ABSTRACT

Excessive occupational exposure to noise results in a well-recognized occupational hearing loss which is prevalent in many workplaces and now it is taken as a global problem. Therefore, this study aims to assess the prevalence of noise-induced hearing loss and associated factors among workers in the Bishoftu Central Air Base in Ethiopia. An institutional-based cross-sectional study was conducted among 260 central air base workers through face-to-face interviews, an environment noise survey, and an audiometric test for data collection. Data were entered by Epi-data version 3.1 and SPSS was used to analyze the data. Finally, a statistical analysis such as descriptive and binary logistic regression analysis was applied. A P-value < 0.05 at 95% CI was considered statistically significant. The overall prevalence of noise-induced hearing loss and hearing impairments was 24.6 and 30.9%, respectively. The highest prevalence of noise-induced hearing loss was recorded for workers who were exposed to noise levels greater than 90 dBA. Out of 132 workers exposed to the average noise level of 75 dB A, only 5% of workers were affected with noise-induced hearing loss, while 128 workers exposed to an average noise level equal to or greater than 90 dB A, 19.6% of workers were identified with noise-induced hearing loss. Regarding sex, around 21.9% of male workers were identified with noise-induced hearing loss. Workers who were exposed to a high noise level workplace previously or before the Central Air Base workplace were five times (AOR = 5.0, 95% CI 1.74-14.36) more likely affected by noise-induced hearing loss than those workers not previously exposed. Those workers who were exposed to greater or equal to 90dBA noise level were 4.98 times (AOR = 4.98, 95% CI 2.59-9.58) more likely to be exposed to noise-induced levels than those who were exposed to less than 90dBA noise level. Moreover, male air base workers were 3.5 times more likely exposed to hearing impairment than female workers (AOR = 3.5, 95% CI 1.01-12.0). This study identified that the prevalence of noise-induced hearing loss and hearing impairments was significantly high. So implementation of a hearing conservation program, giving noise education, and supplying adequate hearing protective devices (HPDs) are essentials.

ABSTRACT

Biogas toilets are one of the most resource-efficient sanitation technologies. The technology has dual purposes of generating energy and stabilizing waste-producing biofertilizers. In Ethiopia, knowledge of human feces' energy potential is limited to optimize the development of biogas toilet facilities. Therefore, this study was aimed to evaluate the biogas and biofertilizer potential of human feces in Jimma City, Ethiopia, which may contribute to the development of sustainable sanitation technologies. The study was lab-based experimental design. In the lab-scale batch experiment, fresh human excreta samples were collected using a urine diversion raised toilet. Using ultimate and proximate laboratory analyses, the theoretical yield of biogas was predicted. Then a series of anaerobic digestion batch experiments were conducted to determine the practical energy yield. The bio-fertilizer potential of human feces was determined by analyzing the nutrient contents of human feces. The findings of this study showed that the bio-methane yield from the experimental results has a mean of 0.393 m3 kg-1, which is 14.16 MJ kg-1. The bio-methane meter cube per capita per head per year were 28.71 (28.03-29.27) from the experimental result and 45.26 for the theoretical yield of methane. In this study, the bio-fertilizer potential of human feces was evaluated using nutrient analysis, specifically the NPK (nitrogen, phosphorus, and potassium). Accordingly, human feces contain potassium (2.29 mg kg-1), phosphorus (1.12 mg kg-1), and nitrogen (3.71 mg kg-1). This finding suggests the bio-methane potential of human feces can be used for energy recovery and alternative sanitation options, providing a positive remedy for the sanitation crisis in urban settings.

