ABSTRACT

The aim of this study is to evaluate COMT2, COMT3, CYP1B1, and ESR1 gene polymorphisms and occurrence of endometrial polyps. In addition, we intended to evaluate the clinical and epidemiological features of patients with and without the presence of the disease, characterizing the possible risk factors. A cross-sectional study was performed, with a total of 309 women, including 236 in the group of women with endometrial polyp confirmed by hysteroscopy and anatomical pathological examination and 73 in the group of people with diagnostic hysteroscopy without abnormal findings from the macroscopic point of view. Polymorphisms of four genes were studied: COMT2 (rs4680), COMT3 (rs5031015), CYP1B1 (rs1056836), and ESR1 (rs2234693). Polymorphism genotyping was determined using real-time polymerase chain reaction. Considering the results, no differences were identified between the two groups with respect to age, body mass index, diabetes, dyslipidemia, or smoking. The group of women without endometrial polyps showed higher use of hormone therapy than the other group (16.4 percent versus 3.8 percent, p < .001). The COMT2, COMT3, CYP1B1, and ESR1 genes exhibited no significant difference for the occurrence of endometrial polyp between the two groups. The research concluded that no correlation was identified between the genetic polymorphisms evaluated and the presence of endometrial polyps.

ABSTRACT

INTRODUCTION: Insulin-like growth factor 1 (IGF-1) is a peptide growth factor that promotes cell proliferation and inhibits apoptosis. The bioavailability of IGF-1 is regulated by the insulin-like growth factor binding protein 3 (IGFBP3). Genetic variations influence the levels of IGF-1 and IGFBP3. The purpose of this study was to examine the association of polymorphisms IGF-1 CA(n) and IGFBP3 rs2854746 with risk of endometrial polyps. MATERIALS AND METHODS: Case control observational study, composed of 104 women with antecedent of endometrial polyp (case group) and 81 postmenopausal women without antecedent of endometrial diseases (control group). Genotyping of IGF-1 CA(n) was performed by PCR and fragment analysis by capillary electrophoresis, and genotyping of IGFBP3 rs2854746 was performed by PCR-HRM. Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated by logistic regression. RESULTS: The genotype IGF-1 CA(19)/CA(19) was associated with an increased endometrial polyp risk (OR=2,57; IC 95%= 1,09 - 6,01); this was also found when combining it with CA(>19)/CA(n) genotypes (OR=2,18; IC 95%= 1,06-4,47). The IGFBP3 rs2854746 analyses showed the CG genotype having a protective effect for endometrial polyp (OR=0,37; IC 95%= 0,19-0,73), fact also observed when grouping CG and GG carriers (OR=0,51; IC 95%= 0,28-0,93). CONCLUSION: The genotypes CA(19)/CA(19) and CA(19)/CA(19) + CA(>19)/CA(n) of the IGF-1 CA(n) may be considered a risk for endometrial polyp, whereas the genotypes CG and CG + GG of IGFBP3 rs2854746 polymorphism have an inverse effect of endometrial polyp risk.

ABSTRACT

Genital leiomyomas are rare tumors that can often be misdiagnosed as Bartholin cyst. We report a case of a 32-year-old patient who had a cystic nodulation in the left labium majus that was suggestive of Bartholin cyst. A resection surgery was performed, and the definitive histopathology diagnosis was vulvar leiomyoma. The macroscopic features of cystic lesions difficult the differential diagnosis between leiomyoma and Bartholin cyst; therefore, a histopathologic examination is often recommended.


Os leiomiomas genitais são tumores raros, e frequentemente são diagnosticados como cisto de Bartholin. Relatamos o caso de uma paciente de 32 anos em que ocorreu nodulação cística no grande lábio esquerdo sugestiva de cisto de Bartholin. Uma cirurgia de ressecção foi realizada, e o diagnóstico histopatológico definitivo foi leiomioma vulvar. As características macroscópicas das lesões císticas dificultam o diagnóstico diferencial entre o leiomioma e o cisto de Bartholin, de modo que o exame histopatológico é frequentemente recomendado.

