ABSTRACT

Próximo a cumplirse el primer centenario de unos trágicos sucesos conocidos por la historiografía como el «Desastre de Annual», un enclave del norte de África donde, en julio y agosto de 1921, miles de soldados españoles murieron masacrados por las hordas rifeñas, hemos pretendido recordar a los veterinarios militares que vivieron aquellas durísimas jornadas cumpliendo la misión que tenían encomendada, dando acreditada muestra de compañerismo, valor y heroísmo. Para este propósito nos hemos apoyado en abundante bibliografía, archivos del cementerio de Melilla, testimonios personales de descendientes, y fuentes documentales relativas al juicio contradictorio para la concesión de la Laureada colectiva al Regimiento de Cazadores Alcántara 14. También hemos realizado consultas en los archivos municipales de las poblaciones de donde eran naturales los protagonistas de este trabajo


Near the frst centenary of some tragic events known to history as the Annual Disaster, an enclave of North Africa where, in July and August 1921, Thousands of Spanish soldiers died massacred by the rifes and hordes, I have tried to remind the military veterinarians who lived those very hard days fulflling the mission they had entrusted, giving accredited show of companionship, courage and heroism. For this purpose I have relied on abundant bibliography, Melilla cemetery archives, personal testimonies of descendants, and documentary sources related to the contradictory trial for the granting of the collective Laureate to the Alcantara Hunters Regiment 14. I have also consulted in the municipal archives of the populations where our fellow veterinary protagonists of this work were natural

ABSTRACT

Our aims were to describe the prevalence of pulmonary hypertension in patients with acute respiratory distress syndrome (ARDS), to characterize their hemodynamic cardiopulmonary profiles, and to correlate these parameters with outcome. All consecutive patients over 16 years of age who were in the intensive care unit with a diagnosis of ARDS and an in situ pulmonary artery catheter for hemodynamic monitoring were studied. Pulmonary hypertension was diagnosed when the mean pulmonary artery pressure was >25 mmHg at rest with a pulmonary artery occlusion pressure or left atrial pressure <15 mmHg. During the study period, 30 of 402 critically ill patients (7.46%) who were admitted to the ICU fulfilled the criteria for ARDS. Of the 30 patients with ARDS, 14 met the criteria for pulmonary hypertension, a prevalence of 46.6% (95% CI; 28-66%). The most common cause of ARDS was pneumonia (56.3%). The overall mortality was 36.6% and was similar in patients with and without pulmonary hypertension. Differences in patients' hemodynamic profiles were influenced by the presence of pulmonary hypertension. The levels of positive end-expiratory pressure and peak pressure were higher in patients with pulmonary hypertension, and the PaCO2 was higher in those who died. The level of airway pressure seemed to influence the onset of pulmonary hypertension. Survival was determined by the severity of organ failure at admission to the intensive care unit.

ABSTRACT

Antecedentes y Objetivos: Aplicar un panel de anticuerpos (anti -receptor de progesterona, -receptor de estrógenos, -receptor del factor de crecimiento epidérmico humano 2 y -citoqueratina 14) utilizando métodos inmunohistoquímicos en tumores mamarios de pequeños animales para analizar su clasificación en subtipos moleculares y su asociación con la invasión, el grado y el tipo histológico de las neoplasias. Material y Métodos: Muestras de tumores mamarios malignos, 10 de la especie canina y 3 de la felina. Control positivo interno: glándula mamaria no tumoral adyacente a las neoplasias. Resultados: El 23% (3/13) de los tumores fueron del subtipo luminal B, el 23% (3/13) fueron HER2 positivos, el 46% (6/13) fueron basales y el 7,6% (1/13) no se pudieron clasificar porque no expresaron ninguno de los marcadores tumorales analizados. Ningún caso fue del subtipo luminal A. Los 6 tumores basales fueron de grado II o III y presentaban o infiltración de solo el estroma o también invasión vascula Dos tercios de los tumores HER2 positivos presentaban infiltración del estroma y 1/2 tumores resultó ser de grado II. Los tumores luminal B, 2/3 fueron de grado II o III. Todos los controles internos fueron positivos. No se encontraron diferencias significativas en la distribución de los subtipos moleculares entre los diferentes grupos de las variables invasión (p-valor=0,26), ni grado de malignidad (p-valor=0,42). Sí hubo diferencias en el límite de la significación estadística en la distribución de los subtipos moleculares entre los diferentes grupos de la variable tipo histológico (p=0,08). Conclusiones: La aplicación del panel de anticuerpos ha permitido descubrir 4 (luminal B, HER2, basal y sin clasificar) de los 5 subtipos moleculares posibles (AU)


