ABSTRACT

L''Adénofibrome est l'affection la plus fréquente des pathologies bénignes du sein. L'objectif était d'identifier les aspects clinique et thérapeutique de l'adénofibrome dans le service de gynécologie obstétrique et chirurgie générale du CHU Gabriel TOURE. Patientes et Méthodes : L'étude était descriptive avec récolte rétrospective des données du 1er juillet 2018 au 31 juillet 2021. Ont été inclus, les dossiers de patientes prises en charge pour adénofibrome selon les principes éthiques garantissant l'anonymat et la confidentialité des données. Résultats : Nous avons recensé 701 cas de pathologie mammaire parmi lesquelles nous avons retenu au total 112 cas d'adénofibrome durant la période d'étude soit 15,9%. Nous avons enregistré 682 cas de tumeur mammaire, l'adénofibrome a représenté 16,42 %. Ainsi parmi toutes les 154 tumeurs bénignes du sein, l'adénofibrome (112 cas) a représenté 72,7%. L'âge moyen était de 27 ans avec un écart type de 14,8 ans (13 ans et 62 ans). La tranche d'âge de 20 à 35 ans était la plus représentée soit 44%. La fréquence de l'adénofibrome diminuait avec l'augmentation de la gestité et de la parité. L'obésité ou le surpoids ont été retrouvés chez 63 des patientes. Le motif principal de consultation était la présence d'une tuméfaction mammaire. L'atteinte unilatérale gauche prédominait dans 46% et la localisation au quadrant supéro- externe représentait 50 %. La résection chirurgicale a représenté la modalité de prise en charge la plus fréquente avec 56,2 % des cas. Conclusion : L'adénofibrome du sein est une affection bénigne assez fréquente. Son diagnostic oblige à une surveillance. Le traitement chirurgical doit être discuté selon l'âge, la taille et la présence de facteurs de risque de malignité


Fibroadenomas are the most common benign breast disorders. The aim of this study was to identify the clinical and therapeutic aspects of fibroadenoma in the obstetric gynecology department and General Surgery of Teaching hospital Gabriel TOURE in Bamako Mali. Patients and Methods: The study was descriptive with retrospective data collection from July 1, 2018 to July 31, 2021. The records of patients treated for fibroadenoma were included in accordance with ethical principles guaranteeing anonymity and confidentiality of data. Results: A total of 112 patients were selected for the study period, representing 15. 9% of all breast pathologies. We recorded 642 cases of breast mass, with fibroadenoma accounting for 16.72% of these breast tumors. Among all 154 benign breast tumors, fibroadenoma accounted for 72%. The mean age was 27 years, with a standard deviation of 14.8 years (13 years and 62 years). The 20-35 age group was the most represented, at 44%. The frequency of fibroadenoma decreased with increasing gestational age and parity. Over 60% of patients were overweight. The main reason for consultation was the presence of breast swelling. Unilateral left breast swelling was predominant in 46% of cases, and location in the upper-outer quadrant accounted for 50%. Surgical excision was the most frequent management modality, accounting for 56,2% of cases. Conclusion: fibroadenoma of the breast are a fairly common benign condition. Diagnosis requires surveillance. Surgical treatment should be discussed according to age, size and the presence of risk factors for malignancy.

ABSTRACT

SETTING: According to reports in South Africa, treatment failure rates for rifampicin-resistant TB (RR-TB) are significant and below the WHO target of ≥70%. HIV infection and the use of highly active antiretroviral therapy (HAART) influence how patients receiving anti-TB drugs respond to therapy. In the treatment of RR-TB, more recent medications, including bedaquiline, pretomanid and linezolid (BPaL), have shown promising results.OBJECTIVE: To assess treatment outcomes in RR-TB patients using BPaL and other second-line anti-TB drugs as recommended by the WHO in the South African population.DESIGN: The databases Medline, PubMed, Google Scholar and Embase were searched for studies between 2015 and 2022, which investigated BPaL outcomes in South Africa.RESULTS: Of the 27,259 participants, 21% were on bedaquiline, 1% were taking pretomanid and 9% were taking linezolid as part of their background regimen. About 68% of the patients were HIV-positive, with 59% of them taking HAART.CONCLUSION: Overall, 66% of patients taking BPaL drugs as part of their background regimen had favourable treatment outcomes. Additionally, patients with RR-TB who were HIV-positive and taking HAART while receiving BPaL drugs as part of a background regimen had improved treatment outcomes.

