ABSTRACT
The paper reports results of sociological analysis of satisfaction of the patients with the medical care provided in in-patient facilities of Arkhangelsk and Vologda regions and Komi Republic. The authors distinguish three groups of factors (professional, organizational, informational) that determine the patients' content with the quality of medical services. The study revealed the generally low level of patients' satisfaction estimated based on selected criteria for medical aid and services.

The characteristics of medical activities in families fostering disabled children are given. The assessment is implemented according the system of specific indicators developed by the authors to determine corresponding social hygienic, medical organizational and social psychological factors. The analysis of medical activities indicators is carried out to reveal the groups of higher medical social risk and to establish the requirements in different types of medical care.

The transition of Russian economy to the market relationship demanded the corresponding alterations in the organization of the population social defence system. The mandatory medical insurance is one of its important components. On the basis of data of the monitoring of defence of rights of insured in the system of mandatory medical insurance in Arkhangelskaya Oblast from 1998 to 2006 a significant increase of complaints of citizen was established. Among major reasons--unlawful actions of medical personnel during execution of their professional duties. It gives evidence of increased legal competence of population. The characteristic of complaints of insured is given according type, reasons and financial consequences for public health institutions. The main factors promoting the optimization of actual system of defense of right of insured citizen are considered.

The materials related to the regional characteristics of health formation in the disabled children are presented. The corresponding evaluation was made using the conventional and new methodological approach based on the so-called "three-dimensional concept". The obtained data is the initial base for developing and implementation of the integrated programs of the medical social, psychological pedagogical and social economical rehabilitation of the disabled children and their families.

The results of medical sociological study of the risk factors in the families bringing up a disabled child is presented. The comparative analysis of the structure of the children's disability considering various methodological approaches and regional characteristics. The description of the social status of the families with disabled children is given. The particular attention is drew to the studying of the medical demographic, social economic and psychological educational risk factors of the studied family contingent.

The financial activity of the rural medical institutions in Northern Russia (Arkhangelsk Region) is analyzed. Recent tendencies in alteration of the expenditures of hospitals of different size and level are noted. Basic trends in the process of reformation of public health are outlined, intended to improve the efficacy of utilizing its financial resources which are always insufficient.

Modern status of laboratory diagnostic service at the state and municipal medical institutions of regional public health is assessed as exemplified by the Archangelsk district. Qualitative parameters of the resource provision and variations thereof at institutions of different type, potential, and localization are analyzed. Availability of diagnostic service for the urban and rural population is assessed. Basic approaches to public health reforms in a region are formulated as the priority condition of improving the diagnostic service, rational utilization of resources, and available and proper medical care of the population.