ABSTRACT

BACKGROUND: COVID-19 pandemic posed a major impact on the availability and affordability of essential medicines. This study aimed to assess the knock-on effects of the COVID-19 pandemic on the supply availability of non-communicable chronic disease (NCD) medicines and paracetamol products in Ethiopia. METHODS: A mixed methods study was conducted to assess the supply and availability of twenty-four NCD drugs and four paracetamol products listed on the national essential medicines list for hospitals. Data were collected from twenty-six hospitals located in seven zones of Oromia region in the southwestern part of Ethiopia. We extracted data on drug availability, cost and stock out for these drugs between May 2019 and December 2020. The quantitative data were entered into Microsoft Excel and exported to statistical package software for social science (SPSS) version 22 (IBM Corporation, Armonk, NY, USA) software for analysis. RESULTS: The overall mean availability of selected basket medicines was 63.4% (range 16.7% to 80.3%) during the pre-COVID-19 time. It was 46.3% (range 2.8% to 88.7) during the pandemic. There was a relative increase in the availability of two paracetamol products [paracetamol 500 mg tablet (67.5% versus 88.7%) and suppository (74.5% versus 88%)] during the pandemic. The average monthly orders fill rates for the selected products range from 43 to 85%. Pre-COVID-19, the average order fill rate was greater or equal to 70%. However, immediately after the COVID-19 case notification, the percentage of order(s) filled correctly in items and quantities began decreasing. Political instability, shortage of trained human resources, currency inflation, and limited drug financing were considered as the major challenges to medicine supply. CONCLUSION: The overall stock out situation in the study area has worsened during COVID-19 compared to pre-COVID-19 time. None of the surveyed chronic disease basket medicines met the ideal availability benchmark of 80% in health facilities. However, availability of paracetamol 500 mg tablet surprisingly improved during the pandemic. A range of policy frameworks and options targeting inevitable outbreaks should exist to enable governments to ensure that medicines for chronic diseases are consistently available and affordable.

ABSTRACT

Micro-nutrients are required in small amounts to maintain growth and cell functioning to complete the life cycle through reproductions. However, pregnant women in developing countries like Ethiopia are vulnerable to multiple micro-nutrient deficiencies. Therefore, this study aimed at assessing the serum level of metals and associated factors like dietary diversity, and health-related problems in pregnant women among small-scale farming in Kellem Wellega, western Ethiopia. A cross-sectional laboratory-based study was conducted from June to August 2021 on 417 pregnant women attending antenatal care at rural healthcare facilities. Data was collected by using per-tested structured questionnaires via face-to-face interviews. The data analysis was conducted using SPSS version 24.0, and multivariate logistic regression analysis was performed to determine the association between predictor and outcome variables. A statistically significant was considered at p value < 0.05 for all the analyses. Our study findings showed that 62.1, 80.9, and 71.9% of the participants were deficient in iron, zinc, and copper micro-nutrient levels, respectively. Pregnant women who could not have formal education were 3.24 (AOR = 3.24, 95% CI 1.33-7.91) and 3.98 (AOR = 3.98, 95% CI 1.2-13.15) times more likely to show zinc and copper deficiency than those who attended secondary school and above, respectively. Furthermore, pregnant women involved only in farming activities were 0.57 (AOR = 0.57, 95% CI 0.36-0.91) and 4.33 (AOR = 5.72, 95% CI 2.34-13.97) times more likely to be exposed to iron and zinc deficiency than those who were engaged in other activities. This study revealed that pregnant women with low income were 6.36 times more likely to be exposed to zinc micro-nutrient deficiencies than those with high-income participants (AOR = 6.36, 95% CI 1.47-27.61). Additionally, those participants who ate a varied diet between 1 and 4 items per day were 2.26 (AOR = 2.26, 95% CI 1.43-3.59) and 2.77 (AOR = 2.77, 95% CI 1.6-4.61) times more likely to suffer zinc and copper micro-nutrient insufficiency than those who consumed 5-10 items per day. Finally, pregnant women who developed diarrhea in the past three months were 1.82 (AOR = 2.77, 95% CI 1.14-2.92) and 2.52 (AOR = 2.52, 95% CI 1.3-4.91) times more likely to be exposed to iron and copper deficiency than those who never show the symptoms, respectively. This study identified low concentrations of zinc, iron, and copper in the blood serum of pregnant women of small-scale farmers.

