ABSTRACT

BACKGROUND: Assess the association between digoxin use and gastrointestinal bleeding (GIB) in a multicenter continuous flow left ventricular assist device (LVAD) cohort. METHODS: Patients implanted with continuous flow LVADs with data on GIB and digoxin use from two centers were included in the analysis (n = 649). GIB events were captured up to 2 years of follow-up. Digoxin use was defined as digoxin prescribed at discharge or within the first 3 months after LVAD implantation. A negative binomial regression model was performed to determine the association between digoxin use and number of GIB events over the follow-up period. RESULTS: Mean age of the cohort was 57 years (±14) and 45% (293/649) were bridge to transplant (BTT). Digoxin was prescribed in 33% of patients. Digoxin use was associated with an unadjusted 32% reduction in the incidence of rate of all cause GIB (IRR 0.68, 95% CI 0.46-0.99, p = 0.049). After adjusting for age, sex, Interagency Registry for Mechanically Assisted Circulatory Support (INTERMACS) profile, renal function, and implanting center there was still a 34% reduction in the incidence rate (IRR 0.67, 95% CI 0.45-0.99, p = 0.048). When limiting the analysis to those with likely arteriovenous malformation associated GIB, the association strengthened (unadjusted: IRR 0.48, 95 % CI 0.26-0.89, p = 0.02, adjusted: IRR 0.47, 95 % CI 0.25-0.9, p = 0.022). CONCLUSIONS: In this multicenter study, inclusive of contemporary devices, digoxin use was associated with reduced GIB events. Prospective data will be required to confirm this association.

ABSTRACT

The European common lizard (Zootoca vivipara) is a widely distributed species across Europe and Asia exhibiting two reproductive modes (oviparity/viviparity), six major lineages and several sublineages. It has been used to tackle a large variety of research questions, nevertheless, few nuclear DNA sequence markers have been developed for this species. Here we developed 79 new nuclear DNA sequence markers using a clonation protocol. These markers were amplified in several oviparous and viviparous specimens including samples of all extant clades, to test the amplification success and their diversity. 49.4% of the markers were polymorphic and of those, 51.3% amplified in all and 94.9% amplified in 5-7 of the extant Z. vivipara clades. These new markers will be very useful for the study of the population structure, population dynamics, and micro/macro evolution of Z. vivipara. Cross-species amplification in four lizard species (Psammodromus edwardsianus, Podarcis muralis, Lacerta bilineata, and Takydromus sexlineatus) was positive in several of the markers, and six makers amplified in all five species. The large genetic distance between P. edwardsianus and Z. vivipara further suggests that these markers may as well be employed in many other species.

ABSTRACT

Dense (>98 th%) and homogeneous ceramic/metal composites were obtained by spark plasma sintering (SPS) using ZrO2 and lamellar metallic powders of tantalum or niobium (20 vol.%) as starting materials. The present study has demonstrated the unique and unpredicted simultaneous enhancement in toughness and strength with very high flaw tolerance of zirconia/Ta composites. In addition to their excellent static mechanical properties, these composites also have exceptional resistance to fatigue loading. It has been shown that the major contributions to toughening are the resulting crack bridging and plastic deformation of the metallic particles, together with crack deflection and interfacial debonding, which is compatible with the coexistence in the composite of both, strong and weak ceramic/metal interfaces, in agreement with predictions of ab-initio calculations. Therefore, these materials are promising candidates for designing damage tolerance components for aerospace industry, cutting and drilling tools, biomedical implants, among many others.

