ABSTRACT

AIM: To describe clinicofunctional features of essential and pulmogenic hypertension in chronic bronchoobstructive syndrome, 24-h profile of arterial pressure (AP), intracardiac hemodynamics and to propose differential diagnostic criteria for these hypertension forms. MATERIALS AND METHODS: 24-h monitoring of arterial pressure (MAP), cardiohemodynamics, external respiration function (ERF) and blood gases examinations were made in 100 hypertensive subjects with chronic obstructive bronchitis and bronchial asthma. RESULTS: Significant differences were found between the groups of essential and pulmogenic hypertension by major values of MAP, echo-CG and ERF. Early disturbances in diastolic function of both the ventricles in essential and pulmonary hypertension in chronic bronchial obstruction arise long before clinical, x-ray and ECG signs of chronic pulmonary heart. CONCLUSION: The findings enable formulation of criteria of differential diagnosis of essential and pulmogenic hypertension and identify the latter as an independent nosological entity.

ABSTRACT

Study of the efficacy of a course of exposures to travelling pulsed magnetic field and magnetic laser sessions in 97 patients with stages I-II essential hypertension showed a high efficacy of travelling pulsed magnetic field in patients with hyperkinetic hemodynamics and initially just slightly shifted blood rheology and platelet hemostasis. Magnetic laser therapy is more effective in patients with eukinetic and hypokinetic hemodynamics and initially sharply expressed disorders of blood rheology and platelet hemostasis.

ABSTRACT

Overall 43 outpatients with arterial ischemia of the lower limbs were treated by emakor, a new Soviet drug that exerts a concomitant alpha- and beta-blocking action. The treatment resulted in a remarkable positive effect which manifested itself in amelioration of circulation in the lower limbs, positive time-course of changes in peripheral hemodynamics and in computer-aided tomography data. The use of emakor is contraindicated in exacerbation of gastrointestinal diseases, chronic pulmonary diseases associated with the obstructive syndrome, and marked signs of venous outflow obstruction in the lower limbs.

ABSTRACT

Coronary arterial affection (CAA) is an objective indicator, closely related to cardiac contractility as well as clinical severity and immediate prognosis of myocardial infarction. The progress of CAA is associated with more pronounced disorders of cardiac contractility, higher rates of severe complications and less optimistic immediate prognosis. Long-term prognosis is not affected by the magnitude of CAA in postmyocardial-infarction patients.

ABSTRACT

The experience of treating 2,500 patients with acute myocardial infarction enabled the authors to prove a diverse character of nitroglycerin effect: it exerts a positive inotropic action on the myocardium, improves elastic properties of the myocardium, normalizes energy balance of the myocardium, influences the system of the blood aggregate state through contractile elements of platelets producing a hypocoagulatory effect.

ABSTRACT

The probability of combined lesion of both ventricles in acute myocardial infarction (AMI) is discussed. In cases of proximal stenosis or occlusion of the right coronary artery, both inferior and right-ventricular (RV) AMI may develop. ECG recordings from the V3R-V6R, V3RIII, V4RIII leads were used for the diagnosis of RV AMI. Elevated ST segment plus the formation of Q waves and a negative T wave in the recordings from these leads indicate RV AMI in the presence of inferior AMI. The extent of myocardial lesion, estimated on the basis of maximum plasma MB CPK activity, is greater in cases of inferior AMI with electrocardiographic signs of RV AMI, as compared to inferior AMI free of such signs.