ABSTRACT

On-site sanitation is the most popular sanitation option for households in many African countries, including Ethiopia. Despite the option being common, there are challenges attributed to the sustainability of those facilities. This community-based exploratory study aimed to explore the challenges in the provision of safe urban on-site sanitation in Jimma town, Ethiopia. Data were collected through key informant semi-structured interviews, transit walks through the village, and focus group discussion (FGD). The FGD was audio recorded and notes were taken by experts. Records and notes were transcribed separately and thematically analyzed. Three major themes or challenges were discovered. Accordingly, the first theme was the community's perception and taboos surrounding human waste. The second theme was urban land use and informal settlements. Ensuring standards separately for sanitation facilities is difficult because of the expansion of illegal settlements. The third theme was poor sanitation planning, operation and maintenance, and institutional setting. Urbanization without infrastructure generally has a complex nature that leads to a fragile sanitation situation in the future. The results suggest the need for multi-dimensional system development and a collaborative sanitation master plan. Furthermore, interventions aimed at breaking the taboo on human waste as a sanitation platform may turn challenges into opportunities.

ABSTRACT

Slums are urban areas with insufficient public services and access to sanitation. Evidence-based selection of sustainable sanitation options is critical for addressing the sanitation crisis in slums. This mixed methods study was conducted in Jimma Town, southwest Ethiopia, to assess sanitation status and prioritize sustainable sanitation options for slums. The study was done in 2 phases: quantitative and qualitative. The quantitative cross-sectional household survey aimed to assess sanitation status and the qualitative exploratory method to explore alternative sanitation options and prioritize sustainable alternatives. A total of 310 households were chosen using systematic random sampling methods, of which 302 participated. Data was gathered through interviews, which were supplemented with questionnaires and observation checklists, and 2 focus group discussions (FGD) were held. First, FGD was with expertise in the sanitation sector, and the second was with community members. The state of sanitation was summarized, and multi-criteria analysis (MCA) was used to prioritize sustainable sanitation options. According to our findings, 68% of households had access to improved facilities, and 22.5% didn't have any form of toilet facility. About 7 off-site and on-site user interface sanitation technology options were considered in the selection of alternative sanitation technologies, and each option was evaluated against 17 health, economic, social, technical, and environmental criteria. In the final analysis, the options with the highest scores for the setting were flush to septic tanks, compost toilets, and biogas toilets. Mobilizing such a promising sanitation option is recommended for future interventions.

BACKGROUND: Adolescents and youths in Ethiopia have limited access to reproductive health services designed to address their needs. Therefore, the study aims to assess adolescents' and youth's sexual practice, contraceptive use, and behavioral patterns towards safe sexual exercise. METHODS: A quantitative cross-sectional study design was used on 374 students selected from high school and vocational colleges to assess their sexual experience and contraceptive use and related perceptions using pre-tested self-administered semi-structured questionnaires. We used descriptive analyses to report their sexual and reproductive health status and logistic regression to examine the association between contraceptive use and other variables. RESULTS: The results show that 52.7% of the respondents (students) migrated or moved from rural to urban to continue their high school and college education at urban. 41.7% of the respondents were with less than 18 years, of which 75.9% of them already initiated sexual intercourse. Of all the students who started sexual intercourse (51.1%), only 30.9% used contraceptives, but condom use accounts for 49.2%. Overall, 84.8% of those who practiced sexual intercourse were at risk of acquiring sexually transmitted diseases due to not using a condom. About 3% of the respondents reported unwanted pregnancy experiences, of which 64% of these pregnancies were reported to be aborted. CONCLUSION: Adolescents and youths attending high schools and College are at increased risk of acquiring HIV and unwanted pregnancy due to low levels of contraceptive use. Specifically, a designed youths-friendly reproductive health service is needed to avert related problems and contribute to sustainable development goals.

