ABSTRACT

This is a report of three cases of complications of acute otitis media. The first case describes coalescent mastoiditis seen in a ten-month-old infant, the second case is petrositis, sigmoid sinus thrombosis and otitic hydrocephalus seen in a five-year-old child and the third case is an extradural abscess encountered in a three-year-old child. The assessment, diagnosis and treatment of each case are illustrated and discussed.

ABSTRACT

In adults, hemangioma is a rare presentation of a parotid mass. In this report, we present a case of a 40-year-old lady complaining of a swelling in the right inferior parotid region. Imaging studies revealed a cystic lesion of the right parotid gland. Fine needle aspiration cytology was not significantly helpful in revealing the diagnosis. In the operating room, a vascular cystic lesion was found in the deep lobe of the right parotid gland. Pathological analysis confirmed the diagnosis of cavernous hemangioma. In this case report, we discuss a rare pathology in the adult population, and review different aspects in its diagnosis and treatment.

ABSTRACT

A 31-year-old lady presented to the otorhinolaryngology clinic complaining of right exophtalmus. Imaging studies revealed a right frontal sinus mass, suggestive of a mucocele, and fronto-ethmoidal ground glass opacification. In the operating room, the frontal sinuses were exposed through an osteoplastic flap, and connected to the nasal cavity through a Lothrop-like drainage pathway. The pathologic analysis revealed a frontal sinus mucocele and fibro-osseous and respiratory epithelial hamartoma of the fronto-ethmoid region. In this case report, this rare pathology is discussed, in addition to a thorough elaboration upon the surgical options as dictated by the anatomic complexity of the region.

ABSTRACT

OBJECTIVE: Investigation of the effects of magnetic resonance fields on commonly used metallic middle ear implants. STUDY DESIGN: Nine middle ear prostheses (seven containing stainless steel and two made of pure gold used as control) were tested in vitro and one stainless steel stapedectomy prosthesis was tested on a cadaveric temporal bone. METHODS: Each metallic prosthesis was placed in an empty Petri dish and introduced into a 1.5-tesla (T) magnetic resonance imaging (MRI) unit. Most of the prostheses were then placed in a water-filled Petri dish and reintroduced into the MRI unit. Eventual in vitro displacement was assessed visually by two means. In situ testing was done by implanting a piston in a cadaveric temporal bone and performing MR sequences ; any possible displacement was then assessed by CT scan and under microscopic vision. RESULTS: None of the prostheses was displaced in the empty Petri dish. However, while in the water-filled Petri dish, three of these moved with the flux. The implanted piston in the temporal bone did not move. CONCLUSION: The displacement of three of the prostheses in water is not relevant in real clinical situations. MRI can thus be considered safe in usual clinical settings, as far as our studied implants are concerned.
