ABSTRACT

Mudanças no estilo de vida global levaram a comportamentos alimentares disfuncionais durante a gravidez. Nesse sentido, a abordagem Mindful Eating é uma prática com potencial para influenciar positivamente os hábitos alimentares, com impacto benéfico na saúde das gestantes. Para avaliar os níveis de Mindful Eating de mulheres antes e durante a gravidez e associá-los ao tipo de assistência pré-natal (Sistema Único de Saúde ou Setor Privado), foi realizado um estudo transversal com 184 gestantes com idade ≥18 anos, acompanhadas por Sistema Único de Saúde (SUS) ou Setor Privado no município de Lavras-MG, Brasil. Foram coletados dados socioeconômicos e obstétricos e informações sobre Mindful Eating foram obtidas por meio do questionário autoaplicável de Mindful Eating. O Mindful Eating foi maior durante a gravidez do que durante o período pré-gestacional. As gestantes atendidas pelo SUS apresentaram menor nível de Mindful Eating em comparação às mulheres do setor privado. As atendidas pelo SUS obtiveram maiores valores durante o período gestacional na subescala Emocional. As gestantes que atenderam no setor privado obtiveram valores mais elevados durante o período gestacional nas subescalas Consciência, Distração e Desinibição. Durante a gravidez, as mulheres apresentaram níveis mais elevados de Mindful Eating, indicando uma melhoria no comportamento alimentar. A pontuação do Mindful Eating das mulheres atendidas no pré-natal pelo Setor Privado foi superior a das mulheres atendidas pelo SUS, necessitando de intervenção individualizada em cada setor para abranger os fatores socioeconômicos inerentes a cada grupo e sua influência na melhoria dos comportamentos alimentares.


Changes in the global lifestyle have led to dysfunctional eating behaviors during pregnancy. In this sense, the Mindful Eating approach is a practice with the potential to positively influence eating habits, with a beneficial impact on the health of pregnant women. To evaluate the levels of Mindful Eating of women before and during pregnancy and to associate them with the type of prenatal care (Unified Health System or Private Sector), a cross-sectional study was conducted with 184 pregnant women aged ≥18 years, accompanied by the Unified Health System (SUS) or the Private Sector in the municipality of Lavras-MG, Brazil. Socioeconomic and obstetric data were collected, and information on Mindful Eating was obtained through the self-administered Mindful Eating Questionnaire. Mindful Eating was higher during pregnancy than during the pre-gestational period. Pregnant women assisted by SUS showed lower Mindful Eating compared with women in the private sector. Those attended by SUS obtained higher values during the gestational period in the Emotional subscale. Pregnant women who attended in the private sector obtained higher values during the gestational period in the Awareness, Distraction and Disinhibition subscales. During pregnancy, women presented higher levels of Mindful Eating, indicating an improvement in eating behavior. The Mindful Eating score of women attended in prenatal care by the Private Sector was higher than that of women attended by SUS, requiring individualized intervention in each sector to cover the socioeconomic factors inherent to each group and their influence on the improvement of eating behaviors.

Estudo ecológico, de série temporal, que teve como objetivo analisar a tendência do estado nutricional de gestantes adolescentes assistidas pela atenção primária à saúde em Minas Gerais. Os dados foram provenientes do Sistema de Vigilância Alimentar e Nutricional, referente ao estado nutricional de 140.969 gestantes adolescentes entre os anos de 2010 a 2019; apresentados em variação percentual anual por meio da regressão linear simples. O baixo peso mostrou tendência decrescente nas macrorregiões Sul e Triângulo do Norte com variação anual de -0,88% (IC95% -1,67; -0,09) e -0,79% (IC95% -1,06; -0,51) e o excesso de peso registrou tendência crescente nas macrorregiões Nordeste, Noroeste, Norte, Triângulo do Sul e Vale do Aço com a variação anual de 0,28% (IC95% 0,04;0,52), 0,37% (IC95% 0,23;0,51), 0,64% (IC95% 0,32;0,95), 0,44% (IC95% 0,18;0,70) e 0,57% (IC95% 0,26;0,88), respectivamente. Conclui-se que, no período avaliado, houve a tendência de aumento do excesso de peso entre as gestantes adolescentes no estado de Minas Gerais, acompanhando o processo de transição nutricional no país (AU).