ABSTRACT

Sustainable development goal seven aims to provide access to affordable, reliable, sustainable, and modern energy for all by 2030, but its progress and energy stacking conditions have not been evaluated. This study aims to assess the urban household energy profile and link its state to sustainable development goals. This study employs a cross-sectional study design on 265 households selected by systematic random sampling from a town called Jimma in Ethiopia and collects the data using interview-based semi-structured questionnaires. The study obtains information from all the selected households. The results show that more than 80% of the households have a grid electricity connection, but more than 85% of the households regularly use firewood and charcoal for cooking. On average, households use about 1236 kg of firewood and 630 kg of charcoal per year. Most households report power interruption, inability to afford electricity costs, and personal preferences for relying on biomass energy for cooking. Over 98% of the households use electricity for lighting, but few use it for appliances. This shows an inherent challenge attributed to the cooking energy services provided by biomass energy sources despite the accessibility of electricity. This shows the significant impacts of biomass energy stacking which connection to electricity alone cannot solve.

Background: The use of plastic shopping bags increases and poses tremendous pressure on the local environment. However, little is known about its utilization among different population categories and their perception of its utilization and willingness to use other sustainable alternatives. This study aimed to assess the community's perception toward the use of plastic shopping bags and its options in Jimma town, Ethiopia. Methods: A community-based descriptive cross-sectional study design was conducted on 351 customers and retailers selected from the town's main marketing areas. The data was collected using a structured questionnaire in a face-to-face interview and analyzed using SPSS v.21. Results: The results show that all the respondents use plastic shopping bags for different shopping services. The majority (147 (41.9%)) of the respondents said that they use 5 to 10 shopping bags, and 66 (18.8%) use more than 10 shopping bags per week. Participants responded with different reasons for the frequent use of plastic bags for shopping. Accordingly, 116 (33.0%) responded that plastic bags are cheap, 92 (26.2%) use them due to lack of alternatives, and 89 (25.4%) responded that plastic bags are light and convenient to use. However, 326 (93%) of the respondents support efforts to reduce single-use plastic bags, 284 (80.9%) support a ban on single-use plastic bags, and 319 (90%) were willing to pay for alternative shopping bags. Conclusion: Communities are aware of the environmental problems associated with the intensive use of plastic shopping bags, their wastes, and the need for alternative options. However, it will continue to pose significant environmental challenges unless low-cost and environment-friendly alternative options are available.

OBJECTIVE: Health extension workers' (HEWs') knowledge and attitude regarding mental health problems are vital for integrating mental healthcare into primary healthcare services. However, information in this context is scarce in Ethiopia. So, this study aimed to assess HEWs' knowledge and attitude towards mental health problems in Jimma, Ethiopia. DESIGN: A cross-sectional study. SETTING: Five districts in Jimma Zone, Ethiopia, 2020. PARTICIPANTS: A total of 259 HEWs working in selected five districts were included in the study. OUTCOME: The knowledge and attitude status of HEWs towards mental health problems. RESULTS: Nearly half of the respondents had inadequate knowledge (122,47.1%) and an unfavourable attitude (125, 48.3%). Most (139, 53.7%) described mental illness as due to evil spirits' possession. Almost all (240, 92.7%) of the respondents reported talking or laughing alone as a manifestation of mental illness. Almost two-thirds (157, 60.6%) of the respondents reported people with mental illness are dangerous. About a quarter (63, 24.3%) of the participants stated witch doctors should manage mental illnesses. CONCLUSIONS: A significant proportion of the study respondents had poor knowledge and attitude towards mental health problems. Short-term and long-term mental health training is needed to improve their perception level and to provide effective community mental health services.