ABSTRACT

INTRODUCTION: Firearm injuries to the lower urinary tract are injuries resulting from an energy transfer from a projectile fired by a firearm or a blast from a firearm, to the bladder, prostate, urethra and terminal ureter. Our objective was to describe the epidemiological and diagnostic aspects in the context of a security crisis. PATIENTS AND METHOD: This was a descriptive cross-sectional study of lower urinary tract firearm injuries at Sominé Dolo Hospital in Mopti from January 2017 to December 2021. RESULTS: In five years, 29 traumas of the lower urinary tract have been collected, about 6 cases per year. The mean age of patients was 27.48 years, the sex ratio was 13.5/1. The main circumstances of occurrence were intercommunal violence 51.72 %, war wounds 31.03 %. Injuries were inflicted by rifles in 62.1 % and improvised explosive devices in 37.9 %. The main modes of discovery were acute urine retention, hematuria and penoscrotal wounds respectively in 31.03 %, 20.69 %, 17.24 %. The organs affected were the urethra in 58.62 %, the bladder, 37.93 %, the prostate 3.45 %. Associated non-urological lesions present in 68.96 % (n=20) were dominated by intestinal lesions and pelvic fractures. CONCLUSION: Firearm injuries to the lower urinary tract are rare despite the context of the security crisis. They affect young men, are mostly linked to intercommunal violence, and are dominated by urethral and bladder injuries.

ABSTRACT

INTRODUCTION: Several authors have suggested an association between the ABO system (ABOS) and the risk of SARS CoV-2 infection. The objective of the present study was to identify the susceptibility of ABOS blood groups to COVID-19 at the National Institute of Public Health (INSP) of Mali. MATERIAL AND METHODS: This was an unpaired case-control study. The cases (402) were COVID-19 patients, and the controls (5438) were Muslim pilgrims, all of whom had undergone ABO blood grouping at the INSP. Logistic regression was performed to identify factors associated with SARS-CoV-2 infection status using SPSS version25 software. RESULTS: Blood types had the following frequencies: O(34%), B(30%), A(28%), AB(8%) for cases and O(42%), B(27%), A(25%), AB(6%) for controls. COVID-19 positive status was not significantly associated with blood groups A(oR=1.17; CI95%[0.93-1.47]), B(OR=1.11; CI95%[0.89-1.39]) and AB(OR=1.41; CI95%[0.97-2.06]). However, blood group O subjects were 28 times less likely to have COVID-19 than non-O(OR=0.72; CI95%[0.58-0.89). CONCLUSION: The results show that blood group O individuals were less likely to have SARS CoV-2 infection than non-O individuals. This should not affect the use of barrier measures in ABOS groups.

ABSTRACT

Introduction : La forme digestive pure de la COVID-19 est possible et initialement considérée comme rare. L'objectif était d'étudier la COVID-19 en milieu chirurgical. Matériels et méthodes: L'étude était descriptive prospective de Mars 2020 à Aout 2021 (18 mois). Tous les patients qui ont été opérés, hospitalisés ou consultés dans le service, au service d'accueil des urgences (SAU) ou dans d'autres services du CHU. GT pour pathologie chirurgicale avec COVID-19 ou infecté au COVID-19 en cours d'hospitalisation étaient inclus dans l'étude. Résultats : 23 cas colligés ; représentant 8,91 % (23/258) des cas de COVID-19 recensés, une fréquence de 1,27/mois. La tranche d'âge 66-75 ans était la plus atteinte, la moyenne d'âge était de 49,13 ± 18,75ans, la prédominance était masculine (sex-ratio de 1,3). Plus de 34,78% ont été recrutés au SAU, 16 malades (69,56%) ont consultés en urgence, les signes digestifs du COVID-19 étaient : Douleur abdominale 20 cas (89,96%), anorexie 19 cas (82,61%), Vomissements 8 cas (34,78%), diarrhée 3 cas (13,04%). Les signes pulmonaires du COVID-19 étaient : Toux 18 cas (78,26%), douleur thoracique 15 cas (65,22%), dyspnée 9 cas (39,13%). Les manifestations du COVID-19 étaient : pulmonaire 9 cas, digestives 9 cas, associées 3 cas, découverte fortuite 2 cas. Le moyen diagnostic a été la TDM Thoracique (100%), le Test-PCR 14 cas (60,86%) avec un test-PCR positif dans 50% des cas. Les pathologies chirurgicales étaient des urgences chirurgicales dans 7 cas (30,43%), des cancers dans 6 cas (26,09%), manifestation digestive COVID-19(30,43%), autres 3 cas (13,04%).Plus de la moitié des malades étaient opérés 12 cas (52,17%). La mortalité globale était de 60,87% et la mortalité des malades opérés était de 41,67%. Conclusion : Les pathologies chirurgicales et COVID-19 n'étaient pas fréquentes. La plupart des malades avaient plus de 50 ans avec une présence moyenne de comorbidité. Les signes pulmonaires étaient les plus marquants, cependant les signes digestifs étaient inaugurales dans la moitié des cas de notre étude. La TDM Thoracique associée ou non au Test-PCR oro-pharyngé permet de faire le diagnostic. Le traitement peut-être médico-chirurgical ou médical. Le risque infectieux de la COVID-19 au cours de l'hospitalisation, pendant ou après la chirurgie est réel et potentiellement grave pour le malade ainsi que les soignants.

