ABSTRACT

The success of adaptation of students to conditions of educational process in Medical University is determined by complex of accompanying socio-psychological factors. The level of personal anxiety determines severity of impact of these factors on psychosomatic state of student. The high level of anxiety among students in conditions of educating in Medical University can significantly and multi-componently impact their physical organism, success in teaching and social well-being. The purpose of the study is to determine prevalence of high levels of anxiety and their impact on health and social well-being of students of different genders, periods of study on budgetary and commercial basis. There is significant prevalence of high levels of personal anxiety among students of first year of education (57.0%), which tends to increase by fourth year (75.5%). The increased anxiety has significant impact on morbidity (correlation with anxiety levels τ=0.69 and τ=0.40 for men and women, respectively) and social well-being of students, their behavior and lifestyle. The female students are the most affected by anxiety factor. The belonging of students with high level of anxiety to budget or commercial group determines specificity their response to conditions of education in the Medical University. The obtained results should be used to improve efficiency of adaptation and psychological support of students in the process of education in Medical University. The high level of anxiety in students should be taken into account in preventive activities of preserving health of students as separate factor, impacting similarly with strong health risk factor on their physical organism and social well-being, considering gender and period of study on budgetary or commercial basis.

ABSTRACT

The motivation of work is based on personnel needs. The directivity of organization management to meeting needs of employees of any professional group is capable to increase their labor motivation and job satisfaction creating at that favorable working environment. The purpose of the study is to determine value of motivational factors-needs for employees of professional groups of medical organization and to justify application of obtained motivational complexes of factors-requirements for increasing labor motivation of personnel. The continuous anonymous survey of 603 employees of six departmental medical organizations was carried out. The respondents were distributed in such professional groups as Administration (42); Experts (97); Nurses (257); Other professionals (86) and Paramedical staff (121). The group Specialists included subgroups Surgical (13) and Therapeutic (84 people) profiles. The group Other specialists included subgroups with higher (51 people) and secondary education (40 people). The factors-needs determining work motivation were estimated by P. Martin and S. Ritchie "Motivational profile Test". The statistical data processing was carried out using "Statistica-10" software. The criteria of Kruskal-Wallis, Kolmogorov-Smirnov and Kendall τ-criterion were applied. The motivational value of factors-needs in employees of professional groups of medical organization, including characteristics of estimates of needs respondents with different work experience considering their gender and marital status is defined. The hierarchical structure of factors-needs according to degree of their motivational value in employees of each professional group can be represented in form of group motivational complex. The indices of motivational value of factors-needs of complex correspond to specific expectations of employees of professional groups. The derived motivational factors-needs complexes can be applied in determining directions of management activities targeted to increasing of work motivation of personnel, creating favorable social and psychological work environment and establishing social responsibility of organization in terms of personnel.

ABSTRACT

Strictures of vesicourethral anastomosis (VUA), urethral strictures, and bladder neck obliteration are frequent complications occurring after treatment for prostate cancer and dramatically reducing the quality of life of the patients. To date, there is no single standard treatment of urethral strictures. One of the promising methods is laser optical urethrotomy using a solid-state Ho:YAG- laser. Since 2012, we treated 12 patients with strictures of VUA, urethral strictures, and bladder neck obliteration. According urethrography, the maximum length of stricture was 4.5 cm. Treatment efficacy was assessed at 6 months after surgery objectively according urethrography, uroflowmetry, and ultrasound of the bladder with the definition of residual urine; and subjectively--by IPSS questionnaire and QoL questionnaire. After removal of the urethral catheter, all patients had recovered independent urination, decreased IPSS scores by 59.5%, IPSS-QoL score by 45.87%, decreased residual urine volume by 89.92%, and increased maximum urinary flow rate by 78.19%. Intraoperative complications and early postoperative complications were not observed. Ho:YAG laser is a minimally invasive and safe tool for urethrotomy of strictures of VUA, urethral strictures, and bladder neck obliteration arising after treatment for prostate cancer. Definitive conclusions about the effectiveness of this method should be based on long-term results of comparative trials.

A procedure was evolved and tested for forming a urinary bladder from the sigmoid in the course of radical cystectomy carried out for invasive bladder cancer (20). A sigmo-sigmoid anastomosis ("end-to-side") was used by means of an apparatus suture.

ABSTRACT

Were examined characteristics of physical development and preparedness of physicians with different value of successfulness of activity in extreme conditions for the period of their first specialization in internship training. Was shown that to the end of study physiological resources of physicians decrease, it could have negative consequences on their activity in extreme conditions. Was made a conclusion that it's necessary to add lessons of physical preparedness, when the main attention would be paid to development of force qualities.

ABSTRACT

To predict the possibilities of use of graduates from civil medical higher schools for bringing up to strength the posts of Russian Federation Armed Forces medical service troop link the sociological cross investigation was conducted. The poll by questionnaire was used in 7522 students of both sexes from 35 Russian civil higher schools. The results of investigation have shown the presence of reserve for bringing up to strength the posts of army and navy medical service at the expense of students from civil higher schools. The rise of social attractiveness of military service by solving the problems socially significant for responders will allow providing the necessary competition among the candidates for substitution of officers' posts--military physicians. At the same time the specific character of military physicians' activities requires the maintenance and further development of its own successfully functioning system for specialists' training.

ABSTRACT

Lately more and more attention is given to the indices of efficiency and safety of operators' activities because of their special complexity and mistake value. Operators' work is connected with intensive processing of encoded information and is accompanied with development of significant fatigue, stereotype cyclic alternation, changes in work and rest regimen--with necessity of working at night. Effect of circadian biorhythms considerably influences on characteristics of operators' activities. The existing organizational approach considers the operator only as being present or absent at working place. The fact that operator can be present but his working capacity is not sufficient is taken into account not enough and that's why requires consideration of operators' work and elaboration of organizational measures.
