ABSTRACT

Using minimally invasive methods of sanitation of bile ducts in 456 patients with high operative risk reduced the number of postoperative complications to 11%, lethality to 1.3%. Two-stage treatment with surgical intervention by means of laparotomy was necessary in 124 patients. In this group of patients the frequency of complications was 32.3%, and lethality--4.8%.

The authors made an analysis of early operations on 6076 patients. This tactics used in 84.3% of the patients allowed to reduce postoperative lethality up to 2.5%.

ABSTRACT

Among the patients with acute cholecystitis, those at the age of 20-45 years account for 12.6%. Of them, 50.9% develop complications: choledocholithiasis, obstructive jaundice, cholangitis, pancreatitis etc. The main method for treatment of the patients is the operation within 24-48 hr from the moment of their admission with intraoperative diagnosis of pathology of the extrahepatic bile ducts. There were no lethality, postoperative complications were noted in 9.3% of the patients.

ABSTRACT

The tactics of early operations for acute cholecystitis in patients with diabetes mellitus gives substantially better results of surgery. In the recent years the incidence of complications in this group of patients has been 20.3%, lethality--6.3%. Control of the carbohydrate metabolism must be made at short terms. It is the operation in combination with the rational insulin therapy which gives the correction of the carbohydrate metabolism.

ABSTRACT

The clinical picture of typhoid peritonitis in 67 patients is described. All the patients were operated upon. General roentgenography of the abdominal cavity and Widal's reaction are thought to be an important help in diagnostics of typhoid peritonitis. An analysis of postoperative complications and lethality in this group of patients is presented.

ABSTRACT

Valuable diagnostic and curative potentialities of fibrocholangioscopy were shown by using the method in the treatment of 50 patients. Fibrocholangioscopy can better differentiate various forms of cholangitis thus determining the mode of finishing choledochotomy. Fibrocholangioscopy is recommended as a method of examining for operations on bile ducts.
