ABSTRACT

The discovery of a square magnetic-skyrmion lattice in GdRu2Si2, with the smallest so far found skyrmion size and without a geometrically frustrated lattice, has attracted significant attention. In this work, we present a comprehensive study of surface and bulk electronic structures of GdRu2Si2 by utilizing momentum-resolved photoemission (ARPES) measurements and first-principles calculations. We show how the electronic structure evolves during the antiferromagnetic transition when a peculiar helical order of 4f magnetic moments within the Gd layers sets in. A nice agreement of the ARPES-derived electronic structure with the calculated one has allowed us to characterize the features of the Fermi surface (FS), unveil the nested region along kz at the corner of the 3D FS, and reveal their orbital compositions. Our findings suggest that the Ruderman-Kittel-Kasuya-Yosida interaction plays a decisive role in stabilizing the spiral-like order of Gd 4f moments responsible for the skyrmion physics in GdRu2Si2. Our results provide a deeper understanding of electronic and magnetic properties of this material, which is crucial for predicting and developing novel skyrmion-based systems.

ABSTRACT

This work aims to study the conditions of topological phase transition (TPT) between the topological and trivial states in the antiferromagnetic topological insulator (AFM TI) MnBi[Formula: see text]Te[Formula: see text] and propose some theory about the relationship of this TPT with the possibility of axion-like state realization in this material. Using the density functional approach we have analyzed the changes in the electronic and spin structure of topological surface states (TSSs) and the nearest conduction and valence bands (CB and VB) including the changes in the bulk band gap as well as the Dirac point (DP) gap in TSSs under variation of the spin-orbit coupling strength in the region of the TPT for infinite crystal and slab with a surface both. We have shown that in both cases the TPT occurs with inversion of the contributions of the Bi-[Formula: see text] and Te-[Formula: see text] states of different parity at the gap edges related to change in the gap sign. In the case of surface calculations, the Bi-[Formula: see text] and Te-[Formula: see text] states at the edges of the bulk band gap and their inversion at the TPT point are transformed into the TSSs with an energy gap at the DP. In this case the TPT takes place without closing the band gap, i.e. with a "jump" through zero and the formation of the nonzero gap during such a transition. Our calculations show that the TPT point is also characterized by an inversion of the out-of-plane spin polarization [Formula: see text] at the [Formula: see text] point for lower and upper parts of the Dirac cone and a significant spatial redistribution of the TSSs between the surface and the bulk. We suppose that the nonzero Dirac gap can have some relationship with the formation of the axion-like state, which presumably couples nonmagnetic spin-orbit and magnetic contributions at the boundary between the topological and trivial phases for a system with parameters close to the TPT conditions. A practically realized system is proposed - the AFM TI with a stoichiometry close to that of MnBi[Formula: see text]Te[Formula: see text]Se[Formula: see text] with partial (about 50%) substitution of Te atoms for Se atoms in MnBi[Formula: see text]Te[Formula: see text] which can be an experimental platform for the implementation and experimental analysis of the TPT and the corresponding possibility of the axion-like state realization in Condensed Matter. Besides, such system could serve as a good platform for studying the dynamic axion effect, where the axion field fluctuations are maximised when a small external field is applied to the system which state is close to the TPT.

ABSTRACT

The orientation of the 4f moments offers an additional degree of freedom for engineering the spin-related properties in spintronic nanostructures of lanthanides. Yet, precise monitoring of the direction of magnetic moments remains a challenge. Here, on the example of the antiferromagnets HoRh2Si2 and DyRh2Si2, we investigate the temperature-dependent canting of the 4f moments near the surface. We demonstrate that this canting can be understood in the framework of crystal electric field theory and the exchange magnetic interaction. Using photoelectron spectroscopy, we disclose subtle but certain temperature-dependent changes in the line shape of the 4f multiplet. These changes are directly linked to the canting of the 4f moments, which is different for the individual lanthanide layers near the surface. Our results illustrate the opportunity to monitor the orientation of the 4f-moments with high precision, which is essential for development of novel lanthanide-based nanostructures, interfaces, supramolecular complexes, and single-molecule magnets for various applications.

