ABSTRACT

Maintaining physical and mental health of older people is one of the important issues to be addressed in the aging society. Social capital, defined as the resources available to members of social groups, has recently attracted attention as a factor influencing public health. Most of the previous studies targeted various communities having different aging rates or population densities at once to examine the associations of social capital and health outcomes. However, the results of those studies are not always consistent. Moreover, because few studies have targeted a particular advanced aging society, associations of social capital and health at such societies have remained unknown. This study examined how social capital associates with health at a particular city having a very high aging rate and low population density. We targeted Iwamizawa city, Hokkaido, Japan, which is one of the most advanced aging areas, with an aging rate of 36.6% and a population density of 165/km2. We analyzed self-administered questionnaire data obtained from "HELLO (HEalth, Lifestyle, and LOcal community of Iwamizawa citizen) Study" in 2018. The sample comprised 1237 individuals aged 65 and older. Following previous studies, we regarded three items-social cohesion, reciprocity, and civic participation-as social capital indices, and targeted two health outcomes: self-rated health (SRH) and degree of depression. Multilevel Poisson regression analyses were used to calculate prevalence ratios (PRs). We found that at the individual-level, the PR (95% confidence interval) of having poor SRH among those with more civic participation was 0.81 (0.71-0.93), and that of being depressed among those with more social cohesion was 0.32 (0.21-0.51), even after adjusting for compositional factors. We also found that the community-level civic participation significantly correlated with aging rate. Our findings indicate that social capital positively associates with older people's health at the advanced aging city.

ABSTRACT

BACKGROUND: Patients hoping to preserve their fertility receive conservative treatment with high-dose medroxyprogesterone acetate (MPA) for well-differentiated endometrioid adenocarcinoma (EC) or atypical endometrial hyperplasia (AEH) . Such treatment generally involves frequent intrauterine operations, including dilation and curettage (D&C) and endometrial biopsy (EMB), which could result in endometritis, endometrial thinning, or intrauterine adhesion. In turn, any of these outcomes could adversely affect implantation and pregnancy development. The current study thus aimed to identify factors that might affect pregnancy following conservative treatment by MPA. METHODS: We compared a pregnancy group (45 patients) with a non-pregnancy group (53 patients) of MPA-treated patients to evaluate the factors affecting clinical pregnancy establishment. We undertook a multivariate logistic regression analysis based on factors shown by univariate analysis to be significantly different between the groups. Univariate analysis identified number of D&C, endometrial thickness, duration of MPA administration, age of pregnancy permission (the age at which a patient was first allowed to attempt pregnancy after disappearance of the lesion), period of disappearance of lesions, and recurrence as independent variables. RESULTS: The odds ratios (95 % confidence interval) of multivariate analysis for disease recurrence, endometrial thickness during ovulation, and age of pregnancy permission were 0.283 (0.102-0.785), 1.677 (1.251-2.248), and 0.889 (0.792-0.998), respectively. There was no significant difference in the other independent variables between groups. CONCLUSIONS: We identified three factors considered to affect pregnancy establishment following conservative treatment with MPA: recurrence, endometrial thickness during ovulation, and the age of the pregnancy permission. Introduction of infertility treatment including assisted reproductive technology (ART) soon after achieving tumor disappearance by MPA would therefore be beneficial for patients with disease recurrence, thin endometrium, or a higher age of pregnancy permission.

ABSTRACT

The wide applications of the subcutaneous pedicle flap "lateral orbital flap" named by the author is presented. The flap is designed between the lateral canthus and the sideburn as a spindle or crescent shaped island 2- to 3-cm wide and 5-cm long. To reconstruct the upper or lower eyelids, the flap is elevated as thin as possible, whereas for the eye socket reconstruction, an adequate volume of subcutaneous fat tissue is attached to the flap. The pedicle is designed inferomedially for the reconstruction of the recipient, any shape of the flap can be designed. As the anatomic structure of the donor site is similar to the eyelid, the excellent result can be obtained both functionally and cosmetically. The donor site is primarily closed without a marked scar. The cases of reconstructions using lateral orbital flap for the upper eyelid and the eye socket in the anophtalmic condition are presented.

ABSTRACT

A compact 4 × 25 Gbps optical transceiver has been fabricated for an optical backplane system, which consists of a 4 × 25 Gbps DFB-LD array, a 4 × 25 Gbps PIN-PD array, and a CMOS transceiver chip. These are directly mounted on 9 × 14 mm(2) multi-layer ceramic package with an electromagnetic shield structure to suppress inner-channel crosstalk effectively. The transceiver includes an analog front-end as well as an electrical interface function to interface with the switch LSI or CPU. Power consumption was as low as 20 mW/Gbps, and a transmission experiment was successfully conducted at 25 Gbps.

ABSTRACT

Two cases of dumbbell-shaped lipoma under the zygomatic arch and the therapeutic courses are described. Both tumours were present in the buccal and temporal processes and the isthmus, which showed dumbbell forms occupying temporal and infratemporal fossae on coronal MR images. According to the location, the tumours seemed to be derived from a masticatory fat pad. The tumours were completely excised via a temporal and small cheek skin incision, with no postoperative complications. Over the 5-year follow-up period, both cases have shown good function and aesthetics, and no recurrences have been observed.
