ABSTRACT

El tutor constituye el eje fundamental de la formación de los estudiantes de las Ciencias Médicas este no es cualquier trabajador o especialista; debe ser un experto en su área de competencia laboral, además de un educador y un gran comunicador capaz de motivar y de ejercer una influencia positiva sobre los educandos. En las esferas cognitivas como afectivas con su dedicación y formación dependen en gran parte de la adquisición de los conocimientos, habilidades y responsabilidades necesarios para ejercer eficazmente el rol que les corresponde y el grado de satisfacción de los estudiantes dentro del proceso educativo. Se plantea que la enseñanza tutelar constituye un pilar esencial en la formación del médico, del enfermero y el tecnólogo, pues le permite al educando lograr un mayor grado de independencia, creatividad y sentido de responsabilidad (AU).


The tutor is a main factor in training Medical Sciences students; he is not any worker or specialist: he should be an expert in his area of occupational competence, besides being an educator and a great communicator able of motivating and positively influencing the students in. Their dedication and training mostly depend on the acquisition of knowledge, skills and abilities to efficaciously play the role they have to play, and to reach the students´ satisfaction level in the teaching process. It is assumed that tutored teaching is an essential pillar in training physicians, nurses and technologists, because it allows the teacher to achieve a higher level of independence, creativity and sense of responsibility (AU).

ABSTRACT

OBJECTIVES: We report a rare case of CD4- cutaneous blastic plasmacytoid dendritic cell neoplasm (BPDCN) with a novel PBRM1 mutation. METHODS: An 11-year-old girl presented with an enlarged mass on her left arm and underwent an incisional biopsy. RESULTS: Histopathologic examination and immunohistochemistry studies showed a monotonous proliferation of blasts that were CD4-, CD56+, and CD123+. There was no evidence of leukemic dissemination. Next-generation sequencing detected PBRM1 and CIC gene abnormalities. We confirmed and validated a novel PBRM1 mutation by conventional polymerase chain reaction and Sanger sequencing. CONCLUSIONS: CD4- variant of BPDCN may be mistaken for myeloid sarcoma or extramedullary lymphoblastic leukemia/lymphoma because of their overlapping morphologic and immunophenotypic features; thus, a careful clinicopathologic evaluation is essential to reach the correct diagnosis. PBRM1 mutation seems to be a driver event in this case. Our study underscores the importance of alterations in chromatin remodeling in the pathogenesis of BPDCN.

ABSTRACT

El objetivo de este estudio fue diseñar y elaborar un Simulador de Movimientos Mandibulares (SMM) que pudiera reproducir enforma precisa los movimientos y fuerzas mandibulares de la Articulación Temporo-Mandibular (ATM) y de la mandíbula. Uncráneo humano de características antropométricas normales se duplicó en fibra de vidrio, con arcos dentales metálicos colados(Aleación de Ni-Cr), con cavidades oclusales previamente preparadas en premolares y molares. La simulación del ligamentoperiodontal se hizo con polivinilsiloxano liviano y la del disco articular con silicona. Se replicaron los movimientos de la ATM yla mandíbula con servomotores de tecnología de robótica: Apertura (45mm), cierre, lateralidad (5mm) y protrusiva (5mm), confuerzas masticatorias promedio de 200 Newtons. Se desarrolló un programa computarizado que simula los movimientosmandibulares en 12 ciclos diferentes, cada ciclo se repitió aproximadamente 4,000 veces durante las pruebas.Este estudio desarrolló una herramienta de investigación de características más reales para realizar estudios de desgaste demateriales dentales en el laboratorio.


The purpose of this study was to design and fabricate a mandibular movement simulator (MMS) that can closely reproduce themovements of the human temporomandibular joint and (TMJ) and the mandible. A human skull was duplicated in fiberglass withcast metallic teeth (Ni-Cr alloy) with occlusal cavities in premolars and molars. Light polyvinyl siloxane was used to simulate theperiodontal ligament and Medigel-Z® to simulate the TMJ disc. The TMJ and mandibular movements were simulated withrobotic technology and servomotors: Opening (45mm), closure, lateral (5 mm) and protrusive (5 mm) with an average chewingforces of 200 Newtons. Computer software was developed to simulate mandibular movements in 12 different cycles; each cyclewas typically repeated more than 4,000 times during testing. This study developed a research tool with more realistic features toperform wear test in the laboratory.
