ABSTRACT

Cooperative behavior represents a situation in which individuals sometimes act in a way that produces a gain to another at a cost to themselves. This may be explained by a history of repeated interactions with others in which such behavior has resulted in reciprocal cooperation from others. Sometimes, even with reciprocal cooperation, gains and costs are unbalanced between partners. In this case, there is evidence that people may present an aversion to both disadvantageous and advantageous distributions of gains. In other words, they may act in such a way as to ensure an equal outcome among all group members. Aversion to inequity that benefits oneself (advantageous inequity (AI) aversion) may be more dependent on social and cultural cues than aversion to inequity that benefits others (disadvantageous inequity (DI) aversion). Using both between-subjects (Experiment 1) and within-subjects (Experiment 2) manipulations, the influence of recent experience with AI on participants' willingness to produce DI was explored within the context of a two-player card game. In initial game phases, the percentage of trials in which the participant experienced AI was manipulated. In subsequent game phases, participants had the opportunity to produce DI to themselves. The results from both experiments suggest that aversion to DI is reduced by recent experience with AI. This procedure allows social influences on DI to be tested, which may be important for providing a psychological explanation of cultural differences in aversion to DI.

ABSTRACT

Portal vein thrombosis (PVT) is a rare condition in the general population that develops serious complications if left untreated for long time. We present a case of a 29-year-old woman who developed PVT due to protein S deficiency versus neonatal funiculitis. Over time, the patient developed upper gastrointestinal bleeding due to esophageal varices and hypersplenism with splenic sequestration that caused minor bleeding episodes. Laparoscopic splenectomy and proximal splenorenal shunt with distal pancreatectomy due to aneurysmal dilatations of the splenic artery were successfully performed to avoid mayor progression of portal hypertension. Patient was discharged with indefinite anticoagulation and after surgery platelets raised up to 200x103/mm3. Laparoscopic splenectomy and proximal splenorenal shunt for portal hypertension due to portal vein thrombosis is an adequate surgery procedure which should be applied in these medical cases.

Introducción: La práctica de la cirugía genital es frecuente en infantes y adolescentes diagnosticados de intersexualidad. Una de sus principales consecuencias se refleja en la personalidad del paciente. Existen numerosos estudios en población adulta, pero son escasos en edades pediátricas. El dibujo constituye un instrumento valioso para la exploración psicológica en edades tempranas. Objetivo: Identificar las características psicológicas de infantes y adolescentes con tratamiento quirúrgico de los genitales, y de su desarrollo psicológico en el momento de la valoración. Métodos: Estudio transversal descriptivo y metodología cualitativa. La muestra la integraron 15 participantes entre 6 y 12 años, con hiperplasia adrenal congénita y cirugía genital. De ellos, 4 con asignación al sexo masculino y 11 con asignación femenina. Todos residentes en La Habana, Cuba y captados de las consultas de seguimiento de los servicios de Endocrinología del Instituto de Endocrinología y hospitales pediátricos. Se aplicaron las técnicas psicográficas (dibujo espontáneo, dibujo temático de la familia y dibujo temático "Así soy yo"). Resultados: El desarrollo psicológico se correspondió con la edad cronológica. El 100 por ciento presentó un pensamiento coherente y estructurado. El 50 por ciento presentó indicadores emocionales que aluden a insatisfacción con el propio yo, angustia (40 por ciento), y falta de aceptación del propio cuerpo (70 por ciento); además, expresaron dificultades en la comunicación familiar (60 por ciento). Conclusiones: Los indicadores globales relevantes de los dibujos denotaron daño emocional, dificultades en la aceptación, percepción y representación del esquema corporal y también en la comunicación social y familiar. Resulta impostergable intervenir en las causas del malestar y los problemas psicológicos de los sujetos estudiados para evitar que se desarrollen enfermedades psiquiátricas en la edad adulta(AU)


