ABSTRACT

BACKGROUND: The prevalence of obesity varies across countries. However, less is known about the geographical, within-country variation. This study investigated and visualized the geographical differences in general obesity defined by body mass index (BMI) and in abdominal obesity defined by waist circumference and waist-to-hip ratio (WHR) in Finland. SUBJECTS AND METHODS: Data for the study consisted of three large population surveys: Health 2000 Survey with a nationally representative sample together with the National FINRISK Study conducted in five areas in 1997 and six areas in 2002. Altogether, 17 816 men and women aged 30-64 years participated in the surveys. In each survey, subjects' weight, height and circumferences of waist and hip were measured. The geographical pattern of mean anthropometric values and obesity prevalence were studied applying a Bayesian hierarchical approach and Geographical Information Systems. RESULTS: Both in men and women, the prevalence of obesity (BMI >/= 30 kg m(-2)) varied little across geographical areas, but it was smaller in cities compared with other areas across the country. In men, the prevalence of abdominal obesity defined both by waist circumference and WHR was higher in western Finland compared with southern and northern Finland. Also in women, the prevalence of abdominal obesity was highest in western Finland, especially as defined by waist circumference. CONCLUSIONS: Geographical variation in BMI was different and less prominent than in waist circumference and WHR. Abdominal obesity was surprisingly high in western Finland, the area seldom investigated. Mapping obesity gives a useful tool for professionals working in the field of health promotion.

ABSTRACT

We performed a nation-wide study on geographical variation in the incidence and prevalence of medicated parkinsonism among the Finns aged > or =30 years using Bayesian spatial conditional autoregressive models. Registry of reimbursed medication for parkinsonism and a prescription database of purchase of these drugs were used to identify the study subjects. They were located by the map coordinates of the place of residence and aggregated into regular 100 km(2) grid cells. A total of 7,190 incident and 10,616 prevalent cases were found. The age-adjusted annual incidence was 32.6/100,000 (95% HDR 31.8-33.4) during the years 1995 to 2000 and prevalence was 268/100,000 (95% HDR 263-274) in 2000. The male to female ratio was 1.45 (95% HDR 1.39-1.51) in incidence and 1.54 (95% HDR 1.47-1.61) in prevalence. There was strong evidence for geographic variation in incidence and prevalence. A zone with high incidence and prevalence was identified in the eastern and central part of Finland. There was no evidence for difference in incidence and prevalence between urban and rural areas. The marked (more than two-fold) geographic variation can hardly be caused solely by practices of the registration and collection of data on diagnosis or by methodological issues, but rather suggests to geographic variation in protective and predisposing factors of Parkinsonism in Finland.

ABSTRACT

Several epidemiologic studies have shown an association between calcium and magnesium and coronary heart disease mortality and morbidity. In this small-area study, we examined the relationship between acute myocardial infarction (AMI) risk and content of Ca, Mg, and chromium in local groundwater in Finnish rural areas using Bayesian modeling and geospatial data aggregated into 10 km times symbol 10 km grid cells. Data on 14,495 men 35-74 years of age with their first AMI in the years 1983, 1988, or 1993 were pooled. Geochemical data consisted of 4,300 measurements of each element in local groundwater. The median concentrations of Mg, Ca, and Cr and the Ca:Mg ratio in well water were 2.61 mg/L, 12.23 mg/L, 0.27 microg/L, and 5.39, respectively. Each 1 mg/L increment in Mg level decreased the AMI risk by 4.9%, whereas a one unit increment in the Ca:Mg ratio increased the risk by 3.1%. Ca and Cr did not show any statistically significant effect on the incidence and spatial variation of AMI. Results of this study with specific Bayesian statistical analysis support earlier findings of a protective role of Mg and low Ca:Mg ratio against coronary heart disease but do not support the earlier hypothesis of a protective role of Ca.
