounted for the results. We also demonstrate that the one-dimensional model is psychologically interpretable, with the model parameters varying sensibly across conditions. We argue that single-process accounts have been prematurely discounted, and formal modeling approaches are important for theoretical progress in the reasoning field.

ABSTRACT

Four experiments examined the claims that people can intuitively assess the logical validity of arguments, and that qualitatively different reasoning processes drive intuitive and explicit validity assessments. In each study participants evaluated arguments varying in validity and believability using either deductive criteria (logic task) or via an intuitive, affective response (liking task). Experiment 1 found that people are sensitive to argument validity on both tasks, with valid arguments receiving higher liking as well as higher deductive ratings than invalid arguments. However, the claim that this effect is driven by logical intuitions was challenged by the finding that sensitivity to validity in both liking and logic tasks was affected in similar ways by manipulations of concurrent memory load (Experiments 1 and 2) and variations in individual working memory capacity (Experiments 3 and 4). In both tasks better discrimination between valid and invalid arguments was found when more working memory resources were available. Formal signal detection models of reasoning were tested against the experimental data using signed difference analysis (Stephens, Dunn, & Hayes, 2018b). A single-process reasoning model which assumes that argument evaluation in both logic and liking tasks involves a single latent dimension for assessing argument strength but different response criteria for each task, was found to be consistent with the data from each experiment (as were some dual-process models). The experimental and modeling results confirm that people are sensitive to argument validity in both explicit logic and affect rating tasks, but that these results can be explained by a single underlying reasoning process. ABSTRACT

A key phenomenon in inductive reasoning is the diversity effect, whereby a novel property is more likely to be generalized when it is shared by an evidence sample composed of diverse instances than a sample composed of similar instances. We outline a Bayesian model and an experimental study that show that the diversity effect depends on the assumption that samples of evidence were selected by a helpful agent (strong sampling). Inductive arguments with premises containing either diverse or nondiverse evidence samples were presented under different sampling conditions, where instructions and filler items indicated that the samples were selected intentionally (strong sampling) or randomly (weak sampling). A robust diversity effect was found under strong sampling, but was attenuated under weak sampling. As predicted by our Bayesian model, the largest effect of sampling was on arguments with nondiverse evidence, where strong sampling led to more restricted generalization than weak sampling. These results show that the characteristics of evidence that are deemed relevant to an inductive reasoning problem depend on beliefs about how the evidence was generated.

ABSTRACT

We present a state-trace analysis of sentence ratings elicited by asking participants to evaluate the overall acceptability of a sentence and those elicited by asking participants to focus on structural well-formedness only. Appealing to literature on "grammatical illusion" sentences, we anticipated that a simple instruction manipulation might prompt people to apply qualitatively different kinds of judgment in the two conditions. Although differences consistent with the subjective experience of grammatical illusion dissociations were observed, the state trace analysis of the rating data indicates that responses were still consistent with both judgment types accessing a single underlying factor. These results add to the existing comparisons between analytic and probabilistic modeling approaches to predicting rating judgments.

