ABSTRACT

OBJECTIVE: The aim is to study the dynamics of prostaglandins and cytokines in the blood and gingival fluid against the background of differential drug correction in patients with generalized periodontitis with different body reactivity. PATIENTS AND METHODS: Materials and methods: 216 people aged 45 between 55 years with a diagnosis of generalized periodontitis of II, III degree of severity, chronic course were examined. Depending on the state of reactivity of the organism, the patients were divided into three groups: the first one consisted of people with normoreaction; the second group included patients with hyperreaction; the third group was made up of people with hyporeaction. The patients underwent patch surgery after the initial therapy. Initially, on the 1st, 2nd, 4th, 6th and 9th day after the operation, the content of prostaglandins (PG) E, E2, F2alpha and cytokines (IL-1ß, IL-6, TNF, IL-4) in the blood and gingival fluid was performed. Patients with impaired body reactivity were treated with the proposed differential drug correction of cytokines and prostaglandins. Statistical processing of the obtained digital data was performed using the computer program Statistica 8.0. RESULTS: Results: The proposed differential drug correction in patients with generalized periodontitis against the background of hyper- and hyporeactivity of the body brings the content of IL-1ß, IL-6, TNF, IL-4 to that of normal body reactivity, which ultimately restores the disturbed balance of pro- and anti-inflammatory cytokines in the blood and gingival fluid. On the 9th day, the content of all proinflammatory cytokines in the main groups was normalized and was commensurate with that of the body's normal response (p>0.05). Differential drug correction led to normalization of the prostaglandin balance index on day 9 after flap surgery (p>0.05), indicating the establishment of a normal balance of eicosanoids in the blood and approximating the values of pro- and anti-inflammatory fractions of prostaglandins to those of normal body reactivity. CONCLUSION: Conclusion: Correction of altered parameters in patients with generalized periodontitis accompanied by impaired (hyper- and hypo-) reactivity of the body with bringing them to values that are typical for normoreactivity is considered to be a condition for optimizing mucosal wound healing after surgery and further stabilization of periodontal tissues.

ABSTRACT

OBJECTIVE: The aim: To study the features of clinical wound healing (intensity and duration of inflammatory reaction) after the indications for patients with generalized periodontitis II, III degrees of severity of patch surgery accompanied by normo-, hyper- and hyporeactivity of the body. PATIENTS AND METHODS: Materials and methods: 216 people aged 45 between 55 years with a diagnosis of generalized periodontitis of II, III degree of severity, chronic course were examined. Depending on the condition of reactivity of the organism, the patients were divided into three groups: the first one consisted of people with normoreaction; the second group included patients with hyperreaction; the third group was made up by people with hyporeaction. The patients after the initial therapy, underwent patch surgery. Initially, on the 2nd, 4th, 6th and 9th day after the operation, the clinical examination of the periodontal tissues (examination, hygienic and periodontal indices) was performed. Statistical processing of the obtained digital data was performed using the computer program Statistica 8.0. RESULTS: Results: Normoreactivity of the body determines the normal time of wound healing after patch surgery, which on the 9th day is characterized by the clinical condition of the gingival mucosa, which corresponds to the concept "healthy" for 98% of patients. In case of hyperreactivity of the organism slowing down of processes of wound healing for 3-4 days in comparison with the time which is typical for normoreactivity with dynamics of the indices which specifies long intensive character of inflammatory reaction in the postoperative period has been observed. In case of hyporeactivity of the body as well as in case of hyperreaction, there was a slowdown in wound healing for 3-4 days compared with normoreactivity with the dynamics of indices that showed slow sluggish nature of the inflammatory reaction in the postoperative period. CONCLUSION: Conclusions: Correction of altered parameters in patients with generalized periodontitis accompanied by impaired (hyper- and hypo-) reactivity of the body with bringing them to values which are typical for normoreactivity is considered to be a condition for optimizing mucosal wound healing after surgery and further stabilization of the periodontal tissues.

