ABSTRACT

Merkel cell tumour is a rare skin tumour of high malignancy, poor prognosis and low survival. It is characterized by its tendency to lymph node and vascular invasion and by a high percentage of locoregional recurrence in the year following surgical removal. It affects adults between 60 and 80 years of age and often occurs in the head and neck. We present the case of an 85-year-old man presenting with an ulcerated gluteal mass of 4 months' evolution. Diagnosis was by histopathological and immunohistochemical study. Early diagnosis and appropriate treatment are important to improve the prognosis of these patients.

ABSTRACT

BACKGROUND: Dyslipidemia is an important risk factor for the development of several diseases. The genetic component of hypertriglyceridemia has been studied in adults, but little is known in children. OBJECTIVE: The objective is to evaluate the association of two variants in APOA5 (rs662799) and APOA1 (rs5072) with triglyceride (TG) levels in Mexican children. METHODS: Anthropometric parameters were measured in 1559 Mexican children 5-14 years of age. DNA was isolated from blood samples. Lipid profiles and glucose concentrations were determined from serum and genotyping of rs662799, and rs5072 was performed using TaqMan® technology. Additive and dominant models adjusted for age, gender and body mass index were used to evaluate the association of these single nucleotide polymorphisms with TG levels. RESULTS: Children with high TG levels were found to have a higher body mass index and waist circumference as well as a worse lipids profile and glucose levels (p < 0.001). Additive and dominant models demonstrated a significant association between the rs662799 and rs5072 with TG. The dominant model showed the strongest significant association (OR = 1.81; 95% CI 1.46-2.24; p = 5.40 × 10-08 for rs662799 and OR = 1.54; 95% CI 1.05-2.25; p = 2.60 × 10-02 for rs5072). CONCLUSION: The minor alleles of rs662799 (APOA5) and rs5072 (APOA1) modulate TG levels in Mexican children.

ABSTRACT

Con objeto de evaluar la eficacia y seguridad terapeutica de la B-metil digoxina en el tratamiento de los enfermos con insuficiencia cardiaca, llevamos a cabo un estudio abierto con 30 pacientes adultos de la Sala de Urgencias del Hospital General "Lic. Adolfo Lopez Mateos", del ISSSTE Los pacientes fueron tratados durante un periodo de seis semanas, habiendose administrado durante este lapso una dosis media individual de 9.4 mg del farmaco. La evaluacion terapeutica se hizo fundamentalmente con base clinica y mediante mediciones objetivas. Todos los pacientes tratados mantuvieron niveles de digoxina circulante dentro de limites terapeuticos.Los resultados obtenidos con este farmaco fueron excelentes y muy buenos en el 86.6% de los casos. Se concluye la utilidad terapeutica de la B-metil digoxina para el manejo de la insuficiencia cardiaca

ABSTRACT

The authors studied 10 cases of amoebic hepatic abscess documented by clinical evidence and confirmed by laboratory tests, liver scan and a good response to treatment. The immunological state of the patients was determined by protein electrophoresis, immunoelectrophoresis, counter-immunoelectrophoresis, radial immunodiffusion and roset formation for T and B lymphocytes. It is concluded that the alterations of cellular and humoral immunity is evident in cases of amoebic hepatic abscess; this alterations are more clear in the acute form of the illness and the immunological deficiency is more significant in the celular immunity.

ABSTRACT

Through 230 consecutive studies, 180 from patients and 50 from healthy persons, we obtain simultaneous figures of the spleen and the liver. The best position to obtain better scintigraphies of the spleen was the posterior left oblique. The normal dimensions of the spleen image was 11.1 mm. +/- 1.2 mm. (1 sigma) long diameter, and 6.9 mm. +/- 1.1 mm (1 sigma) broad diameter. In a great number of patients we found an enlargement of the spleen in the scintigraphic image without clinical demonstration. The study is easy, rapid and without troubles for patients; moreover it afford an important knowledge in the study of the spleen and the liver in simultaneous form.