Antecedents and objectives: to apply an antibodies panel (anti-progesterone receptor -estrogen receptor, -human epi-dermal growth factor receptor 2 and cytokeratin 14) using immunohistochemical methods in mammary tumors of small animals to classify them in molecular subtypes and their association with the invasion, grade and histological type of the malignancies. Material and Methods: samples of malignant mammary tumors, 10 from canine species and 3 from feline ones. Internal positive control: non-tumoral mammary gland adjacent to the malignancy. Results: 23% (3/13) of the tumors were of the luminal B subtype, 23% (3/13) were HER2 positive, 46% (6/13) were basal types and 7,6% (1/13) were unclassifiable because they did not express any of the tested tumor markers. None of the cases belonged to the luminal A subtype. The 6 basal tumors were grade II or III and presented only stromal infiltration or vascular invasion as well. Two thirds of the HER2 positive tumors presented stromal infiltration and hal he tumors were grade II. Two thirds of luminal B tumors were grade II or III. All internal controls were positive. There were no signifi-cant differences in the distribution of the molecular subtypes among the different groups of the invasion (p-value=0.26) and malig-nancy grade variables (p-value=0.42). There were differences of borderline statistical significance in the distribution of the molecular subtypes among the different groups of the histological type variable (p-value=0.08). Conclusions: the application of the antibodies panel has allowed to find 4 (luminal B, HER2, basal and unclassified) out of 5 possible molecular subtypes (AU)

ABSTRACT

The purpose of the present study was to explore the usefulness of the Mexican sequential organ failure assessment (MEXSOFA) score for assessing the risk of mortality for critically ill patients in the ICU. A total of 232 consecutive patients admitted to an ICU were included in the study. The MEXSOFA was calculated using the original SOFA scoring system with two modifications: the PaO2/FiO2 ratio was replaced with the SpO2/FiO2 ratio, and the evaluation of neurologic dysfunction was excluded. The ICU mortality rate was 20.2%. Patients with an initial MEXSOFA score of 9 points or less calculated during the first 24 h after admission to the ICU had a mortality rate of 14.8%, while those with an initial MEXSOFA score of 10 points or more had a mortality rate of 40%. The MEXSOFA score at 48 h was also associated with mortality: patients with a score of 9 points or less had a mortality rate of 14.1%, while those with a score of 10 points or more had a mortality rate of 50%. In a multivariate analysis, only the MEXSOFA score at 48 h was an independent predictor for in-ICU death with an OR = 1.35 (95%CI = 1.14-1.59, P < 0.001). The SOFA and MEXSOFA scores calculated 24 h after admission to the ICU demonstrated a good level of discrimination for predicting the in-ICU mortality risk in critically ill patients. The MEXSOFA score at 48 h was an independent predictor of death; with each 1-point increase, the odds of death increased by 35%.

ABSTRACT

Los neurinomas son tumores de la membranade las células de Schwann (neurilema) poco frecuentes ehistológicamente benignos. Pueden presentarse en cualquier lugar del sistema nervioso, siempre en relación con nervios periféricos o craneales. Representan aproximadamente un tercio de todos los tumores espinales y su localización más habitual es la extramedular e intradural. Dicha localización predispone a complicaciones compresivas y quirúrgicas.Se presenta el caso de un varón de 55 años intervenido deun neurinoma cervical de localización extramedular e intradural al que no se pudo realizar una extirpación completa. El posoperatorio cursó con la afectación de la rama externa del nervio espinal.Se describen las características clínico-anatomopatológicas de dicho tumor y la importancia de la electromiografía en el diagnóstico de la lesión del nervio espinal accesorio


Neurinomas are uncommon and histologicallybenign tumors of the Schwann cell membrane (neurilemma).They may occur in any site of the nervous system, alwaysin relationship with peripheral or cranial nerves. Theyrepresent approximately one third of all the spinal tumors, their most common site being extramedullary and intradural. This location predisposes to compression and surgical complications.The case of a 55 year old man operated on for intradural,extramedullary located cervical neurinoma that could notbe completely excised is presented. The post-operative period occurs with involvement of the external branch of the spinal nerve.The clinical and anatomical characteristics of this tumorand the importance of the electromyography in the diagnosis of the accessory spinal nerve injury are described

ABSTRACT

Presentamos un estudio sobre la evaluación objetiva de la voz en 100 adultos sanos y en 60 sujetos con disfonía por nódulos o pólipos vocales. A todos se les realizó una laringoestroboscopia y un análisis acústico vocal. Se registró y digitalizó la vocal "a" con el programa Dr. Speech Science. Se estimaron los parámetros: frecuencia fundamental (Fo, expresada en función del sexo), jitter, shimmer y Ruido Glótico (NNE, HNR y SNR). La Fo en el grupo normal fue de 139.72 en varones y 267.33 en mujeres, jitter 0.24 y shimmer 2.10. En pacientes disfónicos por nódulos Fo fue 126.96 en varones y 240.72 en mujeres, jitter 0.35 y shimmer 3.25. En pacientes disfónicos por pólipos Fo fue 119.75 en varones y 218.26 en mujeres, jitter 0.50 y shimmer 4.34. Todas estas diferencias fueron estadísticamente significativas. El Ruido Glótico en población normal fue de: NNE -13.62, HNR 24.07 y SNR 24.49; en pacientes con nódulos: NNE -10.65, HNR 25.21 y SNR 25.55; en pacientes con pólipo vocal: NNE -8.24, HNR 29.63 y SNR 28.22. Sólo existió diferencia estadísticamente significativa en el parámetro NNE. Destacamos la importancia de la valoración objetiva de los trastornos de la voz (AU)