OBJECTIVE: This study aims to assess the prevalence of pesticide use and its occupational exposure among small-scale farmers in the Kellem Wellega Zone of western Ethiopia. METHODS: A cross-sectional study design using a structured questionnaire was used to collect data from 249 small-scale farmers' households through face-to-face interviews. Statistical analysis such as descriptive statistics, Chi-square test, and binary logistic regression analysis was applied, and a P-value <.05 at 95% CI was considered statistically significant. RESULTS: The prevalence of pesticide use was 87.15%. About 44.2%, 40.2%, and 43.8% of the study participants were classified as having poor knowledge, poor practice, and negative attitude toward pesticide use, respectively. Thus, small-scale farmers whose age was greater than 40 years were 7.87 times more likely to be exposed to skin irritation than those whose age was less than 20 years (AOR = 7.87; 95% CI: 1.75-35.45) and skin contact (AOR = 0.37; 95% CI: 0.15-0.91). Most farmers who were directly involved in agriculture were 2.22 times more likely to be exposed to the inhalation of pesticide chemicals than those involved in another activity (AOR = 2.22; 95% CI: 1.14-4.33). Based on educational level, small-scale farmers who have a primary school and above were 81% less likely to inhale pesticide chemicals than those who did not have formal education (AOR = 0.19; 95% CI: 0.09-0.41). Furthermore, low-income small-scale farmers were 2.62 times more likely to be exposed to coughing (AOR = 2.62; 95% CI: 1.25-5.51) than high-income participants. Furthermore, farmers with good knowledge were 1.79 times more likely to be exposed to skin irritation than those with poor knowledge (AOR = 1.79; 95% CI: 1.0-3.17). Farmers with poor practice were 1.85 times more likely to show coughing symptoms than those with good practice (AOR = 1.85; 95% CI: 1.08-3.2), and farmers with good practice were 48% less likely to be exposed to headache than those with poor practice (AOR = 0.52; 95% CI: 0.31-0.88). CONCLUSIONS: This study shows that small-scale farmers were exposed to pesticides through coughing, headache, skin irritation, inhalation, and skin contact. Low level of knowledge, poor practice, job, low income, older age, and educational level.

ABSTRACT

BACKGROUND: Trachoma is a worldwide infectious disease causing blindness. Trachoma continued as a public health problem in Ethiopia due to a lack of sanitation and inadequate prevention strategies. This study aimed to identify the impact of water supply and sanitation intervention on preventing active trachoma among children. METHODS: Systematic literature searches were performed from 4 international databases. The search involved articles published from January 1995 up to March 2019. The Cochran Q and I2 statistical tests were used to check heterogeneity among the studies. A random-effect meta-analysis was employed to determine the pooled estimates with a 95% confidence interval (CI). Data analysis was performed using the CMA V.3 and RevMan 5 software program, and the result of the systematic review was reported using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. FINDINGS: Out of 211 studies screened for the analysis, only 29 studies were finally included in this systematic review and meta-analysis. The result revealed factors that are significantly associated with increased odds of active trachoma. Accordingly, households with no access to toilet facilities (odds ratio [OR]: 2.04, 95% CI: 1.75-2.38), no access to improved water (OR: 1.58, 95% CI: 1.27-1.96), and do not practice regular face washing for children (OR: 4.19, 95% CI: 3.02-5.81) have shown increased odds of active trachoma. Besides, the results show a higher prevalence of active trachoma among children who did not wash their faces with soap and frequently. CONCLUSIONS: The study found strong evidence that lack of access to water, sanitation, and hygiene (WASH) was associated with increased prevalence of active trachoma among children. Therefore, a comprehensive and partnership-oriented program is needed to tackle the problem, but further study will be required to strengthen its implementation.

BACKGROUND: Medical students often experience chronic stress. Self-esteem is one of the most important factors in the process of psychosocial growth and has remarkable effect on thoughts, feelings, values, and goals. The aim of this study was to assess the prevalence and associated factors of low self-esteem (LSE) and mental distress among Medical Students of Jimma University. METHOD: This cross-sectional study was conducted in Jimma University from June to July, 2018. Self-esteem was measured with Rosenberg self-esteem scale. Self-administered Short form with General Health Questionnaire was used to assess presence of mental distress. RESULT: Out of 422 students enrolled to the study, 279 (66.1%) were male, and 413 (97.9 %) were 18 to 25 years of age. The prevalence of LSE and mental distress were 19.0%, and 19.7 %, respectively. Students who had LSE had 5 times higher risk of having mental distress, AOR= 5.1 (95% CI, 2.9-8.9). Moreover, female students had higher risk of developing mental distress (AOR=1.9, 95% CI, 1.1-3.3). Students who had poor social support were 4.3 times at higher risk of developing LSE, AOR=4.3 (95% CI, 1.9-9.8). Those who reported to have poor academic performance were also more likely to have LSE AOR= 3.7 (95% CI, 1.3-10.0). CONCLUSION: One in five medical students had LSE and it is strongly associated with metal distress. Female students were at higher risk of mental distress. Preventive, curative and rehabilitative mental health services should be available for medical students with particular attention to those with poor social support.

