ABSTRACT

BACKGROUND: Cerebral stroke is the third leading cause of death after cardiovascular disease, cancer and the leading cause of disability for patients. Hyperbaric oxygen is a non-drug treatment that has the potential to improve brain function for patients with ischaemic stroke. The objective of this study was to evaluate the results of treatment of acute cerebral infarction with hyperbaric oxygen therapy (HBOT). MATERIALS AND METHODS: This was a case-control study. One hundred ninety-five patients diagnosed with cerebral infarction, with signs of onset within 24 hours, were treated at the Centre for Underwater Medicine and Hyperbaric Oxygen of Vietnam National Institute of Maritime Medicine during the period from January 2020 to December 2022. Study group included 100 patients with acute cerebral infarction treated with a combination of HBOT and medication and reference group included 95 patients treated by medication only (antiplatelets drugs, statins, control of associated risks factors) RESULTS: After 7 days of treatment with hyperbaric oxygen (HBO), symptoms such as headache, dizziness, nausea, sensory disturbances, and Glasgow score of the study group improved better than that of the reference group (p < 0.01). Movement recovery in the study group was better than the reference group: the percentage of patients with mild and moderate paralysis in the study group increased higher than that of the reference group (86.0% and 68.4%), the degree of complete paralysis of the study group decreased more than that of the reference group (14.0% and 31.6%). The degree of independence in daily activities in the study group was better than the reference group. In the study group, the percentage of patients with complete independence in daily life increased from 27.0% to 84.0%. In the reference group, the rate of patients who were independent in their daily activities increased from 37.9% to 51.6%. The average number of treatment days of the study group was 10.32 ± 2.41 days and it the reference group 14.51 ± 3.24 days. CONCLUSIONS: Hyperbaric oxygen therapy is a non-drug treatment with many good effects in the treatment of cerebral infarction, especially acute cerebral infarction. HBOT reduces and improves functional symptoms, improves mobility, and reduces treatment time for patients.

ABSTRACT

Background: Dioxin affects nervous system via the system of nervous communicative hormones and endocrine hormones, causing disorders and nervous behavior wrongs. Reproductive health is one of the serious effects of exposing Agent Organe/Dioxin. Objectives: To evaluate the birth defects of the nervous system of children, from the generation of veterans exposed to Agent Orange/Dioxin during wartime and to assess congenital nervous defects related factors, characteristics in these subjects. Subject and methods: A retrospective cohort, interview and examine 12.369 exposed and 7.659 unexposed Hanoi veterans, their birth defective children were examined by the pediatrics. During the 2002-2003 timescale. Results: A statistically high significant rate of reproductive failures and birth defects for exposed veterans, compared to unexposed ones, spontaneous abortion with relative risk RR = 4.18, hydatidiform mole with RR = 3.0, still births with RR = 2.2, birth defects with RR = 2.12. Nervous birth defects were highest; representing more than 40% of live births; the relative risk of the exposed group is 2.3 fold higher than the unexposed group, with p < 0.01. The birth defects increased significantly: mild mental retardation with RR = 3.5, severe mental retardation with RR = 3.5, Down's syndrome with RR = 2.16. Conclusions: Nervous birth defects of children of the Hanoi veterans exposed to Agent Orange during wartime accounted for the highest rate. Congenital nervous defects in male were higher than female. Exposed time, exposed levels and direct exposure to Agent Orange were significant related factors in this study. The increase of birth defects was in correlation with exposed levels.

ABSTRACT

Background: Chromosome mutation type t(8;21) has quite a high frequency in acute myelogenous leukemia, which accounted for about 15% among adult patients. From 2001, the WHO has a new classification for acute myelogenous leukemia based on genetic mutations. Form had AML1/ETO were arranged into genetic mutation group with better prognosis and ability to fully recover after chemotherapy with a high dose of cytarabin. Objective: Study AML1/ETO fusion gene on the patients diagnosed with Acute Myelogenous Leukemia (AML), as well as the clinical features and some haematologic parameters of the AML1/ETO positive group. Subject and methods: 76 patients with AML were treating in the National Institute of Hematology & Blood Transfusion and the Department of Hematology & Blood Transfusion of Bach Mai Hospital from April 2007 to July 2008. These patients were studied for clinical examination, morphology and RNA were extracted from leukemic cells and PCR for AML1/ETO fusion transcript was performed. Results and conclusions: The incidence of AML1/ETO positive in the AML patients was 24%. The incidence of AML1/ETO positive in AML-M2 was 28%. In the AML1/ETO positive group: median age was 26.94+/-9.22; rate of severe anemia, hemorrhage, fever, infection, hepatomegaly, splenomegaly, lymphadenopathy and gum hypertrophy was 44%, 33%, 28%, 11%, 44%, 28%, 17% and 6%, respectively. Median hemoglobin, WBC, platelet, bone marrow cell count, % blast in peripheral blood and in bone marrow was 84.41+/-28.97 g/l, 29.42+/-31.36 g/l, 42.12+/-33.83 g/l, 215.93+/-134.42 g/l, 56.21+/-26.58% and 65.14+/-16.12%, respectively.

ABSTRACT

Background: In Vietnam, there are a number of studies on the application of ATRA in treating acute promyelocytic leukemia (AML – M3) but they have still faced with certain difficulties. Objectives: (1). Study PML/RAR alpha fusion gene on the patients diagnosed with AML – M3. (2). Study the index of hematology of the PML/RAR alpha positive group. Subject and Method: 21 patients with acute promyelocytic leukemia (M3) were studied. All patients were examined with morphology, coagulation and cytogenetic tests and RNA were extracted from leukemic cells and PCR for PML/RAR alpha fusion transcript. Result and conclusion: PML/RAR alpha positive in 67% including 4 patients which were not discovered t(15; 17) by cytogenetic technique. Rates of three subtype (bcr1, bcr2 and bcr3) of PML/RAR alpha were 7 patients (50%), 3 patients (21,5%) and 4 patients (28,5%), respectively. WBC and bone marrow cells of PML/RAR alpha positive group were 5.08+/-3.87 and 155.82+/-106.21. D – Dimer level was 1954.89+/-1575.28; 93% of patients in the PML/RAR alpha positive group had DIC.

ABSTRACT

Background: In a chemical war during 1961-1971, about 94,000 tons of the herbicides were sprayed over Southern Viet Nam.A main component of the herbicides is dioxin that is very harmful to humans. There are many studies focusing on people’s health due to exposure to dioxin residuals. Objective: To investigate the health issues related to exposure to dioxin residuals. Subjects and method: A retrospective comprehensive survey performed on 4018 people in Cam Chinh commune, Cam Lo district, Quang Tri province and compared with 3987 people in Cam Phuc commune, Cam Xuyen district, Ha Tinh province (controls) based on epidemiological interviews. Results: The morbidity was higher at 6.4% in the herbicides-applied area than in the herbicides-non applied area. In herbicides-applied area, there was a high risk of diseases such as bone connective, genitourinary, blood endocrine system, skin and infectious diseases. Factors such as age, sex, familiar income, drinking, smoking and agricultural chemical usage contributed towards the health of the affected population. Conclusions: Agent orange/dioxin pollutions still have the potential to be hazardous to human health in the herbicides-applied areas.