An ecological and time-series study to analyze trends in the nutritional status of pregnant adolescents assisted in Primary Health Care in Minas Gerais. The data came from the Food and Nutrition Surveillance System, referring to the nutritional status of 140,969 pregnant adolescents between 2010 and 2019, presented in annual percentage change using simple linear regression. Low weight showed a decreasing trend in the South and Northern Triangle macro-regions with annual variations of -0.88% (95% CI: -1.67;-0.09) and -0.79% (95% CI: -1.06;-0.51); in turn, excess weight recorded an increasing trend in the Northeast, Northwest, North, Southern Triangle and Vale do Aço macro-regions, with annual variations of 0.28% (95% CI: 0.04;0.52), 0.37% (95% CI: 0.23;0.51), 0.64% (95% CI: 0.32;0.95), 0.44% (95% CI: 0.18;0.70), and 0.57% (95% CI: 0.26;0.88), respectively. It is concluded that, during the evaluated period, there was an increasing trend in excess weight among pregnant adolescents in the state of Minas Gerais, following the nutritional transition process in the country (AU).

ABSTRACT

Abstract Objectives: to investigate sociodemographic and economic factors associated with food insecurity among pregnant women assisted by the universal healthcare network of Lavras, Minas Gerais. Methods: a cross-sectional study investigated socioeconomic and demographic, obstetric, and nutritional conditions experienced by pregnant women. Households in which pregnant women lived were classified as food secure or food insecure using the Brazilian Food Insecurity Scale. Prevalence estimates and prevalence ratios with 95% confidence intervals were generated to test for associations between food insecurity and several socioeconomic and demographic indicators using Poisson regression analysis. Results: among 173 pregnant women who participated in the study, 48% lived in households with some level of food insecurity. Adjusted models showed that pregnant women living in food-insecure households had higher prevalence ratios of lower education attainment (aPR = 1.43, CI95% = 1.07-1.91), woman being the head of the household (aPR = 1.39, CI95% = 1.02-1.87), having family monthly income lower than 1 ½ MW (aPR = 1.68, CI95% = 1.11-2.52) and participating in the government cash transfer program (aPR = 1.47, CI95% = 1.08-1.99). Conclusions: the high prevalence of food insecurity in pregnant women assisted by the public healthcare system was associated with structural social factors. Results of this study will contribute to develop an intersectoral health and nutrition policy in order to promote food security among marginalized communities and vulnerable populations, such as pregnant women.


Resumo Objetivos: investigar fatores sociodemográficos e econômicos associados à insegurança alimentar entre gestantes atendidas pelo sistema único de saúde de Lavras - Minas Gerais. Métodos: características socioeconômicas, demográficas, obstétricas e nutricionais de gestantes foram coletadas em estudo transversal. Os domicílios das gestantes foram classificados em segurança ou insegurança alimentar utilizando a Escala Brasileira de Insegurança Alimentar. Estimativas de prevalência e razão de prevalência com intervalos de confiança de 95%, testaram associações entre insegurança alimentar e indicadores socioeconômicos e demográficos utilizando modelos de Poisson. Resultados: entre as 173 gestantes participantes deste estudo, 48% viviam em domicílios com algum nível insegurança alimentar (IA). Modelos ajustados indicaram que gestantes em IAapresentaram maior prevalência de escolaridade inferior a oito anos de estudo (aRP = 1.39, IC95% = 1.07-1.91), ser chefe de família (aRP = 1.39, IC95% = 1.02-1.87), ter renda mensal inferior a 1½ salário mínimo (aRP = 1.68, IC95% =1.11-2.25), e ser participante do Bolsa Família (aRP = 1.47, IC95% = 1.08-1.99). Conclusão: a alta prevalência de insegurança alimentar em gestantes do sistema público de saúde de Lavras está associado a fatores socio-estruturais. Os resultados desta pesquisapoderão contribuir com o desenvolvimento de políticas intersetoriais de saúde e nutrição para a promoção da segurança alimentar entre comunidades marginalizadas e populações vulneráveis.