ABSTRACT

OBJECTIVE: The aim: The purpose of the study is to characterize the influence of quantitative and qualitative composition of gingival microbiota on the status of the main immune system cells, localized in the gums, in chronic generalized catarrhal gingivitis in children. PATIENTS AND METHODS: Materials and methods: The study involved 26 children aged 9 to 16 years, patients with chronic generalized catarrhal gingivitis mild to moderate severity (CGCG) and 18 children with intact gums were comparison group. We determined the hygienic indices Fedorov, has been received, Silness-Loe, PMA, bleeding index for Myuleman and intensity of caries index CFD + cf, CFD. Histological and immunohistochemical studies were performed on serial sections kriostatnyh who made biopsy of gingival papillae. Microbiological study gingival part of crown plaque was performed by multiplexed PCR in real time. RESULTS: Results: Value hygienic indices in children with CGCG higher than in healthy, indicating the difficulty of care in the presence of periodontal inflammation. As a result of immunohistochemical studies revealed that HLA-DR + cells under conditions of active disease migrate to mucosal lamina propria epithelium. Number of CD3 + cells in the epithelium CGCG was significantly higher than the number in the intact epithelium and was the most numerous of population. In the biopsy of affected children significantly reduced the number of CD4 + cells. When CGCG quantitative total bacterial mass, Lactobacillus spp., Enterobacteriaceae, Gardnerella vaginalis / Prevotella bivia / Porphyromonas spp. in the sample CROWN dental plaque was significantly higher than rates under physiological conditions, and may serve as diagnostic criteria of dysbiosis. CONCLUSION: Conclusions: So, CGCG is a disease in the etiology of which is one of the leading roles played by microbial factor, namely, the value of the quantitative ratios of certain types of microorganisms of dental plaque compared to the total bacterial mass of plaque. Therefore, it is reasonable to include comprehensive treatment CGCG drugs in children, leading to natural immunostimulation which causes restoration of local immunity in the gum tissue and drugs to restore quantitative and qualitative composition of normal microflora of the child, thus providing a high therapeutic effect and serve as justification their choice.

ABSTRACT

OBJECTIVE: The aim of out research was to determine the effectiveness of the sealing property of adhesive systems V and VII generations by studying the depth of curing, water absorption and water solubility of their samples. PATIENTS AND METHODS: Materials and methods: The study was performed according to ISO / TS 11405: 2015 at the Central Factory Laboratory of JSC "CTOMA", Kharkiv, Ukraine, Certificate of Attestation No. 100-062 / 2015 dated 03/04/2015. For the study we used specially prepared laboratory samples which were made in the form of plates from adhesive system "DC Adhesive NF" (Dental Central) of V generation "Single bond Universal" (3M ESPE) of VII generation. А monitor and analyze the results obtained used micrometer MK-25, №4694, №03 / 5321 from 08/18/17; caliper SC-1, No. 267447, No. 03/5322 from 08/18/17; scales AP 210, №112144137, №87027 / 9 from 24.11.2017. RESULTS: Results: According to the results of the investigation the following results were obtained: 1) the average curing depth of V and VII generation of adhesive systems was 4.08mm and 3.51mm respectively at the rate of 2mm; 2) average water absorption: 45.97 µg/mm3 and 30.89 µg/mm3 for V and VII generations respectively; 3) the average water solubility was 6.94 µg / mm3 and 4.17 µg / mm3, respectively. CONCLUSION: Conclusions: According to the results of our laboratory studies, it can be concluded that adhesive systems of V and VII generations can be used for restoration of lesions of tooth hard tissued. Along with this, it can be recommended to use V generation adhesives in closed defects where direct polymerization should be performed and oral fluid access is minimal. Adhesive sysyems of VII generation can be recommended in such cases as: opened restorations, pregingival areas of teeth, cases where a permanent contact of restorations with oral fluid be found along with the cases of autonomic application of adhesive system.

ABSTRACT

OBJECTIVE: The aim: To study possibilities of prognosis of pathological wear of tooth hard tissues development depending on the functional activity of masticatory muscles. PATIENTS AND METHODS: Materials and methods: In order to achieve the goal of the study, a survey of patients at the age of 19 to 69 years was conducted for precision of number of persons who have increased tooth wear. During the examination of all patients, three groups were selected: control and two researched ones. Patients in the control group (30 patients between the age of 18 and 60 years old) had an intact dentition without any visible sign of increased tooth wear. The researched groups included 25 patients aged from 18 to 60 years old. The secondresearch group included patients who had increased tooth wear of I-III level. The children of the patients of the II group with or without any sign of increased tooth wear were referred to the third research group. RESULTS: Results: The method of electromyography was performed for the study of peculiarities of the muscular activity of the maxillofacial area of the patients of the control and research groups. In our opinion, all manifestations of increased tooth wear are related to changes in the muscle system, the motor apparatus and the nervous activity of an organism. CONCLUSION: Conclusions: The study of direct muscular apparatus and related bone system may predict the possibility of developing of an increased tooth wear of young patients in the future and prevent its development, as well as to justify the application of a prevention and treatment plan of the studied pathology.

