ABSTRACT

In the courses of six years a severe hypophosphataemic osteomalacia, painful motor impairment and multiple rib fractures developed in a 51-year-old man. The symptoms gradually improved within one year under treatment with 3 micrograms daily of 1,25-dihydroxycholecalciferol, 3 g phosphorus and 3 g calcium, and biochemical parameters and the bone scintigram became normal. Ultimately, computed tomography, scintigraphy and digital subtraction angiography revealed a highly vascularized tumour in the condylar aspect of the right femur, and it was chiselled out. Histologically it was a mesenchymal phosphaturic tumour of haemangiopericytoma type of questionable benignity. After the operation the patient was symptom-free for some weeks without any drug treatment, but the latter was then resumed because of renewed bone pain. By now, two years later, he is essentially without pain and has full mobility. However, repeat scintigraphy and angiography revealed renewed tumour growth in the right femoral condyle.

ABSTRACT

The potential of magnetic resonance tomography (MRT) to demonstrate the mediastinal veins was evaluated retrospectively in 6 patients with superior vena cava obstruction. In each instance, MRT provided detailed information about the extent of venous obstruction and the precise site of the causative oncological pathology. Transaxial images most clearly and unequivocally depicted obstructed superior vena cava. MRT was also able to indicate some secondary effects of superior vena cava obstruction such as slow intravenous flow. The latter could be demonstrated by a significant increase of signal intensity within venous structures proximal to the obstruction. Peripheral venous collaterals in the chest wall were better seen with contrast enhanced computed tomography scans.

ABSTRACT

The most important angiographic features of atresia of the aortic isthmus are illustrated by three patients. The first patient to have reached an advanced age despite atresia of the aortic isthmus is reported and the diagnosis was confirmed at autopsy. The haemodynamic effects, the age at operation and the incidence of other associated cardiovascular malformations are discussed with reference to the literature.

ABSTRACT

Digital video subtraction angiography is a valuable method for the diagnosis of the relatively rare glomus caroticum tumour. This is shown at one case and proved with conventional angiography of the carotid artery and operation.

ABSTRACT

The article reports on a 56-year old patient with the very rare preoperative diagnosis of a cyst wall hypernephroma. This diagnosis was confirmed both surgically and histopathologically. Diagnosis with the help of roentgenography showed that the diagnosis which had been suspected on the basis of selective arteriography, received a high measure of certainty by the additional use of non-invasive sonography and computerized tomography, thus finally justifying surgical approach to lay open the kidney.