ABSTRACT

AIMS: To evaluate the phytotoxic, antifungal and antioomycete activity; and, determine the chemical composition of the volatile organic compounds (VOCs) and semi-volatile metabolites produced by the endophyte Hypoxylon anthochroum strain Blaci isolated from Bursera lancifolia. METHODS AND RESULTS: Based on its macro- and micro-morphological features, the strain Blaci was identified as Nodulisporium sp.; partial analysis of its ITS1-5.8-ITS2 ribosomal gene sequence revealed the identity of the teleomorphic stage of the fungus as H. anthochroum. Phytotoxic and antimicrobial activities of VOCs, and culture medium and mycelium organic extracts from H. anthochroum Blaci were determined by simple and multiple antagonism bioassays, and gas phase and agar dilution bioassays respectively. The volatile and semi-volatile metabolites were identified by gas chromatography-mass spectrometry. VOCs from a 5-day H. anthochroum strain Blaci culture caused the inhibition of seed germination, root elongation and seedling respiration on Amaranthus hypochondriacus, Panicum miliaceum, Trifolium pratense and Medicago sativa. In addition, extracts, phenylethyl alcohol and eucalyptol main compounds present in the VOCs and extract displayed a high phytotoxic activity, inhibiting the three physiological processes on the four test plants in a concentration-dependent manner. CONCLUSIONS: The results revealed that H. anthochroum strain Blaci produces a mixture of VOCs. These VOCs showed a strong phytotoxic activity on seed germination, root elongation, and seedling respiration of four plants and slightly affected the growth of phytopathogenic fungi and oomycetes. Also, the culture medium and mycelium extracts of H. anthochroum showed a high phytotoxic activity on the four test plants and, generally, the culture medium extract was more phytotoxic than the mycelium extracts. SIGNIFICANCE AND IMPACT OF THE STUDY: This work firstly reports the phytotoxic activity of volatile and semi-volatile compounds produced by the endophyte H. anthochroum strain Blaci on seed germination, root elongation, and seedling respiration of four different plants; consequently, these compounds could be useful in biocontrol of weeds and plant pathogens.

ABSTRACT

Dry eye and meibomian gland dysfunction are common ocular surface disorders. Discrimination of both conditions often may be difficult given the overlapping of signs and symptoms, and the lack of correlation with clinical parameters. A total of 144 individuals were included in this study. To search for proteome differences, tear proteins were collected by Merocel sponge and analyzed using 2D-PAGE. Comparative tear protein profile analysis indicated changes in the expression levels of fifteen proteins. Subsequent to MALDI-TOF/TOF protein identification, network analysis revealed expression/interaction connections with other proteins, thereby identifying additional putative markers. A screening validation assay demonstrated the discriminative power of six candidate biomarkers. A further validation study using multiplexed-like ELISA assays in tear samples collected with both sponge and capillary confirmed the high discriminatory power of five biomarkers: S100A6, annexin A1 (ANXA1), annexin A11 (ANXA11), cystatin-S (CST4), and phospholipase A2-activating protein (PLAA) with an area under ROC curve (AUC)≥ 97.9% (sensitivity ≥ 94.3%; specificity ≥ 97.6%) when comparing dry eye and control individuals. This panel also discriminated between dry eye, meibomian gland dysfunction and control individuals, with a global correct assignment (CA) of 73.2% between all groups. Correct assignment was not found to be significantly dependent on the tear collection method.

ABSTRACT

AIM: The aim of this paper was to report clinical, functional and radiological results of 80 patients surgically treated with a combined mini-open and percutaneous surgical repair as proposed by Kakiuchi. METHODS: All patients were evaluated with a physical examination, evaluation scales, a functional test (Ergo-jump Bosco System), and an ultrasonographic exam along with Power Doppler Ultrasonography (PDU) (S/S). RESULTS: At a mean follow-up of 58 months no cases of rerupture were detected. VISA-A evaluation scale showed an excellent score in 63 patients (78.75%), a good score in 14 patients (17.5%), a fair score in two patients (2.5%), and a poor score in one patient (1.25%). Hannover scale showed an excellent score in 63 patients (78.75%), and a good score in 17 patients (21.25%). Ergo-Jump evaluation showed a 2.07% mean deficit of the affected limb at the Squatting Jump test, a 3.26% mean deficit at the Counter Movement Jump test, and a 0.0062% mean improvement at the Repetitive Jump test. Ultrasonographic exam showed in all cases a satisfactory recovery of the integrity of the operated tendon. The mean AP and LL widths showed a significant increase of 7.13±2.97 mm (+56.1%) and of 4.01±2.36 mm (+43.81%) respectively. According to the modified Öhberg score scale, PDU exam showed a grade +1 in 16 patients (20%) and a grade +2 in seven cases (8.7%). CONCLUSION: The absence of rerupture cases, the satisfactory functional and ultrasonographic results of the patients included in this study cause us to consider this technique as reliable and effective even in young high-demand patients.

