ABSTRACT

The article presents the analysis of efficiency of medical services support related to laboratory analyses in case of alteration of system of Federal clinic financing. The analysis was implemented using the example of functioning of three departments of endoprosthesis replacement of the Federal State Budget Institution "The Academician G. A. Ilizarov Russian Research Center 'Restorative Traumatology and Orthopedics' " in 2013-2015. It is established that during examined period in the structure of laboratory analyses of the Center decreasing of total number of prescriptions to all types of laboratory analyses occurred. The observed decreasing of laboratory analyses had no impact to quality of treatment of patients of the departments of endoprosthesis replacement. In particular, percentage of post-surgery complications among patients of these departments was decreasing during the examined period. The study demonstrated that to the end of 2015 decreasing of average costs in rubles per patient of departments of endoprosthesis replacement related to laboratory analyses occurred up to 20.28% as compared with 2013. The proposals were made concerning structuring of laboratory analyses for patients with endoprosthesis replacement of large joints.

ABSTRACT

The investigation included 56 patients with MI of lower localization and acute left ventricle failure (ALVF). Group A included 33 patients with primary coronary balloon angioplasty (PCBAP), in group B there were 23 patients with MI and ALVF without revascularization. Group A was divided into 2 subgroups--A1 (patients with history of IM) and A2 (patients without previous MI). On the first and on the 14th days echocardiographic investigation was performed in all the patients. Ejection fraction (EF) of the LV in dynamics by the 14th day of the disease increased in group A from 32.8 +/- 1.8% to 36.1 +/- 1.6% (p < 0.05) at the expense of subgroup A2 (32.1 +/- 2.6% on the first day and 37.5% +/- 1.9 on the 14th day, p < 0.05), while in subgroup A1 there were no substantial changes of EF. In group B, on the contrary, there was decreased EF of the LV. Most of the group A patients had many injuries of the coronary bed vessels. After PCBAP of the right coronary artery (RCA) the increase of EF was followed by an improvement of the local contractility of the LV myocardium not only in the infarction zone, but also in the segments with ischemic dysfunction in which blood flows from the infarction-independent artery with its collateral filling from RCA.

ABSTRACT

The paper presents the results of a study of the gravimetric and counting concentrations of respirable asbestos fibers while working with sand-and-crushed stone mixtures, obtained from the concentration of chrysotile asbestos, at distances of 25, 50, and 100 m from the working place, as well as in a car saloon when the electric train passes along the area of these operations following an hour, a day, and a year after the completion of work, in warm and colds seasons of a year. It is concluded that the use of asbestos-containing sand-and-crushed stone mixtures on the railway leads to a higher anthropogenic asbestos load on the population living in the railway right-of-way, on railway workers and passengers. In this connection, it is necessary to evaluate risk factors of asbestos-induced diseases among the above contingents. The authors consider that due to the fact that asbestos-containing sand-and-crushed stone mixtures are well wetted with water, followed by the formation of a firm surface crust that prevents dust formation, as well as the short duration and rare frequency of operations relating their change, it is necessary to irrigate the repair areas with water or surfactant liquids after work termination.

ABSTRACT

The paper presents the results of field tests of a hopper wagon after carriage of mineral fertilizers, which are further to be used for grain carriage. The tests were carried out step by steps: washings from the inner surface of the hopper wagon after carriage of mineral fertilizers in order to identify chemical substances and 12 hours after its abrasive sandblasting; washings from the inner surface, air sampling for filters and into absorbable solutions after completion of wagon blowing 2 hours after dust removal and 30 days after application of an Epovin enamel. It has been concluded that most ions as components of mineral fertilizers were found in the amounts greater than the maximum acceptable concentrations on the walls and inside the hopper wagon after mechanical cleaning of its inner surface of the wagon that carries minerals, as well as its subsequent abrasive sandblasting and blowing.

ABSTRACT

The functions of interhemispheric EEG coherence were analyzed in 12 healthy subjects with the right individual profile of motor and sensor asymmetry and 7 subjects with the left profile in 2 experimental conditions: the state of rest and photostimulation of the central visual field. It was shown that in the rest condition the right-handed subjects have the higher values of EEG coherence in the thetal band in symmetrical frontal and central areas than the left-handed. These differences decreased for the frontal and central areas during activation caused by photostimulation but increased in the theta 2 and betal bands in symmetrical temporal areas (the coherence in the left-handed being higher). The difference in the EEG coherence between conditions was greater for the frontal and central areas in the right-handed than in the left-handed, especially, for the theta 1 and theta 2 bands. These findings suggest that the left-handed subjects have a less developed hierarchy of subcortical control of the functional state shifts than the right-handed.

ABSTRACT

Asbestos dust associated with other factors induces more hazardous effects, than if acting solely. That is seen especially in women whose morbidity grows 1.5-2.0 times higher than in men of the same occupations. Further studies are recommended for better knowledge asbestos dust influence under provocative action of other hazards.
