ABSTRACT

At Vila Pouca de Aguiar area, northern Portugal, crops out a post-tectonic Variscan granite pluton, related with the Régua-Vila Real-Verín fault zone, comprising three types of biotite granites. Among these granites, PSG granite yield the highest average contents of U, probably due to its enrichment in accessory U-bearing minerals such as zircon. In the proximity of faults and joints, these granites are often affected by different degrees of hydrothermal alteration, forming reddish altered rocks, commonly known as "episyenites". These altered rocks are probably associated to the occurrence of hydrothermal processes, which led to uranium enrichment in the most advanced stages of episyenitization. In these granites, both average gamma absorbed dose rates in outdoor and indoor air are higher than those of the world average. Furthermore, even in the worst usage scenario, all these granites can be used as a building material, since their annual effective doses are similar to the limit defined by the European Commission. The geometric mean of radon activity of 91 dwellings located at the Vila Pouca de Aguiar pluton is 568Bqm(-3), exceeding that of other northern Portuguese granites. Measurements carried out during a winter season, indicate that 62.6% of the analysed dwellings yield higher indoor radon average values than the Portuguese legislation limit (400Bqm(-3)), and annual effective doses due higher than the world's average value (1.2mSvy(-1)). The interaction of geogenic, architectural and anthropogenic features is crucial to explain the variance in the geometric mean of radon activity of dwellings from Vila Pouca de Aguiar pluton, but the role of geologic faults is probably the most important decisive factor to increase the indoor radon concentration in dwellings. Hence, the development of awareness campaigns in order to inform population about the incurred radiological risks to radon exposure are highly recommended for this specific area.

ABSTRACT

Literature suggests that parental cancer can provoke aversive emotional arousal in adult children, who may perceive caregiving as a traumatic experience. Limited research has been conducted on emotional and physiological impact of family caregiving for cancer patients undergoing chemotherapy. The aim of the present study was to examine psychological and physiological responses in parental cancer's caregivers. Two matched groups of adult children, with 78 participants each (parental cancer vs. control), completed psychological measures of distress, post-traumatic stress disorder (PTSD) symptoms, and burden. Additionally, each participant visualised standardised pictures with different emotional valences, while cardiovascular (heart rate) and electrodermal responses (skin conductance) were recorded. Between-group analysis showed significant differences on all psychological variables, and on skin conductance for all types of pictures. However, for the heart rate responses, differences were found only for pictures with unpleasant emotional arousal. In the parental cancer group, the heart rate peak response stood out as a predictor of PTSD symptoms, after controlling for distress and burden. This study highlights the important role of psychophysiological measures of family caregiving in oncology. Physiological responses may explain a higher prevalence of PTSD symptoms. Therefore, biofeedback combined with targeted psychosocial interventions for relaxation could be of great clinical value for this population.

ABSTRACT

The aim of this study was to describe the ovarian structure (OS) and its relationship with hyperandrogenism in girls with premature pubarche (PP). A pelvic ultrasound was carried out in 23 girls with PP and in 57 prepubertal age-matched controls (C), and the OS was subdivided into five classes (c): 1-homogeneous; 2-microcystic, 3-multicystic, 4-polycystic and 5-follicular. In the girls with PP, an ACTH test was performed, and the presence of hormonal levels >3 SD of postpubertal normal levels and not compatible with late-onset congenital adrenal hyperplasia were considered an exaggerated response. The fasting levels of glucose (G) and insulin (I) were measured and the fasting I to G ratio (FIGR) was calculated. FIGR >22 was suggestive of I resistance (IR). The microcystic structure (c2) was more frequently found in the PP than in the C group (63% vs 35%, p=0.03). In the PP group, we observed the following OS: cl (n=6), c2 (n=15), c3 (n=1) and c4 (n=1). 11-Deoxycortisol--both basal and after ACTH--was greater in the PPc2 group than in PPc1 (p=0.04, p=0.0008, respectively). We also observed an exaggerated response to ACTH in 87% of the girls with PP, greater in the PPc2 group than in PPc1 (p=0.04). The FIGR showed IR in 44% of girls with PP, but I levels and FIGR were similar between PPc1 and PPc2. These findings suggest generalized adrenocortical hyperresponsiveness in girls with PP, which is more accentuated in PPc2. Long-term follow-up of girls with PP into adulthood is warranted to ascertain whether microcystic ovarian structure precedes functional ovarian hyperandrogenism.

ABSTRACT

This paper reviews the clinical findings, pituitary gonadotrophin reserve, plasma estradiol and androgens, radiological findings and pelvic ultrasound appearance in 17 girls with true precocious puberty (PP), and attempts to asses the value of these tests diagnosis in the clinical management of such patients and better understanding of the pathogenesis of this disorder. As noted in other series, acceleration of growth is one of the earliest features of PP and at the time of diagnosis bone age can be already significantly advanced. In 3 (18%) patients intracranial abnormalities were present. Ultrasound examination showed changes similar to those seen during normal puberty. To conclude, the introduction of high-resolution methods (CT scan and RM) and techniques for ultrasound examination have greatly simplified the clinical investigation of female precocious puberty.