INTRODUCTION AND HYPOTHESIS: Stress urinary incontinence (SUI) can occur simultaneously with anterior vaginal prolapse (AVP) in up to 30% of patients. We studied a monoprosthesis that combines prepubic and transobturator arms for simultaneous treatment of AVP and SUI. METHODS: One hundred four women with AVP underwent surgical treatment using a monoprosthesis (NAZCA TC). POP-Q was used for anatomical evaluation. Functional results were evaluated by quality of life questionnaires, stress test and Stamey score. Patients were evaluated at 1, 3, 6 and 12 months postoperatively. RESULTS: There were significant and sustained improvements in points Aa, Ba and C. Positive stress test was observed in 29.8% preoperatively and decreased to 1.9% after 12 months. There was no significant impact in sexual symptoms. Mesh exposure was noted in 5.7% of patients. CONCLUSIONS: Monoprosthesis with combined prepubic and transobturator arms presented high success rates for AVP repair and simultaneous SUI treatment. It has also been shown to be safe and appears to preserve sexual function.

ABSTRACT

INTRODUCTION AND HYPOTHESIS: This study aims to analyze comparatively the efficacy and safety of synthetic transobturatory and aponeurotic retropubic slings, in the treatment of stress urinary incontinence (SUI) in women. METHODS: Patients were separated in a randomized way. Twenty-one patients were submitted to the operatory correction by the transobturatory sling technique, whereas 20 patients were operated by the retropubic sling technique. All patients were submitted to complete physical exam and urodynamic test. The "T" test and the Mann-Whitney U test were applied to establish comparisons between the two groups. Patients were followed-up for 12 months. RESULTS: Healing rate was 90.5% (19/21) and 95% (19/20), respectively after 12 months. The transobturatory group presented lesser complications rate than the retropubic group. CONCLUSIONS: The transobturatory and the aponeurotic slings techniques were equally effective for the treatment of SUI. The transobturatory sling has shown fewer complications and lesser surgical time than the aponeurotic sling technique.

ABSTRACT

Safyre-t é um sling híbrido, utilizado no tratamento cirúrgico da incontinência urinária aos esforços (IUE), constituído de faixa monofilamentar de polipropileno entre duas colunas de silicone auto-fixável. A utilização da via transobturatória vem ganhando espaço no tratamento cirúrgico da IUE, devido ao fato desta via apresentar menores índices de complicações e maior reprodutibilidade cirúrgica. As publicações internacionais têm monstrado um interesse crescente em relação a esta nova via. Os resultados demonstram índices de cura semelhantes aos slings retro-púbicos, tidos como "padrão-ouro" para tratamento da IUE. O fato de se tratar de uma intervenção minimamente invasiva beneficia as pacientes em relação ao menor tempo cirúrgico, menor período de internação e não há necessidade de realização de cistoscopia intra-operatória. Os resultados aqui apresentados confirmam os benefícios acima citados

ABSTRACT

O prolapso de cúpula vaginal continua sendo um dos maiores desafios da ginecologia cirúrgica contemporânea. Quando se estudam as técnicas consagradas pelas meta-análises, considerando as recidivas e o tempo de seguimento, verifica-se que apenas duas delas conseguem bons resultados a longo prazo: colpossacro fixação (abdominal ou laparoscópica) e colpopexia sacroespinhal. Outras técnicas não conseguem manter os resultados iniciais e tornam-se parte da história cirúrgica atual. Neste estudo aborda-se uma técnica minimamente invasiva, promissora e anatômica; conhecida como sacropexia infracoccígea, que utiliza material sintético (polipropileno multifilamentar) nos casos de prolapso da cúpula vaginal. Este tema é importante no contexto atual, pois essas técnicas são reprodutíveis e de baixo custo, além de propiciarem menor tempo de hospitalização, retorno mais rápido às funções normais e melhorarem a qualidade de vida destas pacientes que, dia a dia, tornam-se mais longevas
