ABSTRACT

The development of intensive care medicine is inseparable from the diversified monitoring data. Intensive care medicine has been closely integrated with data since its birth. Critical care research requires an integrative approach that embraces the complexity of critical illness and the computational technology and algorithms that can make it possible. Considering the need of standardization of application of big data in intensive care, Intensive Care Medicine Branch of China Health Information and Health Care Big Data Society, Standard Committee has convened expert group, secretary group and the external audit expert group to formulate Chinese Experts' Consensus on the Application of Intensive Care Big Data (2022). This consensus makes 29 recommendations on the following five parts: Concept of intensive care big data, Important scientific issues, Standards and principles of database, Methodology in solving big data problems, Clinical application and safety consideration of intensive care big data. The consensus group believes this consensus is the starting step of application big data in the field of intensive care. More explorations and big data based retrospective research should be carried out in order to enhance safety and reliability of big data based models of critical care field.

ABSTRACT

In order to obtain more accurate parameters required for the simulation of asphalt mixtures in the discrete element method (DEM), this study carried out a series of cross-functional asphalt mixture experiments to obtain the DEM simulation meso-parameters. By comparing the results of simulation and actual experiments, a method to obtain the meso-parameters of the DEM simulation was proposed. In this method, the numerical aggregate profile was obtained by X-ray CT scanning and the 3D aggregate model was reconstructed in MIMICS. The linear contact parameters of the aggregate and the Burgers model parameters of the asphalt mastic were obtained by nanoindentation technology. The parameters of the parallel bonding model between the aggregate and mastic were determined by the macroscopic tensile adhesion test and shear bond test. The results showed that the meso-parameters obtained by the macroscopic experiment provide a basis for the calibration of DEM parameters to a certain extent. The trends in simulation results are similar to the macro test results. Therefore, the newly proposed method is feasible.

ABSTRACT

The formation of severe dendritic sodium (Na) microstructure reduces the reversibility of anode and further hinders its practical implementation. In this work, an ionic-electronic dual-conducting (IEDC) scaffold composed of Na3 P and carbon nanotubes is in situ developed by a scalable strategy with subsequent alloying reaction, for realizing dendrite-free Na deposition under high current density and large areal capacity. The in situ formed Na3 P with high sodiophilicity not only sets up a hierarchically efficient ionic conducting network, but also participates in the construction of reinforced solid electrolyte interphase, while carbon nanotubes can assemble an electronic conducting framework. As a result, the multifunctional IEDC scaffold contributes to smooth Na plating and exceptionally reversible Na stripping. High average Coulombic efficiency of 99.8% after prolonged 1200 cycles at 3 mA cm-2 and small overpotential of 20 mV over 250 h (equals to 530 cycles) at high rate of 5 mA cm-2 are obtained. The high availability of Na in IEDC scaffold enables the impressive performance of full cell with limited Na, using Na3 V2 (PO4 )3 (NVP) cathode at practical level. More importantly, the as-developed anode-free full cell with IEDC||NVP configuration delivers a high capacity retention with long lifetime, indicating its great potential for practical Na metal batteries.

ABSTRACT

Sepsis-induced acute lung injury is associated with dysregulated inflammatory reactions. MiR-19b-3p level was reported to be downregulated in patients with sepsis. To evaluate the role of miR-19b-3p in sepsis, cecum ligation and puncture-induced mouse sepsis model and lpopolysaccharide (LPS)-treated pulmonary microvascular endothelial cells (PMVECs) were used. For in vivo study, lung tissue was harvested for hematoxylin and eosin (H&E) staining, tumor necrosis factor-α, interleukin-6 (IL-6), IL-1ß, and p-p65, p-IκB measuring. Cell apoptosis was assessed by TUNEL assay. For in vitro study, cell proliferation and apoptosis were detected by CCK-8 and flow cytometry, respectively. Methylation of miR-19b-3p promoter was measured by methylation-specific PCR (MSP) assay. The target of miR-19b-3p was determined by dual-luciferase reporter gene assay. The level of miR-19b-3p was determined to be downregulated in vitro and in vivo. In addition, miR-19b-3p protected mice from inflammation injury through inhibiting NF-κB signaling pathway. Overexpression of miR-19b-3p increased cell viability, decreased apoptosis, and proinflammatory cytokines secretion in LPS-treated PMVECs. Besides these, Krüppel-like factor 7 (KLF7) was confirmed as the target of miR-19b-3p. And methylation of miR-19b-3p was the reason of decreased miR-19b-3p level. In conclusion, miR-19b-3p protected cells from sepsis-induced inflammation injury via inhibiting NF-κB signaling pathway, and KLF7 was a potential target.