ABSTRACT

This Expert Council focuses on the meta-analysis of studies on the risk of atrial fibrillation (AF) in patients taking omega-3 polyunsaturated fatty acids (PUFA) and of data on the omega-3 PUFA treatment in patients with cardiovascular and kidney diseases.The major statements of the Expert Council: the meta-analysis of AF risk in patients taking omega-3 PUFA showed an increased risk of this arrhythmia. However, it should be taken into account that the risk of complications was low, and there was no significant increase in the risk of AF when omega-3 PUFA was used at a dose of ≤1 g and a standard dose of the only omega-3 PUFA drug registered in the Russian Federation, considering all AF episodes in the ASCEND study.At the present time, according to Russian and international clinical guidelines, the use of omega-3 PUFA can be considered in the following cases: • for patients with chronic heart failure (CHF) with reduced left ventricular ejection fraction as a supplement to the basic therapy (2B class of recommendations according to the 2020 Russian Society of Cardiology guidelines (RSC) and the 2022 AHA / ACC / HFSA guidelines); • for patients with hypertriglyceridemia (>1.5 mmol/l) as a part of combination therapy (IIb class of recommendations and B level of evidence according to the 2021 European guidelines on cardiovascular disease prevention, etc.); • for adult patients with stage 3-4 chronic kidney disease (CKD), long-chain omega-3 PUFA 2 g/day is recommended for reducing the level of triglycerides (2C class of recommendations). Data on the use of omega-3 PUFA for other indications are heterogenous, which can be partially explained by using different form and doses of the drugs.

ABSTRACT

OBJECTIVE: To study the effect of Unifuzol (L-arginine sodium succinate) on cognitive impairment, cerebral blood flow, and damage to the tissues of the hippocampus and cerebral cortex during a 10-day course of administration to rats with chronic cerebral ischemia (CCI) caused by bilateral stenosis of the common carotid arteries (CCA). MATERIAL AND METHODS: The study was conducted on male rats with CCI caused by bilateral stenosis of the CCA by 60%. 40 days after surgery, rats received Unifusol (21, 42 and 84 ml/kg), nicergoline (10 mg/kg), citicoline (500 mg/kg) or placebo (0.9% NaCl) for 10 days. Next, cognitive impairments were assessed in the Morris Water Maze and the New Object Recognition (NOR) test, as well as the level of motor and exploratory activity in the Open Field test. The level of cerebral blood flow was determined immediately after the CCA stenosis and at the end of the experiment. Animals were euthanized in a CO2 incubator, after which the brain was removed and subjected to morphometric analysis. RESULTS: In animals that were modeled with CCA stenosis, pronounced behavioral and cognitive impairments occurred as a result of a decrease in blood flow in the vessels of the brain and subsequent changes in the tissues of the hippocampus and the cerebral cortex. Intravenous course administration of Unifuzol at doses of 42 and 84 ml/kg to animals with CCI was comparable in efficiency to nicergoline and citicoline, which was expressed in greater preservation of the cognitive abilities of animals in the Morris Water Maze and NOR tests. In the Open Field test, animals injected with Unifusol at doses of 42 and 84 ml/kg performed more acts of motor and exploratory activity than animals from the placebo group, and had a higher level of cerebral blood flow (compared to animals that were injected with citicoline). Based on the results of a morphological study, it was found that the most significant neuroprotective effect was provided by nicergoline and Unifuzol (at doses of 42 and 84 ml/kg). CONCLUSION: Unifuzol at a course of administration at doses of 42 and 84 ml/kg, comparable to the reference drugs nicergoline and citicoline, reduces the severity of psychoneurological deficit in animals with CCI, comparable to them improves the microcirculation of brain tissues, preventing damage to brain tissues.