More than 34.78% were recruited at the SAU, 16 patients (69.56%) consulted in emergency, and digestive signs of COVID-19 were: Abdominal pain 20 cases (89.96%), anorexia 19 cases (82.61%), vomiting 8 cases (34.78%) and diarrhea 3 cases (13.04%). The pulmonary signs of COVID-19 were: Cough 18 cases (78.26%), chest pain 15 cases (65.22%), and dyspnea 9 cases (39.13%). Manifestations of COVID-19 were: pulmonary 9 cases, digestive 9 cases, associated 3 cases, incidental discovery 2 cases. The diagnostic mean was Thoracic CT (100%), Test-PCR 14 cases (60.86%) with a positive PCR test in 50% of cases. Surgical pathologies were surgical emergencies in 7 cases (30.43%), cancers in 6 cases (26.09%), COVID-19 digestive event (30.43%) and other 3 cases (13.04%). More than half of patients were operated on 12 cases (52.17%). The overall mortality was 60.87% and the mortality of surgical patients was 41.67%.Conclusion: The infectious risk of COVID-19 during hospitalization, during or after digestive surgery is a real and potentially serious risk for the patient and caregiver

ABSTRACT

Les ingestions de corps étrangers sont des accidents fréquents en pédiatrie. La plupart sont sans grande conséquence sauf ceux qui sont enclavés dans l'œsophage. Nous rapportons 2 cas de pile bouton intraœsophagienne chez 2 enfants âgés de: 4ans de sexe masculin et 2 ans de sexe féminin. Le motif de consultation dans les deux cas était la dysphagie aux solides. La fibroscopie broncho-oesophagienne a été effectuée dans les 2 cas avec échec d'extraction conduisant à une extraction chirurgicale par voie de thoracotomie chez le garçon et de cervicotomie chez la fille. Une sténose œsophagienne est survenue à 2 mois dans les suites opératoires chez le garçon. L'évolution a été favorable après dilatation aux bougies de Rehbein modifiées. Chez la fille en postopératoire un œdème laryngé est survenu et a nécessité des séances de nébulisations. L'évolution a été favorable chez les 2 enfants avec un recul de 5 ans et de 2 mois. Conclusion: Les piles boutons sont des corps étrangers particuliers qu'il faut extraire en urgence. L'oesophagoscopie reste le moyen le plus fréquemment employé mais la chirurgie reste le dernier recourt avec des possibilités de complication.


Ingestion of foreign bodies is a common accident in paediatrics. Most of them are of little consequence except for those that are enclosed in the esophagus. We report 2 cases of intraesophageal button stacks in 2 children aged 4 years' male and 2 years female. The reason for consultation in both cases was solid dysphagia. Bronchoesophageal fibroscopy was performed in both cases with failed extraction leading to surgical extraction by thoracotomy in boys and cervicotomy in girls. Esophageal stenosis occurred at 2 months of age in the postoperative period in boys. The development was favourable after expansion at the modified Rehbein candles. In the postoperative girl, laryngeal edema occurred and required nebulization sessions. The evolution was favorable in the 2 children with a follow-up of 5 years and 2 months. Conclusion: Button batteries are special foreign bodies that need to be removed urgently. Esophagoscopy remains the most frequently used method, but surgery remains the last resort with the possibility of complications.