ABSTRACT

OBJECTIVES: To explore the effects of blood glucose levels on liver functions in patients with sepsis treated in intensive care unit (ICU). METHODS: We performed a retrospective study on 93 patients with sepsis between March 2014 to January 2015. Patients were divided into two groups according to the initial blood glucose levels when admitted to ICU: high glucose group (blood glucose>10 mmol/L) and normal glucose group(blood glucose≤10 mmol/L). Routine blood test results, liver functions and coagulations function were compared between the two groups. RESULTS: On the first day of ICU admission, patients in normal glucose group had lower blood platelet count, higher total bilirubin (TB) and direct bilirubin (DB) levels, lower high density lipoprotein cholesterol (HDL-C) level, lower low density lipoprotein cholesterol (LDL-C) level, longer prothrombin time (PT), longer international normalized ratio (INR), longer activated partial thromboplastin time (APTT) and lower antithrombin Ⅲ (AT Ⅲ) level ( P<0.05). However, seven days after ICU admission, there were no significant differences between the two groups ( P>0.05). CONCLUSIONS: In patients with sepsis, normal blood glucose level might represent severe liver damage.

ABSTRACT

OBJECTIVES: To determine the association between the value of early diastolic transmitral velocity to early mitral anulus diastolic velocity ratio (E/E')measured by bedside ultrasound and pulmonary edema in severe sepsis (including septic shock) patients. METHODS: Data were obtained from the critical ultrasound database between November 2014 and August 2015. The severe sepsis (including septic shock) patients were eligible, but those with chronic heart disease or acute heart failure were excluded. Pulmonary edema was quantified by lung ultrasound score. The patients were divided into two groups according to lateral E/E'. E/E'-high group was defined as the lateral E/E'>8, and the others were in E/E'-low group. The severity and distribution of pulmonary edema were compared between the two groups. The correlation analysis and logistic regression analysis were performed to determine whether E/E' was the independent risk factor of pulmonary edma. RESULTS: There were 82 cases included. The lung ultrasound score of E/E'-high group was significantly higher than that of E/E'-low group ( P=0.007). There was no statistic difference of pulmonary edema severity in the bilateral 5 th and 6 th ultrasound exam areas, but E/E'-high group were more severe in the bilateral 1 th to 4 th ultrasound exam areas ( P=0.004). Linear-regression analysis demonstrated that E/E' and inferior vena cava diameter were independent risk factors of pulmonary edema (standardized regression coefficients were 0.425 and 0.249, respectively, P<0.05). CONCLUSIONS: E/E' is the independent risk factor of pulmonary edema in severe sepsis and septic shock patients.

ABSTRACT

OBJECTIVE: To explore the aesthetic result of dense-packing single-hair grafting for eyelash. METHODS: Twenty-six female cases were treated with the donor sites at the occipital safe area. The single-hair grafts were made with the hair of 4cm in length. The grafts were transplanted to the upper lid margin with needle technique, 30-40 grafts at each side. RESULTS: The hair grafts survived very well with good growth direction. It looked even better when it had a permanent wave or the mascara was applied. The survival rate of hair grafts was more than 95%. CONCLUSIONS: It is feasible to improve the shorter and thinner eyelash by dense-packing single-hair grafts.

ABSTRACT

A protocol was developed to fractionate soil particles down to the nanocolloid scale by combining sieving, sedimentation, centrifugation, and cross-flow filtration (CFF). The validity of the method and the performance of the CFF system were tested by characterizing fractions using laser granulometry, electron microscopy, and chemical analysis. The 0.1-microm-pore-size membrane CFF system effectively retained nanocolloids (<0.1 microm) as shown by laser granulometry and observed directly by transmission electron microscopy. However, environmental scanning electron microscopy images of freeze-dried colloids were very different from their TEM counterparts, suggesting that sample preparation influenced microscopy imaging. Chemical analysis of Cu, Cd, and organic carbon in each fraction showed that the concentrations of these components increased as particle size decreased, indicating colloids and nanocolloids play an important role in retaining trace metals. Particle-size fractionation combined with chemical analysis and electron microscopy can provide insight into the nature and properties of nanocolloids in soil.