BACKGROUND: The presence of agricultural pesticide residue can cause adverse health effects.The main objective of this study was to evaluate the level of pesticides in the blood of small-scale farmers (SSFs) and associated risk factors in western Ethiopia. METHODS: Across sectional study was conducted in June 2020 using 240 blood samples, 140 from the exposed small-scale farmers (ESSFs) and 100 from non-exposed small-scale farmers (NESSFs). The blood sample analysis was mad for 5 organochlorines (OCs) and 3 synthetic pyrethroids (SPs) pesticides by gas-liquid chromatography with an electron capture detector (GC-ECD) methods. Extraction, and clean up of the samples were made by using standard analytical methods. To define the relationships between the outcomes and explanatory variables, logistic regression models were used. RESULTS: The results shows that p,p'-DDT, heptachlor and deltamethrin were the most frequently detected pesticides with 96.4%, 95%, and 100% in both ESSFs and NESSFs, respectively. The ESSFs blood samples have shown the highest mean concentrations of permethrin and p,p'-DDT(1.26 ± 0.15) and (0.28 ± 0.4) mgL-1, respectively. SSFs under the age of 40 were 21% less likely to be exposed to permethrin than those above the age of 40 (Adjusted Odd Ratio, AOR, 0.21; 95% CI: 0.1-0.44). Male SSFs were 17 times more likely to be exposed to heptachlor than females (AOR, 17.36; 95%CI: 7.34-41.09) and farmers with no formal education were 18 times more likely to be exposed to deltamethrin than those with primary schools and beyond (AOR, 18.1; 95 %CI: 4.53-72.06). Furthermore, SSFs that did not use PPE appropriately were 3.6 and 6.21 times more likely to be exposed to cypermethrin (AOR, 3.6; 95%CI: 1.94-6.54) and p,p'-DDE(AOR, 6.21; 95%CI: 3.38-11.41) blood levels than those who did, respectively. SSFs that perform different activities like eating and drinking (11%), chewing (10%), and diverse activities (8%) were more likely to be exposed to p,p'-DDT than those farmers who didn't use pesticides. CONCLUSIONS: This study identified a high concentration p,p'-DDE, p,p'-DDT, heptachlor, cypermethrin, permethrin, and deltamethrin in the blood of small scale farmers. The older age, less education, and farmers with inappropriate PPE use are more likely exposed to pesticides.

ABSTRACT

BACKGROUND: Knowledge of the community regarding mental health problems has a remarkable impact on the attitude, the help-seeking path, and prevention of stigma and discrimination against patients with mental health problems. It is also the cornerstone for designing evidence-based community mental health interventions. However, the evidence is scarce in developing countries like Ethiopia. This study aimed to assess the knowledge regarding mental health problems and associated factors among communities of Jimma Zone, Oromia, Ethiopia. METHODS: A community-based cross-sectional study was conducted in the Jimma zone from March 1 to 22, 2020. A structured, pretested, and interviewer-administered questionnaire was used to collect data from 420 study participants selected through a systematic sampling technique. The knowledge about mental health was measured by the adapted version of the Mental Health Knowledge Schedule tool. Data were entered into Epi-data version 3.1 and exported to SPSS version 23.0 for analysis. Multivariate logistic regression analysis was done, and p-value < 0.05 and 95% CI were used to determine the predictors of the outcome variable. RESULTS: The overall knowledge score showed (188, 44.8%) of the respondents had inadequate knowledge. Moreover, (75, 17.9%) of the respondents reported psychiatric disorders are contagious, and (138, 32.9%) mentioned leaving alone is the treatment for mental illness. Talking or laughing alone and showing strange or unusual behaviors were described as symptoms of mental illness by the majority (407, 96.9%) and (403, 96.0%) of respondents, respectively. Brain dysfunction was attributed to the cause of mental illness by most (390, 92.9%) of the study participants. Similarly, the percentage of responses that attributed the causes of mental illness to possession by an evil spirit, God's punishment, and witchcraft were significantly high (368, 87.6%), (321, 76.4%) and (259, 67.1%), respectively. Furthermore, regression analysis showed that respondents who were able to read and write were 64% less likely to have adequate knowledge than those in secondary school and above educational status (AOR = 0.34, 95% CI (0.16-0.69)). CONCLUSION: Knowledge of mental illness among the general public was relatively poor and higher levels of education were associated with good knowledge of mental health problems; this suggests the need for due emphasis on public education to improve the mental health literacy status of the community.