INTRODUCTION: Central corneal thickness measurement permits an evaluation of physiological and pathological corneal changes. It allows for an intra-ocular pressure correction factor. Our purpose was to determine the mean central corneal thickness in children aged 0 to 72 months and to examine factors that might affect it. PATIENTS AND METHODS: This was a cross-sectional hospital study which took place from February to December 2019 in the ophthalmology service of YO University Medical Center. It included healthy eyes of children 0-72 months of age examined under general anesthesia. Ultrasonic pachymetry was used to measure the central corneal thickness. The studied variables were age, sex, CCT, corneal diameter, intraocular pressure, and cup/disc ratio. RESULTS: One hundred and twenty-seven healthy eyes of 78 children 0-72 months of age were included. The mean age was 22 months. The overall mean CCT was 554.01±34.21µm. It was 556.58µm for children from 0 to 36 months of age and 536.12µm for subjects over 36 months. The mean CCT was 555.96±32.96µm in boys and 551.80±36.16µm in girls. In patients who underwent bilateral CCT measurement, the mean CCT was 554.74±35.67µm in the right eye and 549.76±24.08µm in the left eye. Lower IOP's were found in patients with thicker corneas (p=0.00). CONCLUSION: The CCT values for black children over 36 months of age was similar to the CCT in black adults.

ABSTRACT

OBJECTIVE: To determine the main causes of visual impairment according to age in patients admitted to the ophthalmology department. MATERIALS AND METHODS: This was a retrospective study of 501 cases of visual impairment. Included were patients of both sexes whose best-corrected visual acuity in the better eye was strictly less than 5/10. The parameters studied were age, gender, place of residence, corrected visual acuity in the better eye, and diagnosis. The data were entered and processed with the Epi info 7.2 software. RESULTS: Subjects over 50 were the most prevalent (48.5%). In patients over 50 years of age, cataracts (54.32%) were the leading cause of visual impairment, followed by glaucoma (24.69%) and non-glaucomatous optic neuropathy (26.19%). Trauma (17.62%) was frequently found in patients aged 16 to 50 years. Tropical endemic limbo-conjunctivitis (25.00%) was the leading cause of visual impairment in subjects aged 5 to 15 years. CONCLUSION: This study confirms the results of the literature, which have shown that cataracts are the leading cause of visual impairment in developing countries. A major effort to raise awareness, provide information and therapeutic care would reduce the incidence of visual impairment.

Surgical pathologies were surgical emergencies in 7 cases (30.43%), cancers in 6 cases (26.09%), COVID-19 digestive event (30.43%) and other 3 cases (13.04%). More than half of patients were operated on 12 cases (52.17%). The overall mortality was 60.87% and the mortality of surgical patients was 41.67%. CONCLUSION: The infectious risk of COVID-19 during hospitalization, during or after digestive surgery is a real and potentially serious risk for the patient and caregivers.


INTRODUCTION: La forme digestive pure de la COVID-19 est possible et initialement considérée comme rare. L'objectif était d'étudier la COVID-19 en milieu chirurgical. MATÉRIELS ET MÉTHODES: L'étude était descriptive prospective de Mars 2020 à Aout 2021 (18 mois). Tous les patients qui ont été opérés, hospitalisés ou consultés dans le service, au service d'accueil des urgences (SAU) ou dans d'autres services du CHU. GT pour pathologie chirurgicale avec COVID-19 ou infecté au COVID-19 en cours d'hospitalisation étaient inclus dans l'étude. RÉSULTATS: 23 cas colligés ; représentant 8,91 % (23/258) des cas de COVID-19 recensés, une fréquence de 1,27/mois. La tranche d'âge 66-75 ans était la plus atteinte, la moyenne d'âge était de 49,13 ± 18,75ans, la prédominance était masculine (sex-ratio de 1,3). Plus de 34,78% ont été recrutés au SAU, 16 malades (69,56%) ont consultés en urgence, les signes digestifs du COVID-19étaient : Douleur abdominale 20 cas (89,96%), anorexie 19 cas (82,61%), Vomissements 8 cas (34,78%), diarrhée 3 cas (13,04%). Les signes pulmonaires du COVID-19 étaient : Toux 18 cas (78,26%), douleur thoracique 15 cas (65,22%), dyspnée 9 cas (39,13%). Les manifestations du COVID-19 étaient : pulmonaire 9 cas, digestives 9 cas, associées 3 cas, découverte fortuite 2 cas. Le moyen diagnostic a été la TDM Thoracique (100%), le Test-PCR 14 cas (60,86%) avec un test-PCR positif dans 50% des cas. Les pathologies chirurgicales étaient des urgences chirurgicales dans 7 cas (30,43%), des cancers dans 6 cas (26,09%), manifestation digestive COVID-19(30,43%), autres 3 cas (13,04%).Plus de la moitié des malades étaient opérés 12 cas (52,17%). La mortalité globale était de 60,87% et la mortalité des malades opérés était de 41,67%. CONCLUSION: Les pathologies chirurgicales et COVID-19 n'étaient pas fréquentes. La plupart des malades avaient plus de 50 ans avec une présence moyenne de comorbidité. Les signes pulmonaires étaient les plus marquants, cependant les signes digestifs étaient inaugurales dans la moitié des cas de notre étude. La TDM Thoracique associée ou non au Test-PCR oro-pharyngé permet de faire le diagnostic. Le traitement peut-être médico-chirurgical ou médical. Le risque infectieux de la COVID-19 au cours de l'hospitalisation, pendant ou après la chirurgie est réel et potentiellement grave pour le malade ainsi que les soignants.

