ABSTRACT

BACKGROUND: The pathologies of the salivary glands are very diverse. Salivary stone disease occupies a special place among diseases of the salivary glands of a non-neoplastic nature. The incidence of salivary stone disease is extremely extensive and accounts for up to 85% of all pathologies of the salivary glands. OBJECTIVE: Improvement of minimally invasive methods of treatment of salivary stone disease, which does not lead to excision of the gland. MATERIAL AND METHODS: On the basis of the Clinics of the Samara State Medical University of the Ministry of Health of Russia, in the period from 2010 to 2020, a clinical group was formed, consisting of 193 patients with salivary stone disease of various localization. The age of the patients ranged from 16 to 84 years. All patients were therapeutically examined at the preoperative stage, voluntary informed consent was signed. Shockwave sialotripsy was performed in 95 patients of the main group (in 64 patients the calculus was located in the submandibular salivary gland, in 31 patients - in the parotid salivary gland). For lithotripsy, we used a lithotripter apparatus «Edap¼ (France) and «Wolf¼ (Germany). RESULTS: The analysis of the results obtained as a result of the study made it possible to draw a conclusion about reasonably better results of removal of calculi in the ducts of the salivary glands using the lithotripter «Swiss LithoClast® Master¼ (Switzerland). At the first visit and the therapeutic effect of the lithotripter, all patients suffering from bouts of salivary colic showed complete cessation. CONCLUSION: Thus, our study, which was devoted to the study of the results of various methods of treating patients with salivary stone disease, showed that sialolithotripsy using special narrowly targeted devices for crushing and subsequent excretion of salivary calculi allows to achieve the best results at present.

ABSTRACT

One of the important factors for creating a beautiful smile is the color of the teeth, which for most people has a more important role than anatomical characteristics. The exact reproduction of the color of the tooth affects the appearance and psycho-emotional state of the patient. The purpose of this work is to review domestic and foreign literature on methods for determining the color of teeth. Methods for determining color are divided into visual, spectrophotometric, colorimetric, digital photo analysis and computer method. The most common in the practice of a dentist is visual, which is based on comparing the examined tooth with color templates. Determination of the color of teeth using scales does not give a reliable result, which is due to the small number of shades in the colors, the phenomenon of metamerism, the impossibility of converting the obtained data into digital values, etc. The hardware method for determining the color of teeth compensates for the disadvantages of the visual and has a high adaptability, which requires many interrelated factors. Due to the complexity and high cost, the devices are used mainly for research purposes. Currently, innovative methods for determining the color of teeth, based on digital and computer analysis, are being actively developed. Thus, hardware methods for determining the color of teeth significantly increase the accuracy of choosing the shade of a future aesthetic restoration or orthopedic construction, excluding subjective factors, however, they require mandatory certification. Correct color assessment and identification helps to reduce costly re-treatments and reduce warranty costs. Despite this, visual methods using scales remain in demand among practicing doctors, due to their low cost and ease of use. The accuracy of determining the color of teeth in this case depends on the professionalism of the dentist, as well as on the correctness of the photo protocol.

ABSTRACT

THE AIM OF THE STUDY: Was to conduct an experimental study of the bearing capacity and determination of the Young's modulus of lyophilized allogeneic bone material. MATERIALS AND METHODS: The study used 5 prototypes of lyophilized allogeneic bone blocks, which were alternately subjected to static loading by compressive force. The experimental values of Young's modulus of all bone samples were determined. When applying the Student's probability distribution in statistical processing of experimental data, the average indicators of the forces of destruction of the samples were obtained and Young's modulus was determined. RESULTS AND DISCUSSION: As a result of the study, the indices of the maximum loading (6012H) and Young's modulus (147.2±0.228 MPa) of lyophilized allogeneic bone material were obtained. CONCLUSION: The obtained Young's modulus of lyophilized allogeneic bone material is of great importance in scientific activity, providing the possibility of further investigation of the properties of lyophilized allogeneic bone tissue to form a preoperative forecast of the behavior of an osteoplastic material and a dental implant installed in it.

ABSTRACT

AIM: To study the morphological state of the maxillofacial system in children with cerebral palsy. MATERIAL AND METHODS: The frequency and prevalence of dental anomalies was studied in 30 9-15 years old children with cerebral palsy. For the registration of maxillofacial anomalies, the Bjork assessment method was used. The severity of sagittal incisive disocclusion and the reverse sagittal incisal disocclusion was measured by the magnitude of the sagittal slit. RESULTS: The study revealed no age-dependent correlation of lower canines and second premolars roots growth. The parameters of the cerebral part of the skull measured on the CT-bases lateral cephalograms were significantly less than normal rates except for the length of the posterior part of the skull. CONCLUSION: The morphological changes that determined the formation of sagittal and vertical incisive disocclusions were defined: the reduction of total anterior morphological face height (N-Me) and anterior upper morphological face height (N-SpP) and the increase in the anterior lower morphological face height (SpP-Me) which are characteristic features for patients with incisive sagittal and vertical diocclusion.

ABSTRACT

AIM: To develop the technology of manufacturing of reconstructive allogenic bone graft with individual parameters by using of digital prototyping and modeling. MATERIAL AND METHODS: Allogeneic cadaveric bone blocks were used as material. The following methods were used to develop a manufacturing technology for personalized allogenic reconstructive bone implant: harvesting and special preparation of cadaveric material, primary disinfection, viral inactivation, ultrasound management, lyophilization of bone material, CT-examination of patients with various bone defects, digital prototyping and modeling, milling on a machine with numerical program control, X-ray sterilization. RESULTS: The technology of manufacturing of reconstructive allogenic bone graft with individual parameters was successfully tested in the treatment of 24 patients with post-traumatic, destructive-dystrophic and degenerative defects of jaws in 2012-2015. Final reconstructive allogenic bone implant has a personalized 3D-geometry in accordance with individual maxillofacial anatomy of particular patient. One of implant surfaces is fully congruent to the jaw bone defect, the others form the width, height and volume of reconstructive bone implant considering personified maxillofacial features. All above-mentioned features of bone implant are important to restore chewing function of dental system. CONCLUSION: The problem of bone implant personification may be resolved by using of computed tomography followed by digital analysis of jaw structure, technologies for digital reconstruction of bone defects (digital prototyping and modeling) and digital manufacturing.