ABSTRACT

When asked to determine whether a syllogistic argument is deductively valid, people are influenced by their prior beliefs about the believability of the conclusion. Recently, two competing explanations for this belief bias effect have been proposed, each based on signal detection theory (SDT). Under a response bias explanation, people set more lenient decision criteria for believable than for unbelievable arguments. Under the alternative argument strength explanation, believability affects the reasoning stage of processing an argument, with believable and unbelievable arguments differing in subjective strength for both valid and invalid items. Two experiments tested these accounts by asking participants to make validity judgments for categorical syllogisms and to rate their confidence. Conclusion-believability was manipulated both within group (Experiment 1) and between groups (Experiment 2). A novel two-step version of the signal detection model was fit to receiver operating characteristic (ROC) curves for believable and unbelievable arguments. Model fits confirmed that in both experiments there was a shift in decision criterion but not argument discriminability as a function of argument believability. Crucially, when believability is manipulated between groups, this shift is expected under the response bias account but not under the argument strength account. Therefore, the results support the view that belief bias primarily reflects changes in response bias: people require less evidence to endorse a syllogism as valid when it has a believable conclusion. Delayed feedback during categorization training has been hypothesized to differentially affect 2 systems that underlie learning for rule-based (RB) or information-integration (II) structures. We tested an alternative possibility: that II learning requires more precise item representations than RB learning, and so is harmed more by a delay interval filled with a confusable mask. Experiments 1 and 2 examined the effect of feedback delay on memory for RB and II exemplars, both without and with concurrent categorization training. Without the training, II items were indeed more difficult to recognize than RB items, but there was no detectable effect of delay on item memory. In contrast, with concurrent categorization training, there were effects of both category structure and delayed feedback on item memory, which were related to corresponding changes in category learning. However, we did not observe the critical selective impact of delay on II classification performance that has been shown previously. Our own results were also confirmed in a follow-up study (Experiment 3) involving only categorization training. The selective influence of feedback delay on II learning appears to be contingent on the relative size of subgroups of high-performing participants, and in fact does not support that RB and II category learning are qualitatively different. We conclude that a key part of successfully solving perceptual categorization problems is developing more precise item representations, which can be impaired by delayed feedback during training. More important, the evidence for multiple systems of category learning is even weaker than previously proposed. (PsycINFO Database Record

Three-experiments examined the number of qualitatively different processing dimensions needed to account for inductive and deductive reasoning. In each study, participants were presented with arguments that varied in logical validity and consistency with background knowledge (believability), and evaluated them according to deductive criteria (whether the conclusion was necessarily true given the premises) or inductive criteria (whether the conclusion was plausible given the premises). We examined factors including working memory load (Experiments 1 and 2), individual working memory capacity (Experiments 1 and 2), and decision time (Experiment 3), which according to dual-processing theories, modulate the contribution of heuristic and analytic processes to reasoning. A number of empirical dissociations were found. Argument validity affected deduction more than induction. Argument believability affected induction more than deduction. Lower working memory capacity reduced sensitivity to argument validity and increased sensitivity to argument believability, especially under induction instructions. Reduced decision time led to decreased sensitivity to argument validity. State-trace analyses of each experiment, however, found that only a single underlying dimension was required to explain patterns of inductive and deductive judgments. These results show that the dissociations, which have traditionally been seen as supporting dual-processing models of reasoning, are consistent with a single-process model that assumes a common evidentiary scale for induction and deduction. Single-process accounts of reasoning propose that the same cognitive mechanisms underlie inductive and deductive inferences. In contrast, dual-process accounts propose that these inferences depend upon 2 qualitatively different mechanisms. To distinguish between these accounts, we derived a set of single-process and dual-process models based on an overarching signal detection framework. We then used signed difference analysis to test each model against data from an argument evaluation task, in which induction and deduction judgments are elicited for sets of valid and invalid arguments. Three data sets were analyzed: data from Singmann and Klauer (2011), a database of argument evaluation studies, and the results of an experiment designed to test model predictions. Of the large set of testable models, we found that almost all could be rejected, including all 2-dimensional models. The only testable model able to account for all 3 data sets was a model with 1 dimension of argument strength and independent decision criteria for induction and deduction judgments. We conclude that despite the popularity of dual-process accounts, current results from the argument evaluation task are best explained by a single-process account that incorporates separate decision thresholds for inductive and deductive inferences. Unfamiliar, one-to-one face matching has been shown to be error-prone. However, it is unknown whether there is a strong relationship between confidence and accuracy in this task. If there is, then confidence could be used as an indicator of accuracy in real-world face matching settings such as border security, where the objectively correct decision is typically unknown. Two experiments examined the overall confidence-accuracy relationship, as well as the relationship for positive (match) and negative (mismatch) decisions. Furthermore, they tested whether these relationships were affected by factors relevant to applied face matching settings: the proportion of mismatching trials (PMT), and the task orientation of the decision-maker (look for matches, or look for mismatches). Both calibration analyses and signal detection methods were applied to assess performance. The results showed that confidence can have a high correspondence with accuracy overall, regardless of task orientation but with small effects of PMT. Thus, confidence is promising as an indicator of accuracy in face matching. However, PMT systematically produces large detrimental effects on the confidence-accuracy relationships for positive and negative decisions, when considered separately. Signal detection measures help with understanding these effects and proposing future research directions for improving the relationships. (PsycINFO Database Record