ABSTRACT

PURPOSE: The purpose of this study was to study hepatitis B virus infection in pregnant women. METHOD: This was a one-year descriptive cross-sectional study carried out in the Gynecology-Obstetrics Department of the Gabriel Touré University Hospital. RESULT: During this period, 796 pregnant people were seen in antenatal consultations. Ween rolled 500 pregnant women in whom the search for HBs Ag was carried out, a rate of 62.81%. Of these pregnant 85 had HBs Ag a prevalence of 17%. The average age of these women was 26.9±5.6 years. Our patients were paucipare sin 52.9%. Of these, 17.7% had a family history of chronic liver disease and 37.6% had once given birth in a referral health centre. Blood transfusion, polygamous focus and tattooing/scarification were the risk factors associated with HBs Ag carriage. Alarming clinical signs were absent in 95.2% of cases. Hepatic cytolysis and anaemia were foundin 28.8% and 76.3% of cases respectively; viral replication was observed in 13.6% of pregnant women with a high viral loadin 37.2%. Abdominal ultra sound was normal in 90.8% of cases and esophageal varices were present in 6% of women who performed eso-gastroduduedenal fibroscopy. Fibrosis was significant according to an APRI score in 3.4%. CONCLUSION: The prevalence of HBs A in pregnant women followed remains high.


BUT: Le but de cette étude était d'étudier l'infection par le virus de l'hépatite B chez la femme enceinte. METHODE: Il s'agissait d'une étude transversale descriptive sur un an réalisée au service de Gynécologie-Obstétrique du CHU Gabriel Touré. RESULTAT: Durant cette période, 796 gestantes ont été vues en consultation prénatale. Nous avons enrôlé 500 femmes enceintes chez qui la recherche de l'Ag HBs a été effectuée soit un taux de 62,81%. Parmi ces gestantes 85 avaient l'Ag HBs soit une prévalence de 17%.L'âge moyen de ces femmes était de 26,9±5,6 ans. Nos patientes étaient des paucipares dans 52,9%. Parmi elles 17,7% avaient un antécédent familial d'hépatopathie chronique et 37,6% avaient une fois accouché dans un centre de santé de référence. La transfusion sanguine, le foyer polygamique et le tatouage/scarification étaient les facteurs de risque associés au portage de l'Ag HBs. Les signes cliniques alarmants étaient absents chez 95,2% des cas. Une cytolyse hépatique et une anémie étaient retrouvées chez respectivement 28,8% et 76,3% des cas ; une réplication virale était observée chez 13,6% des gestantes avec une charge virale élevée chez 37,2%. L'échographie abdominale était normale dans 90,8% des cas et les varices œsophagiennes étaient présentes chez 6% des femmes ayant réalisé la fibroscopie œsogastroduodénale. La fibrose était significative selon un score APRI chez 3,4%. CONCLUSION: La prévalence de l'AgHBs chez les gestantes suivies reste élevée.

INTRODUCTION: Diabetes is a chronic hyperglycemia which can be discovered with specific organic complications particularly affecting the eyes, kidneys, nerves, heart and vessels. Globally, the prevalence of stroke in people with diabetes is approximately 10%. OBJECTIVE: Study the epidemiological and clinical aspects of stroke in patients with type 2 diabetes. METHODOLOGY: We made a descriptive and analytical study by a retrospective survey over a period of 10 years, in the department of internal medicine at the university Hospital ofPoint G. We focused on all hospitalized patients with type 2 diabetes, in whom the diagnosis of stroke was retained. RESULTS: At the end of the study 24 patients out of 492 diabetes hospitalized in to the period met our inclusion criteria, representing a frequence of 4.9%. The mean age was 64.67 ± 13.409 years with a sex-ratio of 1.4. The reason for hospitalization was muscle weakness in 54.2% of patients. Eighty-seven point five percent (87.5%) of patients were already known to have diabetes before the weakness. Stroke was the mode of discovery of diabetes in 12.5%. Over 70% of our patients had hyperglycemia at the time of admission. Ischemic stroke was seen in 75% of patients. CONCLUSION: Stroke is a great complication of diabetes witch, can be a way discovery of type 2 diabetes.