ABSTRACT

PURPOSE: To evaluate the concentration of allergic mediators in tears of children with seasonal allergic conjunctivitis (SAC) and perennial allergic conjunctivitis (PAC) compared with controls. METHODS: Twenty children with allergic conjunctivitis (17 SAC, and 3 PAC) and sixteen healthy children were included in this study. Tear samples were collected using a Merocel sponge (Oasis, 0525), and immediately eluted by incubation in elution buffer and subsequent centrifugation at 20,000 rpm for 30 min at 4°C. Concentrations of histamine (HIS), tryptase (TPS), eosinophil chemotactic factor (ECF), major basic protein (MBP), eosinophil cationic protein (ECP), eosinophil-derived neurotoxin (EDN), IgE and E-selectin were measured using enzyme linked immunosorbent assays (ELISA). Data were compared with the Mann-Whitney U test (P<.05), and multivariate analyses were also performed. RESULTS: Tear levels of TPS (P=.014), MBP (P=.032), ECP (P=.0041), IgE (P=.014) and EDN (P=.00077) showed significant differences in children with SAC and PAC compared to controls. CONCLUSION: The simultaneous analyses of allergic mediators in the tears of children with SAC and PAC showed a significant elevated concentration in EDN, ECP and MBP in allergic group and decreased levels in IgE and TPS. Statistical analyses showed a diagnostic accuracy of 94.4% using the eight molecules panel.

ObjetivoEvaluar la concentración de diferentes moléculas mediadoras de alergia en lágrima de niños con conjuntivitis alérgica estacional (CAE) y conjuntivitis alérgica perenne (CAP) frente a controles.MétodosSe incluyeron 20 niños con conjuntivitis alérgica (17 CAE y 3 CAP), y 16 niños sanos utilizados como controles. Las muestras de lágrima se obtuvieron mediante una hemosteta de acetato de polivinilo (OASIS, 0525) colocándola en el canto externo. Se eluyó la lágrima de la hemosteta utilizando un tampón y centrifugación a 20.000rpm durante 30 minutos a 4°C. Una vez cuantificadas las muestras se determinaron las concentraciones de histamina (HIS), triptasa (TPS), factor quimiotáctico del eosinófilo (ECF), proteína básica (MBP), proteína catiónica del eosinófilo (ECP), neurotoxina (EDN), IgE y E-selectina mediante la técnica ELISA (enzyme-linked immunosorbent assay). Los resultados se compararon con el test no paramétrico U de Mann-Whitney (p<0,05) y se realizaron análisis multivariantes mediante regresión logística.ResultadosPresentaron diferencias estadísticamente significativas la TPS (p=0.014), MBP (p=0,032), ECP (p=0,0041), IgE (p=0,014) y EDN (p=0,00077), cuando se comparó la lágrima de niños con CAE y CAP frente a la de niños control.ConclusionesEl análisis simultáneo de mediadores alérgicos en la lágrima de niños con CAE y CAP indica que existe una alteración en la lágrima referida principalmente a un aumento significativo de la concentración de EDN, ECP, MBP, y disminución de IgE y TPS. Los análisis estadísticos revelan una exactitud diagnóstica del 94,4% usando el panel de las 8 moléculas analizadas(AU)

ABSTRACT

Programmed cell death is a crucial process in neural development that affects mature neurons and glial cells, as well as proliferating precursors and recently born neurons at earlier stages. However, the regulation of the early phase of neural cell death and its function remain relatively poorly understood. In mouse models defective in homologous recombination or nonhomologous end-joining (NHEJ), which are both DNA double-strand break (DSB) repair pathways, there is massive cell death during neural development, even leading to embryonic lethality. These observations suggest that natural DSBs occur frequently in the developing nervous system. In this study, we have found that several components of DSB repair pathways are activated in the developing mouse retina at stages that coincide with the onset of neurogenesis. In short-term organotypic retinal cultures, we confirmed that the repair pathways can be modulated pharmacologically. Indeed, inhibiting the DNA-dependent protein kinase (DNA-PK) catalytic subunit, which is involved in NHEJ, with NU7026 increased caspase-dependent cell death and selectively reduced the neuron population. This observation concurs with an increase in the number of apoptotic neurons found after NU7026 treatment, as also observed in the embryonic scid mouse retina, a mutant that lacks DNA-PK catalytic subunit activity. Therefore, our results implicate the generation of DSB and DNA-PK-mediated repair in neurogenesis in the developing retina.

ABSTRACT

La fractura de calcáneo es la más frecuente del tarso; producidas por caída vertical sobre el talón, generalmente intraarticulares 6,3, asociadas a fracturas de columna y lesiones viscerales 1,2,3,6,7. El examen clínico debe complementarse con radiografías. El tratamiento puede ser cerrado, reducción incruenta con o sin fijación y reducción abierta y fijación interna. Las fracturas intraarticulares producen incapacidad. Se presenta paciente masculino de 42 años, posterior a caída de 3 metros de altura, de pie, presenta factura polifragmentaria de ambos calcáneos y de astrágalo derecho. Se realizó reducción incruenta (Método Essex-Lopresti). Obteniéndose consolidación e integridad sin secuelas postraumáticas.