Resumo A Terapia Nutricional Enteral Domiciliar é um método eficaz para garantir o aporte nutricional de um indivíduo em seu domicílio. Sendo assim, o objetivo do estudo foi investigar os aspectos clínicos comumente recorrentes em pacientes pediátricos em terapia nutricional enteral domiciliar. A pesquisa de artigos foi realizada por meio das bases de dados: Scientific Eletronic Library Online (SciElo) e US National Library of Medicine National Institutes of Health (PubMed) utilizando as seguintes palavras-chaves: 'home enteral nutrition', 'home tube feeding' e 'children'. Foram selecionados 16 estudos realizados nos países: Brasil, Chile, Espanha, França, Holanda, Itália, Polônia e Monte Sinai, os quais trouxeram evidências que as principais condições clínicas relatadas para a indicação na terapia nutricional enteral domiciliar são as doenças neurológicas, neoplasias em geral e doenças do trato gastrointestinal. Apenas dois estudos abordaram os aspectos negativos da terapia domiciliar que são os episódios de engasgos, tosse, vômitos, diarreia e constipação intestinal. Deste modo, por meio das condições clínicas relatadas observa-se que a indicação da TNED é essencial para estes pacientes, podendo preveni-los da desnutrição hospitalar, diminuição do tempo de permanência, reinternações e redução dos custos hospitalares, além de trazer maior conforto para o paciente pediátrico por estar mais próximo dos familiares.


Abstract Home Enteral Nutrition is an effective method to ensure the nutritional compliance of an individual at home. The aim of this study was investigate the clinical aspects commonly recurrent in children and youth patients in home enteral nutrition. Research of articles was carried out through the databases: Scientific Electronic Library Online (SciElo) and US National Library of Medicine National Institutes of Health (PubMed) using the following keywords: 'home enteral nutrition', 'home tube feeding' and 'children'. Sixteen studies were selected in countries as Brazil, Spain, France, Netherlands, Italy, Poland and Monte Sinai, in which it was presented evidence that the main clinical conditions related to enteral nutritional therapy are neurological diseases, neoplasia in general and the upper and lower gastrointestinal tract diseases. Only two studies had reported the negative aspects of home therapy as episodes of gagging, coughing, vomiting, diarrhea, and constipation. Thus, through the clinical conditions reported, it is observed that the indication of home enteral nutrition is essential for these patients, which can prevent them from hospital malnutrition, decrease length of stay, rehospitalizations and reduce hospital costs, as well as bring greater comfort to patients by being closer to their family.

INTRODUCTION: Baby-led weaning (BLW) is an alternative approach to introducing food to infants that has become popular. OBJECTIVE: To investigate the scientific evidence regarding the BLW method and its modified version, Baby-Led Introduction to Solids (BLISS). METHODS: The research was carried out between June 2018 and June 2019, through the databases US National Library of Medicine National Institutes of Health (PubMed) using the following keywords: 'BLW', 'Baby-led weaning', 'BLISS', and 'Baby-Led Introduction to Solids'. All original articles from in English published between 2011 and 2019 have been included. RESULTS: Twenty-five studies were included in the analysis. Research on BLW/BLISS focused mainly on the themes: characteristics, behaviors and attitudes of mothers and children, knowledge of mothers and health professionals about feeding methods, weight and body mass index evaluation of children, evaluation of energetic and nutrient intake of children and evaluation of episodes of choking and gag reflex. CONCLUSION: Most research deals with BLW-related experiences and there is still little evidence on the method in terms of adequacy of energy and nutrient intake and growth in childhood. Positive points to child can be observed such as prolonged breastfeeding, greater independence when feeding and better response to satiety. The methods of food introduction do not differ in relation to the occurrence of episodes of choking.


INTRODUÇÃO: O Baby-led weaning (BLW) é uma abordagem alternativa para a introdução alimentar aos lactentes que tem se tornado popular. OBJETIVO: Investigar as evidências científicas a respeito do método BLW e sua versão modificada Baby-Led Introduction to Solids (BLISS). MÉTODOS: A pesquisa de artigos foi realizada entre junho de 2018 e junho de 2019, por meio da base de dados US National Library of Medicine National Institutes of Health (PubMed) utilizando as seguintes palavras-chaves: 'BLW', 'Baby-led weaning', 'BLISS', e 'Baby-Led Introduction to Solids'. Foram incluídos todos os artigos originais encontrados no idioma inglês, publicados entre 2011 e 2019. RESULTADO: Vinte e cinco estudos foram incluídos na análise. As pesquisas sobre BLW/BLISS focaram principalmente nos temas: características, comportamentos e atitudes de mães e crianças, conhecimento de mães e profissionais de saúde sobre os métodos de introdução alimentar, avaliação de peso e de índice de massa corporal, avaliação da ingestão energética e de nutrientes e avaliação de episódios de engasgo e reflexo de gag das crianças. CONCLUSÃO: A maioria das pesquisas aborda experiências relacionadas ao BLW/BLISS e ainda existem poucas evidências sobre o método em termos de adequação de ingestão de energia e nutrientes e de crescimento na infância. Pontos positivos para a criança podem ser observados como aleitamento materno prolongado, maior independência ao se alimentar e melhor resposta à saciedade. Os métodos de introdução alimentar não diferem entre si em relação à ocorrência de episódios de engasgos.

