ABSTRACT

Effects were studied of ultraviolet irradiation of blood and of hemosorption on nonspecific signs of inflammation and endotoxic syndrome such as serum level of medium-weight molecules and percentage of their hydrophobic portion, the content of sialic acids and seromucoid, the binding capacity of blood proteins, and extent of conformational alterations in albumin. It has been ascertained that medium-molecular weight substances are a factor in the pathogenesis of endotoxic syndrome in patients with bronchial asthma, which fact is a reflection of exacerbation of the illness. It is suggested that medium-molecular weight substances might have a part in the decline of the blood protein transport function and in progressing of the conformational alterations in the principal transport protein albumin. Hemosorption and ultraviolet irradiation of blood make for alleviation of endotoxic syndrome and offer advantage of shortening the time the remission supervenes in. The above techniques have direct antiinflammatory and detoxicating effects.

ABSTRACT

On the basis of the analysis of plasmapheresis (PPh) influence on clinical and functional and immunological indices in 105 patients with bronchial asthma the authors have come to the conclusion that PPh effect is the number-of-procedures-dependent. The most pronounced effect occurred in those groups having been given three to five PPh procedures. The above effect begins to reveal itself during the first postoperative week but the greatest positive changes take place between the 7th and the 14th days. As it won't make any difference whether three or five PPh sessions are conducted, the authors recommend plasmapheresis to be done on a three-procedure basis.

ABSTRACT

A comprehensive clinical and functional and allergo-immunological comparative evaluation was done of hemosorption involving the KAU, SKN hemosorbents, "Gemosfer" and a DNA-containing sorbent in 341 patient with bronchial asthma. SKN sorbent were found out to have a more pronounced positive effect on clinical signs and functional activity of immunocompetent cells. The above sorbents appeared to significantly enhance sensitivity of bronchial beta-adrenoreceptors; besides, they allow the dosages of systemic corticosteroids to be substantially brought down. The KAU and "Gemosfer" hemosorbents caused, to a certain degree, a blockade of the phagocyte system of blood, and, to a considerably lesser extent, affected the level of autoimmune processes. Thus, hemosorption should involve SKN sorbents or an SKN-based DNA-containing sorbent.

ABSTRACT

The questionnaire data of 860 patients with bronchial asthma (BA) and the 7-year catamnestic investigation depth were analysed. According to clinical effect achieved the extracorporal methods of the patients with BA treatment were ranked in such a manner: splenosorption, immunosorption to DNA, which contains the sorpents, the conventional method of hemosorption (HS), using the SKN sorpent, plasmapheresis and the ultra-violet irradiation of blood (UVIB). The applicational efficacy of HS using KAU and "Gemosfer" sorpents is significantly lesser than such of SKN sorpents. UVIB do not effects late follow-up results of the hormonally-dependent patients treatment.

ABSTRACT

It was noted while examination of 290 patients with chronic bronchial asthma (BA) that application of plasmapheresis (PA), contrary to hemosorption (HS), permits to achieve more rapid, but less durable remission. The conducted HS, contrary to PA, permits to normalize the common immunoglobulin E level in the blood serum. The advantages of the HS application in patients with atopic form of BA were established.

ABSTRACT

Based on a comprehensive comparative characterization relying on clinical, functional and allergological indices in 110 patients with bronchial asthma, a conclusion was drawn to the effect that traditional hemosorption (HS) involving the use of a sorbent 400 ml in capacity is more efficient procedure than commonly named "lesser" hemosorption with a 100 ml capacity sorbent. It is advisable that in patients with bronchial asthma, HS should be done only with 400 ml volume sorbents.

ABSTRACT

A comprehensive clinical-and-immunological evaluation was done of efficiency of treatment of patients with severe forms of bronchial asthma (BA) by hemosorption through donor porcine spleen, xenosorption (XS), in 63 patients. XS was found out to have a marked positive clinical effect manifested by a considerable decrease of frequency of asphyxiation bouts and amounts of inhalation broncholytics being taken within the first 24 hours after the procedure. By day 14 from the start of XS we succeeded in lowering the systemic corticosteroids maintenance dose on the average 2.5 times as little. Positive clinical dynamics correlated with positive immunologic changes which revealed themselves in normalization of numbers as well as functional activity of T-cells, rise in T-helpers, normalization of content of serum immunoglobulins A, M and G, with the parameters characterizing the system of phagocytosis approaching the norm, the processes of autoimmunization getting more attenuated.

ABSTRACT

The authors studied comparative clinical, immunologic, laboratory and functional effectiveness of hemosorption (HS) on charcoals and xenosorption (XS) on donor pig spleen in bronchial asthma patients. XS was administered in 60 patients, HS was applied to 91 patient. There was no significant difference in immunologic characteristics between the patient groups. However, XS had a more apparent positive influence on the external respiratory function, degree of allergy and unspecific inflammation, as well as on general clinical effectiveness.

ABSTRACT

Clinical investigation was carried out with the view to study the effect of ultraviolet-irradiated autoblood on bronchial spasm induced by acetylcholine in 11 patients with moderate and severe bronchial asthma in out-of-attack period. Reinfusion of autoblood was established to reduce bronchial hyperresponsiveness in asthma thus allowing to control one of the main pathogenetical mechanisms.

ABSTRACT

Ultraviolet irradiation of blood studied in 201 patients with various forms of bronchial asthma proved to be the most effective in "aspirin" and infectious-allergic forms of the disease. Analysis of immunological changes showed that ultraviolet irradiation of blood brings to the norm content of IgG in serum, influence beneficially quantity and functional activity of T-lymphocytes, reduces auto-immunization and sensibilization to Streptococcus.