A study of the objective evaluation of voice was made of 100 voices of healthy adults and 60 with disphonia (nodules and polyps). A laryngostroboscopy and an acoustic analysis was made to everyone. A sustained vowel "a" was carried out and digitalized with Dr. Speech Science software. These parameters were estimated: fundamental frequency (Fo, according to gender), Jitter, Shimmer and glottic noise (NNE, HNR, SNR). In the healthy group Fo was 139.72 in men and 267.33 in women, jitter 0.24 and shimmer 2.10. In disphonic patients (nodules) Fo was 126.96 in men and 240.72 in women, jitter 0.35 and shimmer 3.25. In disphonic patients (polyps) Fo was 119.75 in men and 218.26 in women, jitter 0.50 and shimmer 4.34. These difference were statistically significant. Glottic Noise in healthy group was: NNE -13.62, HNR 24.07 and SNR 24.49; in disphonic patients (nodules) was: NNE -10.65, HNR 25.21 and SNR 25.55; in disphonic patients (polyps) was: NNE -8.24, HNR 29.63 and SNR 28.22. Only the difference in the NNE was statistically significant. We highlight the importance of objective evaluation of voice disorders (AU)

En los últimos años el manejo de la enfermedad cerebrovascular ha cambiado considerablemente y hoy es considerada como una emergencia médica comparable al infarto agudo de miocardio. El tratamiento trombolítico con rt-PA es el primer paso efectivo en el manejo terapéutico del ictus isquémico. El beneficio del rt-PA ha sido claramente demostrado en distintos ensayos clínicos (NINDS; ECASS I y II), donde se demostró una reducción en la mortalidad-morbilidad de estos pacientes en el grado de dependencia posterior, aunque el número de hemorragias sintomáticas fue superior claramente a la de los grupos placebo. Tanto de forma individual como en los metaanálisis, los estudios con rt-PA indican que, a pesar del número elevado de hemorragias cerebrales, no se incrementan la mortalidad o la morbilidad y mejora la evolución clínica. La administración del trombolítico rt-PA requiere una profunda reorganización de los servicios extrahospitalarios de urgencias/emergencias, que posibilite el acceso de estos pacientes a los centros especializados en las mejores condiciones y en un tiempo inferior a tres horas desde iniciados los síntomas; asimismo, es también necesaria la reestructuración de los servicios de urgencias hospitalarios mediante la creación de unidades de ictus, donde personal especializados realice un diagnóstico precoz y un tratamiento específico (AU)


The management of cerebrovascular disease has changed markedly over the last few years, and this condition is nowadays considered to represent a medical emergency skinto acute myocardial infarction. Thrombolytic therapy with rt-PA is the first effective step in the management of acute ischaemic stroke. The benefits or rt-PA therapy have been clearly demonstrated in a number of clinical trials (NINDS, ECASS I and II), which evidenced reduced mortality and morbidity in these cases at the level of ensuing dependence, even though the number of symptomatic haemorrhages was clearly higher than in the placebo-treated groups. Both individually and in a meta-analysis, the rt-PA trials show that despite the high incidence of cerebral haemorrhages mortality or morbidity are not increased and the clinical evolution improves. The administration of rt-PA requires a far-reaching reorganisation of the Home Emergency Care services rendering the access of these patients to specialised centres possible under optimal conditions and within three hours from the apparition of symptoms. The Hospital Outpatient Emergency Department services must also be organised through the creation of Stroke Units where specialised personnel may establish an early diagnosis specific therapy (AU)

ABSTRACT

Structures for 2,5-disubstituted decahydroquinolines (DHQs) are reported for the two diastereomeric pairs cis-275B (14) and cis-275B' (15) and 5-epi-trans-269AB (18) and trans-269AB (19), all isolated from skin extracts of dendrobatid frogs, and for 5-epi-cis-275B' (16) and 5-epi-trans-275B (17) found in the extracts of virgin queens of a myrmicine ant [Solenopsis (Diplorhoptrum) azteca]. Detection of such DHQs in an ant, their first reported occurrence, strengthens a dietary hypothesis for the origin of the approximately 30 DHQs that have been detected in extracts of frog skin. NMR data on the two conformers of cis-decahydroquinoline permit assignment of ring conformations and stereochemistry to cis-DHQs of the "N-endo" type or the "N-exo" type. These conformations are also assigned on whether H-8a is equatorial or axial as determined with E-COSY or 1D-HOHAHA spectra.