Introduction: The practice of genital surgery is frequent in infants and adolescents diagnosed with intersex. One of the main consequences is reflected in the patient´s personality. There are numerous studies in the adult population; however, they are rare in pediatric ages. Drawing is a valuable tool for psychological exploration in early ages. Objective: Identify the psychological characteristics of infants and adolescents with surgical treatment of the genitalia, and to characterize their psychological development. Method: Descriptive cross-sectional study and qualitative methodology. The sample was made up of 15 infants and adolescents between 6 and 12 years old with congenital adrenal hyperplasia and genital surgery. 4 of them with male sex assignment and 11 with female assignment, coming from the primary care level, residents in Havana, Cuba, recruited in the follow-up consultations of the endocrinology services of the Institute of Endocrinology and pediatric hospitals. The psychographic techniques (spontaneous drawing, thematic drawing of the family and thematic drawing called "I am like this" were applied). The study complied with the basic ethical aspects of scientific research. Results: Psychological development corresponded with chronological age. 100 percent of the patients presented a coherent and structured thinking. 50 percent presented emotional indicators that allude to dissatisfaction with one's own self, anguish (40 percent), and lack of acceptance of one's own body (70 percent); in addition, they expressed difficulties in family communication (60 percent). Conclusions: The relevant global indicators of the drawings denoted emotional damage, difficulties in the acceptance, perception and representation of the body scheme and also in social and family communication. It cannot be postponed an intervention in the discomfort causes and psychological problems of the patients studied, in order to avoid that psychiatric diseases can be developed in adults ages(AU)

Cooperation among unrelated individuals can evolve through reciprocity. Reciprocal cooperation is the process in which lasting social interactions provide the opportunity to learn about others' behavior, and to further predict the outcome of future encounters. Lasting social interactions may also decrease aversion to unequal distribution of gains - when individuals accept inequity payoffs knowing about the possibility of future encounters. Thus, reciprocal cooperation and aversion to inequity can be complementary phenomena. The present study investigated the effects of cooperative and uncooperative interactions on participants' aversion to disadvantageous inequity. Participants played an experimental task in the presence of a confederate who acted as a second participant. In reality, the participant interacted with a computer programed to make cooperative and uncooperative choices. After interacting with a cooperative or uncooperative computer, participants chose between blue cards to produce larger gains to the computer and smaller for him/her or green cards to produce equal and smaller gains for both. Results confirmed our first hypothesis that uncooperative interactions would produce aversion to disadvantageous inequity. Lastly, half of the participants were informed that points received during the experiment could be later exchanged for money, and half were not. Results indicated that information about monetary outcomes did not affect aversion to inequity, contradicting our second hypothesis. We discuss these results in the light of theories of reciprocal cooperation, inequity aversion, and conformity.

La trombosis de la vena porta (TVP) es una afección poco común en la población general que desarrolla complicaciones graves si no se trata durante mucho tiempo. Presentamos el caso de una mujer de 29 años que desarrolló TVP por deficiencia de proteína S versus funiculitis neonatal. Con el tiempo, la paciente desarrolló hemorragia digestiva alta por varices esofágicas e hiperesplenismo con secuestro esplénico que provocó episodios hemorrágicos menores. La esplenectomía laparoscópica y la derivación esplenorrenal proximal con pancreatectomía distal por dilataciones aneurismáticas de la arteria esplénica se realizaron con éxito para evitar una mayor progresión de la hipertensión portal. La paciente fue dada de alta con anticoagulación indefinida y tras la cirugía se elevaron las plaquetas hasta 200x103/mm3. La esplenectomía laparoscópica y la derivación esplenorrenal proximal para la hipertensión portal por trombosis de la vena porta es un procedimiento quirúrgico adecuado que debe aplicarse en estos casos médicos.

