ABSTRACT

A 60 year-old female presented to our clinic with a two-year history of progressive nasal obstruction and one isolated episode of epistaxis that did not respond to medical treatment. Endoscopic evaluation revealed roundish, dumbbell-shape, smooth-surfaced, submucosal mass with prominent vessels of approximately 2 cm in the posterior nasal septum,. A paranasal CT scan revealed a well-circumscribed oval shape mass (1.7 cm x 1.6 x 1.5 cm) arising from the posterior border of the bony nasal septum. The tumor was removed en-bloc with 1-centimeter free margins confirmed by intra-operative frozen sections through a completely endoscopic approach. Pathology results confirmed the diagnosis of a pleomorphic adenoma.

ABSTRACT

A 9 year-old male sustained multiple maxillofacial fractures after falling from a two-store building. Frontal sinuses suffered a bilateral non-displaced linear fractures extending into the anterior and posterior walls. Magnetic resonance imaging (MRI) at this time showed a small encephalocele extending into the right frontal sinus. Operative repair was performed using an Endoscopic-Assisted Trephination approach.

ABSTRACT

This is a case series with chart review of 59 consecutive pediatric patients with a diagnosis of cervical esophageal coin who underwent anterior-commissure laryngoscope (ACLA) extraction during apnea. The purpose of this study was (1) to evaluate the efficacy and safety of coin extraction and (2) to analyze foreign body features and intraoperative physiological parameters (apnea time, O2 saturation and end-tidal CO2 (ETCO2) of apnea, minimum O2 during procedure, and heart rate). The technique was completed in 94.9% of the sample. The mean of the length of apnea was 57.7 ± 25.2 seconds. The median minimum O2 saturation was 99.5% (minimum = 93, maximum = 100), and the median of the ETCO2 at the end of the procedure was 35.7 ± 4.8 mm Hg. Heart rates remained at baseline values during the procedure (P < .001). This technique represents an efficient and secure modality for treatment. If successful, the patient can be safely discharged after clearance from anesthesia and a swallowing trial.

ABSTRACT

Congenital arteriovenous fistulas (AVFs) result from inadequate differentiation of the vascular system during fetal development. This case report describes an AVF of the neck, which possibly manifested as noise interference during a newborn hearing-screening test by otoacoustic emissions (OAEs). This report is in compliance with the institutional review board regulations of the University of Puerto Rico School of Medicine.