Elle a porté sur tous les patients diabétiques de type 2 hospitalisés dans le service de médecine interne, chez qui le diagnostic de l'AVC a été retenu. RÉSULTATS: Au terme del'étude 24 patients répondants à nos critères sur 492 diabétiques soit une fréquence de 4,9%. L'âge moyen était de 64,67 ± 13,409 ans avec un sex-ratio de 1,4. Le motif d'hospitalisation était un déficit moteur chez 54,2% des patients. Quatre-vingt-sept virgule cinq pourcent (87,5%) des patients étaient déjà connus diabétiques avant le déficit. L'AVC a été le mode de découverte du diabète chez 12,5%. Plus de 70% de nos patients avaient une hyperglycémieau moment à l'admission. L'AVC ischémique était observé chez 87,5% des patients. CONCLUSION: L'AVC est une complication redoutable du diabète, il peut être le mode de découverte d'un diabète de type 2.

INTRODUCTION: Elderly people with diabetes represent a large and growing part of the general practitioners patient database, but their state of frailty compared to the non-diabetic population is poorly understood. OBJECTIVE: To study the relationship between diabetes and frailty in people aged 65 and over in primary care. METHOD: Frailty syndrome was assessed by the Fried Scale and compared between populations of diabetic and non-diabetic patients; 268 patients were studied, including 129 diabetic patients. RESULTS: Frailty was found in 27.9 % of older diabetic patients. The observed mean Fried score was 1.76 in people with diabetes versus 1.39 in non-diabetics, with more frail diabetic subjects (p = 0.007). In this study, HbA1c levels showed no significant association to frailty. CONCLUSION: Larger studies in several general practice clinics should be performed on subjects over 65 years of age with or without diabetes.

Méthode : La fragilité a été évaluée par l'échelle de Fried et comparée entre les populations de patients diabétiques et non diabétiques; 268 patients ont été inclus dont 129 patients diabétiques. Résultats : La fragilité concernait 27,9 % des diabétiques. ABSTRACT

Blood and faecal samples from 2086 Sahelian and Djallonke sheep have been collected to assess the epidemiology and spatio-temporal distribution of gastrointestinal parasites. A survey with a questionnaire allowed collecting data on individual animal and farms. Faecal samples were analysed using the McMaster technique, while anaemia was assessed using FAMACHA score test and packed cell volume. The overall prevalence of gastrointestinal parasites was 91%, and strongyles (70.7%), Eimeria sp. (60.4%), Moniezia sp. (5.7%), Stronyloides sp. (0.9%) were identified. There was no significant association between the region and strongyles infection (P ˃ 0.05). Animals sampled during the rainy season (n = 565; P = 82.2%, mean EPG = 154.8 ± 12.02) were mostly and heavily infected than those from the hot dry (n = 518; P = 74.2%; mean EPG = 97.7 ± 17.4) and the cold dry season (n = 392; P = 55.5%; mean EPG = 24.5 ± 19.5) (P ˂ 0.05). Females (n = 1027; P = 72.1%; mean EPG = 77.6 ± 17) were most infected than males (n = 448; P = 67.7%; mean EPG = 61.6 ± 19.5) (P ˂ 0.05). Animals that were treated, at most 3 months before sampling (n = 166; P = 61.5%; mean EPG = 41.7 ± 21.4), were less infected than those, which received this treatment after three months before sampling (n = 199; P = 74.8%; mean EPG = 89.1 ± 15.8) (P ˂ 0.05). Data from blood samples indicate that 6.5% of sheep was anaemic (PCV ≤ 19%), while 59.7% was noted anaemic regarding FAMACHA scores. PCV values varied significantly for FAMACHA scores, and the highest mean value was noted for score 1 (27.6) (P ˂ 0.05). The Cohen's kappa score between FAMACHA and PCV was 0.02. These findings suggest that strongyles infections are significantly associated with season, and heavier infections were noted during rainy season.

