ABSTRACT

In a 66-year old woman, progressive worsening of an exertional dyspnoea revealed the presence of an abundant right pleural effusion. The patient had a right ovarian mass, highly suspect at ultrasonography. Thoracoscopy showed a macroscopically normal pleura with normal cytology and histology. The ovarian tumour was very irregular, but it was found to be benign at histology. This gonadic goitre of the right ovary was entirely removed, and the pleural effusion did not recur.

ABSTRACT

The authors have estimated the maternal plasma volume ine 243 patients by undertaking 282 calculations. The samples were taken often a fairly long time before the delivery, the mean being 4.05 weeks and the maximum being 16 weeks. The results were compared between normal pregnancies and those with hypertension that had no effect on the mother or fetus. The increase in the plasma volume was however significantly far less when the hypertension was severe or when there was intra-uterine growth retardation, either by itself or in association with hypertension. There is a positive correlation between the plasma volume and the fetal weight and thus a good index of normal growth can be worked out. Measuring the plasma volume, therefore, can be used as an early screening technique for severe hypertension with high risk for the fetus.

ABSTRACT

This work reviews 78 case histories of urinary incontinence in women treated surgically. There are two series compared with one another: Burch's operation (indirect colpocystopexy) carried out on 37 patients. The placing of an aponevrotic sling under the neck of the bladder by Cukier's method in 41 patients. Urodynamic studies were carried out in each case systematically before and after operation in order to make a diagnosis and indicate the correct treatment to use in each particular patient. The results as far as the incontinence is concerned, clinically and urodynamically, were good, with a cure rate or a great improvement in 94% to 97.5% of cases. The results seem to be better when strips were used; but these increased the numbers of patients who suffered from dysuria. A protocol for treating these cases of stress incontinences in women can be worked out from this study.

ABSTRACT

Experience gained by the authors in the use of external metacarpo-radial fixation for compound fractures of the lower extremity of the radius has led to their proposal of a technique that is less aggressive to the soft tissues. They employ a systematic approach to the radius and second metacarpal, followed by the use of atraumatic plates applied by means of a simple screw.