ABSTRACT

Background: The global burden of mental health problems is high and is predicted to rise. At present, mood symptoms are the foremost common psychological problems worldwide, yet little is known regarding their magnitude and associated factors in developing countries. Therefore, this study aimed to assess the magnitude and associated factors of anxiety, depressive, manic symptoms, and suicidal behavior among the rural Jimma community, Ethiopia. Methods: A community-based quantitative cross-sectional survey was employed on 423 households selected through systematic random sampling. An adapted version of the Mini International Neuropsychiatric Interview tool was used for the structured face-to-face interview. The collected data were checked for completeness, coded, and inserted into Epi Data version 3.1 and exported to SPSS version 23 for analysis. Variables with P < g0.05 and odds ratio (OR) [95% confidence interval (CI)] on multivariate logistic regression analysis were considered as factors associated with the outcome variable. Results: Overall, 185 (44.0%), 55 (13.1%), 44 (10.5%), and 23 (5.5%) of the respondents had anxiety, depressive, manic symptom, and suicide behavior, respectively. The odds of having anxiety symptoms were nearly 5 times higher among those who had perceived discrimination and racism experience compared to their counterpart [adjusted OR (AOR), 5.02; 95% CI, 1.90-13.26]. Likewise, recently bereaved participants had 4-fold higher odds of reporting depressive symptoms (AOR, 3.9; 95% CI, 1.4-10.4) than the non-bereaved ones. Furthermore, respondents who had depressive symptoms were almost four and a half times more likely to have manic symptoms compared to those who did not (AOR, 4.3; 95% CI, 1.71-11.02). Conclusion: Anxiety, depressive, manic symptoms, and suicidal behavior were prevalent in the community and positively associated with multiple psychosocial factors. Implementing accessible and affordable community-based mental health services is recommended to mitigate the problems.

ABSTRACT

BACKGROUND: Cosmetic products emits Total Volatile Organic Compound (TVOC) and Particulate Matter with an aerodynamic diameter of 10 micrometers (PM10) of different sizes and characteristics with adverse health effects. Despite the increasing need for cosmetic products, related pollutants level of concentration from beauty salon is not well understood in developing countries. OBJECTIVE: This study aims to assess indoor air pollutant concentrations in the beauty salon and self-reported health problems among the salon workers in Jimma town. METHODS: A cross-sectional study design was used on 87 beauty salons from May 13-24, 2019. The concentrations of PM10, TVOCs, CO2, room temperature, and relative humidity were measured and triangulated with the survey data collected through measurements and questionnaires. A statistical software package, SPSS v.21, was used to analyze the data. A binary logistic regression was used to analyze categorical data and linear regressions to predict pollutants level and associated health outcomes. RESULTS: The results show that 93.1% of the respondents are females, and 85% were below 30 years old. More than 60% of the respondents were married individuals. 56.3% and 44.8% of the workers work over 10 hours per day and work the whole week. 34.6% of the workers reported as worked during pregnancy. About 70% of the workers know the harmful effects of cosmetics, benefits of ventilation, and Personal Protective Equipment (PPE) use, but only 19.4% use face masks. The majority (88.5%) reported health problems after starting work in the beauty salon. The mean volume of the beauty salon was 36.3 m3, with a mean PM10 concentration of 0.465 mg/m3 and a mean TVOC concentration of 1034.2 µg/m3. These air pollutants have shown a statistically significant association with self-reported health problems. Hence, urgent intervention with subsequent continuous awareness creation is needed to reduce the health consequences of a beauty salon's indoor air pollutants.