OBJECTIVE: The aim of this case-control study was to identify risk factors associated with necrotizing fasciitis (NF) of the lower limbs. PATIENTS AND METHODS: We conducted a prospective case-control study in hospital dermatology departments in 5 sub-Saharan African countries over a 2-year period (April 2017 to July 2019). The cases were patients with NF of the lower limbs and the controls were patients with leg erysipelas. Each case was matched with two controls for age (±5 years) and sex. We analyzed local and general factors. RESULTS: During the study period, 159 cases (73 females, 86 males) were matched with 318 controls. The mean age was 48.5±15.8 years for cases and 46.5±16.2 years for controls (P=0.24). The main local signs of NF were cutaneous necrosis (83.7%), pain (75.5%) and induration (42.1%). Multivariate analysis showed the following to be independent risk factors associated with NF of the lower limbs: obesity (odds ratio [OR]=2.10; 95% confidence interval [CI]: 1.21-3.42), diabetes (OR=3.97; 95% CI: 1.95-6.13), nicotine addiction (OR=5.07; 95% CI: 2.20-11.70), use of non-steroidal anti-inflammatory drugs (NSAIDs) (OR=7.85; 95% CI 4.60-14.21) and voluntary cosmetic depigmentation (OR=2.29; 95% CI: 1.19-3.73). CONCLUSION: Our study documents the role of NSAID use at the onset of symptoms as a risk factor for NF of the lower limbs. However, the originality of our study consists in the identification of voluntary cosmetic depigmentation as a risk factor for NF of the lower limbs in sub-Saharan Africa patients. Our results also identified typical overarching factors such as diabetes, obesity and nicotine addiction. Knowing these factors and taking them into account will enable optimization of management strategies for these conditions.

ABSTRACT

COVID-19 was declared a pandemic in March 2020. For case management, Mali has created several treatment sites including the site of the CHU Gabriel Touré. AIMS: The objective of the study was to analyse drug prescriptions for the COVID-19 treatment at the CHU Gabriel Touré. METHODS: We performed a retrospective and descriptive study from April to September 2020. Drug prescriptions and hospital records were used to collect data. Prescriptions and hospital records were used to collect data. RESULTS: A total of 29 patients were hospitalized. The median age was 44 years, 75.90% of patients had at least one pathology associated with COVID-19. The number of prescriptions was 333 comprising 870 lines of prescriptions including 33.21% for standard treatments, and 66.79% for associated pathologies. with 86.23% available at the Hospital Pharmacy. Chloroquine, dosed at 250mg, was administered at 500mg twice a day. The national guidelines from the treatment of COVID-19 recommends 200mg of chloroquine in two doses. Vitamin C was prescribed for all patients although not included in the national guidelines. The class of drugs for the blood and blood-forming organs was the most prescribed (31.49%). The average cost of treatments was 65,602 ± 106,858 FCFA with a maximum of 567,860 FCFA. An evaluation of prescriptions in other treatment sites is necessary.


La COVID-19 est une infection virale qui s'est propagé dans tout le monde. Pour la prise en charge des cas, le Mali a créé des sites de traitement dont celui du CHU Gabriel Touré. OBJECTIF: C'était d'analyser les prescriptions médicamenteuses pour le traitement de la COVID-19 au CHU Gabriel Touré. MÉTHODOLOGIE: L'étude était rétrospective et descriptive et a concerné la période d'avril à septembre 2020. Elle s'est déroulée au Centre de Traitement COVID-19 du CHU Gabriel Touré et à la Pharmacie Hospitalière de cet hôpital. Les ordonnances et les registres d'hospitalisation ont servi à collecter les données. RÉSULTATS: Au total, 29 patients ont été hospitalisés. L'âge médian était de 44ans, 75,90% des patients présentaient au moins une pathologie associée à la COVID-19. Le nombre d'ordonnances était de 333 comportant 870 lignes de prescriptions dont 33,21% de traitements standards et 66,79% pour les pathologies associées avec 86,23% disponibles à la Pharmacie Hospitalière. Le phosphate de chloroquine, dosé à 250mg, était administré à 500mg deux fois quotidiennement. Dans les directives nationales le phosphate de chloroquine était à 100mg pour 200mg trois fois quotidienne. La vitamine C non prévue dans les directives a été prescrite à tous les patients. La classe des médicaments du sang et des organes hématopoïétiques a été les plus prescrits (31,49%). Le coût moyen des traitements était de 65602±106858 FCFA avec maximum de 567860 FCFA. Une évaluation des prescriptions dans les autres sites de traitement est nécessaire.

ABSTRACT

OBJECTIVE: To assess the pertinence and the cost of preoperative examinations prescriptions by surgeons at CHU-YO for programmed surgery. PATIENTS AND METHODS: This was a prospective and observational study of 145 patients received in preanesthetic consultations between February 4 and April 4, 2016 in the department of anesthesiology, intensive care unit and emergency of CHU-YO. It consisted of an evaluation of 1106 preoperative examinations prescribed by the surgeons of this hospital compared to the formalized recommendations of French experts. RESULTS: Out of a total of 1,106 prescribed preoperative examinations, 1069 (96.6%) have been achieved. The six most commonly prescribed tests were hemogram (95.9%), blood grouping (90.3%), prothrombin (PT) (91.7%), active cephalin time (ACT) (86.9%), blood sugar (92.4%) and creatininemia (89%). Ninety-two results (8.6%) examinations were abnormal. Prescriptions for preoperative examinations were relevant in 30.2% of cases. The average cost of performing preoperative examinations per patient was 19832±14023 FCFA. CONCLUSION: Prescriptions for preoperative examinations by surgeons are mostly non-rational, the application of a repository is essential to improve this practice.

