ABSTRACT
The trapdoor deformity has been thought to be the result of contraction of the semicircular scar of the skin surface. The hypothesis is presented that a sheet of internal scar is primarily responsible for raising the skin inside the semicircle.

Deformity of the orbital region and contraction of the eye socket were encountered in 3 patients who in their infancy underwent exenteration of the orbit and postoperative irradiation for the treatment of retinoblastoma. These major problems were attributed to the less-vascularized cicatricial conjunctiva left in place. To solve these disadvantages, a microvascular technique using a free radial forearm flap was adopted. A large permanent eye socket was achieved and depression deformity of the orbital region was corrected in a one-stage operation. This is, to our knowledge, the first report on the use of a free vascularized skin flap for "malignant contracture" of an anophthalmic eye socket. The excellent cosmetic results of this method are demonstrated.

A 23-year-old female patient with ameloblastoma on the right side of the mandible is presented. The affected mandible was excised and reconstructed with a free vascularised scapular bone graft followed by dental reconstruction with an artificial dental root. This is a preliminary report and the benefits of the procedure are discussed.

A case of unilateral athelia with a subcutaneous dermoid cyst in the area in question is reported. The histologic features of the cyst wall suggest that it is derived from a base in the nipple-areola complex. Dozens of cases of congenital absence of the breast have been reported, some of them associated with congenital anhidrotic ectodermal dysplasia, others with additional musculoskeletal abnormalities, and still others normal in other respects. But to our knowledge, the combination of athelia and corresponding subcutaneous dermoid cyst has not been reported.

We used an orbicularis oculi muscular pedicle musculocutaneous flap to close the defect resulting from removal of a large xanthoma from an Oriental upper eyelid. This is an excellent method not only to save eyelid skin but also to make a superior palpebral fold at the same time.