ABSTRACT

BACKGROUND: To compare capecitabine/cisplatin with 5-fluorouracil/cisplatin as first-line treatment for advanced gastric cancer (AGC). PATIENTS AND METHODS: In this randomised, open-label, phase III study, patients received cisplatin (80 mg/m(2) i.v. day 1) plus oral capecitabine (1000 mg/m(2) b.i.d., days 1-14) (XP) or 5-FU (800 mg/m(2)/day by continuous infusion, days 1-5) (FP) every 3 weeks. The primary end point was to confirm noninferiority of XP versus FP for progression-free survival (PFS). RESULTS: A total of 316 patients were randomised to XP (n = 160) or FP (n = 156). In the per-protocol population, median PFS for XP (n = 139) versus FP (n = 137) was 5.6 versus 5.0 months. The primary end point was met with an unadjusted hazard ratio (HR) of 0.81 [95% confidence interval (CI) 0.63-1.04, P < 0.001 versus noninferiority margin of 1.25]. Median overall survival was 10.5 versus 9.3 months for XP versus FP (unadjusted HR = 0.85, 95% CI 0.64-1.13, P = 0.008 versus noninferiority margin of 1.25). The most common treatment-related grade 3/4 adverse events in XP versus FP patients were as follows: neutropenia (16% versus 19%), vomiting (7% versus 8%), and stomatitis (2% versus 6%). CONCLUSIONS: XP showed significant noninferiority for PFS versus FP in the first-line treatment of AGC. XP can be considered an effective alternative to FP.

ABSTRACT

Los tejidos blandos comprenden los músculos, tendones, la grasa, el tejido fibroso, el tejido sinovial, los vasos y los nervios. Alrededor del 60 por ciento de los sarcomas de tejidos blandos se originan en los miembros, siendo 3 veces más frecuentes en los miembros inferiores que en los superiores. En 30 por ciento de los casos se localiza en el tronco y en 40 por ciento son retroperitoneales. En el resto del 10 por ciento se trata de tumores de cabeza y cuello. La entidad denominada fibrohistiocitoma comprenden un gran número de tumores calcificados anteriormente como fibrosarcomas o como variedades polimorfas de otras sarcomas, y se caracteriza por una mezcla de células fusiformes (fibrosas) y células redondas (histiocitarias) dispuestas en un patrón estoriforme, junto con abundantes células gigantes y zonas de polimorfismo. La importancia de los estudios de inmunohistoquímica en los tumores de partes blandas radica en la necesidad que tiene el patólogo de precisar el diagnóstico histopatológico de lesiones benignas y malignas pseudosarcomatosas o sarcomatosas y poder diferenciarlas de neoplasias de otro origen. Se trata de paciente masculino de 65 años quien consulta por presentar desde agosto del 2006, caracterizado por lesiones ulcerosa en cuero cabelludo sangrantes quién se le tomo una primera biopsia que reporta. La nueva Biopsia tomada en enero en este centro reporta Carcinoma Epidermoide moderadamente a poco diferenciado en región fronto parietal de cuero cabelludo recibe 1 ciclo de quimioterapia, se toma una biopsia nueva reportando su inmunohistoquímica Fibrohistiocitoma Maligno (fibroxantosarcoma)/Tumor de Cuero Cabelludo, en vista de resultado se planifica nueva quimioterapia, se observo mejoría satisfactoria en la lesión en cuero cabelludo, por lo que decide oncología y el servicio de Medicina Interna dar de alta y seguir sus quimioterapias por consulta externa.

ABSTRACT

Paciente femenina de 57 años portadora de Telagiectasia hemorragia hereditaria con antecedentes de padre y dos hermanos portadores de igual patología. Enfermedad genética con transmisión dominante caracterizada por telangiectasias (malformaciones vasculares pequeñas) en la piel y en el revestimiento de las mucosas, epistaxis y malformaciones arteriovenosas en varios órganos internos incluyendo cerebro y pulmones. En 1909, Hanes le da el nombre de HHT. En 1999 se definió los criterios diagnósticos: epistaxis, Telangiectasias, lesiones viscerales, historia familiar. Se han descrito cuatro tipos: HHT1, HHT2, HHT3, poliposis juvenil y HHT4. El TGFâ-1 ha sido más comúmente en su fisiopatología.