ABSTRACT

JUSTIFICACIÓN: la derivación al médico es una de las posibles actuaciones del farmacéutico a través del servicio de indicación farmacéutica. Dicha derivación permite el cribado de pacientes de alto riesgo desde la farmacia comunitaria (FC). La colaboración multidisciplinar permite establecer criterios de derivación de pacientes consensuados entre profesionales. El estudio INDICA+PRO tiene como objetivo implantar el SIF protocolizado en la FC española a través del uso de protocolos consensuados en una fase previa de codiseño del servicio. OBJETIVOS: describir las características de los pacientes derivados al médico a través del SIF en el estudio INDICA+PRO Implantación. MATERIAL Y MÉTODOS: estudio con diseño híbrido de efectividad-implantación tipo 3 llevado a cabo inicialmente durante 14 meses. La intervención codiseñada constaba de: procedimiento general del SIF establecido por Foro de Atención Farmacéutica en FC, protocolos consensuados entre sociedades médicas (Semergen y SemFyC), asociaciones farmacéuticas (SEFAC y MICOF) y universidad (GIAF-UGR) específicos para síntomas menores (dermatológicos, digestivos, relacionados con el dolor, respiratorios y otros) incluidos en una plataforma digital (SEFAC e_XPERT®) y la formación de los farmacéuticos con el seguimiento de 33 farmacéuticos facilitadores del cambio de práctica. Cada protocolo específico incluía criterios de derivación divididos en: duración del síntoma, edad del paciente, síntomas de alarma, medicamentos, problemas de salud y estado fisiológico del paciente y otros. ABSTRACT

OBJECTIVE: To analyze the effectiveness of emergency surgical care under COVID-19 pandemic and its impact on postoperative outcomes. MATERIAL AND METHODS: We analyzed the results of treatment of 1.749 patients who were treated from May 01 to September 30 in 2018-2020. The study group consisted of 48 COVID-19 patients with urgent surgical diseases. The control group consisted of patients with emergency surgical diseases and no coronavirus infection. RESULTS: The new coronavirus infection significantly increases in-hospital mortality by 8 times and postoperative mortality by 6.5 times compared to pre-pandemic period. CONCLUSION: Emergency surgical care to COVID-19 patients is accompanied by increase of in-hospital mortality up to 31.3% and postoperative mortality up to 33.3%.

ABSTRACT

AIMS: Metformin, rosiglitazone and sulfonylureas enhance either insulin action or secretion and thus have been used extensively as early stage anti-diabetic medication, independently of the aetiology of the disease. When administered to newly diagnosed diabetes patients, these drugs produce variable results. Here, we examined the effects of the three early stage oral hypoglycaemic agents in mice with diabetes induced by multiple low doses of streptozotocin, focusing specifically on the developmental biology of pancreatic islets. METHODS: Streptozotocin-treated diabetic mice expressing a fluorescent reporter specifically in pancreatic islet α-cells were administered the biguanide metformin (100 mg/kg), thiazolidinedione rosiglitazone (10 mg/kg), or sulfonylurea tolbutamide (20 mg/kg) for 10 days. We assessed the impact of the treatment on metabolic status of the animals as well as on the morphology, proliferative potential and transdifferentiation of pancreatic islet cells, using immunofluorescence. RESULTS: The effect of the therapy on the islet cells varied depending on the drug and included enhanced pancreatic islet ß-cell proliferation, in case of metformin and rosiglitazone; de-differentiation of α-cells and ß-cell apoptosis with tolbutamide; increased relative number of ß-cells and bi-hormonal insulin + glucagon + cells with metformin. These effects were accompanied by normalisation of food and fluid intake with only minor effects on glycaemia at the low doses of the agents employed. CONCLUSIONS: Our data suggest that metformin and rosiglitazone attenuate the depletion of the ß-cell pool in the streptozotocin-induced diabetes, whereas tolbutamide exacerbates the ß-cell apoptosis, but is likely to protect ß-cells from chronic hyperglycaemia by directly elevating insulin secretion.

ABSTRACT

Exocytosis of peptides and steroids stored in a dense core vesicular (DCV) form is the final step of every secretory pathway, indispensable for the function of nervous, endocrine and immune systems. The lack of live imaging techniques capable of direct, label-free visualisation of DCV release makes many aspects of the exocytotic process inaccessible to investigation. We describe the application of correlative scanning ion conductance and fluorescence confocal microscopy (SICM-FCM) to study the exocytosis of individual granules of insulin from the top, nonadherent, surface of pancreatic ß-cells. Using SICM-FCM, we were first to directly follow the topographical changes associated with physiologically induced release of insulin DCVs. This allowed us to report the kinetics of the full fusion of the insulin vesicle as well as the subsequent solubilisation of the released insulin crystal.