ABSTRACT

The three-component reaction of 5-aminotetrazole with aliphatic aldehydes (formaldehyde, acetaldehyde) and acetoacetic ester derivatives in water under microwave irradiation leads to the selective formation of 4,7-dihydrotetrazolo[1,5-a]pyrimidine derivatives. Under similar conditions using 4,4,4-trifluoroacetoacetic ester 5-hydroxy-4,5,6,7-tetrahydrotetrazolo[1,5-a]pyrimidines are obtained. The analogous reaction with acetylacetone requires scandium(III) triflate as catalyst. The antioxidant activity of selected compounds was assayed with 1,1-diphenyl-2-picrylhydrazyl.

ABSTRACT

OBJECTIVE: Introduction: The condition of hard teeth tissues while choosing treatment method becomes particularly important due to the large development of adhesive technologies and the choice of filling materials. The quality of adhesion is one of the key factors that affects effectiveness of the entire range of therapeutic interventions, that determines the success of therapeutic treatment and the reliability of therapeutic restorations fixation. The aim of the work is to study the condition of hard teeth tissues in carious and non-carious pathology for further rationale of the choice of filling material and adhesive system. PATIENTS AND METHODS: Materials and methods: A thorough examination included teeth that were removed according to the indications. The age of patients, whose teeth were examined, was 25-55 years old according to the classification of age periods adopted at the International Symposium on the Age Periodization (Moscow, 1965). We conducted a morphological study of enamel and dentin of teeth with carious and non-carious lesions (increased erasability, cuneal defects), by obtaining chips and microsections of teeth, followed by examination of the number of enamel prisms, dentinal tubules,their diameter, the thickness of the gaps between these structures. The studies were carried out using a scanning electron microscope (SEM) "Mira 3 LMU" ("Tescan", Czech Republic) with a maximum resolution of 1 nm and a maximum magnification of 1,000,000. Dentally removed teeth were washed in running water, cleaned of plaque, dried by filter paper and crushed by vice, that was followed by the selection of the wreckage for the study. Then, markings in the dentine and enamel zones were made to fix the sizes of enamel prisms and dentinal tubules. RESULTS: Results: After the completion of the determination of enamel prisms and dentinal tubules diameter, counting of their number was carried out according to the method proposed by I.M. Tkachenko, by obtaining a series of digital images of the examined tooth's enamel and dentin with varying degrees of magnification. The number of prisms per unit area characterized the enamel density, on which permeability, hardness, tissue resistance to fracture, bending and modulus of elasticity depends. The number of dentinal tubules characterized the density of dentin and the peculiarities of trophic processes that occur in dentin. Also, in our opinion, the amount of dentinal tubules determines the damping properties of dentin and strength enamel values. CONCLUSION: Conclusions: Thus, the structure of enamel and dentin has peculiarities of the morphological structure at various clinical conditions of hard teeth tissues. This peculiarity can be associated with a decrease of enamel density, that is associated with a decrease in the number of prisms in the non-carious teeth pathology and an increase of the gaps between them.

ABSTRACT

OBJECTIVE: Introduction: Periodontal diseases take second place in the world among dental diseases and tend to be ubiquitous regardless of age, sex and place of residence. According to the modern point of view inflammatory periodontal diseases refer to infectious chronic inflammatory diseases, therefore, the normalization of the microflora of the oral cavity is an essential condition for their rational therapy. The aim: To increase the effectiveness of conservative treatment of chronic generalized periodontitis of the II-nd and III-rd severity using the medication based on the antiseptic Troklosene. PATIENTS AND METHODS: Materials and methods: 34 patients of age 45 to 55 years who were diagnosed with chronic generalized periodontitis of the IIInd and IIIrd degree of severity were examined and treated. All patients reported signs of the disease for 3-15 years. Patients of the study group by simple randomization were divided into two subgroups, depending on the therapeutic treatment performed. The I-st subgroup -16 patients treated with the additional use of the medication based on the antiseptic Troklosene, the IInd subgroup - 18 patients who were treated without using this drug. RESULTS: Results: Analyzing the results after the treatment in the patients of the I -st and II -nd subgroup, they noted a decrease in PMA index, Green-Vermillion index, bleeding index. Pathological mobility of teeth decreased to the I degree. The hygienic condition of the oral cavity was characterized in patients of both subgroups after treatment as satisfactory. In patients of the Ist subgroup (with the additional use of the medication based on the antiseptic Troklosene), after the 5th visit there were signs of remission, and in the IInd subgroup (using standard base therapy), after 7 visits, there were signs of clinical remission. With the additional use of the drug based on the antiseptic Troklosene, in contrast to the traditional therapy, a change in the bacterial ratio of microorganisms occurred, which influenced a significant decrease in the total bacterial mass. CONCLUSION: Conclusion: The best results were obtained in the Ist subgroup, where along with the standard base therapy, the supplement of based on the antiseptic Troklosene was used additionally. Introduction to the treatment regimen for patients with chronic generalized periodontitis with II and III severity of the drug based on the antiseptic Troklosene contributed to a stable clinical effect, as evidenced by the dynamics of clinical indicators and the reduction in the number of visits. Also, a change in the bacterial ratio of microorganisms with a significant decrease due to Enterobacterium spp. is noted. To maintain this result, it is recommended to continue the use of the drug based on the antiseptic Troklosene with a frequency of 4 times per day within 7 days after the end of treatment.