PATIENTS ET MÉTHODES: Il s'est agi d'une étude prospective observationnelle portant sur 145 patients reçus en consultation préanesthésique entre le 4 février et le 4 avril 2016 dans le service d'anesthésie-réanimation et d'urgences du CHU-YO. Elle a consisté en une évaluation de 1106 examens préopératoires prescrits par les chirurgiens de cet hôpital, par comparaison aux recommandations formalisées d'experts français. RÉSULTATS: Sur un total de 1106 examens préopératoires prescrits, 1069 (96,6%) ont été réalisés. Les six examens les plus fréquemment prescrits étaient l'hémogramme (95,9%), le groupage sanguin (90,3%), la détermination du taux de prothrombine (TP) (91,7%), le temps de céphaline activée (TCA) (86,9%), la glycémie (92,4%) et la créatininémie (89%). CONCLUSION: Les prescriptions d'examens préopératoires par les chirurgiens sont majoritairement non rationnelles, l'application d'un référentiel y relatif est indispensable pour améliorer cette pratique.

BACKGROUND: Human norovirus (NoV) is an etiological agent associated with acute gastroenteritis (AGE) in both children and adults worldwide. However, very few studies have been reported on the prevalence and genetic diversity of NoV strains in children older than 5 years of age and adults with little or inadequate water and sanitation conditions. OBJECTIVES: The aim of this study was assessing the prevalence of the human norovirus in older children and adults suffering with diarrhoea from rural communities in the Vhembe district, Limpopo province. METHODS: Between August 2017 and October 2018, stool samples were collected from outpatients suffering from AGE and screened for NoV strains using the RIDA©GENE norovirus I and II real-time one-step RT-PCR. RNA extracts of NoV-positive samples were subjected to RT-PCR amplification and nucleotide sequencing to genotype the positive NoV strains. RESULTS: Out of 80 collected stool samples, 13 (16%) were tested positive for norovirus. Genogroup GII was identified in 6/13 (46%) samples and genogroup GI in 7/13 (54%) samples. The sequence analyses showed multiple genotypes including GII.Pg, GII.1, GII.2, GII.4, and GI.3. Phylogenetic analysis revealed the relatedness of NoV genotypes identified with other strains reported globally. CONCLUSION: Continued systematic surveillance to evaluate norovirus association with diarrhoea is needed to assist with epidemiological surveillance and disease burden in people of all the age groups.

ABSTRACT

Background: Breast cancer (bca) is the type of cancer most frequently diagnosed among women in Canada. Breast cancer is categorized into various molecular subtypes by the expression of estrogen receptor (er), progesterone receptor (pgr), and her2 (human epidermal growth factor receptor 2). Currently, Canada has no national cancer registry with epidemiology data by subtype. Thus, we conducted a study to determine incidence, survival, and clinicopathologic characteristics by bca subtype [triple negative breast cancer (tnbc); her2+; and hormone receptor-positive (hr+), her2-] in Canadian women newly diagnosed with bca. Methods: Female patients diagnosed between 1 April 2012 and 31 March 2016 (fiscal 2012-2015) were identified in the Ontario Cancer Registry, and individual patient data were linked to data in provincial health administrative databases. Descriptive statistics and Kaplan-Meier curves were generated. Results: In this cohort, 3277 women (9.5%) had tnbc, 4902 (14.3%) had her2+ bca, and 22,247 (64.8%) had hr+, her2-breast cancer. The annual incidence was 15 per 100,000 for the tnbc group, 21-23 per 100,000 for the her2+ group, and 97-105 per 100,000 for the hr+, her2- group. The lowest median overall survival (mos) of 8.9 months was observed in women with clinical stage iv tnbc. In comparison, the mos was 37.3 months in those with her2+ disease and 35.2 months in those with and hr+, her2- metastatic bca. Conclusions: In the present study, the most recent and largest administrative database analysis of a Canadian population to date, we observed a subtype distribution consistent with previously reported data, together with comparable annual incidence and overall survival patterns.