ABSTRACT

Se determinaron niveles de hemoglobina glicosilada (HbA1c) como parámetro de control metabólico en diabéticos hospitalizados o en consulta en el Hospital del Seguro Social y medio privado, como parte de estudio multicentrico de OMS para conocer este parámetro en los venezolanos. Se analizó el número de diabéticos atendidos, los niveles de HbA1c, sexo, tipo de tratamiento e índice de masa corporal (IMC). Se atendieron 124 diabeticos, 48 hombres (38,70 por ciento) y 76 mujeres (61,29 por ciento), 7 eran diabéticos tipo 1 y 117 diabéticos tipo 2, encontrándose niveles promedio HbA1c 8,5 por ciento, IMC 29,7. Se concluye que el análisis de HbA1c permite evaluar el control metabólico de los pacientes diabéticos, evidenciándose mal control en el grupo estudiado. Se hace necesario ajustes en estrategias de control en estos pacientes.

ABSTRACT

Isolated tungsten nanoparticles (α-W and ß-W phase) were synthesized and epitaxially grown on alumina and spinel particle surfaces with an average tungsten size of ≤20 nm for a low tungsten content (of ≤1.5 vol%). Using tungsten (VI) ethoxide alcoholic solutions, tungsten trioxide hydrated precursors were attached to a ceramic grains surface as a nanoparticle coating. High-resolution transmission electron microscopy (HRTEM) micrographs showed epitaxial interfaces between alumina, spinel and metallic tungsten. This epitaxial growth is assumed to be due to the effect of water vapour on the sublimation of ortho-tungstic acid during the reduction process in a hydrogen atmosphere. The planes involved in the epitaxy were found to be [Formula: see text] and [Formula: see text].

ABSTRACT

En este trabajo se presenta una revisión de 5 años (2001-2006) de pacientes en área pediátrica con el diagnóstico de apendicitis aguda ingresados en el servicio de cirugía pediátrica del Hospital Dr. Patrocinio Peñuela Ruíz del Estado Táchira para determinar las complicaciones que se presentaron en los casos estudiados. Para tal fin se diseño un estudio retrospectivo el cual reporto un total de 143 pacientes que corresponde al total de pacientes ingresados de los cuales 6,2 por ciento presentaron alguna complicación y las mas frecuente fueron las de tipo infeccioso en herida operatoria.

ABSTRACT

El feto acardio constituye una entidad rara, de etiología incierta, que complica las gestaciones múltiples y especialmente las gestaciones gemelares univitelinas. Tiene una incidencia de 1 de cada 35.000 embarazos, constituye el 1 por ciento de las gestaciones gemelares monocigóticas. Se presenta el caso de femenina de 19 años, I gesta, con embarazo simple clínicamente a término, referida con diagnóstico ecográfico de malformación fetal (tumoración bilobulada) en miembro inferior y oligoamnios. Se le realiza cesárea segmentarea, obteniéndose Recién Nacido vivo femenino, con apgar de 7 puntos al 1' y 9 puntos a los 5' con peso al nacer de 2.800 grs y talla al nacer de 49 cms, sin evidencia de lesión en miembro inferior izquierdo, se encontró tumoración sólida amorfa de 10 por 10 cm en útero, la cual se envió para estudio anatomopatológico. La paciente es dada de alta por buena evolución clínica a las 48 horas de post-operatorio. Reporte de anatomía patológica indica feto acardio mielocéfalo.

En este trabajo se presenta una revisión de cinco años (2001-2006) de pacientes pediátricos con diagnóstico de apendicitis aguda ingresados al servicio de pediatría del Hospital Dr. Patrocinio Peñuela Ruíz para determinar datos epidemiológicos para tal fin se diseño un estudio respectivo y descriptivo, el cual reporto un total de 143 pacientes el grupo etario mas afectado estuvo entre los 10-11 años con 50 de los casos revisados que representa 34.96 por ciento.

ABSTRACT

La histerectomía es la operación ginecológica más frecuentemente practicada en el mundo y la segunda intervención quirúrgica, en general, después de la cesárea. Se presenta una Revisión de 6 años de pacientes a quienes se les realizó Histerectomía ingresadas al servicio de Gineco-obstetricia del Hospital "Dr. Patrocinio Peñuela Ruíz" periodo 2000-2005, para determinar edad predominate, causas y tipo de histerectomía realizada. Para tal fin se analizaron en forma retrospectiva 251 historias clínicas siendo el grupo etario más afectado 46-50 años con un 31.87 por ciento la principal causa fue fibromatosis 69.72 por ciento y el tipo de histerectomía más realizada fue Panhisterectomia 46.6 por ciento.