ABSTRACT

AIM: To study prognostic factors for survival in patients of liver cirrhosis (LC) with portal hypertension (PG). MATERIALS AND METHODS: 155 patients (women - 49.4%, men - 50.6%, median age 56.4 years [51.3; 61.6]) were follow up for 36 months. Viral LC was diagnosed in 33.8%, alcohol LC - 22.1%, autoimmune LC - 15.6%, other causes and cryptogenic causes noted in - 28.5%. More than half of the patients had a class В of Child-Pugh (51.9%). RESULTS: During the study period 42 patients (28.3%) died. The analysis of survival by various factors for Kaplan-Mayer was carried out. Significant predictors: Child-Pugh class, LC etiology, comorbidity in Charlson scores, adherence to endoscopic treatment of esophageal varices were included in the model of proportional Cox risks. In a single-factor analysis, the risk of death is higher in patients who violate the timing of endoscopic treatment, but this factor has lost significance in the multifactor model. Patient with Child-Pugh C, alcoholic LC, comorbidity more 5 points by Charlson score have a higher mortality risk. CONCLUSION: Identification of risk factors that affect the survival of LC patients will allow to use of an individual plan for the LC treatment and prevention of PG complications.

ABSTRACT

AIM: To assess an effectiveness of surgical treatment of gastroesophageal reflux disease (GERD) combined with hiatal hernia (HH). MATERIAL AND METHODS: The trial included 96 patients with GERD and HH who were divided into 2 groups. The principal difference between groups was the use of surgery in the main group and therapeutic treatment in the comparison group. RESULTS: The effectiveness of surgical treatment is superior to therapeutic treatment of GERD by more than 2.5 times. CONCLUSION: HH combined with GERD is an indication for surgical treatment. Fundoplication cuff should not lead to angular and rotational esophageal deformation. Nissen procedure in Donahue modification (Short Floppy Nissen) simulates optimally the geometry of esophago-gastric junction and His angle.

ABSTRACT

The sphere of application of biomarkers is expanding every year and already comprises their using as indicator of presence or absence of disease, response to therapy, efficiency of medications or pre-clinical model of diagnostic parameter and even participant of process of search of mechanism of effect of medications. Hence, it is impossible to overestimate significance of studying of biomarkers. The article is dedicated to systematization and structuring of present information concerning biomarkers, starting from primary screening and completing with validation of chosen molecule or characteristic.

ABSTRACT

The aim of the study is to evaluate a preventive effectiveness of food products with elevated content of biologically active substances by community-acquired pneumonia with unknown causative agent. The authors studied two groups of youngsters (18 years old) during period of adaptation to conditions of organized group: with normal nutritional status [with body mass index (BMI) = 19.5-22.9 kg/m2] and decreased one (with BMI = 18.5-19.4 kg/m2). Tumor necrosis factor (TNFα) and interleukin 1α (IL-1ß) levels in blood serum were criteria of degree of inflammatory process. Functional activity of phagocytes was also evaluated with the use of nitroblue tetrasol test (NTT-test). To prevent pneumonia natural concentrated food product made of vegetable raw materials (3.0 g per day) by cryogen technology was added in diet of persons with normal nutritional status during two periods (15 days) with an interval (30 days). But persons with decreased nutritional status got the product made of protein-vegetable raw materials during 15 days only and additionally product made of protein-vegetable raw materials by cryogen technology one time per day (35.0 g) during 15 days. The persons with normal nutritional status were divided in three group: persons (480 men) in the first group got the product only, persons (100 men) of the second group underwent vaccination with polysaccharide polyvalent «Pneumo-23¼ vaccine only, persons (316 men) of the third group underwent vaccination in combination with the product intake. The persons (231 men) with decreased nutritional status composed one group and got the food product only. Control group consisted of 263 men with normal nutritional status. The authors evaluated incidence rates during three months of observation and duration of in-patient treatment. The results were compared with data obtained in baseline period (the same time periods during three years before the study). The authors received data on the decrease of a reserve ability of phagocytes; it provided evidence about an initial deficiency of immune protection factors in persons with decreased nutritional status. In adaptation period, this phenomenon was observed in persons with normal nutritional status too. The including of protein-vegetable product in diet promoted the increase of bactericidal function of neutrophils and body weight among youngsters with decreased nutritional status; but including of vegetable product kept bactericidal function of neutrophils on normal level. In adaptation period, TNFα levels increased in both groups by 21.0 and 35.1%, respectively; IL-1ß level did not significantly change in group with normal nutritional status, but increased 2.25 fold in persons with decreased nutritional status. The levels of studied cytokins decreased to baseline values in 30 days after the product intake. To this time, TNFα increased 1.8 fold and IL-1ß increased 2.86 times in control group. A preventive effectiveness of the especially created products in nidus with unknown causative agent was confirmed by the decrease of incidence rate (82.4-78.4%) in persons with normal and lower nutritional status that coincided with effectiveness of a vaccinal prevention only. The food product with antioxidant properties potentiated a vaccinal prevention. The positive effect consisted in lack of fatal outcomes and outbreaks of the disease; mean bed-day decreased on 3.5 days; predominance of mild form of the disease in clinical picture in comparison with same parameters during basic period of observation.

