ABSTRACT

Hypopituitarism is an illness of the hypophysis with deficit of one or more pituitary hormones. It can be congenital or acquired. Etiologic factors of acquired hypophyseal insufficiency are numerous. Pituitary gland infarction is a frequent cause of hypopituitarism. Extracorporeal circulation, applied in cardiac surgery procedures, may also, by mechanism of ischemic necrosis, cause hypophyseal lesions and development of hypopituitarism. This paper presents a case of a female patient suffering from panhypopituitarism which developed after extracorporeal circulation during aortocoronary bypass surgery. The authors point to the extracorporeal circulation as a rare etiologic factor in development of hypopituitarism, as well as unusual clinical picture with dominant psychic disorders and diagnostic and treatment procedures.

ABSTRACT

Occurrence of various forms of thyroiditis is getting higher in the last ten years. Subacute or granulomatous or thyroiditis de Quervain has, most certainly, the greatest practical and clinical importance. We present a case of subacute thyroiditis in a female patient 50 years of age in whom Q-fever was positively diagnosed. The disease had had a benign course nevertheless the cause and it passed without any consequences.

ABSTRACT

The ultrasonographic diagnosis of parathyroid glands was introduced by the Japanese in 1975. Only after more perfect machines have been introduced, first of all by advancement of "real-time" technique, this method started being used as a routine. The paper shows results of ultrasonographic investigation in 200 patients separated into two groups. In the first group 100 patients were examined with a clinical suspect of primary hyperparathyroidism from which in 30 the existence of parathyroid gland adenoma was diagnosed. Scintigraphy of parathyroid glands was performed in the smaller group of patients; the comparison of these two methods was done, and in 77.7% cases coincidence was found. In the group of 10 operated patients comparison of the two mentioned methods was carried out, with surgical or pathohistologic findings and the coincidence was complete. In the second group 100 patients were examined belonging to the chronical program of hemodialysis, from which in 24 cases existence of swollen parathyroid glands was found. Comparison of ultrasonographic and scintigraphic findings was also done and the coincidence was 68.75%. At the end, the importance of ultrasonography in the first diagnostic stage of swollen parathyroid glands was pointed out.

ABSTRACT

The paper presents the importance of cytochemical research in the diagnostics of thyroid gland diseases. It is considered that this research usefully supplements the standard cytodiagnostics of the thyroid, especially in clarifying functional and morphological changes in goiter. Given are the basic principals of cytochemical analyses as well as the valuing, in other words, the quantification of cytochemical results. The following methods are more closely described: PAS, alpha-naphthilacetate esterase, naphthol AS-D acetate esterase, peroxidase, beta-glucuronidase and alkaline phosphatase.

ABSTRACT

Ultrasonography takes an important place in the diagnostics of thyroid gland diseases. It is a contemporary, uninvasive method of examination, very comfortable for the patient. By applying this examination method it is possible to attain useful data which are related to morphological changes in the gland, while in regard to the functional condition of this organ, it does not present any information. The paper presents ultrasonographic features of individual thyroid gland pathological conditions. The use of puncture under the control of ultrasound can considerably improve the aspiration cito-diagnostics of the thyroid gland.

ABSTRACT

Although psychic changes are an integral part of the clinical manifestations of hypothyrosis, in traditional literature it is always especially pointed out that this category of patients usually acquires the conditions for developing rough disorders, psychoses. Nevertheless, the results of this study indicate that the most frequent psychic changes are mild and atypical depressions, and that their frequency isn't greater in relation to previously studied patients with hyperthyrosis.

ABSTRACT

Hyperthyroidism has always been the challenge for psychosomatic investigations. In traditional scientific literature the specific structure of hyperthyroid personality was pointed out. Severe and important emotional influences can induce disease in such, prone personalities. This investigation fails in featuring the specific and unique psychosomatic patterns in hyperthyroid patients.