Making accurate predictions about events is an important but difficult task. Recent work suggests that people are adept at this task, making predictions that reflect surprisingly accurate knowledge of the distributions of real quantities. Across three experiments, we used an iterated learning procedure to explore the basis of this knowledge: to what extent is domain experience critical to accurate predictions and how accurate are people when faced with unfamiliar domains? In Experiment 1, two groups of participants, one resident in Australia, the other in China, predicted the values of quantities familiar to both (movie run-times), unfamiliar to both (the lengths of Pharaoh reigns), and familiar to one but unfamiliar to the other (cake baking durations and the lengths of Beijing bus routes). While predictions from both groups were reasonably accurate overall, predictions were inaccurate in the selectively unfamiliar domains and, surprisingly, predictions by the China-resident group were also inaccurate for a highly familiar domain: local bus route lengths. Focusing on bus routes, two follow-up experiments with Australia-resident groups clarified the knowledge and strategies that people draw upon, plus important determinants of accurate predictions. For unfamiliar domains, people appear to rely on extrapolating from (not simply directly applying) related knowledge. However, we show that people's predictions are subject to two sources of error: in the estimation of quantities in a familiar domain and extension to plausible values in an unfamiliar domain. We propose that the key to successful predictions is not simply domain experience itself, but explicit experience of relevant quantities.

ABSTRACT

We exploit the morphological stereotypy and relative simplicity of the Drosophila nervous system to model the diverse neuronal morphologies of individual motor neurons and understand underlying principles of synaptic connectivity in a motor circuit. In our analysis, we use images depicting single neurons labeled with green fluorescent protein (GFP) and serially imaged with laser scanning confocal microscopy. We model morphology with a novel formulation of Conditional Random Fields, a hierarchical latent-state CRF, to capture the highly varying compartment-based structure of the neurons (soma-axon-dendrites). In the training phase, we follow two approaches: (i) hierarchical learning, where compartment labels are given, and (ii) latent-state learning, where compartment labels are not given in the samples. We demonstrate the accuracy of our approach using wild-type motor neurons in the larval ventral nerve cord. However, our method can also be used for the identification of motor neuron mutations, as well as the automated annotation of the motor circuitry in wild type and mutant animals. Our method is directly applicable to the recognition of compartment-defined structures.

ABSTRACT

INTRODUCTION: Health-information-seeking behavior is the purposive search for health-related information. The literature contains few articles examining this behavior as it relates to orthodontics; therefore, the aim of this study was to investigate how adolescent orthodontic patients seek information about their treatment, why they search for this information, and how they prefer the information to be available. METHODS: In-depth interviews were conducted with 15 subjects, 10 to 16 years old, who were considering orthodontic treatment. The purpose of the interviews was to identify themes that were then used to develop a questionnaire to investigate information-seeking behavior. The questionnaire was distributed to 50 adolescent orthodontic patients. RESULTS: The main sources of information were talking to a dentist or an orthodontist (84%), talking to peers (66%), and reading information leaflets (64%). Surprisingly few respondents had used the Internet (8%). The preferred sources of information were talking to an orthodontist (26%) and family members (12%), followed by talking with the general dentist (10%), viewing a DVD (10%), or reading information leaflets (10%). CONCLUSIONS: The preferred method of obtaining information was verbal, followed by audiovisual and written. Audiovisual information could be beneficial for those who find reading difficult or who struggle to retain verbal information. Few patients accessed information via the Internet, and this was attributed to concerns about the reliability of the information.