ABSTRACT

We investigated the first laboratory-confirmed human case of cowpox virus infection in Russia since 1991. Phylogenetic studies of haemagglutinin, TNF-α receptor-like protein and thymidine kinase regions showed significant differences with known orthopoxviruses, including unique amino-acid substitutions and deletions. The described cowpox virus strain, taking into account differences, is genetically closely related to strains isolated years ago in the same geographical region (European part of Russia and Finland), which suggests circulation of viral strains with common origin in wild rodents without spread over long distances and appearance in other parts of the world.

ABSTRACT

Gastric antral vascular ectasia (GAVE) is an uncommon but often severe cause of upper gastrointestinal bleeding. GAVE may be asymptomatic or accompanied by clinical anemia or overt gastrointestinal bleeding. The diagnosis is mainly based on pathognomonic endoscopic pattern, defined as «watermelon stomach¼, located in antrum. Autoimmune disorders aye co-existing in about 60% of patients with GAVE, chirrosis in about 30%, and cardiac or renal failure in 10%.The "golden standarden" treatment of GAVE is endoscopic argon plasma coagulation (APC). There is poor information about diagnostics and treatment of GAVE in the domestic literature. The analysis of the presented materials shows that correct diagnostics takes much time and the choice of the optimum optimal treatment strategy encounters difficulty. Our experience with diagnostics and therapy of GAVE is based on the treatment of 4 patients. The diagnosis of GAVE was established within the period from 5 months to 1.5 years after onset of the disease. In two cases, GAVE was asymptomatic. Two patients with severe anemia completed endoscopic treatment; in one case, APC was supplemented by laser coagulation and bipolar coagulation. The implementation of laser coagulation caused some technical difficulties. Treatment of both patients was successful. Endoscopy pivotal for diagnosis of GAVE is the main method of its treatment. Due to the large number of diagnostic errors and the importance of correct diagnosis of GAVE, it is necessary to pay attention to this issue in training programs for endoscopists.

ABSTRACT

UNLABELLED: A hallmark of malignancy is excessive tumor glycolysis, even in the presence of oxygen, which causes lactacidosis in the tumor microenvironment and favors tumor cell proliferation and survival. For this reason antimetabolic agents which target tumor cell metabolism are being researched extensively as promising anticancer drugs. AIM: To study the effect of lactacidosis on survival of Lewis lung carcinoma (LLC) cells at the conditions of nutritional substrate deficiency in vitro and evaluate antitumor and antimetastatic activity against LLC/R9 in vivo. MATERIALS AND METHODS: LLC variant LLC/R9 was used as experimental tumor model. Tumor cell viability was determined using trypan blue staining. Apoptosis level was counted with the use of Hoechst 33258 dye. Lactate content in the tumor tissue was evaluated by enzyme method with the use of lactate dehydrogenase. Reactive oxygen species was determined using 2.7-dichlorofluorescein diacetate. Effects of dichloroacetate (DCA) on the growth and metastasis of LLC/R9 were analyzed by routine procedures. Evaluation of DCA effect toward electron-transport chain (ETC) components was performed using EPR. RESULTS: It has been shown that at the conditions of lactacidosis and glucose deficiency, LLC/R9 cell viability in vitro was higher by 30% (p < 0.05) and apoptosis level was triply lower (p < 0.05) than these indices at the conditions of glucose deficiency only. In mice with transplanted LLC/R9 tumors treated for 3 weeks per os with DCA at the total dose of 1.5 g/kg of body weight starting from the next day after tumor transplantation, the primary tumor volume was just by 30% lower than that in control group. At the same time, the number and volume of lung metastases in animals treated with DCA were by 59% (p < 0.05) and 94% (p < 0.05) lower, respectively, than these indices in the control group. DCA treatment resulted in nearly 30% increase (p < 0.05) of lactate content in tumor tissue compared to that in the control, but did not affect significantly the levels of heme iron complexes with NO (at g med = 2.007) in mitochondrial ETC proteins and Fe-S cluster proteins (at g = 1.94) in tumor cells. CONCLUSIONS: It has been shown that lactacidosis significantly promoted LLC/R9 cell survival at the conditions of glucose deficiency in vitro. If LLC/R9 developed in vivo, DCA as the compound with antilactacidosis activity did not suppress significantly the primary tumor growth but exerted significant antimetastatic activity.

