ABSTRACT

BACKGROUND: Acquired immunodeficiency syndrome (AIDS) remains a major global public health problem. This study aimed to obtain current epidemiological data on the Human immunodeficiency virus (HIV) infections in Mogadishu, Somalia. METHODS: This study included 92,270 anti-HIV test results reported for 82,954 different individuals between 2015 and 2019. HIV tests were performed using the Architect HIV Ag/Ab Combo assay and retested with the Elecsys HIV combi PT assay. RESULTS: HIV seropositivity was found to be 0.32% (269/82,954) in all individuals over a period of four years. Anti-HIV seropositivity in the 0 - 14, 15 - 19, 15 - 24, 15 - 49, and > 15 age groups were as follows: 0.17% (11/6,441), 0.17% (12/7,131), 0.15% (35/24,132), 0.37% (212/56,895), and 0.34% (258/76,513), respectively. In HIV-infected patients, anti-HBs, HBsAg, anti-HCV, and anti-TP (syphilis) seropositivity was found to be 30.3% (56/185), 9.54% (23/241), 1.24% (3/242), and 3.45% (2/58), respectively. CONCLUSIONS: The findings from this study provide comprehensive data on the HIV epidemiology in Somalia. We believe that the results presented in this study will contribute to the risk analysis and planning of preventive policies of national and global health organizations.

BACKGROUND: Oxidative stress increases in many systemic and endocrine diseases. The effect of increased parathyroid hormone levels (PTH) and the effects of this hormone on oxidative stress in patients with primary hyperparathyroidism (pHPT) is unknown. We aimed to investigate the change of Thiol-disulfide (SH-SS), one of the oxidative stress parameters, in patients diagnosed with pHPT and the usability of this parameter in patients with pHPT. METHODS: Forty-six patients who recently diagnosed with asymptomatic pHPT and 40 healthy controls were included in this prospective study. In addition to routine examinations for pHPT, serum SH-SS measurements were recorded. The pHPT patients included in the study were divided into two groups as patients with and without surgical treatment indication. RESULTS: It was observed that the pHPT group had lower total SH and native SH values and higher SS values compared to the control group (P<0.05 for each). Native SH values were found to be lower in pHPT patients who were indicated for surgical treatment compared to those who did not (P<0.05). An independent relationship was found between Native SH and serum calcium, urine calcium and T scores in DEXA level in asymptomatic pHPT patients with surgical treatment indication (P<0.05). CONCLUSIONS: In our study, native SH level decreases in patients with pHPT, especially in patients with surgical treatment indication for pHPT. The decrease in SH levels, which is a natural antioxidant that protects the body against oxidative stress, and the increase in SS levels in pHPT patients may be another metabolic effect of this disease. Native SH may be helpful in determining the indication for surgical treatment in asymptomatic pHPT patients.

BACKGROUND: In this study, we planned to determine the frequency of postoperative AF and its related parameters in morbidly obese patients who underwent bariatric surgery. METHODS: The study included 300 patients with morbidly obese who had no history of AF and underwent successful bariatric surgery. Routine anamnesis, physical examination and laboratory parameters of the patients were recorded. Patients with postoperative AF were detected. The participants were grouped as patients with and without AF. RESULTS: Postoperative AF occurred in 19 (6.3%) patients. Age, BMI and LAd diameter parameters independently determined the presence of AF. When the ROC curve was performed to identify patients with AF, the area under the ROC curve was found to be 0.841, 0.785 and 0.902 for age, BMI and LAd diameter, respectively. According to this analysis, 50 years for age, 43 kg/m2 for BMI and 40 mm for LAd were used to determine patients with AF with acceptable sensitivity and specificity (>70% each). CONCLUSIONS: The most important determinants of postoperative AF are age, LAd and basal BMI. Therefore, morbidly obese patients with LAd >40 mm, BMI >43 kg/m2 and >50 years of age should be followed up more closely for postoperative AF development and preoperative precautions should be taken to prevent AF development.