ABSTRACT

OBJECTIVE: Introduction: Periodontal diseases, arising against a background of stomach pathology at tobacco addiction remain an acute problem of modern dentistry. The aim of the work is an experimental study of efficiency of application of the new preparation for oral care during treatment of periodontitis simulated against a background of hyperpeptic gastritis under conditions of intoxication by tobacco smoke. PATIENTS AND METHODS: Materials and methods: At the first stage all experimental animals were divided into 4 groups: І - intact, II - with simulated periodontitis, III - with simulated periodontitis against a background of simulated hyperpeptic gastritis, IV - with simulated periodontitis against a background of hyperpeptic gastritis under conditions of tobacco smoking. Biochemical researches at periodontitis in rats were conducted for determination of influencing stomach pathology and tobacco smoke. On the II stage efficiency of local therapy was studied with the use of the new preparation for oral care and a comparator agent. RESULTS: Results: At experimental periodontitis against a background of hyperpeptic gastritis under conditions of smoking the considerable changes in periodontal tissues typical for the inflammatory process develop. The local therapy at rats with the use of the new preparation resulted in accelerating removing harmful influence of damaging factors and restoring the state of periodontal tissues, than in case of application of the comparator agent. CONCLUSION: Conclusions: The efficiency of the new preparation consists in normalizing influence on processes of lipid peroxidation, inflammation and activation of the protective systems of oral cavity during periodontitis which arises up against a background of hyperpeptic gastritis.

ABSTRACT

Inflammatory cytokines interleukin-1 (IL-1) and tumor necrosis factor-α (TNF-α) regulate the activity of the hypothalamo-pituitary-adrenal (HPA) axis at several levels. Although hypothalamic CRH secretion may be the primary mechanism by which these cytokines activate the HPA axis, IL-1 expression is increased within the adrenal glands in models for systemic inflammation, and IL-1 may augment adrenal glucocorticoid production. Our aim was to investigate the direct effects of IL-1α and IL-1ß on adrenal steroidogenesis and expression of three key steroidogenic genes in human adrenocortical cells using the NCI-H295R cell line as a model. mRNAs encoding receptors for IL-1, TNF-α, and leukemia inhibitory factor (LIF) were detectable in the cell line (Affymetrix microarray analysis). Both IL-1α and IL-1ß increased cortisol, androstenedione, dehydroepiandrosterone and dehydroepiandrosterone sulfate production, and the accumulation of mRNAs for steroidogenic acute regulatory protein (STAR), 17α-hydroxylase/17,20-lyase (CYP17A1) and 3ß-hydroxysteroid dehydrogenase 2 (HSD3B2) in these cells (P<0.05 for all). Both ILs augmented TNF-α- and LIF-induced STAR and CYP17A1 mRNA accumulation, and TNF-α-induced cortisol production (P<0.05 for all). Both ILs also increased the apoptotic index of the cells (P<0.05), which was efficiently neutralized by their specific antibodies. The IL-induced changes in the STAR, HSD3B2, and CYP17A1 protein levels were not as evident as those in the respective mRNA levels. In conclusion, the combined effect of inflammatory cytokines at the adrenal level in acute or chronic inflammatory states could significantly stimulate glucocorticoid production, and thus explain the observed discrepancy between the cortisol and ACTH concentrations sometimes seen in sepsis and chronic inflammatory states.