BACKGROUND: Food allergy is an abnormal immune response to antigens introduced into the body through food. Its prevalence has increased in developed and developing countries. Natural products are traditionally used to alleviate and treat diseases, and diet can play a role in both the prevention and management of food allergy. The effects of capsaicin as an anti-oxidant, anticarcinogenic, and anti-inflammatory in the energy expenditure and suppression of fat accumulation have been demonstrated. This study evaluated the effect of oral supplementation with capsaicin on a food allergy model. METHODS: OVA-sensitized mice received ovalbumin solution, and they were fed with chow supplemented with capsaicin for 7 days. The control group received AIN-93 chow with no supplementation. IgE anti-ova, inflammatory infiltration, oxidative stress and metabolic analysis were performed. RESULTS: The results showed that capsaicin supplementation is not able to reduce characteristic signs of food allergy, such as production of IgE and weight loss. However, macrophages infiltration and IL-33 in proximal jejunum was reduced in OVA capsaicin group. In addition, hepatic triglycerides and intestinal hydroperoxides were reduced in both capsaicin groups. CONCLUSION: Oral supplementation with capsaicin attenuated important factors associated to food allergy such as inflammation and oxidative stress, suggesting better prognosis and evolution of the disease.

BACKGROUND: Mucositis is a common complication in patients undergoing radiotherapy and chemotherapy. It is associated with pain, poor quality of life, and malnutrition, leading to an increased number of hospital admissions and prolonged hospitalization. The use of immunonutrients may be an alternative treatment option, which may help to improve patient outcome. OBJECTIVE: Here we assessed the impact of L-citrulline (CIT) on a murine model of 5-fluorouracil (5FU)-induced mucositis. METHODS: Swiss male mice were randomized into 4 groups: control, CIT, 5FU, and 5FU+CIT. Mice were fed with commercial chow and supplemented with an oral solution of alanine (control and 5FU groups) or CIT (CIT and 5FU+CIT groups). On the seventh day, mice received intraperitoneal phosphate-buffered saline or 5FU (200 mg/kg, single dose) to induce mucositis. On the 10th day, mice were euthanized, and the blood and small intestines were harvested. Body weight, morphology, histopathology score (hematoxylin and eosin) of the small intestine (from 0-12), myeloperoxidase activity, oxidative stress level, and intestinal permeability were assessed. RESULTS: We observed significant weight loss after the administration of 5FU in both treated and control animals. CIT administration contributed to a partial recovery of the mucosal architecture as well as an intermediate reduction of the histopathologic score, and functional intestinal permeability was partially rescued. CONCLUSIONS: CIT administration attenuated 5FU-mediated damage to the mucosal architecture of the small intestine, decreasing the size of the injured areas and promoting decreased intestinal permeability.

Gluten exclusion (protein complex present in many cereals) has been proposed as an option for the prevention of diseases other than coeliac disease. However, the effects of gluten-free diets on obesity and its mechanisms of action have not been studied. Thus, our objective was to assess whether gluten exclusion can prevent adipose tissue expansion and its consequences. C57BL/6 mice were fed a high-fat diet containing 4.5% gluten (Control) or no gluten (GF). Body weight and adiposity gains, leukocyte rolling and adhesion, macrophage infiltration and cytokine production in adipose tissue were assessed. Blood lipid profiles, glycaemia, insulin resistance and adipokines were measured. Expression of the PPAR-α and γ, lipoprotein lipase (LPL), hormone sensitive lipase (HSL), carnitine palmitoyl acyltransferase-1 (CPT-1), insulin receptor, GLUT-4 and adipokines were assessed in epidydimal fat. Gluten-free animals showed a reduction in body weight gain and adiposity, without changes in food intake or lipid excretion. These results were associated with up-regulation of PPAR-α, LPL, HSL and CPT-1, which are related to lipolysis and fatty acid oxidation. There was an improvement in glucose homeostasis and pro-inflammatory profile-related overexpression of PPAR-γ. Moreover, intravital microscopy showed a lower number of adhered cells in the adipose tissue microvasculature. The overexpression of PPAR-γ is related to the increase of adiponectin and GLUT-4. Our data support the beneficial effects of gluten-free diets in reducing adiposity gain, inflammation and insulin resistance. The data suggests that diet gluten exclusion should be tested as a new dietary approach to prevent the development of obesity and metabolic disorders.