Introducción: La microcirugía de laringe incluye el dilema de un campo quirúrgico que coincide espacialmente con el del acceso a la vía respiratoria del paciente. Los mixomas, sobre todo numerosos y supra-infraglóticos, representan un reto terapéutico. Si aunado a este hecho, el paciente presenta una vía respiratoria anatómicamente difícil, la fórmula de la catástrofe sólo precisa mezclarlos con un plan de ventilación convencional. En este caso, la ventilación jet puede ser la alternativa para evitar una situación de desastre. Objetivo: Describir la secuencia de hechos y los resultados al aplicar por primera vez en el país la ventilación jet vía transcricotiroidea. Caso clínico: Ante el fracaso previo de una intervención mediante un método tradicional de ventilación, en una segunda intención para exéresis de múltiples mixomas laríngeos en una paciente con vía respiratoria anatómicamente difícil, después de obtener su consentimiento informado, se procedió a anestesiar y obtener un acceso transcricotiroideo a la vía respiratoria, para ventilar con flujos jet a través de un trocar 16G. La intervención, pensada para 15 min, se extendió durante 90 sin complicaciones ventilatorias o de oxigenación. La paciente fue dada de alta sin secuelas. Conclusiones: Aplicar ventilación jet vía transcricotiroidea fue oportuno, seguro y efectivo. Las complicaciones cardiovasculares menores fueron fácilmente controlables. Se usó por primera vez la ventilación jet transcricotiroidea en el país. Este resultado estimula la asimilación de las tecnologías de ventilación jet en contextos electivos o emergentes, como el desafío de una vía respiratoria anatómicamente difícil(AU)


Introduction: The larynx microsurgery involves the dilemma of a surgical field that coincides spatially with that of access to the patient's airway. Myxomas, especially numerous and supra/subglottic, represent a therapeutic challenge. If together with this fact, the patient presents an anatomically difficult airway, the formula for the catastrophe just needs to mix both with a conventional ventilation plan. In this case, jet ventilation can be the choice to avoid a disaster situation. Objective: To describe the sequence of events and the outcomes when transcricotyroid jet ventilation was applied for the first time in the country. Clinical case: In the face of previous failure of an intervention using a traditional method of ventilation, in a second intention for exeresis of multiple laryngeal myxomas in a patient with anatomically difficult airway, after obtaining her informed consent, we proceeded to anesthetize to obtain a transcricothyroid access to the respiratory tract, to ventilate with jet flows through a 16G trocar. The intervention, planned for 15 min, was extended for 90 minutes without ventilatory or oxygenation complications. The patient was discharged without sequelae. Conclusions: Transcricotyroid jet ventilation was timely, safe and effective. Minor cardiovascular complications were easily controllable. Transcricotyroid jet ventilation was used for the first time in the country. This outcome stimulates the assimilation of jet ventilation technologies in elective or emerging contexts, such as the challenge of an anatomically difficult airway(AU)

OBJECTIVES: Determine the prevalence of prenatal opioid and other drug positivity among women delivering infants in Utah and compare the findings with national data. STUDY DESIGN: Umbilical cord tissue samples and nonidentifiable demographic data were collected anonymously in 13 labor and delivery units throughout Utah. Samples were analyzed for opioids, amphetamines, cannabinoids, cocaine, phencyclidine, barbiturates, benzodiazepines, propoxyphene, and alcohol biomarkers. RESULTS: Fifty-eight (6.8%) of 850 umbilical cord samples were positive for one or more substances. Opioids were the most frequently detected drugs (4.7%). Fewer samples were positive for alcohol (0.4%), methamphetamine (0.1%), cocaine (0.1%), and marijuana (0.4%). CONCLUSION: Opioids were the most frequently detected drugs at delivery. Although some of the samples positive for opioids might have been a result of intrapartum exposure, a significant number were positive for opioids that are not given during labor. This parallels the increasing nonmedical use of prescription pain medications in the general population and has important implications for neonates because of the potential for significant morbidity secondary to neonatal abstinence syndrome.
