ABSTRACT

The article presents the scientific biography of L. E. Golubinin, the prominent therapist, prepared on the basis of publications' review and rich archive materials, including documents of the Central Historical Archive of Moscow (f.418) and personal archive of V. D. Shervinsky from the Department of History of Medicine of The Federal State Budget Scientific Institution "N. A. Semashko National Research Institute of Public Health". For the first time the role L. E. Golubinin played in organization of the scientific school of Shervinsky-Golubinin asd a largest therapeutic school in the Moscow University is revealed. Also a high moral level of the personality of L. E. Golubinin is presented.

ABSTRACT

The materials are analyzed characterizing scientific therapeutic schools organized in the Moscow university at the turn of XIX-XX centuries by G.A. Zahar'in, A.A. Ostroumov and V.D. Shervinskii (collaboratively with L.E. Golubinin). It is demonstrated that in the 1890s the leadership passed from faculty therapy of Zahar'in to hospital therapy of Ostroumov. The school of Shervinskii-Golubinin played decisive role in further development of therapeutic clinic in Russia and after that in its restoration in the USSR. It is emphasized that the case in question is just scientific schools but not their founders - Zahar'in and Ostroumov. The both of them are justifiably referred to initiators of national clinic of intern diseases.

ABSTRACT

Contemporary data on mechanisms of development, types, and clinical picture of hypertensive crisis (HC) are presented. Algorithms of rational therapy of uncomplicated and complicated HC are considered. Appropriateness of the use in HC of antihypertensive drugs with multifactorial action is stressed. These drugs include urapidil - an antihypertensive agent with complex mechanism of action. Blocking mainly the postsynaptic 1-adrenoreceptors urapidil attenuates vasoconstrictor effect of catecholamines and decreases total peripheral resistance. Stimulation of 5HT1-receptors of medullary vasculomotor center promotes lowering of elevated vascular tone and prevents development of reflex tachycardia.

ABSTRACT

Twenty four hour blood pressure and ECG monitoring, assessment of sexual function (questionnaires, penile artery Doppler flowmetry) were carried out in male hypertensives treated with atenolol, metoprolol, or bisoprolol. Compared with atenolol and metoprolol bisoprolol was found to be more effective and safe. The use of bisoprolol was associated with no negative changes and even with improvement of some parameters of sexual function. Sildenafil could be used for correction of male sexual dysfunction occurring during treatment with other beta-blockers.