ABSTRACT

The development of neonatal surgery is an important task of health care system, because birth defects have been the 2nd most prevalent cause of infant mortality for many years. In order to improve the quality of care for neonates with surgical diseases we studied the main causes of neonatal deaths during the period from 1995 to 2014, on the basis of data from the Children's Surgery Department of Yakutsk. In 77% of cases, the causes of lethal outcomes in neonates with surgical pathology were conditionally preventable. We singled out the basic organizational problems, the solution of which led to a 3.5 times reduction in mortality of infants with surgical pathology during the second period of the study (2005-2014). The main organizational aspects of the regional model of improving medical care of infants with surgical pathology are: antenatal diagnosis of malformations and prenatal consultation with children's surgeon, competent and timely transportation of newborns from district hospitals, the centralization of aid at level 3 hospitals, the introduction of modern diagnostic and treatment algorithms, methods of minimally invasive endosurgery.

ABSTRACT

The article presents our experience of treatment of congenital diaphrogmatic hernia at newborns and hernia of an esophageal opening of a diaphragm among children of early age. Since 2010 surgical tactics has been changed: the prolonged preoperative preparation with use of the device of high-frequency ventilation of lungs, correction of symptoms of pulmonary hypertensia is carried out, operation is carried out according to plan after stabilization of the patient in parameters of oxygenation and an acid-base state, operative treatment is executed by method of low-invasive endosurgery. Over the last 5 years 25 children with good functional and cosmetic results have been operated. The lethality in the case of congenital diaphragmatic hernia at newborns decreased to 12%, it was noted generally among patients with the expressed hypoplasia of lungs which died during preoperative preparation. In recent years we place emphasis on antenatal diagnosis of pathology and prenatal diagnostics of degree of a hypoplasia of a lung on indicators of a pulmonary and head index at a fetus. It wasn't noted a postoperative lethality.

ABSTRACT

A Desulfovibrio strain physiologically similar to and phylogeneticall related to "D. caledoniensis" SEBR 7250, D. portus MSL79, and D. dechloracetivorans ATCC 700912 (96.9, 95.9, and 95.8% similarity of the 16S rRNA gen sequences, respectively) was isolated from marine biofouling in the coastal zone of the South China Sae (Nha Trang, South Vietnam). The cells of strain ME were gram-negative motile vibrios (0.4-0.6 x 1.3-2 µm) with a single flagellum. The strain grew at 20 to 39 degrees C (growth optimum at 34-37 degrees C), pH 5.8 to 8.5 (pH optimum at 6.8-7.5), and salinity from 0.08 to 1.1 M Na+ (optimum at 0.2-0.3 M Na+). In the presence of sulfate, the strain grew autotrophically with hydrogen or on lactate, formate, pyruvate, fumarate, and malate. Weak growth occurred on succinate, glycerol, and fructose. In the absence of sulfate, the strain was able to ferment pyruvate, malate (weakly), but not lactate. Sulfate, sulfite, thiosulfate, elemental sulfur, and dimethyl sulfoxide were used as electron acceptors. Vitamins and yeast extract were not required for growth. The G+C content was 52.4 mol %. Predominant fatty acids were C18:0 (13.9%), C16:0 (9.6%), iso-C16:0 (9.5%), C18: 1w7 (8.8%), anteiso-C15:0 (8.1%), and iso-C 17:1 (7.2%). The fatty acid composition was close to that of D. dechloracetivorans BO and has some similarity to that of D. portus. Based on its genotypic and phenotypic characteristics, strain ME maybe considered as a new species, for which the name Desulfovibrio hontrensis sp. nov. is proposed.