ABSTRACT

PURPOSE: The Ethiopian Ministry of Health has integrated mental health services into the community health service, but it has not yet been implemented. Therefore, this study aimed to explore the barriers and facilitators of mental health service implementation in the Ethiopian community health program. METHODS: A qualitative case study was conducted in the Ethiopian primary health care system in 2019. We have conducted about eight key informant interviews with policymakers, service managers, and service providers. The interviews were tape-recorded, transcribed, translated, and analyzed manually using the World Health Organization building blocks framework. RESULTS: The Health Ministry of Ethiopia has recently included mental health services into the health extension package but not yet implemented as part of integrated services. The identified barriers were low political commitment, shortage of resources, non-functional referral system, lack of interest from private health service organizations, attitudinal problems from both the society and service providers, and lack of consistent reporting system of the mental health problems. However, the well-designed primary health care system, trained health extension workers, changing political commitment and attitude of the community could facilitate the mental health service implementation. CONCLUSION: A series of activities are expected, especially from the healthcare system managers to implement, follow, and evaluate mental health services implementation at the health extension programs.

ABSTRACT

BACKGROUND: Governments have imposed physical and social distancing measures together with hand hygiene to contain COVID-19. However, maintaining the recommended social distancing less likely applicable in developing countries where the majority of the people depend on daily income. Countries like Ethiopia imposed universal use of face masks as an additional measure. AIM: This study aims to review and present the advantage and challenges of imposing universal use of face masks in the community and to forward possible recommendations. METHODS: The study was synthesized using a narrative literature review approach involving COVID-19 databases, published literature on COVID-19, and relevant news. RESULTS: The first confirmed case of COVID-19 in Ethiopia was reported on March 13, 2020. Consequently, the government has taken swift actions including social distancing and strict hygienic measures to contain its transmission. However, new cases have started to increase with local transmission. Hence, the country imposed additional measures including universal use of face masks at public places. The universal use of masks requires a sufficient supply of masks with affordable costs. Moreover, awareness of the proper use of face masks and its disposal is another challenge affecting its effectiveness against corona virus infection. CONCLUSION: Universal use of face masks can contribute to the containment of the virus in the community if adequately available and properly used and managed after use. Encouraging local production with clear guidance, demonstration, and awareness creation helps to improve the accessibility and effectiveness of face masks particularly among the vulnerable population.

ABSTRACT

BACKGROUND: Recent studies have shown an increase in open defecation and slippage of open defecation-free certified villages in Ethiopia, despite significant progress the country made on sanitation programs. Hence, realizing of existing facts, this study was conducted aiming at a critical review of available literature and to provide consolidated data showing the level of slippage and its associated factors in Ethiopia. METHODS: Systematic literature searches were performed from four international databases. The search involved articles published from December 1, 2013, up to June 4, 2019. The Cochran's Q and I2 test statistics were used to check heterogeneity among the studies. To negotiate heterogeneity from qualitative data, we used a mixed-method approach. The researchers also conducted a publication bias assessment and sensitivity analysis. A random effect meta-analysis was employed to determine the pooled estimates of open defecation free slippage rate with a 95% confidence interval (CI). The data analysis was performed using the CMA V.3 software program. RESULT: After screening 1382 studies, 12 studies were finally included in this systematic review. The estimated pooled rate of open defecation-free slippage in Ethiopia was 15.9% (95% CI 12.9-19.4%). The main contributing factors for open defecation-free slippage were lack of technical support, financial constraints, low-quality building materials, improper program implementation, and lack of sanitation marketing. CONCLUSION: It was estimated that 1 out of 6 Ethiopian households engaged in open defecation after they have certified open defecation-free status, implying the low possibility of achieving sustainable development goals of 2030, which aims to ensure sanitation for all. Therefore, the government of Ethiopia and donors should better give special attention to the following options: (1) awareness for open defecation-free slippage, (2) launch a post-open defecation-free program, and (3) encourage research on pro-poor sustainable sanitation technologies.
